A liver fluke, Opisthorchis viverrrini (OV), is the major cause of the high incidence of cholangiocarcinoma in Thailand. The prevalence of OV infection remains high in various parts of the country, especially in Northeast Thailand and particularly in wetland rural areas where a large proportion of the community work in agriculture and continue the traditional practice of eating raw or uncooked cyprinoid fish products. The national control program seems to have had little impact in many of these areas, and it has been difficult to make precise assessments of the overall effectiveness of the program. This paper is the first report of prospective research project designed to monitor the impact of the national control program in rural communities located in a northeastern province and at high risk of OV infection. The participants in this initial survey were 1,569 villagers, aged 20-65 years, living in two subdistricts of Yasothon Province. Stool examinations showed that 38.68% were infected with OV. Males were slightly more likely to be infected than females, but the difference was not statistically significant. Infection was found to be positively associated with age in both males and females. The preliminary data indicate that the population selected for study is suitable for the purpose of the monitoring project.
A retrospective cohort study was carried out with 340 female breast cancer at a teaching university in northeast of Thailand recruited and followed-up until the end of 2006. Survival probabilities were estimated using the Kaplan-Meier method. 161 cases were alive after five years and 58 patients were lost to follow-up. The overall observed survival rates at 1, 3 and 5 years were 83.3%, 59.9% and 42.9%, respectively. When analysis was conducted for stage combined into 2 groups, early (stage I, II and unknown) and late (stage III and IV), the 5-year survival rate for early stage (60%; 95%CI: 0.51-0.67), was higher than for late stage (27%; 95%CI: 0.19-0.34) with high statistical significance (p<0.001). The hazard ratio of patients with stage IV was 11.6 times greater than for stage I (p=0.03). The findings indicate that the different stages of breast cancer markedly effect the overall survival rate.
Radiolarian assemblages from KODOS area were analyzed in order to understand the biostratigraphy and paleoceanography of deep-sea sediment from the Northeast Equatorial Pacific. The sediment core was divided into two or three units on the basis of the chemical and physical properties. In the upper sediment, mixtures of Quaternary and Tertiary radiolarians are found indicating active reworking processes. Dissolution of radiolarians seem to increase with depth. Radiolarians are seldom in Unit III presumably due to dissolution and corrosion. The middle part of unit I appears to correspond to Collosphaera invaginata Zone (0.21 Ma). Unit II belongs to Collosphaera tuberosa Zone. Based on the absence of Stylatractus universus, we estimate its age to be younger than 0.42 Ma. Based on our analyses of radiolarians in Unit I and II, we estimated the age of unit III as Tertiary, particularly from Oligocene to Miocene. There may to be hiatuses of more than 3 My from late Miocene to Pliocene, which probably resulted from erosion and dissolution by the Antarctic Bottom Water Sedimentation rates during Quaternary range from 0.15 to 0.50 mm/ky with significant variabilities among stations. Radiolarians in the study area were mostly warm-water species.
The incidence and mortality rates of breast cancer in Northeast Brazil are increasing and little is known about prevailing reproductive factors contributing to this increase. A case-control study was conducted in a public hospital of Campina Grande, state of $Para{\acute{i}}ba$, including 81 women with diagnosed invasive breast cancer and 162 age matched (${\pm}5years$) controls. Binominal logistic regression analysis was applied to estimate odds ratio (OR) and confidence intervals (CI) of risk factors. In this model, age at menarche ${\leq}12$ (OR=2.120; CI: 1.043-4.308; p=0.038), single parity (OR=3.748; CI: 1.459-9.627; p=0.06) and reproductive period >10 years (OR=3.042; CI: 1.421-6.512; p=0.04) were identified as independent variables that significantly increased breast cancer risk of parous women. Compared to parous women who never practised breastfeeding, total breastfeeding time > 24 months decreased the risk of breast cancer (OR=0.258; CI: 0.084-0.787; p=0.017). The results indicated that modifiable reproductive factors contribute to breast cancer risk in women included in the present study. Women's knowledge about factors such as the protective effect of breastfeeding could reduce the risk of breast cancer.
