Investigation of Perceived Discomforts and Suggestion of a Procedure for the Universal Design of Housing (주거시설의 유니버설 디자인을 위한 활동실태조사 및 불편 설계요소 개선 절차 제안)

  • This study examined perceived discomforts encountered in residential facilities such as individual houses and apartments, and suggested a procedure for improving the problems in the living residence. The participants consisted of two groups of people: normal and underprivileged people; the underprivileged group was classified into 3 sub-groups: elderly people (over 65 years old), handicapped people and pregnant women. To identify inconvenient places and design factors in the residential housing, a total of 200 subjects (50 for each group) participated in the survey using the 7-point Likert scale. As a result, all the user groups mainly answered a high degree of discomfort due to narrow parking places, doorsills, and height differences between floors. The elderly, the handicapped, and the pregnant women replied that it is uncomfortable to use ramps and utility rooms. In a follow-up study, one-to-one in-depth interviews were conducted with some of the participants to analyze the detailed causes of inconvenient places and design factors. This study also suggested the procedure for solving the problems in various design factors from perspective of universal design, by considering prior studies which focused on a certain user group like the elderly. This procedure was suggested to consider as many user groups as possible at the same time. The results can be used to understand the discomforts of various user groups on residential housing. The procedure can also be used to develop universal design guidelines for the living residence.

Elderly normative study of the short form Korean-Boston Naming Test (한국판 보스톤 이름대기 검사의 단축형에 대한 노인 규준 연구)

  • An increase in senile dementia population has caused social concerns in Korea. The Korean-Boston Naming Test(K-BNT) has been used in assessing naming ability of patients with diffuse brain damage as well as senile dementia. This study was conducted to develop a short from of the K-BNT. to present normative data, and to demonstrate clinical utility of the shortform K-BNT. The participants were 142 healthy elderly adults, ages between 55 and 84, who were sampled in the Seoul-Kyungki area. Twenty patients with mild probable DAT participated in the study. We developed four norms based on education(0∼6 years and more than 6 years) and age(55∼64 and 65∼84 years). The study showed high internal consistency among the items. Further. DAT patients and normal controls showed significant difference in the K-BNT short form scores(F(1, 158) = 23,216, p < .0001). Normative data presented in this study will be useful in evaluating the naming ability of elderly patients in clinical setting.

The Effect of Compression Knee Band and Heat Treatment on Blood Velocity of the Elderly with Osteoarthritis (무릎밴드 가압과 가온요법이 퇴행성 무릎관절염 노인의 혈류속도에 미치는 효과)

  • The purpose of this study was to investigate the change in blood velocity(mm/sec) when compression and/or heat were applied to the knee joint for the elderly who has both normal and painful legs with osteoarthritis(OA). Experimental compression knee band was prepared from the 3D knee data of the average women in 60's. 3D replica of knee was reduced by 7, 10, and 13% from the nude pattern in course direction. Clothing pressure was measured at the front and back of each healthy and painful knee of elderly women for one minute while standing and sitting on the chair. Blood velocity was measured at 13 cm upper from the mid-patella for 15 minutes. Results are as follows: first, compression or heating treatment itself did not change blood velocity of both legs; second, combination treatment with heating and compression was effective to increase blood velocity. In details, for healthy legs, combination treatments with compression by 10% reduced pattern(about 1.3kPa) and heating($43^{\circ}C$) induced the maximal blood velocity, however, for knees with OA, 7% reduced pattern(about 1.0kPa) with simultaneous heating($43^{\circ}C$) was more effective than other cases. These results indicated that pain and spasticity of knee joint with OA could be reduced by applying heat and compression therapy, where the compression level of painful knee should be slightly lower than of healthy leg.

Apolipoprotein E Polymorphism and Cognitive Function Change of the Elderly in a Rural Area, Korea (일개 농촌지역 노인의 아포지단백 E 다형성과 인지기능 변화)

  • Objectives : The aim of this study is to examine the cognitive function change related to aging, the incidence of cognitive impairment, and the association between apolipoprotein E polymorphism and cognitive impairment through a follow-up of the elderly with normal cognitive ability at baseline. Methods : Two hundred and fifteen subjects aged 65 and over were surveyed in February, 1998 (baseline survey), and their cognitive function was assessed again in 2003 1st follow-up) and the once again in 2006 (2nd follow-up). Ninety one subjects completed all surveys up through the 2nd follow-up and their cognitive function scores using MMSE-K (Korean Version of the Mini-Mental State Examination) and the distribution of apolipoprotein E allele were analyzed. Results : The cognitive function scores decreased with aging and the difference between baseline and the 2nd follow-up scores of the study increased with the age group. The incidence rate of cognitive impairment through an 8-year follow-up was 38.5% and higher in older age groups. Age was the only significant factor for incidence of cognitive impairment, but there was no significant association between apolipoprotein E genotype and incidence of cognitive impairment. Conclusions : The cognition of the elderly decreased with aging and the association of apolipoprotein E genotype with incidence of cognitive impairment was not significant in this study. To confirm the association between apolipoprotein E polymorphism and incidence of cognitive impairment further studies will be needed.

