Objectives : There are many kinds of method to evaluate neural decompression during operation. They are direct visual and manual inspection, intraoperative ultrasound, endoscope, intraoperative computed tomography and intraoperative myelography. We used intraoperative myelography to evaluate the proper decompression of neural elements during the decompressive surgery. Methods : We injected 10-20cc of nonionic water-soluble contrast materials through direct puncture site of exposed dura during operation or lower lumbar level or lumbar drain inserted preoperatively. 12 patients were included in this study. They were 7 patients of centrally herniated lumbar disc disease, 1 patient of multiple lumbar spinal stenosis, 2 patients of thoracic extradural tumor and 2 cervical fracture & dislocations. Results : 5 of 12 patients showed remained neural compression through intraoperative myelography, so they were operated further through other approach. Myelographic dye is heavier than CSF, so the dependent side of subarachnoid space was visualized only. In one case, CSF leakage through hemovac was detected, but it was treated only bed rest for 5 days after hemovac removal. Conclusion :Intraoperative myelography is an effective method to evaluate neural decompression during spinal surgery. This technique is easy and familiar to us, neurosurgeons.
Tween 80 (Polysorbate 80) is a hydrophilic nonionic surfactant commonly used as an ingredient in dosing vehicles for pre-clinical in vivo studies (e.g., pharmacokinetic studies, etc.). Tween 80 increased apical to basolateral permeability of digoxin in Caco-2 cells suggesting that Tween 80 is an in vitro inhibitor of P-gp. The overall objective of the present study was to investigate whether an inhibition of P-gp by Tween 80 can potentially influence in vivo absorption of P-gp substrates by evaluating the effect of Tween 80 on the disposition of digoxin (a model P-gp substrate with minimum metabolism) after oral administration in rats. Rats were dosed orally with digoxin (0.2 mg/kg) formulated in ethanol (40%, v/v) and saline mixture with and without Tween 80 (1 or 10%, v/v). Digoxin oral AUC increased 30 and 61% when dosed in 1 % and 10% Tween 80, respectively, compared to control (P<0.05). To further examine whether the increase in digoxin AUC after oral administration of Tween 80 is due, in part, to a systemic inhibition of digoxin excretion in addition to an inhibition of P-gp in the GI tract, a separate group of rats received digoxin intravenously (0.2 mg/kg) and Tween 80 (10% v/v) orally. No significant changes in digoxin IV AUC was noted when Tween 80 was administered orally. In conclusion, Tween 80 significantly increased digoxin AUC and Cmax after oral administration, and the increased AUC is likely to be due to an inhibition of P-gp in the gut (i.e., improved absorption). Therefore, Tween 80 is likely to improve systemic exposure of P-gp substrates after oral administration. Comparing AUC after oral administration with and without Tween 80 may be a viable strategy in evaluating whether oral absorption of P-gp substrates is potentially limited by P-gp in the gut.
In this study, a reactive emulsifier with vinyl groups was synthesized by using 3-butenoic acid and polyoxyethylene(20) stearyl ether. The synthesized reactive emulsifier was confirmed by FT-IR and $^1H-NMR$. In addition, the reactive emulsifier synthesized in the preparation of aqueous acrylic pressure sensitive adhesives was used and the properties of the respective pressure sensitive adhesives were compared to those of using commonly used nonionic emulsifiers. The solid content was measured in the range of 56.8~57.4%. In the case of the initial adhesion, the S20BA made with a reactive emulsifier was measured as $^{\sharp}13$. Peel strengths of the prepared adhesives were measured in the range of $0.66{\sim}1.05kg_f$ and the highest peel strength was observed for S20BA. As a result of the heat resistance test, S20BA showed the highest as $840^{\circ}C$. In order to evaluate the applicability of adhesives for skin, the pH value was measured as 7, neutral and also it was found to be non-irritation from primary skin irritation test results.
