이상운동질환에서의 도파민 신경계 영상 (Imaging of Dopaminergic System in Movement Disorders)

  • Parkinson's disease is a common neurodegenerative disorder that is mainly caused by dopaminergic neuron loss in the substantia nigra. Several radiopharmaceutics have been developed to evaluate the integrity of dopaminergic neuronal system. In vivo PET and SPECT imaging of presynaptic dopamine imaing are already applied to Parkinson's disease and other parkinsonism, and can demonstrate the dopaminergic dysfunction. This review summarized the use of the presynaptic dopaminergic imaging in PD as biomarkers in evaluation of disease progression as well as in diagnosis of PD.

악성 흑색종에서 $^{18}F-FDG$ PET의 임상 이용 (Clinical Application of $^{18}F-FDG$ PET in Malignant Melanoma)

  • $^{18}F-FDG$ PET showed a high sensitivity and specificity in the initial staging of malignant melanoma, and it also predicted the prognosis correctly. In addition, it had a higher sensitivity and specificity in the detection of recurrence and restaging than conventional imaging modalities. Meanwhile, there's less clinical evidence to support the use of $^{18}F-FDG$ PET in the response evaluation for chemotherapy and the diagnosis/differential diagnosis of malignant melanoma.

  • Xanthogranulomatous pyelonephritis is an uncommon chronic renal infection, which is usually found on middle-aged women and is rare in infant. Sometimes it forms focal mass like lesion of kidney with pathologically characteristic lipid-laden macrophage. A 1-month female infant was admitted for fever and moaning sound. On work-up of urinary tract infection, abdomen ultrasonography and computed tomography revealed a large mass on the upper portion of right kidney and PET/CT showed homogeneously increased $^{18}F$-FDG uptake. The radical nephrectomy of right kidney was performed and histology revealed a focal xanthogranulomatous pyelonephritis. To our knowledge, this is the first report presenting the finding of $^{18}F$-FDG PET/CT in the childhood xanthogranulomatous pyelonephritis.

  • The goals of developments in nuclear medicine instrumentation are to offer a higher-quality image and to aid diagnosis, prognosis assessment or treatment planning and monitoring. It is necessary for physicists and engineers to improve or design new instrumentation and techniques, and to implement, validate, and apply these new approaches in the practice of nuclear medicine. The researches in physical properties of detectors and crystal materials and advances in image analysis technology have improved quantitative and diagnostic accuracy of nuclear medicine images. This review article presents recent developments in nuclear medicine instrumentation, including scatter and attenuation correction, new detector technology, tomographic image reconstruction methods, 511 keV imaging, dual modality imaging device, small gamma camera, PET developments, image display and analysis methods.

약물부하 검사법의 현재와 미래 (Current Trends and Future Development in Pharmacologic Stress Testing)

  • Pharmacologic stress testing for myocardial perfusion imaging is a widely used noninvasive method for the evaluation of known or suspected coronary artery disease. The use of exercise for cardiac stress has been practiced for over 60 years and clinicians are familial with its using. However, there are inevitabe situations in which exorcise stress is inappropriate. A large number of patients with cardiac problems are unable to exercise to their full potential due to comorbidity such as osteoarthritis, vascular disease and pulmonary disease and a standard exercise stress test for myocardial perfusion imaging is suboptimal means for assessment of coronary artery disease. This problem has led to the development of the pharmacologic stress test and to a great increase in its popularity. All of the currently used pharmacologic agents have well-documented diagnostic value. This review deals the physiological actions, clinical protocols, safety, nuclear imaging applications of currently available stress agents and future development of new vasodilating agents.

  • Monoclonal antibodies have become widely investigated in the Nuclear Oncology, especially in the radioimmunosassay of tumor markers and in vivo radioimmunoimaging of cancer. However, there are numerous factors as to whether radioimmunoimaging will ultimately successful. For imaging of tumors, metallic radionuclides such as In-111, Ga-67, Tc-99m have favorable nuclear properties than widely used I-131. These radioistopes have characteristics of the useful radiation for imaging, convenient short half-lives and the simple and rapid radiolabeling of monoclonal antibodies by using bifunctional chelaing agents. The obtained chelate-tagged antibodies are quite stable both in vitro and in vivo, without interfering antibody activities and animal experiments provided a good basis for its clinical applicability for the radioimmunoimaging of cancer. Much attention has also been given to the possibility, only beginning to be exploited, of the specific treatment of malignant neoplasms with these agents. Although specific antibody has not been developed that is uniquely specific for cancer alone and there are still many questions to be answered and problems to be overcome before radioimmunoimaging can be successfully used in ptients with cancer, these methods can be applied to the coupling of monoclonal antibodies with anti-neoplastic drugs or radionuclides suitable for internal radiation therapy of cancer.

분자영상의 윤리 및 규제 (Ethical and Regulatory Problems of Molecular Imaging)

  • As a molecular imaging is the most up-to-date technology in Nuclear Medicine, it has complicate ethical and regulatory problems. For animal experiment, we have to follow institutional animal care committee. for clinical experiment, we have to get approval of Institutional Review Board according to Helsinki declaration. In addition, approval from Korea Food and Drug Administration (KFDA) is essential for manufacturing and commercialization. However, too much regulation would suppress development of new technology, which would result in the loss of national competitive power. In addition, most new radioactive ligands for molecular imaging are administered to human at sub-pharmacological and sub-toxicological level. In conclusion, a balanced regulation is essential for the safety of clinical application and development of new technology.

  • 두경부의 악성 종양의 진단에서 $^{99m}Tc$-(V)-DMSA 영상술은 비교적 높은 양성율을 보여 주었고 (Planar 65%, SPECT 90%), 원격 전이부위의 영상진단에도 도움이 되었다. 그러나 $^{99m}Tc$-(V)-DMSA는 두경부의 양성 병변에도 섭취가 될 수 있어, 판독 시 임상검사 및 다른 영상소견과의 비교검토가 필요할 것으로 판단된다.

  • Evaluation of dementia in patients with early symptoms of cognitive decline is clinically challenging, but the need for early, accurate diagnosis has become more crucial, since several medication for the treatment of mild to moderate Alzheimer' disease are available. Many neurodegenerative diseases produce significant brain function alteration even when structural imaging (CT or MRI) reveal no specific abnormalities. The role of PET and SPECT brain imaging in the initial assessment and differential diagnosis of dementia is beginning to evolve vapidly and growing evidence indicates that appropriate incorporation of PET into the clinical work up can improve diagnostic and prognostic accuracy with respect to Alzheimer's disease, the most common cause of dementia in the geriatric population. in the fast few years, studios comparing neuropathologic examination with PET have established reliable and consistent accuracy for diagnostic evaluations using PET - accuracies substantially exceeding those of comparable studies of diagnostic value of SPECT or of both modalities assessed side by side, or of clinical evaluations done without nuclear imaging. This review deals the role of functional brain imaging techniques in the evaluation of dementias and the role of nuclear neuroimaging in the early detection and diagnosis of Alzheimer's disease.

  • Recent progress in the development of non-invasive imaging technologies continues to strengthen the role of biomedical research. These tools have been validated recently in variety of research models, and have born shown to provide continuous quantitative monitoring of the location(s), magnitude, and time-variation of gene delivery and/or expression. This article reviews the use of PET technologies as they have been used in imaging biological processes for molecular imaging applications. The studies published to date demonstrate that noninvasive imaging tools will help to accelerate pre-clinical model validation as well as allow for clinical monitoring of human diseases.
