Bartter syndrome (BS) is an autosomal recessively inherited rare renal tubular disorder characterized by hypokalemic metabolic alkalosis and hyperreninemic hyperaldosteronism with normal to low blood pressure due to a renal loss of sodium. Genetically, BS is classified into 5 subtypes according to the underlying genetic defects, and BS is clinically categorized into antenatal BS and classical BS according to onset age. BS type I is caused by loss-of-function mutations in the $SLC12A1$ gene and usually manifests as antenatal BS. This report concerns a male patient with compound heterozygous missense mutations on $SLC12A1$ (p.C436Y and p.L560P) and atypical clinical and laboratory features. The patient had low urinary sodium and chloride levels without definite metabolic alkalosis until the age of 32 months, which led to confusion between BS and nephrogenic diabetes insipidus (NDI). In addition, the clinical onset of the patient was far beyond the neonatal period. Genetic study eventually led to the diagnosis of BS type I. The low urinary sodium and chloride concentrations may be caused by secondary NDI, and the later onset may suggest the existence of a genotype-phenotype correlation. In summary, BS type I may have phenotype variability including low urine sodium and chloride levels and later onset. A definitive diagnosis can be confirmed by genetic testing.
Computation solutions for the flow around a cube, which were generated as part of the Computational Wind Engineering 2000 Conference Competition, are compared with full-scale measurements. The three solutions shown all use the RANS approach to predict mean flow fields. The major differences appear to be related to the use of the standard $k-{\varepsilon}$, the MMK $k-{\varepsilon}$ and the RNG $k-{\varepsilon}$ turbulence models. The inlet conditions chosen by the three modellers illustrate one of the dilemmas faced in computational wind engineering. While all modeller matched the inlet velocity profile to the full-scale profile, only one of the modellers chose to match the full-scale turbulence data. This approach led to a boundary layer that was not in equilibrium. The approach taken by the other modeller was to specify lower inlet turbulent kinetic energy level, which are more consistent with the turbulence models chosen and lead to a homogeneous boundary layer. For the $0^{\circ}$ case, wind normal to one face of the cube, it is shown that the RNG solution is closest to the full-scale data. This result appears to be associated with the RNG solution showing the correct flow separation and reattachment on the roof. The other solutions show either excessive separation (MMK) or no separation at all (K-E). For the $45^{\circ}$ case the three solutions are fairly similar. None of them correctly predicting the high suctions along the windward edges of the roof. In general the velocity components are more accurately predicted than the pressures. However in all cases the turbulence levels are poorly matched, with all of the solutions failing to match the high turbulence levels measured around the edges of separated flows. Although all of the computational solutions have deficiencies, the variability of results is shown to be similar to that which has been obtained with a similar comparative wind tunnel study. This suggests that the computational solutions are only slightly less reliable than the wind tunnel.
Homologous recombination repair (HRR) plays an important role in protection against carcinogenic factors. Genes regulating the HRR mechanisms may impair their functions and consequently result in increased cancer susceptibility. RAD 51 and XRCC3 are key regulators of the HRR pathway and genetic variability in these may contribute to the appearance and progression of various cancers including head and neck cancer (HNC). The aim of the present study was to compare the distribution of genotypes of RAD51 (135G/C, 172 G/T) and XRCC3 (Thr241Met) polymorphisms between HNC patients and controls. Each polymorphism was genotyped using the polymerase chain reaction-restriction fragment length polymerase (PCR-RFLP) technique in 200 pathologically confirmed HNC patients along with 150 blood samples from normal, disease free healthy individuals. We observed that homozygous variant CC genotype of RAD51 135G/C was associated with a 2.5 fold increased HNC risk (OR=2.5; 95%CI=0.69-9.53; p<0.02), while second polymorphism of RAD 51 172 G/T, heterozygous variant GT genotype was associated with a 1.68 fold (OR=1.68; 95%CI=1.08-2.61; p<0.02) elevation when compared with controls. In the case of the Thr241Met polymorphism of XRCC3, we observed a 16 fold (OR=16; 95% CI=3.78-69.67; p<0.0002) increased HNC risk in patients compared to controls. These results further suggested that RAD51 (135G/C, 172 G/T) and XRCC3 (Thr241Met) polymorphisms may be effective biomarkers for genetic susceptibility to HNC. Larger studies are needed to confirm our findings and identify the underlying mechanisms.