Studies on the side effects of contrast agents are being discussed based on various cases, but studies analyzing the chemical structure of the underlying contrast agents are difficult to understand as the manufacturers have not disclosed them. Therefore, in this study, the chemical structure of the contrast medium was analyzed using 1H-NMR spectrometer for Omnipaque contrast medium prepared from Iohexol, which is a nonionic iodide contrast medium, Xenetix contrast medium from Iobitridol, and Iomeron contrast medium from Iomeprol. As a result, it was found that the Omnipaque contrast medium of Iohexol had 6 carboxyl groups, 3 carbonyl groups, 4 amine groups, 1 methyl group, and 2 cyano groups. It was found that the Xenetix contrast medium of the iobitridol formulation had 6 carboxyl groups, 3 carbonyl groups, 2 amine groups, and 4 cyano groups. It was found that the Iomeron contrast agent of the Iomeprol formulation had 5 carboxyl groups, 3 carbonyl groups, 4 amine groups, 1 methyl group, and 2 cyano groups. As shown in this study, the chemical structure of the non-ionic monomer contrast agent increases its affinity with water by binding a number of hydroxyl groups (OH) to the carboxyl group. It is necessary to accurately identify each of these factors and analyze the physical and chemical changes of the contrast medium according to various environmental factors.

Objective: To compare the incidence of aspiration pneumonia, nausea, and vomiting after intravascular administration of nonionic iodinated contrast media (ICM) between patients who fasted before contrast injection and those who did not. Materials and Methods: Ovid-MEDLINE and Embase databases were searched from their inception dates until September 2022 to identify original articles that met the following criteria: 1) randomized controlled trials or observational studies, 2) separate reports of the incidence of aspiration pneumonia, nausea, and vomiting after intravascular injection of non-ionic ICM, and 3) inclusion of patients undergoing radiological examinations without fasting. A bivariate beta-binomial model was used to compare the risk difference in adverse events between fasting and non-fasting groups. The I2 statistic was used to assess heterogeneity across the studies. Results: Ten studies, encompassing 308013 patients (non-fasting, 158442), were included in this meta-analysis. No cases of aspiration pneumonia were reported. The pooled incidence of nausea was 4.6% (95% confidence interval [CI]: 1.4%, 7.8%) in the fasting group and 4.6% (95% CI: 1.1%, 8.1%) in the non-fasting group. The pooled incidence of vomiting was 2.1% (95% CI: 0.0%, 4.2%) in the fasting group and 2.5% (95% CI: 0.7%, 4.2%) in the non-fasting group. The risk difference (incidence in the non-fasting group-incidence in the fasting group) in the incidence of nausea and vomiting was 0.0% (95% CI: -4.7%, 4.7%) and 0.4% (95% CI: -2.3%, 3.1%), respectively. Heterogeneity between the studies was low (I2 = 0%-13.5%). Conclusion: Lack of fasting before intravascular administration of non-ionic ICM for radiological examinations did not increase the risk of emetic complications significantly. This finding suggests that hospitals can relax fasting policies without compromising patient safety.
Binary system of water and polyoxyethylene dodecyl ether, $C_{12}H_{25}(OCH_2CH_2)nOH$, have been studied by $^1H$ NMR techniques. For n=5($C_{12}EO_5$) and n=8($C_{12}EO_8$), the self-diffusion coefficients of nonionic surfactants in the isotropic phase($L_1$) have been measured by using pulsed field gradient technique for a range of temperature and concentrations. In addition the line widths of the different proton signals have been monitored, and samples of some liquid crystalline characteristic were also studied. Dramatic Broadening of the methylene signals of the alkyl($C_{12}H_{25}$) chain is observed as the hexagonal liquid crystalline phase is approached in the $C_{12}EO_5-$water system, while only small broadening is observed in the $C_{12}EO_8-$water system. It was shown that there was a growth of $C_{12}EO_5$ micelles to rods with increasing concentrations, while the $C_{12}EO_8-$ micelles at low temperature remain small in the concentration range. The self-diffusion coefficients of the surfactants decrease rapidly with increasing concentration until a minimum is reached after which there is slow increase. The location of the minimum point occurs at lower concentrations the temperature is close to the cloud point, where the system separate into two isotropic phase. In the line width studies, broadening is found at a certain temperature interval when the concentration is increased in the $C_{12}EO_5$ system. The results indicate that the surfactant aggregates grow in size at the cloud point is approached. The aggregates seem to be flexible and probably not to be of a definite shape close to the cloud point. In the $C_{12}EO_8$ system, the micelles are much less affected by an increase in temperature and micellar growth can't be unambiguously established. The methylene signals of the ethylene oxide moieties consistantly show narrower $^1H$ signals, showing that in the aggregates they are less ordered than the chain methylenes. The various changes in aggregate size and shape are correlated with the stability ranges of the isotropic and liquid crystalline phases according to phase diagrams from the literature. Both aggregate size and phase structure are in qualitative agreement with concentration based on the effective shape of the molecules at different temperature and concentration.

