암환자에게 제공된 다빈도 간호중재 분석 - 5개 종합병원을 중심으로 - (Analysis of Nursing Interventions Frequently Used with Cancer Patients)

  • Purpose: The purpose of this study was to identify nursing interventions frequently used with cancer patients. Nursing records from 5 general hospitals for patients with stomach, liver, lung or colon cancer were analyzed. Method: A descriptive study methodology was used and nursing records for 15 patients in each disease category at each hospital, who were admitted and discharged during June 2007 were analyzed. Results: Five domains of NIC were found and the physiological(basic) domain was most frequent (31.52%). Twenty two classes of NIC were identified with risk management for safety being most frequent (22.49%). For the 119 nursing interventions identified, the most frequent was pain management with 7,827 (12.31%), followed by prevention of falls (11.76%), surveillance (6.79%) and wound care (5.12%). Nursing activities of pain management and prevention of falls were comparable to activities listed in literature on guidelines for evidence based and best practices in nursing care. Eight of the 17 nursing activities for pain management, and 9 of 14 for fall prevention were consistent with these guidelines. Conclusion: In this study, nursing interventions were found to be focused on physical care, monitoring patients' condition and education. We have to develop diverse nursing interventions and a convenient recording process.

간호대학생의 간호전문직관에 미치는 영향 요인 (Affecting Factors of Nursing Professionalism Perceived by Nursing Students)

  • Purpose: The purpose of this study was to investigate nursing professionalism and affecting factors of nursing professionalism in freshman nursing students. Methods: Pre and post-test study design was used to compare the differences of sutdy variables at the completion of a fundamental nursing course. A convenience sample of 164 were recruited. The date were collected from September 1st to December 1st in 2012. Results: The mean scores of the nursing professionalism were significantly increased from a pre-test (3.73) to post-test (3.93). The mean scores of the self-esteem were significantly increased from a pre-test (3.05) to post-test (3.13). Self-esteem and satisfaction with nursing were factors that affect nursing professionalism. Conclusion: Attention should be given to nursing education to cultivate nursing professionalism by improving self-esteem and satisfaction with nursing in nursing students.

  • Purpose: Ego-integrity in Erikson's stage theory is used frequently among health team members related to the care of the elderly and has specific meanings within the context of quality of life in later life. However, the concept of ego-integrity in the elderly has not been well articulated in the literature. This study was conducted clarify and conceptualize the phenomena of ego-integrity in the elderly. Method: A Hybrid Model of concept development was applied to develop a concept of ego-integrity, which included a field study carried out in Seoul, South Korea using in-depth interviews with old adults who were admitted as a right person for research subject according to attributes of ego-integrity analysed in the theoretical phase. Results: The concept of ego-integrity emerged as a complex phenomenon having meanings in several different dimensions which encompassed several attributes. Conclusions: Ego-integrity is a concept having needs that should be treated in a specific way and it is possible to enrich the meaning and methods to manage ego-integrity in nursing interventions for promoting quality of life so that its application may have effects that have positive impacts on the elderly's well being.

간호대학생의 저혈당 응급관리 시뮬레이션 실습 수행 평가를 위한 임상판단 루브릭 적용 (Evaluation of Lasater Clinical Judgment Rubric to Measure Nursing Student' Performance of Emergency Management Simulation of Hypoglycemia)

  • Purpose: To evaluate the applicability of Lasater Clinical Judgment Rubric (LCJR) as an evaluation tool for hypoglycemia simulation practicum on Korean nursing students. Methods: The methodological study was done to evaluate the reliability and validity of the LCJR. Based on Benner's 4 levels of nursing grading rubric, ten items of the LCJR was evaluated for interrater reliability and internal consistency. The content validity was tested by eight experts and concurrent validity was done by Clark (2006)'s clinical simulation grading rubric. Fifty five video-taped cases of senior nursing students in Y University were used for the reliability and concurrent validity of the LCJR. Results: The interrater reliability was r=.90 (p<.001); Kendall tau b=.87 (p <.001), and Cronbach's alpha was .90. A value of item content validity index of the LCJR was .97 and correlation coefficient between the LCJR and Clark's instrument was .90 (p<.001). The mean (${\pm}SD$) of the nursing students' clinical judgment was 2.04 (${\pm}50$). Conclusion: The LCJR is a useful tool to examine the simulation performance evaluation for improving competency among nursing students. The results indicated that the LCJR may provide valuable information regarding clinical judgment of nursing students and thus, suggested to use to develop a simulation-based education program.

