주의집중 피로회복이론의 장으로 본 경주 옥산서원 강학 및 유식공간의 일원적 공간성 (A Study on the Coexistance of Ganghak(講學) and Yusik(遊息) space of Oksan Confucian Academy, Gyeongju: Directed Attention Restoration Theory Perspectives)

  • 조선 중기 대표적 서원중의 하나인 옥산서원을 사례대상으로, Kaplan의 "주의집중회복이론 틀"을 적용시켜, 서원의 기호학적 의미체계로서의 강학과 유식공간 상호 간의 보완적 특성을 파악함으로써 전통조경의 함축적 의미 도출과 경관해석의 지평을 넓히고, 현대적 학교 공간계획과 조경설계에 일조할 수 있는 논거를 구축하고자 한 본 연구의 결론은 다음과 같다. 옥산서원의 강학과 유식공간의 공간과 경관의 일원성을 통해 주의집중과 회복, 자연과의 합일 그리고 심성론과 수양론을 관통하는 성리미학적 속성이 확인되었다. 이는 정보처리과정을 위한 주의집중을 효과적으로 재개하는 회복적 공간특질로 설명할 수 있다. 옥산서원과 그 주변 환경은 입지와 공간구성, 공간포치 그리고 당호 등을 통한 기호체계의 일부로서, 강학과 유식공간의 일원성을 설명하는 유기체적 환경이며 이는 주의집중으로 부터의 네 가지 회복적 환경요소과 결부되어 나타난다. 즉 자연완상은 강학의 연장인 유식을 통해 강학의 궁구함을 가져오는 체험을 넓히게 한다. 요컨대 '공부'와 '쉼'의 과정은 완물적정을 통해 존재의 의미를 발견하고 인간의 심성을 수양하며 도덕을 체득하게 유도되고 있다. 옥산서원은 입지적으로 읍치로 부터 일정 거리를 두고 떨어진 '벗어남'과 서원의 조망경관과 공간 포치를 이용한 영역 간의 일체감을 통한 '확장감'이 부상되며, 폐쇄성 강한 서원내부에서 자연 우월적 외부공간으로 이르는 다양한 출구 또한 자연으로의 확장을 돕고 있다. 뿐만 아니라 옥산서원에서는 강학과 유식의 호환 및 양립성을 통해 본 '부합성' 등에 기반한 서비스스케이프가 잘 드러난다. 회복관경의 극점은 학문적 존양자인 회재의 자연 유상공간인 사산오대 및 옥산구곡과 관련된 경물 체험을 통해 환기되는 '매혹감'에서 더욱 여실히 나타난다. 이는 강학과 유식이라는 서로 다른 기능공간을 일원적으로 통합하는 내적 질서이자 중요한 회복환경의 가치임을 일깨운다. 특히 옥산서원 주변에 펼쳐져, 회재로부터 정의되고 의미를 부여 받은 사산오대는 성리학적 인식론의 관점에서 자연의 숭고미와 격물치지를 이해하는 매우 유효한 학습환경일 뿐 아니라 주의집중능력을 강화시킬 수 있는 서비스스케이프로써, 최적화한 회복환경이 되고 있음을 실증적으로 보여주었다.

산업장 근로자의 건강증진행위와 자아개념 및 건강의 중요성 인식에 관한 연구 (The Determinants of Health Promoting Behavior of Industrial Workers)

