Application of the Evaluation Tool for the Performance Outcomes in Fundamental Nursing Practicum: A Case Study Focused on Evaluating of Communication Ability (기본간호학실습에서 학습성과 평가도구의 적용 사례: 의사소통능력 평가를 중심으로)

  • This is the report of a case study to describe the results of applying an evaluation tool for communication ability when performing core basic nursing skills in fundamental nursing practice education. The communication ability evaluation tool was constructed based on a literature review and expert advice. The tool was applied to test 94 students who were taking fundamental nursing practice courses from October to November 2019. As a result, five factors (self-introduction, eye contact, emotional support, information provision, and therapeutic touch) were derived as evaluable items of communication ability to be evaluated when performing core basic nursing skill, and were evaluated when performing core basic nursing skills to measure vital signs. The average communication ability was 3.96 out of 5 points. According to the rubric, 95.8% of all students attained 'medium' and reached their goal achievement level. The findings of this study are meaningful in providing an important basis for improving the performance outcome evaluation process and for constructing a systematic evaluation system in fundamental nursing practicum. Further studies to secure the validity and reliability of this communication ability evaluation tool and comparative studies with various evaluation tools are suggested.

Work Stress and Nursing Performance of Clinical Nurses (임상간호사의 업무스트레스와 간호업무수행)

  • Purpose: The purpose of this study was to examine the work stress and nursing performance of clinical nurses' and to identify relationship between work stress and nursing performance. Method: This descriptive study queried 316 clinical nurses, selected by convenient sampling from four hospitals. Demographic characteristics, work stress and Nursing performance were measured. Descriptive statistics, t-test, ANOVA were performed to identify relationship between work stress and nursing performance on SPSS WIN 10.0. Result: The study subjects reported that the younger they were, the higher work stress they experienced and the lower nursing performance they behaviored. Also, the longer they worked as a nurse, the lower work stress they experienced and the higher nursing performance they hehaviored. The nurses working at ICU experienced more work stress. The nurses working at pediatric ward reported higher nursing performance. The married nurses' nursing performance was higher than unmarried nurses'. There was significant negative correlation between work stress and nursing performance among nurses. Conclusion: These results suggest that the nurse who experience higher work stress couldn't perform optimistic leveled nursing performance. Strategic planning for nursing administer and avoidance of work stress were discussed.

Development of the Program Evaluation Measurement of Continuing Nursing Education Programs (간호사 보수교육 프로그램 평가도구 개발)

  • Purpose: This study was done to develop a measurement tool for evaluation of continuing nursing education programs and to verify its validity for effective management and quality of education programs. Methods: The draft of the evaluation measurement was developed from consultation with professionals, focus group interviews targeting groups of nurses, and individual interviews with education program planners. After 6 professionals examined content validity, 46 items were retained. A pilotsurvey was conducted to confirm the time required to complete the questionnaire and the level of understanding of general content and each item in the questionnaire. Construct validity was verified through exploratory factor analysis of data from a survey with 44 items completed by 452 nurses and 59 education program planners. Results: The final evaluation measurement for continuing nursing education programs consisted of 6 evaluation factors and 36 evaluation items. The 6 evaluation factors included identifying program goals and target groups, program planning, performance, operation and management, program outcomes, and program effectiveness. Conclusion: The evaluation measurement for continuing nursing education programs developed in this study is considered suitable to utilize as an evaluation measurement of the quality of continuing education programs for nurses.

The Development and Evaluation of a Clinical Practice Nursing Students' Microbiology Program Based on the Mastery Learning Model (완전학습모델기반 간호 미생물학 이론 및 실습프로그램의 개발과 효과평가)

  • Purpose: The purpose of this study was to develop a clinical practice nursing students' microbiology program based on the mastery learning model, and to evaluate the effects of the program on nursing students' knowledge, self-efficacy, performance, and satisfaction related to the nursing students' microbiology program. Methods: The program was developed by using the processes of the mastery learning model. The pre-experimental research design involved a one group pretest-posttest design. The setting was a university located in Incheon, Korea. A total of 130 nursing students participated in the program including a theoretical lecture, clinical practice, and formative and summative evaluation. Results: Using the program that was designed and developed, results for the total score of self-efficacy, knowledge, and performance in the post-test application were significantly higher than in the pre-test application (p<.05). The satisfaction of hand hygiene and disinfection/contaminated hand microbial culture and disinfection test received the highest ratings. Conclusion: The application of a clinical practice nursing students' microbiology program was effective, and can be expanded to other nursing students. Future research with other study designs was warranted in order to prove the effect of a microbiology program based on the mastery learning model.

The Study of the Knowledge and Performance of Nursing Students for the Nosocomial Infection Control (병원감염관리에 관한 간호대학생의 지식 및 실천에 관한 연구)

  • Purpose: This study was designed to investigate nursing students' exposure experience to infectious diseases and their knowledge and performance for the nosocomial infection control. Method: The data was obtained from 292 nursing students having clinical experience at two university hospitals in Seoul from November 2005 to February 2006. Specific questionnaire was constructed and used for this study. The obtained data was analyzed by using the SPSS window program. Result: The results were as follow : 1. Among the evaluation scores of knowledge for the nosocomial infection control, the score of sterilization and the score of the intravenous therapy management were low(.14). 2. Among the evaluation scores of performance for the nosocomial infection control, the score of hand washing is low(1.63). 3. 86.3 percent of the nursing students had exposure experience to infectious disease. Many of them did not use the protective equipments in clinical experience nor took any treatment after the exposure to infectious diseases. Conclusion: Based on these results, we suggest that the programs for the nosocomial infection control should be developed for nursing students. It is also suggested that nursing students should be educated before their clinical experience to keep protected from the infection. In particular, the hand washing, sterilization and intravenous therapy management should be emphasized in the educational programs.