Prevalence of Metabolic Syndrome and its Related Factors of Elderly People in Korea (우리나라 70세 이상 고령자의 연령대별 대사증후군의 분포 및 관련요인)

  • This study examined the distribution of metabolic syndrome according to the age groups among the elderly people to reveal the sociodemographic and health related factors. The survey in 2011~2014 from the National Health Insurance Corporation under regular medical check-ups, which received a recognition survey targeted 1,756 people aged over 70. Multiple logistic regression was performed on the relation metabolic syndrome and its related factors. As a result, the risk ratio for metabolic syndrome increased significantly in females than in males, living with a family than living alone, high economic status than in the low group, obese than in the normal weight group, have a history of stroke group than the no history group, smoking group than the non-smoking group, and drinking group than the non-drinking group. The distribution of metabolic syndrome differed significantly according to the sociodemographic characteristics and health-related variables.

Effects of Vitamin E Supplementation on Immune Response and Antioxidant Defense Parameters in Healthy Korean Elderly Women (노인에서 비타민 E 보충이 면역능력과 항산화상태에 미치는 영향)

  • This study investigated the effects of vitamin E supplementation on immune responses and antioxidant status in healthy Korean old and young women. Blood samples were obtained from 15 healthy old women (over 60 years old) and from 15 healthy young women(20 years old) before and 4 weeks after vitamin E( tocopherol acetate) supplementation(400IU/day). Daily nutrient intakes were calculated, and plasma vitamin E concentration, numbers and percentages of white blood cell and their subpopulation, percentages of lymphocytes and subpopulation, NK cell percentages, plasma immunoglobulin A, G, M and C3 concentration, proliferation of PMN with mitogen were measured. Also plasma TBARS concentration and radical scavenger activity of erythrocytes were investigated. Plasma vitamin E concentrations were significantly increased after supplementation in both groups. In elderly women, vitamin E supplementation restored the per centages of neutrophils, lymphocytes, and eosinophils which had been out of normal ranges before supple mentation. And after vitamin E supplementation, helper T cell percentages significantly increased in elderly. Plasma immunoglobulin and complement C3 concentrations were not affected by vitamin E supplementation in both groups. PMN proliferations with mitogen were significantly lower in old women than in young women, and there was no effect of vitamin E supplementation. Vitamin E supplementation significantly decreased plasma TBARS concentrations in old and young women. RSA of erythrocytes was increased in both groups, but the statistical significant was only found in young women group. Therefore, these results suggest that the moderate vitamin E supplementation in old women improves immune responses, especially nonspecific immunity and cell mediated immunity, via protection of oxidant stress.

Physiological Parameters Related to Health of The Elderly (노인의 건강관련 생리적 지수에 관한 연구)

  • The purpose of this study was to identify the health status of elderly. Subjects were 132 older people who live in home and institution located Taejon metropolitan city. Data were collected from May 1997 to April 1998. To obtain data about health status of elderly. pulse, respiration, systolic and diastolic blood pressure. grip strength. pinch pressure. flexibility, arm circumference. triceps skin fold thickness were measured. Data were analyzed for frequency. percentage. t-test using SPSS pc+ program. The results were as follows: 1. $74.4\%$ of subjects was perceived as 'good' in their health status. 2. Mean pulse, mean respiration, systolic and diastolic blood pressure were with in normal limits. There were no statistical differences between men and women in pulse, respiration, systolic and diastolic pressure. 3. Left and right grip strength were 24.89 psi and 25.23psi. The grip strength in men was higher than that of women. It showed statistically difference between men and women in grip strength. 4. Left and right pinch pressure were 7 pound and 7.32 pound. There was statistically difference of pinch pressure between men and women. 5. left flexibility was better than right flexibility of subjects. There was no statistically significant difference between men and women in flexibility. 6. Arm circumference was 24.96cm and there was no statistically significant difference between men and women. 7. Mean skin fold thickness was 12.83 em. Skin fold thickness in men was lower than that of women. It showed statistically difference between men and women. From these results, further study should be considered gender differences in health status of elders and carried in larger sample than this study.