  • This descriptive-correlational study was conducted to identify the major factors affecting health promoting behaviors. 344 workers who employed in four different manutacturing plants in Taegu and Kyungbuk area were selected by convenience sampling method. Data were collected from April let to April 18th, 1998 by ready structured questionaires. The purpose of this study was to offer the basic data for health promotion theory development and health promotion strategy planning. This study was based on Pender's Health Promotion Model and examined three variables health promoting behavior, self-concept and perceived importance of health. The Life Style and Health Habit Assessment scale(LHHA) developed by Pender(1982).The Self-concept scale developed by Choi(1972) and the Health Value scale developed by Wallston, Maides and Wallston(1980) were used for this study. Data was analyzed by percentage, mean. t-test. ANOVA, Pearson Correlation Coefficient, and Stepwise Multiple Regression. The major findings of this study are as follows ; 1. The average level of health promoting behavior practice was 63.2% and possible range was from 62 to 248 point. The mean score of respondent's positive self-concept was 75.8. 81.4% of respondents put a high priority on the importance of health. 2. There was a significant difference between the practice level in the category of general self care and less amount of working hours per day(P=0.000), less amount of working hours per week(P=0.000). There was a significant difference between the practice level in the category of nutrition and age(0.002), marital status(0.000), working hour per day(0.008), working hours per week(0.001), There was a significant difference between the practice level in the category of nutriton and sex(0.000), age(0.000), marital status(0.025), education level(0.000), working hours per day(0.002), working hours per week(0.006). There was a significant difference between the practice level in the category of sleep and rest and age(0.003), marital status(0.002), working hours per day(0.001), working hours per week(0.001). There was a significant difference between the practice level in the category of stress management and working hours per day(0.001), working hours per week(0.002). There was a significant difference between the practice level in the category of self-actualization and working hours per day(0.050). 3. General characteristics influencing the respodent's self-concept were level(P=0.009) and worksite(P=0.001). 4. The results of the hypothesis tests are as follows The first hypothesis, that "The respondent who have more positive self-concept will have higher scores in the practice of health promoting behavior." was supported(r=0.2973, P=0.0001). The second hypothesis that "The respondent who have higher perception level on importance of health will have higher scores in the practice health promoting behavior." was rejected(r=- 0665, P=0.2225). 5. The most important factor that affects health promoting behavior practice was working hours per week(6.0%). The combination of working hours per week, age, education level accounted for 10.0% of the variance in health promoting behavior. In conclusion, the results of this study on industrial workers supported Pender's health promotion model in partial and showed the relatedness between self concept and the practice of health promoting behavior. Further research is required to find factors influencing health promoting behaviors of industrial workers.

폐경에 대한 적응 과정 (Adaptation Process to Menopause)

  • Although the average menopausal age has not changed, women's life span has increased. Today's women live longer after their menopause than those in the past, and this calls for attention in both nursing and medical fields. Many studies have revealed how women reacted to menopause and suffered from it. But they did not discriminate the menopausal meaning and effects from the climacteric phenomena. So, this author tried to clarify what menopause itself meant to the climacteric women, by means of grounded theory methodology. The interviewees were 21 women, whose ages were between 46 and 60 years. They were selected by theoretical sampling technique, and the author tried to include all levels of important variables such as age, educational background, religion and job. Data were collected by the author through in -depth interviews and observations in July, 1994. The interviews were mostly done in the homes of the subjects, or in some cases at the author's office or in a hospital. Interviews took from 30 minutes to 2 hours. Interviews were tape recorded and transcribed later by a research assistant. Data were analyzed as gathered, by the constant comparative method proposed by Strauss and Corbin. Eleven concepts were discovered from the data, and they were grouped under six higher order categories. These six categories were "to give menopause a meaning", "to experience value change", "to have self-help strategies", "to have no strategies", "to live a life worth living", "to have a sense of powerlessness" Among these "to experionce value change" was . selected as the core category. Five major categories were systematically integrated around the core category. Women's adaptation to menopause was defined as proceeding as follows : Most women felt relief and sorrow at the same time when they faced menopause, and some only sorrow or agony. Then, they consulted with others about menopausal symptoms, or tried to think of them by themselves. Finally, they gave menopause a meaning, which was that menopause and its symptoms were natural phenomena. But menopause made women reflect on them-selves and their past lives. As they reflected on themselves, their value on life began to change. As their value changed, some women seeked self help strategies. Those self help strategies were what they had learned from collegues, professionals or mass media. The quality of their lives depended on whether they practiced self help strategies or not. Three types of lives were found. Twelve women enjoyed a life worth living, and practiced the self help strategies, because they accepted menopause a chance to change. They were characterized by a high educational level, having a professional job and a sincere faith in God. Seven women were living as usual, because they did not have the necessity to change. They were high school graduates and house wives. Two women recognized menopause a chance to change, but they did not try self help strategies. Their characteristic was low educational level. Those who did not try self help strategies complained of powerlessness to varying degrees. The educational background, full-time jobs and faith helped women adapt to menopause positively. But social support was not helpful to women's adaptation to menepause. Three hypotheses were derived from the analysis. (1) The higher the educational level, the more theneed to change. (2) Women with higher educational background will practice self help strategies more than those with lower edcational background. (3) The more women practice self help strategies, the worthier lives they will live. Suggestions for further studies are as follows. (1) Studies to test hypotheses are needed. (2) A study to find the relationship between the degree of practicing self help strategies and locus of control. (3) Spiritual approaches would better be applied to help menopausal women. (4) Education through mass media should be given mere frequently.

