Background: The aim of this study was to examine the correlation between the number of remaining natural teeth (NRT) and the appendicular skeletal muscle mass index (SMI) in older adults. Methods: This study was based on data from the 2008-2010 Korea National Health and Nutritional Examination Surveys. The participants were 2,378 older participants (984 men and 1,394 women) aged over 65 years. Survey dentists conducted oral health examinations, and ASM was measured using dual-energy X-ray absorptiometry. Results: The participants with $NRT{\geq}20$ had more ASM and SMI than those with NRT<20 in both sexes. SMI was correlated with NRT in men (r=0.018, p<0.001) and in women (r=-0.007, p<0.001). The positive correlation between the NRT and SMI remained significant in men even after adjusting for age, marital status, income, smoking, drinking, physical activity, protein intake, energy intake, calcium intake, body mass index, fasting blood glucose level, medications, and prostheses (${\beta}=0.011$, p=0.001). In women, the correlation disappeared after adjustment for smoking, alcohol, physical activity, protein intake, energy intake, calcium intake, marital status, income, fasting basal glucose, medication administration, and prostheses. Conclusion: This study showed a correlation between NRT and SMI in those ${\geq}65$ years of age in Korea. The relationship persisted in men, but not in women, even after adjusting for confounders.
본 연구는 구강건강을 중심으로 노인의 삶의 질에 관련된 요인을 찾아내어 향후 노인의 삶의 질을 향상시키기 위한 국가 정책 수립 평가에 기초자료로 제시하고자 한다. 연구 분석 자료는 2007년 7월부터 2009년 12월에 실시한 국민건강영양조사 4기 원시자료로부터 구강검진을 받은 만65세 이상 노인 3,882명의 자료를 활용하였으며 IBM SPSS Statistics 19.0 프로그램을 이용하여 분석하였다. 연구 결과는 잔존치아 수가 많을수록, 국소의치 및 총의치를 필요로 하지 않는 경우 삶의 질이 높았다. 따라서 보철물이 없는 치아 상실이 삶의 질에 크게 작용하므로 잔존치아 수를 증가시키는 노력과 함께 노인에게 치과 보철물 장착을 하여 구강건강을 회복시키는 것이 시급한 과제라고 생각된다.
Statement of problem : A scientific examination and understanding of overall aspects of the natural dentition is the first step involved in making a satisfactory selection in the shade of an aesthetic prosthesis. Proper natural shade selection of the prosthetic restoration that is in harmony with the remaining dentition is as important aesthetically, as harmony of form and function in the anterior dentition. Clinically, the most commonly applied method of shade selection has been visual, but because of the subjective nature inherent to this method, shade selection results are variable and can be influenced by such factors as the technician, the type of shade guide used, and the type and intensity of the lighting. Purpose : The purpose of this study was to develop a more objective and scientific approach to examining and understanding the shade of teeth, which has in turn lead to the development of a number of shade analysis devices that present a more objective method of shade analysis. Material and Method : In this study, the shades of healthy anterior teeth were examined and analyzed using the recently developed digital shade analysis of the $ShadeScan^{TM}$ System. The study examined 80 individuals in their twenties, 40 males and 40 females, presenting 6 healthy, unrestored maxillary anterior teeth. Tooth brushing and oral prophylaxis were performed prior to evaluation. The ShadeScan handpiece was used to acquire images of the 6 maxillary anterior teeth. These images were analyzed using the Vita/Classical mode of the $ShadeScan^{TM}$ Software, and shade maps of each tooth were acquired and divided into cervical, middle, and incisal thirds. The shade distribution of each third, left and right symmetry, and gender differences were investigated and analyzed. Results : The results of the study are as follows : 1. An overwhelming majority of the examined teeth were found to possess shades belong to Group A, with the greatest variations occurring at the middle and cervical thirds of the maxillary central and lateral incisors, in both male and female subjects. 2. Canines of both male and female subjects showed left and right symmetry with uniform shade distribution of A4 and C4, while the lateral and central incisors showed left-right symmetry of the incisal 1/3 with a uniform shade distribution of A2 and A3 shades 3. No significant differences in shade distribution were seen between genders in maxillary canines, whereas maxillary central and lateral incisors showed differences at the middle and cervical thirds between male and female subjects The results of this study show that with the exception of maxillary canines, maxillary anterior teeth display a diverse shade distribution as well as gender differences. Conclusion : Clinically, when making a shade selection using the existing shade guide, one must consider the fact that even a single tooth consists of a variety of shades. The results of this study show that when selecting a shade from a number of groups is difficult, shades from A group are the most consistent with the natural shade or maxillary anterior teeth.

