15세 여아에서 발생한 췌장의 비기능성 신경내분비종양 - 1예 보고 - (Nonfunctioning Neuroendocrine Tumor of the Pancreas in a 15-year-old Girl - a Case Report -)

  • We report a case of nonfunctioning neuroendocrine tumor of the pancreas in a 15-year-old girl who presented with back pain. On physical examination, there was mild tenderness in the left upper quadrant of the abdomen. The patient had no pancreatic hormone-associated symptoms. An abdominal ultrasonography showed a well-demarcated hypervascular solid mass with calcification in the tail of the pancreas. An abdominal computed tomography scan showed a $6{\times}5cm$ sized well-encapsulated enhancing solid mass with cystic component in the tail of the pancreas. Distal pancreatectomy was performed. Pathology revealed awelldifferentiated nonfunctioning low grade malignant neuroendocrine tumor of the pancreas. The postoperative course was uneventful.

종격동 부신경절종;1례 보고 (Nonfunctioning Paraganglioma of the Anterior Mediastinum - A case eport -)

  • A case of nonfunctioning paraganglioma of the anterior mediastinum in a 64-year-old woman is presented. The tumor was adherent to the pericardium and parietal pleura but not to the phrenic nerve. Complete excision of the tumor is the treatment of choice, but partial excision may provide long-term survival. A new classification and terminology suggested by Glenner and Grimley is emphasized. This aorticopulmonary paraganglioma is the first case in the Korea.

Comparison of Surgical Outcomes of Laparoscopic Adrenalectomy Between Patients With Nonfunctioning Adrenal Adenomas and Subclinical Cushing Syndrome: A Single-Center Experience

  • Purpose: To examine and compare the perioperative outcomes of laparoscopic adrenalectomy (LA) and to determine its efficacy in patients with nonfunctioning adrenal adenomas (NFA) and subclinical Cushing syndrome (SCS). Materials and Methods: We retrospectively analyzed the clinical data obtained from 79 consecutive patients who underwent LA for the treatment of either NFA (n=35) or SCS (n=44) between 2011 and 2016. All patients had undergone computed tomography, as well as endocrinological tests to confirm the diagnosis prior to the adrenalectomy. The primary endpoint was improved metabolic parameters relating to diabetes, hypertension, dyslipidemia, and obesity. Results: Patients with SCS compared to those with NFA showed a higher occurrence of diabetes (29.5% vs. 11.4%), hypertension (59.1% vs. 34.3%), and dyslipidemia (43.2% vs. 14.3%). Patients with SCS showed a smaller median tumor size compared to those with NFA (2.5 cm vs. 5 cm). No significant perioperative complications ${\geq}$ Clavien-Dindo classification grade III were observed in any patient (SCS or NFA group). In terms of their metabolic profile, patients with SCS showed a significant postoperative improvement in hypertension (50.0%), diabetes (53.9%), dyslipidemia (31.6%), and obesity (29.2%). However, patients with NFA showed a postoperative improvement only in dyslipidemia (40.0%) and obesity (4.8%). Conclusions: Owing to absence of significant perioperative complications and the marked postoperative improvement in metabolic impairment, LA is a useful treatment strategy in patients diagnosed with SCS. In contrast, LA was not observed to show beneficial effects in correcting/improving the metabolic profile in patients presenting with NFA.

  • The pulmonary sequestration is an uncommon congenital anomaly characterized by the presence of nonfunctioning lung tissue supplied by aberrant artery from the aorta or its branches and usually has no communication with the normal bronchial tree. The patient was 35 year old female and had no specific complaints. The lung mass was found incidentally and was continned to be intralobar pulmonary sequestration by aotography whitch showed aberrant blood supply from thoracic aorta at the T. vertebra level. The right lower lobectomy was done.

  • Parenteral nutrition (PN) is effective and relatively safe method providing nutrients to patients with a nonfunctioning or insufficiently functioning gastrointestinal tract via peripheral or central vein. In last several decades, useful steps have been taken in the understanding of nutritional needs, physiological changes and complications of intravenously fed patients. PN includes amino acids, glucose, lipids, electrolytes, vitamins, iron, minerals and trace elements, and must be based on individual circumstances such as patient's age, health status and disease. The purpose of this review is to introduce overall components and recent updates of parenteral nutrition.