만성질환자 배우자의 돌봄 경험에 대한 이론 구축 (A Theory Construction on the Care Experience for Spouses of Patients with Chronic Illness)

  • Chronic illness requiring attention and management during a long period of time puts great burden onto patients, their family and society. For patients with chronic illnesses, providing social support is the most important, and the fundamental support comes from their spouses. Amount and quality of support from spouses seems to differentiated according to the sex of patients. Female patients tend to believe that their spouses are not very supportive. Therefore, the researchers assessed the burden of husbands of female arthritis patients to discover the factors that result in greater burden. Also, they developed a theoretical model of husbands′ care for their wives through a qualitative research into husbands′ experience. Method 1: The study material was 650 female arthritis patients registered in an arthritis clinic. The questionnaire about the disease experience of female arthritis patients and the burden of husbands were sent. Returned questionnaires numbered 210(32.3%) and 27 were excluded because of inadequate answers. The remaining 183 questionnaires were analyzed. The mean age of the patients was 51 years and the mean age of spouses was 55 years. The mean marital period was 28 years. The average duration since diagnosis was 9.1 years. Education level was varied from primary school to graduate school, and average income/month was 1,517,300 won. Method 2: Initial questionnaire studies on the burden of husbands were performed. Among 183 responding husbands, 23 consented to participate for a qualitative research. Data was obtained by direct and telephone interviews. The mean age of participants was 58 years, and the educational level and socioeconomic status also varied. Result: 1. Husbands′ burden: The average burden was 57.68 with a range of 6-96. 2. Burden and general characteristics: The husband′s burden correlated with the age of the patients, numbers in the family, therapy methods, patient′s level of discomfort, patient′s disease severity, patient′s level of dependence and the husband′s understanding of the level of severity. 3. Linear correlation analysis on burden: The husbands′ burden is explained in 22.5% by husband′s recognition of level of severity and husbands′ age. 4. There were four patterns of the burden on husbands: both objectve burden and subjective burden were high(pattern I), both of objectve burden and subjective burden were low(pattern II), objective burden was high but subjective burden was low(pattern III), objective burden was low but subjective burden was high(pattern IV). The pattern was correlated with the family income, educational level of the patients and their husbands, therapy methods, patient′s level of discomfort, patient′s disease severity, patient′s level of dependence and husband′s understanding of level of severity. 5. The core category of the caring experience of the husbands with arthritis patients was "companionship". The causal factor was the patients′ experience due to symptoms : physical disfigurement, pain, immobility, limitation of house chores, and limitation of social activities. Contextural factors are husbands′ identification of housework and husbands′ concern about the disease. The mediating factors are economic problems, fear of aging, feeling of limitation and family support. The strategy for interaction is mind control and how to solve emotional stress. The "companionship" resulted from caring activities, participation of household activities, helping patients′ to coping with emotional experience. 6. Companionship is established through the process of entering intervention, and caring state of mind. Entering intervention is the phase of participation of therapy and involvement of houseworks. The caring phase consists of decision on therapy, providing therapy, providing direct care, and taking over the household role of wife. Through caring phase, the changing phase set a stage in which husbands consolidate the relationship with their wives, and are reminded of the meaning of marriage. As a result, in changing phase, husbands′ companionship is enhanced. In conclusion, nursing care of chronic illnesses should include a family member especially the spouse. All information on disease shoud be provided to patients and whole family member. Strong support should also be provided to overcome difficulties in taking over role of other sex. Then the quality of life of patients and families will be much improved.