Purpose: This study aimed to quantify the influence of tooth position within the field-of-view (FOV) on cone-beam computed tomography (CBCT) imaging artifacts' intensity when assessing teeth restored with various intracanal materials. Materials and Methods: Seventy single-rooted teeth were divided into 7 groups (10 teeth per group): NiCr post (NC), AgPd post (AP), metal core fiberglass post (MCFG), fiberglass post (FG), anatomical fiberglass post (AFG), fiberglass post cemented with core build-up cement (FGCo), and anatomical fiberglass post cemented with core build-up cement (AFGCo). All posts were cemented using a regular dual-curing resin cement (Allcem), except FGCo and AFGCo which were cemented with a core build-up dual-curing resin cement (AllcemCore). Each tooth was scanned on a CS9000 in 5 positions within the FOV: a central position, anterior horizontal peripheral, peripheral superior, peripheral inferior, and posterior horizontal peripheral position. Hyperdense, hypodense, remaining teeth areas and ROI areas were quantitatively analyzed using ImageJ software. Results: Posterior horizontal peripheral position increased the intensity of artifacts on FGCo and AFGCo post groups (P<0.05), and specifically the hypodense artifact intensity on FG and AFG post groups (P<0.05). NC and AP groups presented greater intensity of artifacts than any other post groups(P<0.05). Conclusion: Artifact intensity increases in the presence of high atomic number materials and when the object is not centered within the FOV. The impact of positioning within the FOV on artifact was greater for fiberglass posts cemented with core build-up dual-curing cement than for metal posts and fiberglass posts cemented with regular dual-curing cement.
Objectives : The purpose of this study was to investigate the relationship between the oral health status of elderly people and their oral health-related quality of life. Methods : The subjects were the elderly people over 65 years old in Busan. A total of 479 elderly people participated in the study from general hospital, two research institutes, eight dental clinics, six welfare institutions, sixteen senior centers, and three nursing homes. The oral health impact profile-14 (OHIP-14) was used in the routine dental checkups of the elderly people. Results : Those who had no oral symptoms had a better oral health-related quality of life. Among the subfactors of the OHIP-14, significant differences were shown in functional limitation(p=0.001), physical pain(p<0.001), emotional discomfort(p<0.001), physical disability(p=0.001), emotional disability (p=0.001), social disability(p=0.005), physical handicap(p=0.003) and total OHIP-14(p<0.001). Those who had 18 sound teeth(natural teeth) or more had a better oral health-related quality of life. Among the subfactors of the OHIP-14, significant differences were shown in functional limitation(p<0.001), physical pain(p=0.007), emotional discomfort(p=0.019), physical disability(p=0.018), Emotional disability(p=.032) and total OHIP-14 (p=0.006). Conclusions : The results revealed a close relationship between oral health status and oral health-related quality of life. The number of sound teeth(natural teeth) and frequency of toothbrushing had a more positive influence. Therefore oral health programs for the elderly people can preserve remaining teeth. Toothbrushing is the best way to improve the quality of life in the elderly people.
Objectives: The purpose of this study was to compare the cutting efficiency of a newly developed microprojection tip and a diamond-coated tip under two different engine powers. Materials and Methods: The apical 3 mm of each root was resected, and root-end preparation was performed with upward and downward pressure using one of the ultrasonic tips, KIS-1D (Obtura Spartan) or JT-5B (B&L Biotech Ltd.). The ultrasonic engine was set to power-1 or -4. Forty teeth were randomly divided into four groups: K1 (KIS-1D / Power-1), J1 (JT-5B / Power-1), K4 (KIS-1D / Power-4), and J4 (JT-5B / Power-4). The total time required for root-end preparation was recorded. All teeth were resected and the apical parts were evaluated for the number and length of cracks using a confocal scanning micrscope. The size of the root-end cavity and the width of the remaining dentin were recorded. The data were statistically analyzed using two-way analysis of variance and a Mann-Whitney test. Results: There was no significant difference in the time required between the instrument groups, but the power-4 groups showed reduced preparation time for both instrument groups (p < 0.05). The K4 and J4 groups with a power-4 showed a significantly higher crack formation and a longer crack irrespective of the instruments. There was no significant difference in the remaining dentin thickness or any of the parameters after preparation. Conclusions: Ultrasonic tips with microprojections would be an option to substitute for the conventional ultrasonic tips with a diamond coating with the same clinical efficiency.
The purpose of this study is to determine if a relationship exists among osteoporosis, alveolar bone density and periodontal disease in postmenopausal osteoporotic women and postmenopausal healthy women. Twenty-two women were evaluated for this study. They were attending the postmenopausal clinic, Seoul National University Hospital and generally healthy except osteoporosis. They had experienced menopause not less than one year when we began to examine them. Bone densities of lumbar area(L2-L4) was determined by DEXA(LUNAR-expert Co,. U.S.A). We diagnosed osteoporosis when T-score was below -2.5 and healthy state when T-score was over -1. Osteoporotic(10 female), not hormone-treated group and healthy control group(12 female) were asked for their age, menopausal age, menopausal period and the number of remaining teeth and examined clinically for plaque index(PI), gingival index(GI), clinical attachment loss(CAL) on their 6 Ramfjord index teeth. Intraoral radiographs were taken in maxillary anterior zone. All films were equally exposed and developed. Each films was digitized and analysed using image processing software, Scion image. Alveolar bone regions of interest were selected and Intensity of each pixel was quantized in the array ranging from 0(white) to 255(black). The two groups were comparable with respect age, menopausal age, menopausal period and number of remaining teeth. The osteoporotic women had significantly lower alveolar bone density than controls in maxilla. But no significant difference was found with respect clinical attachment loss, plaque index and gingival index. Supported by the Ministry of Public Health and Welfare, Korea (HMP-00-CH-10-0009).