The Impact of Choline Acetyltransferase Polymorphism on the Expression of Mild Cognitive Impairment (Choline Acetyltransferase 유전자 다형성이 경도인지손상 발현에 미치는 영향)

  • Objectives : The potential association between choline acetyltransferase(CHAT) polymorphism and the risk of mild cognitive impairment(MCI) has not been investigated in Korea. We examined the main effect of CHAT polymorphism and its interaction with apolipoprotein E(APOE) polymorphism in the development of MCI in elderly Korean sample. Methods : We analyzed CHAT 2384G > A polymorphism and APOE polymorphism among 149 MCI subjects with MCI and 298 normal controls. We tested the association between MCI and CHAT A allele status using a logistic regression model. In addition, we employed generalized multifactor dimensionality reduction(GMDR) to investigate the interaction between CHAT and APOE with regard to the risk of MCI. Results : The CHAT A allele was associated with AD risk(OR = 1.59, 95% CI = 1.02-2.48, p = 0.042). No significant gene-gene interaction between CHAT and APOE was found in GMDR method(testing balanced accuracy = 0.540, p = 0.055). Conclusion : The CHAT A allele was associated with MCI risk in the Korean elderly. Its interaction with the APOE ${\varepsilon}4$ allele was not significant with regard to the development of MCI.

Effectiveness Evaluation of Scanogram Using Longbone Detector (Longbone 검출기를 이용한 Scanogram의 유효성 평가)

  • Scanogram is that combine several practical images into one image to observation. So it is an important consideration in many clinical situation such as iliac measurement, leg alignment measurement and Scoliosis. Currently, scanogram examinations are mainly conducted for children and elderly patients. In this study, in order to apply the longbone detector to children or elderly patients who are difficult to cooperate with, we compared the longbone detector from D equipment with the G equipment discovery 656 Puls equipment in reproducibility of images, distribution of irradiation dose, scattering dose, irradiation time and image acquisition time. D equipment took more than twice as much time as G equipment. The scattered dose generated about 50% more G equipment than D equipment. In the whole spine scanogram and the measurement length of the lower leg, D equipment was also measured longer than G equipment. However, both methods did not show much difference from the CT scanogram, so there was no problem in measurement. The height of the thyroid radiation dose of G equipment was produced more radiation than D equipment. However, the longbone detector deviated from the x-ray center line relative to the tube rotation method, and was measured lower by the directionality of the measuring instrument, so that the error could not be corrected. In the conclusion of study, using the longbone detector is excellent for applying to children or elderly patients to reduce scattering dose. However, using CR may be useful to normal patients. Because, the image quality may deteriorate due to an imbalance of dose difference in thickness depending on the body part. So, it is useful to using a compensation filter or tube rotation method when we take a whole spine scanogram.

An Analysis of Somatotype of Elderly Women with Implications for Clothing Construction (노년층여성의 의복구성을 위한 체형분석 연구)

  • Physical measurements of 181 Korean elderly women, aged 60 through 79, living in the Seoul area, were taken in 1981 in order to analyze their body types and to study clothing construction according to their body types. The study was composed of 117 items in all; 35 items in measurements, 1 items in Rohrer index, 68 items in physical index to stature and bust girth, and 14 items in proportion, Coefficients of correlation among body dimensions were used. The results from this analysis are as follows: 1) The most significant changes in body dimensions in advanced age were the decrease of bust height and the increase of both abdomen girth and waist girth. 2) Distinct physical changes did not appear until advanced senescence(70~79 years of age). 3) In the three somatotype groups(thin, average, and plump) categorized by their Rohrer index, the dimensions that remained unchanged in senescence were back waist length and front waist length ; the dimensions that varied most significantly in senescence were the waist measurements. 4) As a result of the analysis of coefficients of correlation among the body dimensions of the 3 figure groups categorized by the Rohrer index, the fundamental dimensions for establishing the size of ready-to-wear garments of elderly women were the stature, bust girth and center back length in upper garments. 5) The result of the analysis of physical index and proportion of the 3 figure groups showed that the more plump a figure might be, the ratio of depth to width measurements increased; the back waist height was higher and trunk enlarged. 6) The 9 figure categorized by their Rohrer index and stature showed that the percentage of women in the category of“normal figure”decreased more in advanced senescence than during early senescence. 7) An analysis of variation of the 9 figure groups, the stature and other items for a short and thin figure were the smallest, and for a tall and plump figure, the largest.