갱년기 여성을 위한 앱 기반의 인지기능훈련 프로그램 개발 (Development of mobile-application based cognitive training for Menopausal Women with Cognitive Complaints)

  • 본 연구는 인지예비능 이론에 입각하여 인지기능감퇴를 호소하는 갱년기 여성들을 위해 인지기능훈련 프로그램을 개발하고자 하였다. Jung의 웹기반 교수-학습 체제 설계의 절차적 모형에 근거하여 분석, 설계 및 개발 단계를 거쳐 개발하였다. 인지기능훈련 프로그램은 8주간의 프로그램으로 모바일 애플리케이션을 활용한 인지훈련과 동영상 교육이며 총 24회기로 매 회기는 20~30분이 소요되며 3회/주 하도록 구성되었다. 인지기능훈련은 갱년기 여성들에 있어서 가장 취약한 인지영역으로 확인된 기억력, 주의집중력, 언어기능과 통합적 집행기능을 위한 시나리오기반 문제해결 등으로 내용을 구성하였다. 각 주차별 교육내용은 갱년기의 정의, 원인 및 증상, 갱년기와 뇌기능 등 총 8가지 주제로 개발되었다. 프로그램 시범운영을 위해 인지기능감퇴를 호소하는 10명의 갱년기 여성을 대상으로 인지기능훈련 프로그램을 적용하여 훈련 전과 훈련 8주후 인지기능의 효과를 측정한 결과 전반적 인지기능이 증가되었고 통계적으로 유의하였다(t=-3.04, p=.014). 모바일 앱 기반 인지기능훈련 프로그램은 기억력 향상뿐만 아니라 잠재적 치매 유병율을 감소시키는 효과도 기대할 수 있을 것이라 기대된다.

Models for Spiritual Care in Hospice and Palliative Care

  • 영성은 인간의 본질적인 부분으로 말기 환자와 가족의 실제적인 영적요구 충족을 위한 영적돌봄은 HPC에서 가장 중요한 영역이다. 말기환자와 가족의 실제적인 영적요구에 부응하는 돌봄모형을 확인하기 위해 최근 보건의료계에서 가장 많이 사용하는 국 내외 database를 검색하여 분석한 결과, 영성은 전인적 존재인 인간의 본질적인 부분으로, 삶의 위기상황에 더욱 충족되어야 할 존재론적 특성으로서, 말기 환자의 영성평가는 HPCT이 기본적으로 갖추어야 할 역량임이 확인되었다. HPC에 evidence based theory로 활용 가능한 총 10개의 모델이 제시되었다. 세 개의 모델들은 영적돌봄 결과에 초점을 둔 모델로서, spiritual health, QOL, and coping을 중요한 outcome으로 보는 모델들이었다. 영적돌봄수행에 초점을 둔 모델은 7개였으며, HPCT의 다학제간 협력을 통한 돌봄과정을 나타낸 'Wholeperson Care Model', 인간의 보편적인 실존적 요구충족이 보완된 'An Existential Functioning Model', 대상자의 문화적, 그 외 다양성을 고려하는 'An Open Pluralism View', HPCT와 말기환자와의 관계중심모델인 'A spiritual-relational view' and 'The Framework of Systemic Organization', 영적돌봄과정의 전체적 역동을 설명하는 'The principal components model' and 'The Actioning Spirituality and Spiritual care in Education and Training model'이 있다. 이러한 모델을 토대로 HPC에서 가장 최선의 영적돌봄 모델 구축을 위한 지속적인 임상연구 노력이 계속될 필요가 있겠다.

