A novel de novo mosaic mutation in PHEX in a Korean patient with hypophosphatemic rickets

  • X-linked hypophosphatemic rickets is caused by loss-of-function mutations in PHEX, which encodes a phosphate-regulating endopeptidase homolog. We report a 26-year-old man with X-linked hypophosphatemic rickets who showed decreased serum phosphate accompanied by bilateral genu valgum and short stature. He had received medical treatment with vitamin D (alfacalcidol) and phosphate from the age of 3 to 20 years. He underwent surgery due to valgus deformity at the age of 14 and 15. Targeted gene panel sequencing for Mendelian genes identified a nonsense mutation in PHEX (c.589C>T; p.Gln197Ter) and a mosaic pattern where only 38% of sequence reads showed the variant allele. This mutation was not found in his mother, who had a normal phenotype. This is a case of a sporadic nonsense mutation in PHEX and up to date, this is the first case of a mosaic mutation in PHEX in Korea.

비증후군성 부분 무치증 환자에서 PAX9 유전자 돌연변이 (A Novel PAX9 Mutation in a Family with Non-Syndromic Oligodontia)

  • 본 연구는 서울대학교 치과병원 소아치과에 내원한 부분 무치증 환자를 대상으로 질환의 원인이 될 수 있는 돌연변이를 규명하고, 그 역할에 대하여 고찰하고자 하였다. 다수의 영구치 결손을 주소로 서울대학교 치과 병원에 내원 한 7세 여아와 어머니를 대상으로 구강검진 및 파노라마 방사선 촬영을 진행하였다. 연구 동의서를 받고, 유전자 검사를 위한 채혈을 시행하였다. PAX9 유전자의 모든 exon에 대해 특이적인 primer를 이용하여 중합효소 연쇄반응을 시행하였으며, 해당 산물을 정제하고 염기서열분석을 진행하였다. 결과는 NCBI Gene Bank와 대조하여 해당 영역의 돌연변이를 조사하였다. 7세 환아는 총 11개의 영구치 결손이 관찰되었으며, 어머니는 총 19개의 영구치 결손이 관찰되었다. 치아 결손 외에 손톱, 모발, 피부, 땀샘과 연관 된 다른 결함은 관찰되지 않았다. 유전자 분석 결과, PAX9의 exon 2 영역에서 nonsense mutation(c.184G>T, $p.Glu62^*$)을 확인하였으며, 대상자 모두에서 이형접합 돌연변이로 관찰되었다. 해당 돌연변이의 결과로 exon 2 내에서 전사 종결이 일어나게 되며, 발현 된 단백질은 정상 단백질에 비하여 280개의 아미노산이 짧은 상태로 발현된다고 추측할 수 있다. 결손된 부분은 paired box domain 및 DNA binding site 등 해당 단백질의 기능에 있어서 필수적인 영역을 포함하고 있으므로, 부분 무치증을 유발했을 가능성이 높다. 또는 해당 돌연변이의 전사체가 nonsense-mediated decay system(NMD system)에 의하여 분해됨으로써 haploinsufficiency를 유발하여 부분 무치증의 원인으로 작용했을 것으로 사료된다.

A compound heterozygous mutation in the FMO3 gene: the first pediatric case causes fish odor syndrome in Korea

  • Trimethylaminuria (TMAuria), known as "fish odor syndrome," is a congenital metabolic disorder characterized by an odor resembling that of rotting fish. This odor is caused by the secretion of trimethylamine (TMA) in the breath, sweat, and body secretions and the excretion of TMA along with urine. TMAuria is an autosomal recessive disorder caused by mutations in flavin-containing monooxygenase 3 (FMO3). Most TMAuria cases are caused by missense mutations, but nonsense mutations have also been reported in these cases. Here, we describe the identification of a novel FMO3 gene mutation in a patient with TMAuria and her family. A 3-year-old girl presented with a strong corporal odor after ingesting fish. Genomic DNA sequence analysis revealed that she had compound heterozygous FMO3 mutations; One mutation was the missense mutation p.Val158Ile in exon 3, and the other was a novel nonsense mutation, p.Ser364X, in exon 7 of the FMO3 gene. Familial genetic analyses showed that the p.Val158Ile mutation was derived from the same allele in the father, and the p.Ser364X mutation was derived from the mother. This is the first description of the p.Ser364X mutation, and the first report of a Korean patient with TMAuria caused by novel compound heterozygous mutations.

Linkage of the Kanamycin Resistance Gene with the Streptothricin Resistance Gene in Staphylococcus aureus SA2

  • The pKH2 isolated from the multidrug-resistant Staphylococcus aureus SA2 is a 40.98-kb plasmid and mediates resistance to ampicillin, clindamycin, erythromycin, kanamycin, and streptomycin. The 3.4-kb HindIII fragment conferring kanamycin resistance was cloned from the pKH2 into pBluescriptII $KS^+$ and partial sequence determination of that fragment was carried out. Sequence analysis revealed that the kanamycin resistance gene which encoded aminoglycoside 3'-phosphotransferase was linked to the streptothricin resistance gene. But a nonsense mutation was found in the streptothricin resistance gene and this mutation resulted in a truncated protein of streptothricin acetyltransferase. Homology comparison with nucleotide sequence databases revealed that the 3.4-kb HindIII fragment of pKH2 had been derived not from S. aureus but from Gram-negative Campylobacter coli.