Purpose: Thermal sensory test as an essential part of quantitative sensory testing (QST) has been recognized as a useful tool in the evaluation of the trigeminal nerve function. Normative data in the orofacial region have been reported but the data on differences in the test site, sex and ethnicity are still insufficient. Thus, this study aimed to investigate the normal range of orofacial thermal QST data in the healthy Korean women, and assess sex difference of thermal perception in the orofacial regions. Methods: Thermal QST was conducted on 20 healthy women participants (mean age, 26.4 years; range, 21 to 34 years). The thermal thresholds (cold detection threshold, CDT; warm detection threshold, WDT; cold pain threshold, CPT; and heat pain threshold, HPT) were measured bilaterally at the 5 trigeminal sites (the forehead, cheek, mentum, lower lip and tongue tip). The normative thermal thresholds of women in the orofacial region were evaluated using one-way ANOVA and compared with the previously reported data from age- and site-matched 30 healthy men (mean age, 26.1 years; range, 23 to 32 years) using two-way ANOVA. One experienced operator performed the tests of both sexes and all tests were done in the same condition except the time variability. Results: Women showed significant site differences for the CDT (p<0.001), WDT (p<0.001), and HPT (p=0.047) in the orofacial region. The CDT (p<0.001) and the CPT (p=0.007) presented significant sex difference unlike the WDT and the HPT. Conclusions: The thermal sensory evaluation in the orofacial region should be considered in the context of site and sex and the normative data in this study could be useful for assessment of the sensory abnormalities in the clinical setting.
Objectives: This clinical study reports the effects of Cheonggansoyo-san (CSS) on three patients with glossodynia. Methods: Three patients with glossodynia were treated with CSS (i.e., an herbal medicine) and acupuncture. Numeric rating scales (NRSs) for glossodynia and dry mouth and hwa-byung questionnaire scores were evaluated, and the results from heart rate variability (HRV) evaluations were analyzed. Results: After treatment with CSS, three patients' glossodynia symptoms, such as burning sensations, tingling, and numbness of the tongue, were improved. NRS ratings for glossodynia and dry mouth and the hwa-byung questionnaire scores also improved, and HRV parameters were closer to the normal range. Conclusion: Cheonggansoyo-san could be used to treat glossodynia.
Seasonal variability in the algal community structure of intertidal zones was examined at six study sites in Marine National Parks, on the western and southern coasts of Korea from March to November 2015. A total of 193 species of algae, comprising 27 green, 40 brown and 126 red algae, were identified. Algal biomass averaged 101.50 g dry wt./m2 with a maximal at Tonggae (168.12 g dry wt./m2) and minimum at Haseom (54.49 g dry wt./m2). The dominant seaweeds by biomass were Sargassum thunbergii at five sites (Tonggae, Haseom, Namdongri, Ando amd Sinjeonri), and S. fusiforme at Soando. Community indices measured as follows: dominance index (DI), 0.31–0.69; richness index (R), 7.30–11.43; evenness index (J'), 0.33–0.49; and diversity index (H'), 1.15–1.86. As evaluated using community indices and environmental states, the EEI (Ecological Evaluation Index) of the Marine National Park was “normal”. The present results indicate that seaweed community structures and indices could be used to evaluate the environmental status of coastal ecosystems.
Purpose: To assess the internal echo intensity and morphological variability of masseter muscles on ultrasonography and to establish diagnostic criterion of estimation. Materials and Methods: Participants consisted of 50 young adults (male 25, female 25) without pathologic conditions and with full natural dentitions. Sonographic examinations were done with real time ultrasound equipment as Logiq 500 (GE Medical Systems, Seoul, Korea) at 3 parts according to lines paralleling with ala-tragus line as reference line. The thickness and area of masseter muscles according to reference line in cross-sectional images were measured at rest and at maximum contraction. The visibility and width of the internal echogenic intensity of the masseter muscles were also assessed and the muscle appearance was classified into 4 types. Data were statistically analyzed by paired t-test and $x^2$-test. Results: 1. When comparing the thickness and area of masseter muscles concerning with gender, there was few significant difference between right and left sides, however, there were significant differences between males and females except for the greatest thickness of left side. 2. The changes of the greatest thickness and the area between rest and maximum contraction showed that the part of the least thickness manifested more increase at maximum contraction. 3. Each part the manifestations of the internal echogenic intensity of the masseter muscles were different depending on the locations. But there was no statistically significance. Conclusion: Changes of muscles thickness with contraction and internal echogenic intensity with locations showed great disparity within the masseter muscles, which will be diagnostic criteria for pathophysiologic and anatomic changes of masseter muscles.