In this study, emulsion polymerizations for synthesizing acrylic pressure-sensitive adhesive(PSA) were carried out using 2-ethylhexyl acrylate(2-EHA), n-butyl acrylate(n-BA), methyl metacrylate(MMA) as fundamental monomers and acrylic acid(AAc) as a functional monomer in the presence of anionic SLS (sodium lauryl sulfate). To obtain the optimized synthetic condition in the polymerization, we analyzed the polymerization variables such as the effect of surfactant concentration and hydrophilic lipophilic values(HLB). At the same time, the final adhesive properties were also analyzed by the function of the initiator concentration and buffer concentration. In the results, the most stable emulsion was obtained at the surfactant concentrations between 3 and 5 wt%. It was also determined the effect of HLB value of nonionic surfactant and the initiator concentrations on the gel content. Stable emulsion is obtained using the surfactant having HLB value of 12.3. The rate of emulsion polymerization was increased at the initiator concentration greater than 1 wt%, but the stability of the emulsion was decreased. Finally, the effect of the buffer concentrations on the pH and the conversion of the acrylic emulsion product were experimentally measured. At the sodium bicarbonate concentration above 0.4 wt%, the buffer infulence was apparent. The buffer effect was fully acceptable at the concentrations between 0.6 and 0.8 wt% regardless of the monomer composition.
The soil remediation by non-ionic surfactant solutions ($C_{12}H_{25}O(CH_2CH_2O)_5H$ and Triton X-100) was studied. Depending on the amounts and use of co-surfactants, MPT(phase inversion temperature), dynamic interfacial tension, and the detergency efficiency of the surfactant solutions in soil were investigated. The oils used were kerosene, n-hexadecane, and paraffin oil. With respect to a higher detergency efficiency, a lower interfacial tension and the MPT was very important. The $C_{12}H_{25}O(CH_2CH_2O)_5H$ was better than Triton X-100 on the oil removal from the soil and the effect of oil kinds was kerosene>paraffin $oil{\geq}n-hexadecane$. The co-surfactant, n-dodecanol, reduced the MPT compared to no addition of this, whereas it did not enhance the detergent efficiency.

For any conceivable desalination process using the gas hydrate formation, the kinetics has to be one of the most important parameters from the economic point of view. We thus were to improve the kinetics of the R-134a (also known as HFC-134a) gas hydrate formation by using promoters and three different kinds of edible surfactants were selected for the desalination process targeted to produce potable water; κ-carrageenan, lecithin, and polysorbate 80 among anionic, amphoteric, and nonionic surfactants, respectively. Then, the kinetics change of the R-134a hydrate formation was monitored by varying the surfactant concentration. Experimental results demonstrated that the rate of R-134a hydrate formation increases with the addition of edible surfactants in general and the effect as a promotor has an order of polysorbate 80 > κ-carrageenan > lecithin. As a supportive measure, the atomic charges of each surfactant were calculated by using a DFT (density functional theory)-based molecular modeling and the results showed a positive relationship between the promotor effect of each surfactant and the number of oxygens available for hydrogen bonding and the negativity of their atomic charge values.
