This Pilot study identifies potential dioxin and furan sources and medical plastic wastes produced from hospital solid waste streams. In this study, air emissions of dioxins and furans from sources in the U.S., which were estimated by the U.S. Environmental Protection Agency (EPA), were summarized. Potential loading routes of dioxins and furans to the U.S.-Canada Great Lakes region have also been identified in trcent surveys. In addition, medical and hospital solid wastes produced in typical large city hospitals were characterized as important potential sources of dioxins and furans. Plastic contents in medical Plastics Characterization Survey (MMPCS), plastics composition data were obtained from a survey of five Massachusetts Hospitals and Medical Centers. By identifying plastic wastes as a percentage of total hospital wastes, we were able to use data from a preliminary study that analyzed the waste stream of 16 major New York City hospitals (NYCMWS) characterizing the plastic content of the wasters. This study determined the types of plastic wastes included in each medical waste stream (regulated medical waste or non regulated medical waste) and it discussed the potential for recyclibility of hospital plastic wastes. The combination of the NYCMWS and the MMPCS surveys provides for the first overview of the size of the recycling task of hospital plastic wastes and the potential of dioxin elimination if dioxin generating plastics were to be eliminated from hospital use.

The development of technology causes a lot of change. Many researchers have insisted that the proliferation of cyber space changes the physical space. Their insistences have been accumulated into three aspects. Firstly, the proliferation of cyber space brings out the concentration in the physical space, secondly the decentralization and lastly both at the same time. In Korea, the concentration of plastic surgery clinics has taken place in Kangnam-gu area at similar period of the Internet proliferation. In this research, we execute empirical study of whether the concentration of plastic surgery in specific areas correlates with the proliferation of cyber space or not. In order to do this, we verified homogeneity of plastic surgery websites between Kangnam-gu and Non-Kangnam-gu areas. Also, we used three statistical and data-mining techniques which are Multi-discriminant analysis, Decision tree analysis and artificial neural network analysis. As a result, there was homogeneity between two different area plastic surgery clinics websites, but there was not big heterogeneity as well. Therefore, in this case of concentration of plastic surgery in Korea, the proliferation of cyber space restrictively correlates with the concentration of physical space.
Concrete is a heterogeneous material containing many weaknesses such as micro-cracks, pores and grain boundaries. The crack growth mechanism and failure behavior of concrete structures depend on the plastic deformation created by these weaknesses. In this article the non-linear finite element method is used to analyze the effect of presence of micro pore near a crack tip on both of the characteristics of crack tip plastic zone (its shape and size) and crack growth properties (such as crack growth length and crack initiation angle) under pure shear loading. The FE Code Franc2D/L is used to carry out these objectives. The effects of the crack-pore configurations and the spacing between micro pore and pre-excising crack tip on the characteristics of crack tip plastic zone and crack growth properties is highlighted. Based on the obtained results, the relative distance between the crack tip and the micro pore affects in very significant way the shape and the size of the crack tip plastic zone. Furthermore, crack growth length and crack initiation angle are mostly influenced by size and shape of plastic zone ahead of crack tip. Also the effects of pore decrease on the crack tip by variation of pore situation from linear to perpendicular configuration. The critical position for a micro pore is in front of the crack tip.

Plastic surgery training worldwide has seen a thorough restructuring over the past decade, with the introduction of formal training curricula and work-based assessment tools. Part of this process has been the introduction of revalidation and a greater use of simulation in training delivery. Simulation is an increasingly important tool for educators because it provides a way to reduce risks to both trainees and patients, whilst facilitating improved technical proficiency. Current microsurgery training interventions are often predicated on theories of skill acquisition and development that follow a 'practice makes perfect' model. Given the changing landscape of surgical training and advances in educational theories related to skill development, research is needed to assess the potential benefits of alternative models, particularly cross-training, a model now widely used in non-medical areas with significant benefits. Furthermore, with the proliferation of microsurgery training interventions and therefore diversity in length, cost, content and models used, appropriate standardisation will be an important factor to ensure that courses deliver consistent and effective training that achieves appropriate levels of competency. Key research requirements should be gathered and used in directing further research in these areas to achieve on-going improvement of microsurgery training.