This study was carried out to analyze the effect of a diet program for adult women on weight loss, BMI, eating habits, sleeping habits, health related indices. The subjects was 415 participants of the 10 weeks (20 time participation program). The data was collected by basic somatometry and HRV (Heart Rate Variability) / APG (Accelerated Plethysmograph) Analyzer. The average age, height, weight and BMI were 28.6 years, 162.1 cm, 62.8 kg and 23.9 $kg/m^2$, respectively. Their body types by BMI were under weight (1.2%), normal (45.8%), overweight (24.8%), mild obesity (22.7%) and obesity (5.5%). There were significant reductions of average weight (4.6 kg) and average BMI (1.75 $kg/m^2$) on the 10th week. There were positive changes in vascular age (50.4%), stress index (44.6%), fatigue index (43.9%), health index (54.5%) of the subjects during the program. There was a meaningful difference of the average variation for the vascular age, stress index, fatigue index, and health index between two groups; one improving the eating habit and the other did not (p<0.05), and also there was a meaningful difference of the average variation for the fatigue index in both groups improving the sleeping habit (p<0.05), but was not a meaningful difference of the average variation for the vascular age, stress index, and health index between two groups. There was a meaningful difference of the average variation for the vascular age, stress index, fatigue index, and health index between two groups improving both eating and sleeping habit (p<0.05). In conclusion, the weight loss program was effective on the weight loss and BMI reduction and health related indices.
A health index was proposed that evaluates personal health state from both measured physiological variables and survey questions. Four health indices were defined such as cardiovascular index, stress index, obesity index, and management index. The total health index was calculated by summing these four health indices. Physiological variables such as blood pressure, heart rate variability(HRV), accelerated photoplethysmograph(APG), and body fat percentage were non-invasively measured and a survey questionnaire that asks personal health state, exercise intensity, and food preference was developed. The suggested health index was applied to thirty eight persons including 30 patients and 8 normal persons with an average age of 51.8. The average health index was estimated to be 75.1 out of 100 points. Young age group(below 50) and men group showed higher health indices than the aged(over 50) and women groups. The correlation coefficient between the cardiovascular index and stress index was found to be 0.513, which means stress is related to cardiovascular health state. The correlation coefficient between the measurements and survey questions was 0.385 for the cardiovascular index. It was as low as 0.182 for the stress index. More case studies may improve correlations between measurements and survey questions, and then, the current health index system may develop as an effective tool to evaluate personal health state.
Objectives : The purpose of this research is to examine the efficacy of Gamisoyosan on anxiety of generalized anxiety disorder according to dosage form. Methods : In this randomized, double blinded, placebo-controlled study, we planned to give Gamisoyosan simple extract mixture or Gamisoyosan compound extract or controlled medication for major symptoms of generalized anxiety disorder. As preparatory research, Hamilton rating Scale for Anxiety(HAM-A) was measured as the 1st evaluative instrument, and Korean State-Trait Anxiety Inventory(STAI-K), Penn State Worry Questionnaire(PSWQ) Korean Beck's Depression Inventory (BDI-K), Symptom Checklist-90-Revised(SCL-90R), WHO Quality of Life Abbreviated(WHOQOL-BREF) and Heart Rate Variability(HRV) were also measured as the 2nd evaluative instrument at the before treatment. Results : Demographic characteristics showed that there are Clinical characteristics-vital signs are within the normal range. The characteristics of disease-chief complaint, pattern Identification and etiological factor of the highest frequency number were worry, heart deficiency with timidity(心膽虛怯), family matters. The average period of disease in subjects is 6.31years. The results of Chest PA, EKG and clinicopathologic examination are within the reference range. The Scores of HAM-A, STAI-K, PSWQ were measured above the cutoff point. There are significant positive correlations among HAM-A, STAI-K, BDI-K and among HAM-A, STAI-K, PSWQ. There are no significant correlations between PSWQ and BDI-K. Conclusions : We considered that selection of subjects in this research is appropriately accomplished And this methodology is expected to be applied to the subsequent research. And also, we hope to make up for this study through various study and discussion.
