임신부에서 측두하악장애의 진단과 치료 (Diagnosis and Treatment of Temporomandibular Disorder in Pregnant Women)

  • In this case report, I discussed the diagnosis and treatment of two pregnant women with temporomandibular disorders(TMD) who visited the Department of Oral Medicine, PNUH. Also, I reviewed some investigations of diagnosis and treatment of TMD in pregnant women. The obtained results were as follows; 1. No single X-ray diagnostic procedure for TMD results in radiation dose that threatens the well-being of the developing embryo and fetus. 2. Most non-steroidal anti-inflammatory drugs(NSAIDs) have commonly used because these drugs are considered to be nonteratogenic, but these agents are not recommended for routine use after 3rd trimester. 3. Electro-acupuncture stimulation therapy(EAST) is contraindicated for 1st trimester, and ultrasonic deep heat therapy, microwave deep heat therapy, low level laser therapy, myo-monitor are not contraindicated for pregnant women but clinician must consider some risk of adverse fetal effects. 4. The occlusal stabilization splint may be used for pregnant women, if it is fabricated indirectly. 5. Surgical treatment is contraindicated for pregnant women.

Clinical Characteristics of Trauma-Related Chronic Osteomyelitis in 3 Wild Raccoon Dogs (Nyctereutes procyonoides)

  • Osteomyelitis typically occurs because of the direct inoculation of bacteria or fungi after penetrating trauma or surgical contamination or, by extension, from soft tissue infection. Osteomyelitis is rarely reported in wildlife animals, though severe chronic osteomyelitis cases do exist in wildlife owing to the scarcity of medical support in the wild environment. This report describes three cases of chronic osteomyelitis in wild raccoon dogs related to trauma. The typical symptoms of three reported cases were ataxia, stiffness, muscle atrophy, and lethargy. All three cases were relevant to traumatic or severe external injury, and skin infestation caused by ectoparasites was apparent on an ocular inspection. In the radiographic examination, diffuse sites of osteolytic lesions and remarkable periosteal responses were demonstrated around the injured limb in all three cases. Apparent neutrophilia with a left shift, lymphocytosis, and monocytosis in hematological examinations generally indicated chronic infection as shown in case 1 and 3. Treatment was attempted with broad-spectrum antibiotics and non-steroidal anti-inflammatory drugs, such as amoxicillin/clavulanic acid, enrofloxacin, clindamycin, and meloxicam. These treatment options helped improve the overall prognosis of chronic osteomyelitis, but the outcomes did not meet the treatment goal entirely. Osteomyelitis can be extremely challenging to treat, particularly in wild animals, because of their distinctive traits, such as masking phenomenon and uncontrolled exposure to ectoparasites. Earlier diagnosis with a radiographic examination, hematological examinations, and careful patient monitoring, followed by prolonged antibiotic therapy and restricted exercise, are the key factors leading to a better prognosis.

안전상비의약품 판매 이후 중독환자 특성 변화 (Changes in Toxicological Characteristics after Sales of Nonprescription Drugs in Convenience Stores)

  • Purpose: On November 15, 2012, sales of OTC (Over-The-Counter) drugs began at convenience stores, which changed the accessibility of some drugs. As a result, the exposure and access patterns of these drugs could have changed. In this study, we reviewed the changes in the characteristics of drug poisoning patients because of the reposition of nonprescription drugs according to the revised Pharmaceutical Affairs Act. Methods: A retrospective study was conducted to evaluate changes in characteristics of drug poisoning patients between 2008 and 2016. A registry was developed by an emergency medical center in a local tertiary teaching hospital, and patients who visited the center were enrolled in this registry. We compared two periods, from 2008 to 2012 (Pre OTC) and from 2013 to 2016 (Post OTC), for type of intoxicant, time from poisoning to visiting the emergency center, intention, psychiatric history, previous suicidal attempt, alcohol status, and emergency room outcomes. The primary outcome was the number of patients who took acetaminophen and NSAIDs (nonsteroidal anti-inflammatory drugs). Secondary outcomes were ICU admission rate, mortality rate, and number of patients who visited the ER when the pharmacy was closed after taking acetaminophen and NSAIDs (nonsteroidal anti-inflammatory drugs). Results: Among 1,564 patients, 945 and 619 patients visited the emergency room during pre and post OTC periods. The number of patients with acetaminophen and NSAIDs poisoning decreased from 9.2% to 6.1% (p=0.016). The ICU admission rate and mortality rate in the emergency room did not show significant results in the relevant patient groups, and so was the number of patients visiting ER when the pharmacy was closed taking acetaminophen and NSAIDs. Conclusion: Despite the sales of nonprescription drugs at convenience stores, the number of acetaminophen and NSAIDs poisoning patients decreased.

An Analysis on the Effect of Patient-controlled Analgesia Performed by Orthopaedic Department or Postoperative Pain Control after Shoulder and Elbow Surgery

  • Background: We investigated the effectiveness of pain management and the adverse events of intravenous (IV) patient-controlled analgesia (PCA) after orthopedic surgery. Methods: From September 2014 and August 2015, we performed a retrospective analysis of 77 patients who underwent orthopedic surgery of the shoulder or the elbow in our hospital. The composition of the intravenous PCA administered to the patients was as follows: 250 mg of dexketoprofen trametamol, 70 mg of oxycodone, and 0.6 mg of ramosetron, which were made up to 79 ml of normal saline. We evaluated and statistically analyzed the difference in the visual analogue scale (VAS) scores for pain at immediate postoperation, at 24 hours of PCA, at 72 hours of PCA, and after discontinuation of PCA and in the incidence of adverse events. Results: We found that VAS score decreased for 3 postoperative days and that with discontinuation of IV PCA a meaningful change in VAS score was no longer seen. Of the 77 patients, 22 presented with adverse events (28.6%). We terminated IV PCA temporarily in the 21 patients who presented with adverse events; we terminated analgesia permanently in one patient (1.2%). Consequently, 76 of 77 patients carried out IV PCA till the designated period. Conclusions: Intravenous PCA after orthopedic surgery of the shoulder or the elbow may be accompanied with adverse events. Careful assessment of the patient and treatment of the adverse outcomes are key to a successful maintenance of PCA and to a successful management of postoperative pain.