When a ribosome encounters a premature termination codon

  • In mammalian cells, aberrant transcripts harboring a premature termination codon (PTC) can be generated by abnormal or inefficient biogenesis of mRNAs or by somatic mutation. Truncated polypeptides synthesized from these aberrant transcripts could be toxic to normal cellular functions. However, mammalian cells have evolved sophisticated mechanisms for monitoring the quality of mRNAs. The faulty transcripts harboring PTC are subject to nonsense-mediated mRNA decay (NMD), nonsense-mediated translational repression (NMTR), nonsense-associated alternative splicing (NAS), or nonsense-mediated transcriptional gene silencing (NMTGS). In this review, we briefly outline the molecular characteristics of each pathway and suggest mRNA quality control mechanisms as a means to regulate normal gene expression.

A case with GRIN2A mutation and its non-neurological manifestations

  • In epilepsy-aphasia spectrum (EAS) disorders, mutations in the glutamate receptor ionotropic N-methyl-D-aspartate type subunit 2A (GRIN2A) have become important for screening the disease. Research into the phenotypic variability of several types of neurologic impairment involving these mutations is in progress. However, the non-neurological problems related to these mutations are poorly understood. EAS disorders usually have epileptic, cognitive, or behavioral manifestations. In this case report, we present a female patient with epilepsy, delay in expressive language and social development, and intellectual disability with low intelligence quotient and memory quotient, but normal motor development. Through genetic analysis, she was found to have a missense and a nonsense mutation in GRIN2A (c.1770A>C; p.Lys509Asn and c.3187G>T; p.Glu1063∗, respectively) and we consider the nonsense mutation as 'pathogenic variant'. She was also discovered to have congenital hypothyroidism, growth hormone deficiency and Rathke's cleft cyst in the brain, which were previously unknown features of GRIN2A mutation. Our findings should widen understanding of the spectrum of GRIN2A phenotypes, and emphasize the need for more research into the association between GRIN2A mutations and non-neurologic clinical presentations.

A nonsense PAX6 mutation in a family with congenital aniridia

  • Congenital aniridia is a rare ocular malformation that presents with severe hypoplasia of the iris and various ocular manifestations. Most cases of congenital aniridia are known to be related to mutations in the paired box gene-6 (PAX6 ), which is an essential gene in eye development. Herein, we report a familial case of autosomal dominant congenital aniridia with four affected members in 3 consecutive generations and describe the detailed ophthalmologic findings for one of these members. As expected, mutational analysis revealed a nonsense mutation (p.Ser122*) in the PAX6 gene. Thus, our findings reiterate the importance of PAX6 mutations in congenital aniridia.

A Korean Girl with Campomelic Dysplasia caused by a Novel Nonsense Mutation within the SOX9 Gene

  • Campomelic dysplasia (CMD) is a rare, often lethal, genetic disorder characterized by multiple congenital anomalies and abnormal development of the reproductive organs in males. Mutations in the SOX9 gene are known to cause CMD. We present a Korean CMD girl with a normal 46,XX karyotype and a female reproductive organ phenotype. She was born at 2.35 kg at 38 weeks of gestation and showed characteristic phenotypes, including cleft palate, micrognathia, hypertelorism, flat nasal bridge, congenital bowing of limbs, hypoplastic scapulae, deformed pelvis, and 11 pairs of ribs. She also had an atrioseptal defect of the heart and marked laryngotracheomalacia requiring tracheostomy and tracheopexy. SOX9 mutation analysis revealed the presence of a novel nonsense mutation, $p.Gln369^*$, and the patient was genetically confirmed to have CMD. Although she showed marked failure to thrive and neurodevelopmental delay, she is now 40 months of age and is the only surviving patient with CMD in Korea.

한국인 백혈병 환자에서 아데노신 디아미나제 유전자의 새로운 변이의 확인 (Identification of Novel Mutations In Adenosine Deaminase Gene In Korean Leukemia Patients)

  • 백혈병은 조혈모세포의 비정상적인 증식에 의해 일어나서 질환이고, adenosine deaminase (ADA) 유전자는 백혈병의 약물 작용점으로 중요하다. 이러한 연구의 일환으로 한국인 백혈병 환자 20명의 ADA 유전자의 변이를 조사하기 위해 혈액 genomice DNA를 추출하여 염기서열을 결정하였다. 그 결과 nonsense 변이인 F101F 하나, missense 변이 E260K, D8Y 각각 하나, 그리고 외국에서는 보고되지 않은 것으로 정상인에서 IVS6-52 에 GC가 도입된 것을 확인하였다. 백혈병 환자와 유전자 변이간에 통계학적인 차이점은 없지만 이러한 연구는 앞으로 백혈병의 진단 마크 개발에 도움이 될 것으로 사료된다.

Hereditary spastic paraplegia with thin corpus callosum due to novel homozygous mutation in SPG11 gene

  • The most common form of autosomal recessive hereditary spastic paraplegia (HSP) is caused by mutations in SPG11/KIAA1840 gene, which encodes for spatacsin. The clinical presentation of SPG11 is characterized by cognitive impairment, peripheral neuropathy and a thin corpus callosum in brain magnetic resonance imaging. We identified a novel homozygous nonsense mutation (c.6082C>T [p.Q2028]) in exon 32 of SPG11 in Korean siblings. Our findings suggest that this novel homozygous mutation in SPG11 is associated with HSP and with dysgenesis of the corpus callosum.