Job Analysis of Clinical Research Coordinators Using the DACUM Process

  • Purpose: This study was done to analyze the job of clinical research coordinators (CRCs). Methods: Through the "developing a curriculum (DACUM)" workshop, the definition of CRCs' role was described and CRCs' duties and tasks were identified. Finally, the developed duties and tasks were validated for importance, difficulty, and frequency. Results: A CRC is defined as the one who coordinates and performs tasks related to clinical research/trials among investigators, participants, and sponsors according to the Good Clinical Practice at institutions conducting clinical trials. Twelve duties and 78 tasks were identified on the DACUM chart which represented the importance, difficulty, and frequency of tasks represented as A, B, and C respectively. Based on determinant coefficient (DC) of the task, the highest ranked task was confirming the eligibility of participants for research (DC=8.03) and the lowest was inventory management for clinical study materials (3.95). Conclusion: In this study, the job of a CRC was analyzed through the DACUM process and it was found that CRCs were doing various duties and tasks. Based on these results, it is suggested that it is necessary to develop CRC education programs considering the career ladder of CRCs.

보완. 대체요법관련 국내 간호연구의 동향분석 (The Analysis of Trends in Complementary and Alternative Therapy (CAT) in Nursing Research in Korea)

  • Purpose: The purpose of this study was to analyze trends in complementary and alternative therapy in nursing research in Korea. Method: The researchers examined academic theses and dissertations published from 1990 to 2002, and 151 articles were used for the analysis. Result: Nursing research on CAT increased rapidly from 1995 Articles with quantitative research designs made up 93.9% of the total and there were more experimental studies than non-experimental studies. Patients who had surgery, mental disorders, renal failure, hypertension, arthritis, dementia & cancer were the most frequently participants in studies on CAT. The type of CAT used in nursing research were mind-body therapy (65.8%), manual healing therapy (28.7%), phamacologic & biological therapy (3.7%), bioelectromagnetics (0.9%) and herbal therapy (0.9%). In 44 articles both psychological and physiological parameters were used as dependent variables. In 34 articles only physiological parameters were used and in 13 only psychological parameters. The most frequently used physiological parameters in CAT were pain, physiological function and vital signs, while the most used psychological parameters were anxiety, depression and stress. Conclusion: More studies about CAT are needed to extend the role and fields for professional nursing. There is a need to conduct qualitative studies in nursing about the experiences of patients who receive CAT and nurses who use CAT.

한국어판 '암 환자가 인식하는 간호의 질 측정도구'의 타당도와 신뢰도 (Validity and Reliability Test of the Korean Version of the Oncology Patients' Perception of the Quality of Nursing Care Scale)

  • Purpose: The purpose of this methodological study was to examine the validity and reliability of a translated Korean version of The Oncology Patients' Perception of the Quality of Nursing Care Scale. Methods: The translated scale was pilot tested and then administered to 360 patients with cancer. Exploratory and confirmatory factor analyses were utilized to assess the factor structure. Internal consistency reliability was determined using coefficient ${\alpha}$. Results: Two of the 40 items were deleted with a principal component method of exploratory factor analysis because they did not meet the factor-loading criterion. The 38 items were again analyzed and, four factors were validated (KMO=.96, Bartlett ${\chi}^2=10809.81$, df=780, p<.001). The four factors accounted for 60.9% of the variance. Factor loadings of the scale on the four subscales ranged from .65 to .91. The scale showed reliable internal consistency with a Cronbach's ${\alpha}$, total 38 items (${\alpha}=.96$) in four subscales: individualization (18 items, ${\alpha}=.96$), proficiency (10, .91), responsiveness (7, .90), and coordination (3, .80). Conclusion: The findings of this study demonstrate that the scale has satisfactory construct validity and reliability to measure quality of cancer nursing care from the patient's perspective in Korea.