성 건강 교육프로그램 개발과 효과 (The Development of a Sexuality Education Program and Its Effect on High School Girl Students)

  • The purpose of this study is to investigate the seven-hour sexuality education program for senior high school girl students and apply it to them to find out how this program affects their sex-related knowledge, sexual attitudes, and the self-efficacy of sexual behavior practice. The ultimate goal of this study is that high school girl students are to prevent unwanted pregnancy, STD and AIDS, after attending this sexuality education program. A non-equivalent control group pretest-post test design was used for this study. The number of experimental group consisted of 80 girl students and the control group was composed of 80 girls, too. A total of 160 freshmen from two commercial girls' high schools were selected by a convenience sampling method. This study was carried out in commercial high schools located in Daejeon, Korea from July 15th to October 31st, 2002. To discover the need for sexuality education, the researcher surveyed with a pre-designed questionnaire to 160 girls from the two different commercial high schools. The sex education curriculum was developed on the basis of the need of high school girl students for sex education survey results and literature review and a teaching-learning planning model and health belief model. The experimental group was taught the sexuality education program for seven hours from October 19th to 23rd, 2002 and each period continued for 50 minutes. The SPSS(10.0) Windows statistics software was used to analyze the data, along with frequency, percentage, $X^2$-test, t-test, and paired t-test. The instruments' internal consistencies were tested by Cronbach's alpha coefficient. The results of this study were as follows: 1. The increment of sex-related knowledge score after seven hour sexuality education was significantly higher in experimental group (t=-10.88, p=.000). 2. The increment of sex-related attitude score after seven hour sexuality education was significantly higher in experimental group (t=-2.67, p=.009). 3. The increment of the practice of self-efficacy of sexual behavior score after seven hour sexuality education was significantly higher in experimental group(t=-4.95, p=.000). In conclusion, sexuality education programs based on high girl students' educational desire, characteristics of development, a teaching learning planning model, a health belief model were effective to improve sex-related knowledge, sexual attitudes and the self-efficacy of sexual behavior practice of commercial high school girl students. This result can contribute to expand the school nurse's role in the sex education area and to develop theory-based school health nursing.

일부 초등학생의 기본심폐소생술 교육효과에 대한 연구 (A Study of the Effectiveness of Basic CPR (Cardiopulmonary Resuscitation) Education of Primary School Children)

  • Purpose: This research is based on experiments practiced with 6th grade students in primary school as subjects. They were trained in basic cardiopulmonary resuscitation theory and received actual training in CPR. Methods: The subjects were randomly sorted into two classes of the 6th grade students. The experimental group was composed of 35 students. The control group was composed of 32. The experimental group received basic CPR theoretical and practical education once and received a practical evaluation three times, with 4 weeks in between evaluations. The control group received CPR theoretical education before the study. The data was analyzed by ${\chi}^2$-test and t-test using the SPSS/WIN 12.0 program. Results: Hypothesis 1 was that the basic CPR knowledge score would begin to decrease right after the education and continue to decrease as time passed. The experimental group's knowledge score continued to increase 12 weeks after education, but there was no decrease in the control group's knowledge score (F=5.870, p=.000). Hypothesis 2 was that the basic CPR attitude score would decrease right after the education and continue as time passed. There was no significant difference in the experimental group's score after education, nor was there any change in the control group's score on this measure (F=3.986, p=.004). Hypothesis 3 was that the subjects' confidence in practicing CPR would decrease right after education and continue as time passed. There was a significant decrease in the experimental group's score, but no significant change in the control group's score on this measure (F=75.574, p=.000). Hypothesis 4 was that the practical accomplishment evaluation score of CPR would decrease as time passed. There was a significant decrease in the experimental group's score on this measure right after education (F=38.368, p=000). Conclusion: Retraining for basic CPR education will be needed in all aspects of the education/training at least every 4 weeks, to preserve the students' retention of learned material/training. This is because students' scores fell significantly four weeks after education/training.