Human papillomavirus (HPV) is a major cause of cervical cancer. More than 100 HPV genotypes have been identified; however the distribution varies geographically and according to ethnicity. The purpose of this study was to investigate the prevalence and distribution of HPV subtypes among Northeast Thai women. Subjects included 198 cases of SCCA and 198 age-matched, healthy controls. HPV-DNA was amplified by PCR using the consensus primers GP5+/6+ system followed by reverse line blot hybridization genotyping. The prevalence of high-risk HPV infection was 21 (10.1%) and 152 (76.8%) in the controls and in the cases, respectively. High-risk HPV significantly increased the risk for cervical cancer with an OR of 42.4 (95%CI: 22.4-81.4, p<0.001) and an adjusted OR of 40.7-fold (95%CI: 21.5-76.8, p <0.001). HPV-16 was the most prevalent HPV type in the SCCA (56.2%) followed by HPV-58 (17.8%) and HPV-18 (13.6%); whereas HPV-58 (46.4%) was a prominent genotype in the controls followed by HPV-16 (39.3%) and unidentified HPV types (25.0%). These findings indicate that HPV infection remains a critical risk factor for SCCA; particularly, HPV-16, HPV-58 and HPV-18. In order to eradicate cervical cancer, sustained health education, promoted use of prophylactics and a HPV-58 vaccine should be introduced in this region.
Background: Polymorphisms in the MDM2 309 (T>G) and TP53 72 (G>C) genes are reported to increase the susceptibility to head and neck cancer (HNC) in various populations. The risk for HNC is also strongly associated with etiologic habits such as smoking, alcohol consumption and/or chewing of betel quid (BQ). In a case-control study, we investigated the significance of the above polymorphisms alone, and upon interaction with one another as well as with various etiologic habits in determining HNC risk in a Northeast Indian population. Materials and Methods: Genotyping at 309 MDM2 and 72 TP53 in 122 HNC patients and 86 cancer free healthy controls was performed by PCR using allele specific primers, and the results were confirmed by DNA sequencing. Results: Individuals with the GG mutant allele of MDM2 showed a higher risk for HNC in comparison to those with the TT wild type allele (OR=1.9, 95%CI: 1.1-3.3) (p=0.022). The risk was further increased in females by ~4-fold (OR=4.6, 95% CI: 1.1-19.4) (P=0.04). TP53 polymorphism did not contribute to HNC risk alone; however, interaction between the TP53 GC and MDM2 GG genotypes resulted in significant risk (OR=4.9, 95% CI: 0.2-105.1) (p=0.04). Smokers, BQ- chewers and alcohol consumers showed statistically significant and dose-dependent increase in HNC risk, irrespective of the MDM2 genotype. Conclusions: MDM2 genotype could serve as an important predictive biomarker for HNC risk in the population of Northeast India.
Multiple genetic and environmental factors have been reported to play key role in the development of nasopharyngeal carcinoma (NPC). Here, we investigated interactions of XRCC1 Arg399Gln and XRCC2 Arg188His polymorphisms and environmental factors in modulating susceptibility to NPC in Northeast India. One-hundred NPC patients, 90 first-degree relatives of patients and 120 controls were enrolled in the study. XRCC1 Arg399Gln and XRCC2 Arg188His polymorphisms were determined using PCR-RFLP, and the results were confirmed by DNA sequencing. Logistic regression (LR) and multifactor dimensionality reduction (MDR) approaches were applied for statistical analysis. The XRCC1 Gln/Gln genotype showed increased risk (OR=2.76; P<0.024) of NPC. However, individuals with both XRCC1 and XRCC2 polymorphic variants had 3.2 fold elevated risk (P<0.041). An enhanced risk of NPC was also observed in smoked meat (OR=4.07; P=0.004) and fermented fish consumers (OR=4.34, P=0.001), and tobacco-betel quid chewers (OR=7.00; P=0.0001) carrying XRCC1 polymorphic variants. However, smokers carrying defective XRCC1 gene showed the highest risk (OR = 7.47; P<0.0001). On MDR analysis, the best model for NPC risk was the five-factor model combination of XRCC1 variant genotype, fermented fish, smoked meat, smoking and chewing (CVC=10/10; TBA=0.636; P<0.0001); whereas in interaction entropy graphs, smoked meat and tobacco chewing showed synergistic interactions with XRCC1. These findings suggest that interaction of genetic and environmental factors might increase susceptibility to NPC in Northeast Indian populations.