Preparation of a New Chiral Stationary Phase Based on (2S,3S)-O,O'-Bis-(10-undecenoyl)-N,N'-bis-(3,5-dinitrobenzoyl)-2,3-diamino-1,4-butandiol and Its Application

  • A new liquid chromatographic chiral stationary phase based on (2S,3S)-O,O'-bis-(10-undecenoyl)-N,N'-bis-(3,5-dinitrobenzoyl)-2,3-diamino-1,4-butandiol was prepared starting from (2R,3R)-1,4-bis(benzyloxy)-2,3-butanediol. The new chiral stationary phase was applied to the resolution of racemic anilide derivatives of N-acetyl-a-amino acids, 1,1'-bi-2-naphthol and 3,3'-diaryl-1,1'-bi-2-naphthols. The CSP was also applied to the resolution of some chiral drugs including a diuretic, bendroflumethiazide, and non-steroidal anti-inflammatory agents such naproxen and alminoprofen. In every case, the chiral recognition efficiency of the new CSP was quite excellent.

안와부 자가지방이식술 후 시력 저하에 대한 증례보고 (Visual Disturbance following Autologous Fat Injection into Periorbital Area)

  • Purpose: Autologous fat injection into the facial area is a frequently used technique in aesthetic plastic surgery for augmentation of the soft tissue. Fat injection is a very safe procedure because of the advantage of being autologous tissue. Minimal foreign body reaction or infections are noted after fat injection. However, there may be some complications including those as severe as blindness. There have been some case reports on visual disturbances after autologous fat injection reported in the literature. Methods: A 21-year-old female patient underwent autologous fat injection into left eyebrow area to correct depression of soft tissue. Immediately after injection of autologous fat, she complained sudden visual loss on the left eye. She had come to our emergency room and ophthalmologic evaluation showed that the patient could only recognize hand motion. There was no abnormality of the optic nerve on magnetic resonance imaging. Suspecting an ischemic optic neuritis from fat embolism of the central retinal artery, the patient was treated conservatively with occular massage, antiglaucomatic agent, anti-inflammatory drugs and antibiotics. Visual field examination showed visual defect of half the lower hemisphere. Results: While maintaining antiglaucomatic agents and non steroidal anti inflammatory drugs, fundoscopic examination showed no abnormalities on the second day of admission. Visual field examination showed an improvement on the fourth day along with decreased eyeball pain. Significant improvement of vision was noted and the patient was discharged on the fifth day of admission. The patient was followed-up 2 days afterwards with improved vision and visual field defect. Conclusion: We describe an unusual case of sudden unilateral visual disturbance following autologous fat injection into periorbital area.

NCI-H157 폐암 세포주에서 Caspase Cascade 활성을 통한 Arsenic Trioxide와 Sulindac 병합요법의 세포고사효과 (Inducing Apoptosis of NCI-H157 Human Lung Carcinoma Cells via Activation of Caspase Cascade by Combination Treatment with Arsenic Trioxide and Sulindac)

  • 연구배경 : Arsenic trioxide($As_2O_3$)은 재발성 또는 불응성 급성전골수성백혈병의 치료제로 쓰이는 항암제로서 비소세포폐암을 포함한 다른 암세포주에도 효과가 있는 것으로 되어있다. NSAIDs는 항암 예방약제로 사용되고 있고, 세포고사를 통해 다른 항암제나 방사선치료의 반응성을 강화시키는 것으로 알려져 있다. 저자들은 NCI-H157 세포주에서 $As_2O_3$와 sulindac의 병합치료가 그것들의 세포고사를 배가시키는지 여부를 알아보고자 하였다. 방 법 : 세포 독성은 MTT 방법으로 측정하였고, 세포고사를 알아보기 위해 핵산 염색과 유식세포 분석을 시행하였다. 세포고사의 기전을 보기 위해 caspasefamily의 활성을 보았고, PARP와 ICAD의 분절을 western blotting으로 확인하였다. 또한 Fas와 Fas-L의 발현유무를 western blotting을 통해 관찰하였다. 결 과 : NCI-H157 폐암세포에 $As_2O_3$와 sulindac을 병합치료시 단독치료군에 비해 생존율이 의미 있게 감소하였고, 이러한 세포사는 핵산염색을 통한 염색사의 응축과 핵 분절 유도와 유식세포 분석에 의한 $sub-G_0/G_1$ DNA분획의 증가현상을 통해 세포고사에 의해 매개됨을 알 수 있었다. 세포고사의 유도에는 caspase 3, 8, 9를 통한 활성화와 이에 의한 PARP와 ICAD의 절단을 확인하였다. 또한 caspase-8 protease의 활성화에는 Fas와 Fas/L 단백질의 발현증가가 유도되었음을 알 수 있었다. 결 론 :NCI-H157 폐암세포주에 $As_2O_3$와 sulindac의 병합요법은 Fas/FasL 신호전달계의 활성화와 caspase 단백질 활성화 의해 세포고사가 유도되었다.