The purpose of the present study is to inspect the satisfied degree of each oral function in geriatric patients with the shortened dental arch and when their prosthetic treatment is on schedule, provide some references to such treatment. For the approach to such study, 521 subjects were reviewed by grouping them according to the number of their remaining teeth, and masticatory function, phonetic function, facial change, and TMJ disorders were inspected and clarified through some questionnaires. Also through the questionnaires, the correlations between the geriatiric patients with the shortened dental arch and dentition and between the geriatiric patients with the Free-end RDP at the shortened dental arch and their oral function were found out with their satisfied degree of oral function. Results or findings from such study are as follows : 1. With regard to their satisfied degree of oral function, there was a significant difference of satisfaction between or among the group having only the anterior teeth and the group having the part of premolars and the group having even the part of molars, however no significant difference of satisfaction appeard between the group having 1st molars and the group having 2nd molars. 2. With regard to their satisfied degree of phonetic function, no significant difference appeared between or among the group having only the anterior part of teeth and the group having even the part of premolars and the group having even the part of molars, and with regard to their satisfied degree of facial change, no significant difference of satisfaction appeared between the group having the part of premolars and the group having even the part of molars. 3. With regard to their satisfied degree of masticatory function, phonetic function, TMJ disorders, and facial change, no significant difference appeared between the group having both the anterior part of teeth and the part of premolars and the group attached with the Free-end RPD on the same conditions of the afore-said group.
The purpose of this study was to examine the relationship between the dietary habits and oral health of elderly people in an attempt to pave the way for the development of oral health promotion programs geared toward improving the quality of life of the elderly. The subjects in this study were senior citizens who were selected by convenience sampling from Seoul. The findings of the study were as follows: 1. Regarding subjective oral health state, 54.5% of the elderly people, more than the half, considered their mouth to be in good health. The number of their mean remaining permanent teeth was 13.71. 47.5 percent of the senior citizens investigated had no shaking teeth. 2. Those who found themselves to be in good oral health had meals on a regular basis(p=0.022) and ate detergent food often. The gaps between them and the others was significant(p=0.005). In contrast, the elderly people who were in a bad oral health frequently ate cariogenic food(p=0.044). 3. The elderly people who had 21 teeth or more ate detergent food often(p=0.029), and those who owned no teeth had a sweet teeth(p=0.003), ate more cariogenic food(p=0.001) and had a snack frequently(p=0.026). 4. The subjective oral health status had a positive correlation to detergent food intake(r=0.23) and had a slightly negative correlation to preference for sweets(r=-0.14), cariogenic food intake(r=-0.14) and snack intake(r=-0.06). The number of tooth was positively correlated to detergent food intake(r=0.23) and negatively to preference for sweets(r=0.32), cariogenic food intake(r=-0.30) and snack intake(r=-0.21). The presence or absence of shaking teeth had a positive correlation to snack intake(r=0.14). The above-mentioned findings suggested that the dietary habits of the elderly people had a statistically significant relationship to subjective and objective oral health state, which indicated that there was a close relationship between oral health and dietary habits. Therefore how to improve dietary habits as well as oral health should be taken into account when oral health promotion programs are developed for the sake of the elderly. That would contribute to promoting the oral health of elderly people and eventually boosting their quality of life.
Purpose: The purpose of this study was to identify associated factors of chewing discomfort among community-dwelling elderly. Methods: The study was cross-sectional design and secondary data analysis using the 6th Korea National Health and Nutrition Examination Survey. Among the total of 7,550 participants, data was analyzed with 1,126 adults aged 65 years and over. Chewing discomfort was assessed by the perceived chewing discomfort. Multivariate logistic regression analysis was used to find the associated factors of chewing discomfort. Results: Along with 61.7% of the participants reported having chewing discomfort, 85.2% reported to perceive poor oral health and 35.0% had oral pain. In multivariate logistic regression, perceived oral health (OR 3.22, 95% CI 2.24~4.63), oral pain (OR 2.46, 95% CI 1.76~3.43), activity limitation (OR 1.71, 95% CI 1.05~2.80), teeth requiring treatment (OR 1.61, 95% CI 1.14~2.26), number of remaining teeth (OR 1.60, 95% CI 1.22~2.10) and educational level (OR 1.56, 95% CI 1.15~2.12) were the significant predictors of chewing discomfort. Conclusion: The prevalence in chewing discomfort was high in elderly Koreans and various factors were associated with chewing discomfort. To improve chewing ability, it is suggested that the national level of policies offer strategical oral health programs in this population.
