거골하 관절의 관절경술 (Subtalar Arthroscopy)

  • The development of good quality small-diametered arthroscopes and refined arthroscopic techniques has contributed to the improvement of the subtalar arthroscopy. The therapeutic indications are synovectomy, removal of loose bodies, debridement and drilling of osteochondritis dissecans, excision of subtalar impingement lesions and osteophytes, lysis of adhesions for post-traumatic arthrofibrosis, removal of a symptomatic os trigonum, calcaneal fracture assessment and reduction, and arthroscopic arthrodesis of the subtalar joint. The subtalar arthroscopy can be done in supine position using thigh holder or in lateral decubitus position. The arthroscope generally used is a 2.7-mm 30 degrees short arthroscope. Noninvasive distraction with a strap around the hindfoot can be helpful. Usually anterolateral, middle and posterolateral portals are utilized for inspection and instrumentation within the joint. Twoportal posterior subtalar arthroscopy in prone position can be performed as well with 4.0-mm 30 degrees arthroscope, depending on the type and location of the subtalar pathology. The subtalar arthroscopy is a technically demanding procedure, which requires proper instrumentation and careful operative technique. Possible complications are nerve damage and persistent wound drainage.

Real-time photoplethysmographic heart rate measurement using deep neural network filters

  • Photoplethysmography (PPG) is a noninvasive technique that can be used to conveniently measure heart rate (HR) and thus obtain relevant health-related information. However, developing an automated PPG system is difficult, because its waveforms are susceptible to motion artifacts and between-patient variation, making its interpretation difficult. We use deep neural network (DNN) filters to mimic the cognitive ability of a human expert who can distinguish the features of PPG altered by noise from various sources. Systolic (S), onset (O), and first derivative peaks (W) are recognized by three different DNN filters. In addition, the boundaries of uninformative regions caused by artifacts are identified by two different filters. The algorithm reliably derives the HR and presents recognition scores for the S, O, and W peaks and artifacts with only a 0.7-s delay. In the evaluation using data from 11 patients obtained from PhysioNet, the algorithm yields 8643 (86.12%) reliable HR measurements from a total of 10 036 heartbeats, including some with uninformative data resulting from arrhythmias and artifacts.

Review of Photoacoustic Imaging for Imaging-Guided Spinal Surgery

  • This review introduces the current technique of photoacoustic imaging as it is applied in imaging-guided surgery (IGS), which provides the surgeon with image visualization and analysis capabilities during surgery. Numerous imaging techniques have been developed to help surgeons perform complex operations more safely and quickly. Although surgeons typically use these kinds of images to visualize targets hidden by bone and other tissues, it is nonetheless more difficult to perform surgery with static reference images (e.g., computed tomography scans and magnetic resonance images) of internal structures. Photoacoustic imaging could enable real-time visualization of regions of interest during surgery. Several researchers have shown that photoacoustic imaging has potential for the noninvasive diagnosis of various types of tissues, including bone. Previous studies of the surgical application of photoacoustic imaging have focused on cancer surgery, but photoacoustic imaging has also recently attracted interest for spinal surgery, because it could be useful for avoiding pedicle breaches and for choosing an appropriate starting point before drilling or pedicle probe insertion. This review describes the current instruments and clinical applications of photoacoustic imaging. Its primary objective is to provide a comprehensive overview of photoacoustic IGS in spinal surgery.

Mass Spectrometry-based Hair Metabolomics for Biomarker Discovery

  • Metabolomics makes it possible to analyze the interrelationships between various signaling molecules based on the metabolic pathways involved by using high-resolution devices. This approach can also be used to obtain large-scale metabolic information to identify the relevant pathways for disease diagnosis and prognosis and search for potential biomarkers. In the fields of medicine and forensics, hair analysis is used to detect various metabolites in the body. Hair can be harvested readily in a noninvasive manner and is easier to transport and store than blood and urine. Another advantage from a forensic viewpoint is that hair reflects all the components of body fluids. In addition, because of the unique coating structure of hair, it can be used for measurements without changing or destroying its adsorbed components. In this review, the pretreatments for hair analysis, instrumental conditions and clinical applications are discussed. Especially, the clinical use of hair metabolomics in the diagnosis of various diseases and the limitations of the technique are described.