대학 간 통합 웹기반 중환자간호실습 콘텐츠 개발 및 적용 (Development of Web-based Multimedia Contents for the Critical Care Practice of Nursing Students through Inter-College Collaboration)

  • Purpose: This study was conducted to develop Web-based multimedia contents for supporting student nurses' clinical practice on critical care, and to evaluate learners' responses. Methods: Based on the steps of Assessment, Design, Development, Implementation, & Evaluation(ADDIE) model, a total of 13 self-directed learning modules including live lectures and real video clips were developed through faculty collaboration of nine nursing colleges in Gwangju and Chonnam province. The finally developed multimedia contents were published on the Web of the learning management system at a local e-learning center. Results: The Web contents were evaluated after self-learning by 81 junior college nursing students who were encouraged to study it at their own pace during their two-week clinical practice at a medical or surgical intensive care unit of a university hospital and two hospitals. The knowledge (t = -27.66, p < .001) and self-evaluated clinical performance level(t = 7.54, p < .001) were significantly increased after learning of the Web contents and clinical practice, and satisfaction level that measured post-test only was 4.0 out of 5 point. Conclusion: The use of Web contents for critical care need to be extended as a complimentary material in a class room lecture or clinical practice of students to increase their self-learning ability and understandings of clinical knowledge and situation.

Prevalent Signs and Symptoms in Patients with Skin Cancer and Nursing Diagnoses

  • Background: Skin cancer has a remarkable importance given the high incidence in the population. In Brazil, it is estimated that there were 98,420 new cases of non-melanoma skin cancer among men and 83,710 new cases among women in 2014. Objectives: To verify signs and symptoms present in patients with skin neoplasms according to the literature and relate them to the nursing diagnoses of NANDA International. Materials and Methods: Integrative literature review carried out from March to May 2015 in the databases: Cumulative Index to Nursing and Allied Health Literature, SCOPUS, National Library of Medicine and Nattional Institutes of Health, Latin American and Caribbean Sciences of Health and Web of Science. The descriptors used were: 'Signs and Symptoms' and 'Skin Neoplasms'. Sixteen articles were identified as the final sample. After review, the signs and symptoms of skin cancer identified in the literature were related to the defining characteristics present in NANDA International, with the aim to trace possible nursing diagnoses. Results: The most prevalent signs and symptoms were: asymmetric and well circumscribed nodules with irregular borders; speckles with modified color aspect; ulcerations; blisters; pain; itching; and bleeding. The principal nursing diagnoses outlined were: risk for impaired skin integrity; impaired skin integrity; acute pain; risk of shock; and impaired comfort. Conclusions: The identification of signs and symptoms present in patients with skin cancer and the relationships of these with the nursing diagnoses of NANDA International provide a basis for qualified and systematized nursing care to this clientele.

중환자실 간호사의 간호조직문화, 진정성 리더십, 자아존중감이 직장 내 괴롭힘에 미치는 영향에 대한 구조모형 (The Structural Equation Model of Organizational Culture, Authentic Leadership, Self-Esteem, and Bullying in Nurses at Critical Care Units)

  • Purpose: The purpose of this study were to identify the influential factors of bullying of intensive care nurses and to suggest a final structural model based on identified relationships between nursing organizational culture, authentic leadership, self-esteem, and bullying in the workplace. Methods: Data were collected from 221 nurses at intensive care units in eight general hospitals using structured questionnaires and analyzed by structural equation modeling. Results: In this study, the average of bullying in the workplace was 1.34±0.40, nursing organizational culture was 3.31±0.47, self-esteem was 2.79±0.44, and authentic leadership was 3.61±0.60. The factors affecting nursing organizational culture were authentic leadership (β=.54, p<.001) and self-esteem (β=.24, p=.002) that had direct positive effects on the nursing organizational culture. The nursing organizational culture had also a direct effect on reducing workplace bullying (β=-.45, p<.001). Authentic leadership (β=-.24, p=.004) and self-esteem (β=-.11, p=.004) had indirect effects on workplace bullying, which was mediated by the nursing organizational culture. Conclusion: To understand and reduce workplace bullying, evaluating a nursing organizational culture should be preceded. Based on the finding of this study, an intervention for increasing authentic leadership and self-esteem of nurses can positively help to create the nursing organizational culture and then reduce workplace bullying.