Osmidrosis axillae is a distressing problem characterized by foul odor at the axillae due to excessive apocrine sweat gland secretion. We introduce a new non-surgical method using a specially insulated needle and high frequency electrocoagulator. From September 2001 to January 2005, 52 patients were treated with this procedure for osmidrosis axillae. In authors' procedure, insulated needle which was designed to protect skin and dermal layer was inserted into the deep dermis of axilla area and apocrine sweat glands were removed by electrocoagulation. Fifty two patients were evaluated more than 6 months after surgery. Among these patients, 48 patients received secondary coagulation procedure. After second operation, all patients were satisfied with postoperation results. We conclude that our method has several advantages such as 1) short operation time, 2) no necessity of postoperative immobilization of shoulder joints, 3) no need of hospitalization, 4) minimal scarring and no bleeding, 5) prevention of skin necrosis, 6) a safer operative method for recurred cases as a secondary method.
This study performed a precise dynamic finite element time history elastic-plastic seismic analysis considering the welds, which have been not considered in design stage, on the nuclear components subjected to severe seismic loadings such as beyond-design basis earthquakes for sustainable nuclear power plants. First, the dynamic finite element elastic-plastic seismic analysis was performed for a general design practice that does not take into account the welds of the pressurizer surge line system, one of safety class I components in nuclear power plants, and then the reference values for the accumulated equivalent plastic strain, equivalent plastic strain, and von Mises effective stress were set. Second, the dynamic finite element elastic-plastic seismic analyses were performed for the case of considering only the mechanical strength over-mismatch of the welds as well as for the case of considering both the strength over-mismatch and welding residual strain. Third, the effects of the strength over-mismatch and welding residual strain were analyzed by comparing the finite element analysis results with the reference values. As a result of the comparison, it was found that not considering the strength over-mismatch may lead to conservative assessment results, whereas not considering the welding residual strain may be non-conservative.
This paper emphasizes the use of CFT columns in frame structures subjected to strong horizontal forces and shows that the efficiency of using CFT columns is increased when the plastic design approach is adopted. Because the plastic design approach is based on redistribution of the force of the internal member, a double node for the rotational degrees of freedom, where the adjacent two rotational degrees of freedom can be connected by a non-dimensional spring element, is designed and implemented into the formulation. In addition, an accompanying criterion is considered in order to make it possible to describe the continuous moment redistribution in members connected to a nodal point up to a complete plastic state. The efficiency of CFT columns is reviewed in comparison with RC columns in terms of the cost and the resistance capacity, as defined by a P-M interaction diagram. Three representative frame structures are considered and the obtained results show that the most efficient and economical design can be expected when the use of CFT columns is considered on the basis of the plastic design, especially when a frame structure is subjected to significant horizontal forces, as in a high-rise building.
Keloid scars are often considered aesthetically unattractive and frustrating problems that occur following injuries. They cause functional and cosmetic deformities, displeasure, itching, pain, and psychological stress and possibly affect joint movement. The combination of these factors ultimately results in a compromised quality of life and diminished functional performance. Various methods have been implemented to improve keloid scars using both surgical and non-surgical approaches. However, it has proven to be a challenge to identify a universal treatment that can deliver optimal results for all types of scars. Through a PubMed search, we explored most of the literature that is available about the intralesional injection treatment of hypertrophic scars and keloids and highlights both current (corticosteroid, 5-fluorouracil, bleomycin, interferon, cryotherapy and verapamil) and future treatments (interleukin-10 and botulinum toxin type A). The reference lists of retrieved articles were also analysed. Information was gathered about the mechanism of each injection treatment, its benefits and associated adverse reactions, and possible strategies to address adverse reactions to provide reliable guidelines for determining the optimal treatment for particular types of keloid scars. This article will benefit practitioners by outlining evidence-based treatment strategies using intralesional injections for patients with hypertrophic scars and keloids.

Concrete is the most significant material in the construction industry which is required to construct several facilities like roads, buildings, and bridges etc. which leads to the economic development of a country. But now days, in view of sustainable development and environmental problems, plastic waste management is one of the major environmental issues due to its non-biodegradable nature which allows it to stay in the landfills until they are cleaned up. To overcome all these concerns, plastic waste may be used as a substitute of natural fine and coarse aggregate in concrete and a valuable solution to utilize the plastic items which causes several problems. In order to, present study is focused on the affecting properties of concrete as workability, compressive strength, and tensile strength of concrete with using plastic waste and without using plastic waste. Based on the detailed literature, it was observed that the plastic waste is not affecting the quality and consistency of concrete. However, as the number of PVC particles in the mixture increased, the drying shrinkage values decreased and the inclusion of plastic flakes can mitigate drying shrinkage cracking which leads the higher durability of concrete. Based on the comprehensive literature, it was also observed that the plastic aggregate found to be suitable for low and medium strength concrete. However, the investigation on the application of plastic aggregate in the high strength concrete is found limited. It was concluded that the optimum percentage of the plastic aggregate was found about 20%.
Purpose: Central giant cell granuloma is a rare, benign giant cell tumor which commonly develops in areas near the teeth. It accounts for approximately less than 7% of benign tumors of the mandible. Clinically, central giant cell granuloma is classifed into aggressive and non-aggressive type, and usually requires surgical treatment. There has been no report of central giant cell granuloma in plastic surgery field of the country, and we report a case with a brief review of the diagnosis and treatment of the disease. Methods: A 23-year-old male presented with a hard, non-tender, growing mass with the size of $4.0{\times}3.0\;cm$ on mandible for several months. Computed tomography scan showed a solid mass within thinned outer cortex on mandible. The thinned outer cortex was excised with the mass and the inner cortex was partially removed burring. After the tumor removal, mandible was fixed by reconstruction plate. Results: Pathologic report showed numerous large multinucleated giant cells, diffusely distributed in a background of ovoid-to-spindle-shaped mononuclear cells. There was no evidence of recurrence after 1 year follow up. Bony defect was regenerated and we removed the reconstruction plate. Conclusion: Removal of central giant cell granuloma results in defect of outer cortex, which can be reconstructed by using reconstruction plate, autologous bone graft or bone cement. We used reconstruction plate as a conservative method to induce secondary healing of the outer cortical defect area, which resulted in normal mastication and occlusion with no recurrence.