기계식 인공판막 상태 평가를 위한 컴퓨터 보조진단 시스템 (Computer Aided Diagnosis System for Evaluation of Mechanical Artificial Valve)

  • 임상적으로 의사가 청진기를 이용해 초기 혈전이 생긴 기계식 판막 음향신호의 변화를 구분하기는 쉽지 않다. 기계식 판막의 이상은 환자의 죽음을 의미하기 때문에 기계식 판막의 신뢰성과 초기 혈전 현상을 비관혈적으로 조기 진단하는 방법은 매우 중요하다. 이 논문은 컴퓨터 보조진단 시스템과 음향신호의 주파수 스펙트럼의 이동을 관찰하여 기계식 판막의 혈전 현상을 비관혈적으로 평가하는 것을 목적으로 한다. 혈전 모델은 상용화된 기계식 판막에 폴리우레세인과 실리콘을 이용하여 제작하였다. 판막의 표면에는 폴리우레세인을 코팅하고, 봉합링에는 실리콘을 코팅하였다. 봉합링의 주위에서 혈전이 발생하고, 20%, 40%, 60%로 자라나는 현상은 실리콘을 이용하여 제작하였다. 실험 시스템에서 판막의 음향 신호는 마이크로폰과 증폭기를 사용하여 측정하였고, 마이크로폰에는 주위잡음을 제거하기 위해 커플러를 장착하였다. 측정된 음향신호는 A/D 컨버터를 이용하여 샘플링하고, 스펙트럼을 분석하였다. 정상적인 판막과 혈전이 형성된 판막의 주파수 구분을 위해 인공신경망을 구성하였고, 연속적으로 판막의 운동 주기성을 확인하기 위하여 return map을 사용하였다. 생체 내 실험에서는 기계식 판막을 사용하는 순환장치를 장착한 동물과 기계식 판막을 치환 받은 지 1년 이내와 1년이 넘은 환자에게서 데이터를 채집하였다. 실험에서 얻은 데이터 스펙트럼은 두 가지 형태의 첨두치를 보였고, 이중에서 두 번째 첨두치는 혈전의 모델에 따라 변화를 보였다. 생체 내, 외 실험에서 얻은 데이터를 인공신경망에 적용한 결과 정상 판막과 혈전이 생성된 판막을 구분하였고, 환자를 대상으로 한 실험에서는 10명 중 1명이 두 번째 첨두치가 이동하는 결과를 보였지만 다른 방법으로 확인하지는 못했다. 본 논문의 결과는 기계식 판막의 혈전현상을 비침습적으로 조기 진단하고, 상태를 지속적으로 감시할 수 있는 기술적 토대를 제공할 것이다.

Assessment of Malignancy in Brain Tumors by 3T MR Spectroscopy

  • Purpose: To assess clinical proton MR spectroscopy (MRS) as a noninvasive method for evaluating tumor malignancy at 3T high field system. Methods: Using 3T MRI/MRS system, localized water-suppressed single-voxel technique in patients with brain tumors was employed to evaluate spectra with peaks of N-acetyl aspartate (NAA), choline-containing compounds (Cho), creatine/phosphocreatine (Cr) and lactate. On the basis of Cr, these peak areas were quantificated as a relative ratio. Results: The variation of metabolites measurements of the designated region in 10 normal volunteers was less than 10%. Normal ranges of NAA/Cr and Cho/Cr ratios were 1.67$\pm$018 and 1.16:1:0.15, respectively. NAA/Cr ratio of all tumor tissues was significantly lower than that of the normal tissues (P=0.005). Cho/Cr ratio of high-grade gliomas was significantly higher than that of low-grade gliomas (P= 0.001), Except 4 menigiomas, lactate signal was observed in all tumor cases. Conclusions: The present study demonstrated that the neuronal degradation or loss was observed in all tumor tissues. Higher grade of brain tumors was correlated with higher Cho/Cr ratio, indicating a significant dependence of Cho levels on malignancy of gliomas. This results suggest that clinical proton MR spectroscopy could be useful to predict tumor malignancy. Environmental tobacco smoke and children's health