Usefulness of Clinical Performance Examination for Graduation Certification of Nursing Students (졸업인증 임상수행력평가의 유용성 평가)

  • Purpose: The aim of this study was to testify whether clinical performance examination (CPX) was useful to evaluate comprehensive performance for nursing students just prior to graduation. Method: A cross-sectional descriptive study was designed to examine the usefulness. A total of 61 nursing students whose requirement credits were completed for graduation from a C University in G-city, at December, 5, 2007. Data were analyzed by Pearson's Correlation Coefficient and Spearman's rank Correlation Coefficient. Results: This study showed that both of the finals scores with paper and pens and the clinical practicum scores were not correlated with the CPX scores (r=-.031, p=.811; r=.028, p=.831). Consistency of scores between faculties and standardized patients was moderate (r=.752, p=.000). Conclusion: CPX was considered as a different and innovative evaluation from previous testing systems to test the various aspects of performance including knowledge, skill and attitude. Therefore, CPX under high raters' consistency was useful to test nursing students's final performance. Further study would be needed to develop the standard of CPX system.

  • Purpose: This study was conducted to develop the nursing process based performance measurement tool and the evidence based care standards for nursing care in medication management and blood transfusion. Methods: The care standards and performance measurement tool were drafted through comprehensive review of relevant literature, national guidelines, hospital protocols, and standards of recognized international accrediting bodies. The proposed care standards and performance measurement tool were reviewed by the panel of experts and refined based on the panel's suggestions. Final care standards and performance measurement tool were validated by surveying the hospital nurses. Results: All items of the performance measurement tool for medication management and blood transfusion were evaluated appropriate. All contents of care standards and the measurable elements except the evaluation of discharge education were appropriate. The performance measurement tool developed in this study was found to be acceptable as a tool to evaluate quality of nursing care in medication management and blood transfusion. Conclusion: The outcomes of this study including the performance measurement tool and evidence based care standards would be the important indicators to monitor whether necessary nursing care is implemented and be the useful primary resources to improve quality of nursing care services.

Effects of Empowerment and Job Satisfaction on Nursing Performance of Clinical Nurses (간호사의 임파워먼트와 직무만족이 간호업무성과에 미치는 영향)

  • Purpose: This study was conducted to identify factors that affect nursing performance; Those factors are personal characteristics, sub-categories of empowerment and job satisfaction of clinical nurses. Methods: Data were collected from November 24, 2012 to March 11, 2013, and participants were 451 hospital nurses in 6 hospitals. Descriptive statistics, t-test, one-way ANOVA, Pearson correlation coefficient, and stepwise multiple regression were used for data analysis. Results: Nurses who were older, married, highly educated, in the position of charge nurse, and with longer clinical careers showed relatively higher empowerment, higher job satisfaction, and higher nursing performance than others. Also, there were strong correlations between nursing performance and empowerment (r=.576)/job satisfaction (r=.617). Factors predicted nursing performance were relationship with coworkers (${\beta}=.398$), duty (${\beta}=.181$), promotion system (${\beta}=.134$), and turnover intention (${\beta}=-.109$). Factors predicting job satisfaction were competency of empowerment (${\beta}=.249$), and clinical career (${\beta}=.151$). These 6 factors explained 55.2% of the variance in nursing performance (F=93.37, <.001). Conclusion: Regarding human resource management, relationship with co-workers is a changeable factor. Therefore nursing organizations should apply these factors in human resource management to enhance nursing performance and achieve organizational goals.

Analysis of the importance of nursing care and performance confidence perceived by nurses in the neonatal intensive care unit (신생아중환자실 간호사가 인식한 간호업무의 중요도와 수행 자신감)

  • Purpose: Neonatal nurses are expected to have clinical competency to provide qualified and safe care for high-risk infants. An educational intervention to enhance nurses' clinical competence is often a priority in the nursing field. This study was conducted to explore nurses' perceived importance and performance confidence of nursing care activities in neonatal intensive care units. Methods: One hundred forty-one neonatal nurses from seven hospitals across South Korea participated in the online survey study. The scale of neonatal nursing care activity consisted of 8 subdomains including professional practice (assessment, diagnosis, planning, intervention, evaluation, education, research, and leadership). The Importance-Performance Matrix was used to analyze the importance of and confident performance in each of the nursing subdomains. Results: Both importance and performance confidence increased as nurses' age (p=.042 and p<.001) and clinical experience (p=.004 and p<.001). Participants scored relatively higher in importance and performance confidence in the professional practice subdomains (assessment, intervention, evaluation), but scored lower in the education and research subdomains. Conclusion: To provide evidence-based nursing care for high-risk infants in neonatal intensive care units, educational interventions should be developed to support nurses based on the findings of the research.

Comparison of Nursing Performance by Career Levels in a Nurse Career Ladder System (경력개발시스템 경력단계별 수술실 간호사 업무수행능력 비교)

  • Purpose: This study was to compare nursing performance by career levels in a career ladder system. Methods: This survey was conducted with 140 OR staff nurses. Ten unit managers evaluated their own staff with a performance evaluation tool composed of 51 items. Data were analyzed by frequency, t-test, one-way ANOVA, ${\chi}^2$-test, and ANCOVA. Results: In career level CN II, nursing performance showed differences by personal characteristics. However, there was no significant difference in level CN III. CN IIIs' nursing performance was significantly higher than CN IIs' in all domains. Nursing performance of CN III was significantly higher than CN IIs' in 5 to 10 years of clinical experience. Although there was no significant difference in other clinical experience groups, CN IIIs' scores were always higher than CN IIs' Conclusion: The results indicate that nursing performance improves with career ladder advancement. Organizations should encourage advancement to higher levels and eliminate obstacles.