비정상 위치 및 이중혈액공급을 받는 외엽형 폐격리증 -1례 보고- (Extralobar Pulmonary Sequestration of Unusual Location and Dual Blood Supply -A Case of Report-)

  • Pulmonary sequestration is an unusual congenital malformation characterized by the presence of nonfunctioning lung tissue which usually has no communication with the normal bronchial tree and receives its blood supply from an anomalous systemic artery. Extralobar form is a very rare congenital malformation. We have experienced a 54 year old female patient with a mass in the upper lobe complaining of cough and blood tinged sputum. A triangular shaped mass was located in the left upper lobe, medially. The arterial blood supply were from the thoracic aorta and the pulmonary artery but there was no the tracheobronchial communication. The venous drainage was through the pulmonary vein. The mass was confirmed as extralobar pulmonary sequestration associated with a pericardial defect.

폐격리증에 대한 임상적 고찰 (Clinical study of Pulmonary Sequestration)

  • Pulmonary sequestration occurs when some disturbance produces a cystic mass of nonfunctioning lung tissue which lacks normal communication with the tracheobronchial tree. Between 1971 and 1985, pulmonary sequestration was diagnosed in 11 patients, ranging age from 3 to 29 years. All sequestration were intralobar type. Definitive diagnosis can only be obtained by aortography and/or surgical exploration in 10 cases. The other one was confirmed by pathologic examination postoperatively. The presenting complaints were mostly recurrent local pulmonary infection, but in 2 cases mediastinal mass with respiratory symptoms was presented, and cardiac murmur was only finding in one case. Preoperative diagnostic procedure revealed 3 associated anomalies which were funnel chest, right aortic arch, and pulmonic stenosis with vascular ring. Operative treatment for sequestration was lobectomy in 10 cases, and a segmentectomy in one. There was no operative mortality, but 3 complications [empyema, B-P fistula, post-op bleeding] which were controlled by subsequent operations or conservative measure. Aortography is strongly advocated not only for its diagnostic value, but for its preoperative localization of the aberrant vessels that are the major concern to the surgeon.

폐엽내형 폐격리증 수술치험 1례 (Surgical Resection of Intralobal Pulmonary Sequestration - A Case Report -)

  • The pulmonary sequestration is rare congenital pulmonary disease with nonfunctioning lung tissue supplied by aberrant arteries arising from systemic arteries-thoracic aorta, subclavian artery, innominate artery, internal mammary artery, etc. In our country, only 23 cases were reported previously and the majority was intralobar pulmonary sequestration except 2 cases. The patient was 17 year-old man and admitted due to intermittent cough, productive sputum and fever for 8 years. On simple chest P \ulcornerA view, multiple cysts with air-fluid levels were located at left lower lobe area. Aortogram revealed two aberrant arteries arising from thoracic aorta just above the diaphragm. On the operative field, the arteries were 0.7 and 0.3 cm in diameter. Left lower lobectomy was done with ligation of aberrant arteries. The patient was recovered and discharged uneventfully.

  • Pulmonary sequestration Is an unusual congenital malformation characterized by the presence of nonfunctioning lung tissue which usually has no communication with the normal bronchial tree and which receives its blood supply from an anomalous systemic artery instead of a pulmonary arterial branch. Two forms of pulmonary sequestration occur: Intralobar sequestration, in which the abnormal pulmonary tissue is incorporated within the normal lung and shares a common covering of visceral pleura, and extralobar sequestration, in which the abnormal pulmonary tissue is separated from the normal lung and has its own pleural investment. Since 1970, five cases of pulmonary sequestration were operated at the Department of Thoracic and Cardiovascular Surgery, College of Medicine, Seoul National University. 1. Among five cues, one was male and the others were female. 2. All were intralobar type and involved left lower lobe. 3. There was no operative mortality following left lower lobectomy and ligation of the aberrant artery.