  • Passive exposure to tobacco smoke significantly contributes to morbidity and mortality in children. Children, in particular, seem to be the most susceptible population to the harmful effects of environmental tobacco smoke (ETS). Paternal smoking inside the home leads to significant maternal and fetal exposure to ETS and may subsequently affect fetal health. ETS has been associated with adverse effects on pediatric health, including preterm birth, intrauterine growth retardation, perinatal mortality, respiratory illness, neurobehavioral problems, and decreased performance in school. A valid estimation of the risks associated with tobacco exposure depends on accurate measurement. Nicotine and its major metabolite, cotinine, are commonly used as smoking biomarkers, and their levels can be determined in various biological specimens such as blood, saliva, and urine. Recently, hair analysis was found to be a convenient, noninvasive technique for detecting the presence of nicotine exposure. Because nicotine/cotinine accumulates in hair during hair growth, it is a unique measure of longterm, cumulative exposure to tobacco smoke. Although smoking ban policies result in considerable reductions in ETS exposure, children are still exposed significantly to tobacco smoke not only in their homes but also in schools, restaurants, child-care settings, cars, buses, and other public places. Therefore, more effective strategies and public policies to protect preschool children from ETS should be consolidated.

New Diagnostic Tool for Far Lateral Lumbar Disc Herniation : The Clinical Usefulness of 3-Tesla Magnetic Resonance Myelography Comparing with the Discography CT

  • Objective : To prospectively assess the diagnostic and clinical value of a new technique (3-tesla magnetic resonance myelography, 3T MRM) as compared to computed tomographic discography (disco-CT) in patients with far lateral disc herniation. Methods : We evaluated 3T MRM and disco-CT of 25 patients, whom we suspected of suffering from far lateral disc herniation. Using an assessment scale, 4 observers examined independently both 3T MRM and disco-CT images. We analyzed observer agreement and the accentuation of each image. Results : We found complete matching, and observer agreement, between high resolution images of 3T MRM and disco-CT for diagnosing far lateral disc herniation. Conclusion : We think noninvasive 3T MRM is an appropriate diagnostic tool for far lateral disc herniation as compared to disco-CT.

Review of the chemistry of first-generation Tau PET tracers

  • Alzheimer's disease (AD) is one of the challenging conditions that have no cure, yet early diagnosis can help to control the disease. PET imaging of tau has several advantages, such as being a noninvasive, safe diagnostic technique that correlates directly with the disease progression. Many tau tracers have been reported to date; however, the chemical scaffolds of them fall in a narrow chemical window, and none was approved yet as none is entirely selective and sensitive to tau. These problems are being solved as new tracers emerge constantly. In this report, the first-generation tau tracers such as [11C]PBB3, 2-arylquinoline (THK) series, [18F]T808, and [18F]AV-1451 ([18F]T807) are reviewed from an organic and radiochemistry perspective; thus the most effective chemical approach to synthesize these tracers is discussed. This would help to design novel tracers which can meet the challenges faced by the current tracers.

고려수지기맥 레이저 자극의 압통 역치상승 효과 (Increasing Effect of Laser Stimulation to Koryo-Hand Acupuncture Points on Experimental Pressure Threshold)

  • This study was conducted to examine the increasing effects of Ga-As-Al laser Koryo-hand acupuncture on experimental pressure threshold. Forty healthy subjects (female=20. male=20) aged 21 to 30 years were randomly assigned to two treatment groups with same ratio in sex. The subjects in the experimental group (n=20) received Ga-As-Al laser stimulation, and those in the control group (n=20) received sham stimulation on appropriate Koryo-hand acupuncture points M10 on the left hand which is reflex point of upper trapezius portion. Experimental pressure threshold at the contralateral upper trapezius was determined with a pressure algometer and Galvanic Skin Response (GSR) before and after treatment. The change of pressure threshold between pretreatment and posttreatment in the experimental group was greater than that in the control group (p<0.05). The result indicates that Ga-As-Al laser Koryo-hand acupuncture increases experimental pressure threshold and suggests that it is an effective noninvasive pain management technique.