임신 인식 개선 프로그램 개발 및 효과 분석 (Development and Effect Analysis of Pregnancy Recognition Improvement Program)

  • 본 연구는 최근 심각한 저출산에 대한 임신 인식 개선을 목적으로 프로그램을 개발하고 적용하여 그 효과를 분석한 혼합설계연구이다. 연구기간은 2018년 4월 1일부터 10월 26일까지 이었고, 연구에 참여한 대상은 M시와 M군에 거주하고 있는 젊은 여성들(19-21세)로 구성된 총 16명이었다. 프로그램 개발과정은 인식의 변화를 유도하기 위한 Dorothy Johnson 의 행위체계이론을 근간으로 하였으며, 문헌고찰과 국가정책 보고서를 참고하였다. 문헌을 통하여 개발된 프로그램은 가임기 여성 25명으로부터 저출산에 대한 원인 및 대안을 도출한 후 전문가 패널 토의 검증을 통하여 프로그램을 최종적으로 개발하였다. 프로그램 내용은 3영역으로 구성되었다. 1영역은 여성 생식관련 교육, 2영역은 친구 맺기, 3영역은 건강 체크이다. 양적 연구 결과 분석은 SPSS 18.0을 활용하여 Shapiro wilk와 Wilcoxon sign rank으로 분석하였고, 질적 연구결과 분석은 포커스 그룹을 활용하여 내용분석을 하였다. 연구결과, 본 프로그램을 적용하여 임신인식은 사전(M=3.21, SD=.342), 사후(M=4.46, SD=.398)로 통계적 유의 수준 하에서 유의미하게 상승하였고 (p<0.01), 본 프로그램에 대한 내용 분석 결과는 <깊이 생각하게 됨>, <임신은 축복>, <여성 당사자의 현실적인 요구 반영>로 3개의 주제가 도출되었다. 즉, 본 동료 그룹(peer group)을 활용한 임신 인식개선 프로그램을 적용한 결과, 참여 이전보다 임신에 대하여 깊게 생각하게 되었고, 임신은 축복이라는 인식을 하는 동시에 임신을 하는 당사자인 여성들의 현실적인 요구를 정책에 반영하길 원하고 있는 것으로 나타났다. 결론적으로, 본 프로그램은 젊은 여성을 대상으로 한 임신 인식 개선에 효과적이었다.

한국문학에 나타난 죽음과 삶을 통한 웰다잉(well-dying)과 웰에이징(well-aging) 연구 (A study of well-dying and well-aging through death and life appeared in Korean literature)

  • 본 연구는 한국 문학을 고찰한 문학 연구들을 활용하여 문학 작품에서 재현되는 삶과 죽음을 파악하고 웰에이징과 웰다잉을 제시하고자 하였다. 조사 기간 및 대상은 문학 연구 10건에서 다루는 13세기부터 20세기 사이에 기록된 문학 작품이다. 조사 방법은 총론과 각론 구획, 문학 장르 구획, 시대 구획, 키워드 고찰 등이다. 조사 결과 대다수의 문학 연구는 작가론을 포함한 각론에 해당하였고 소설(서사) 갈래와 시(서정) 갈래에 집중되었으며 20세기 문학 작품들을 주로 다루었다. 또한 문학 작품에 내재하는 죽음, 삶 양상과 웰다잉, 웰에이징의 관련성을 전체 집합과 부분 집합의 개념으로서 파악하였다. 결론적으로 본 연구는 특정 시기 문학 작품들을 연구하였다는 제한에도 불구하고 한국 문학에서 출발한 전망을 확보하는 희망으로서의 삶에서 웰에이징의 본질을 고찰하였고, 한국 문학에서 유래한 통과의례로서의 죽음과 의미 있는 죽음에서 웰다잉의 본질을 인식하였다. 21세기 대한민국 사회의 웰다잉, 웰에이징 구현과 융복합 연구 응용 등에 일정 부분 기여할 것을 기대한다.