A strong negative correlation has been detected between the North Pacific Oscillation Index (NPI) and the Effective Drought Index (EDI) in May over Korea. In May of positive NPI year, anomalous patterns caused a drought in Korea as follows: the anomalous south-low, north-high low-level pressure patterns in the northeast and southeast of Korea have strengthened the anomalous northerlies to Korea. In addition, these anomalous northerlies have prevented western North Pacific (WNP) high from moving northward. As a result, anomalous descending flows have strengthened in the mid-latitude region in East Asia. In the WNP, the anomalous south-high, north-low sea surface temperature (SST) has been widely distributed, which has strengthened anomalous south-low, north-high low-level pressure patterns. These anomalous characteristics of pressure and SST patterns observed in May of positive NPI years have already been detected in previous winter (December-February) and early spring (March, April). In addition, the anomalous negative sea ice concentration in the North Pacific during two seasons has strengthened the anomalous anticyclonic circulation in the same region and in turn made a contribution to formation of anomalous south-low, north-high pressure patterns in May.
In order to reveal the vertical variation of physical properties in deep-sea sediments, deep-sea sediment cores were collected at 78 stations using a multiple corer in the KR5 area, one of the Korea contract areas for manganese nodule exploration, located in the northeast equatorial Pacific. Based on the color of sediments, sampled sediment cores were characterized into three lithologic units (unit 1,2, and 3). In all sediment cores, three units appear systematically; unit 1 lies at the top of cores and unit 2 and/or unit 3 appear to underlie unit 1 or alternate with unit 3. Unit 1 layer from the top of cores shows dark grayish brown to dark brown with mean thickness of 10.2cm. Unit 2 and 3 layers show very dark brown to black color and yellowish brown to brown color, respectively. According to the physical properties of the deep-sea sediment cores, sediment column can be divided into three sections. Section A $(0{\sim}15cm)$ in subbottom depth consists mostly of unit 1. Mean values of physical properties of section B $(15{\sim}30cm)$ in subbottom depth are similar to those of section C (>30 cm) in subbottom depth. However, the physical properties of section B were more variable than those of section C because of the high activity of bioturbation in section B. These results will provide valuable information for selecting suitable sites for mining manganese nodules in the Korea contract areas.
Annual variations of inorganic nutrients such as nitrate(+nitrite), phosphate and silicate in association with thermocline were investigated in the upper 200 m of the water column at KODES Long-term Monitoring (KOMO) station in the northeast equatorial Pacific from 1995 to 2002. Global climatic disturbances such as El Nino and La Nina, should have affected KODES area during the study period. In 1995-97 and 2000-2002, a thermocline where temperatures rapidly decrease with depth, was formed at 50-70 m water depth. Nutrient depletion, specially for nitrate and phosphate, was extended down to approximately 50 m depth, which coincided with the surface mixed layer depth. In 1998 and 1999, however a very fluctuating thermocline was observed at 20-100 m water depth. In the photic zone (up to 100 m depth), depth integration of nitrate, phosphate and silicate ranged from 2.02 to $23.14\;gN/m^2$, from 0.87 to $4.05\;gP/m^2$ and from 35.67 to $176.21\;gSi/m^2$, respectively. As a result of changes in the water column structures, nutrient concentrations also showed fluctuation parallel to the changes of thermocline in the study area.
