Breast Cancer Molecular Subtypes Defined by ER/PR and HER2 Status: Association with Clinicopathologic Parameters in Ivorian Patients

  • Background: Breast cancer is a heterogeneous disease that represents a major public health problem. The immunohistochemical determination of breast cancer subtypes with regard to estrogen receptor (ER), progesterone receptor (PR), and human epidermal growth factor receptor (HER2) status can contribute to improved selection of therapy and patientcare. The purpose of this study was to determine the prevalence of the molecular breast cancer subtypes and to assess their associations with classical clinicopathologic parameters for better therapeutic decisions in women with breast cancer in the Ivory Coast. Materials and Methods: Formalin-fixed and paraffin-embedded blocks of patients diagnosed with primary breast carcinoma were subjected to immunohistochemical assay for the assessment of ER/RP and HER2 expression. The one-way analysis of variance evaluated the difference between breast cancer subtypes and mean age of patients. The Chi-square Test was used to compare standard clinicopathologic prognostic parameters with tumor subtypes. Results. Among 302 patients, 57% were premenopausal and 43% were postmenopausal. The invasive ductal carcinoma not otherwise specified (IDC NOS) (82.8%) was the most frequent histological type, and the tumor grade 2 (56%) was predominant followed by grade 3 (20.9%). The proportion of positivity of ER, PR, and HER2 was 56%, 49%, and 15.6%, respectively. Half of patients of this study (51.6%) had luminal A breast tumor type followed by TN (32.1%). Other subtypes were luminal B (10.1% ) and non-luminal HER2+ (6.3%). Conclusions. The findings of the present study are in line with the literature and should assist in management of breast cancer in our country.

A COMPARATIVE ANALYSIS WITH RESONANCE FREQUENCY ACCORDING TO VARIOUS SIMULATED BONE DEFECTS (다양한 가상 골 결함에 따른 공진 주파수의 비교 분석)

  • Purpose: Resonance frequency analysis (RFA) , a non-invasive technique for the clinical measurement of implant stability, was investigated. Peri-implant bony defect may contribute to implant failure. This in vitro study evaluated the resonance frequencies according to various bony defects and determined whether the directional bone defect can affect the value of frequency analysis. Material and Method: Fifteen 3.75 mm in diameter and 10 mm in length, machined self-tapping implant future were used. Twelve types of bone defects that have different horizontal and vertical dimensions were simulated. Embedded implants were attached to the dental surveyor. Then, the transducer was connected with the implant fixture and the ISQ value was measured at four different directions. Two-way analysis of variance and post hoc $Sch\`{e}ffe'$ test were performed at the 95% significance level. Results: The control group showed the highest ISQ value and 5 thread-$360^{\circ}$ group had the lowest one. As the vertical exposure of implants in each angle was increased, the ISQ value was decreased. Although the horizontal exposure in each thread was increased, the ISQ value was not significantly decreased. Conclusion : Although the simulated defect type was different from each other, the ISQ value was similar among groups.

Comparative Study of Tongue Color in Common Cold Patients and Controls (감기 환자와 건강대조군 간의 설 특성 비교연구)

  • Tongue diagnosis is convenient and non-invasive method to examine the body's functional condition, and it has been frequently used in traditional Korean Medicine (KM). The aim of this study was to investigate the difference of the tongue color assessed by computerized tongue image analysis system (CTIS) between the common cold (CC) patients and healthy subjects. A total of 85 participants, including 45 CC patients without organic diseases and 40 healthy subjects, were asked to complete the CC symptom questionnaire. A tongue image was acquired by using CTIS. Color differences in Commission Internationale de l'Eclairage (CIE) L*, a* and b* between the CC patient group and the control group were analyzes by using paired t-test analysis. The variable CIE b* of the tongue body was significantly lower in CC than that in controls (P=0.019). The variable CIE L* of the tongue coating was significantly higher in CC than that in controls (P=0.032). In CC, the color of the tongue body seems to be changed to intense red color. The color of the tongue coating seems to be changed to thick fur. The present study demonstrated that the CTIS can be used as a diagnostic and monitoring tool for the objective and standardized evaluation of common cold in clinics.

The Relationship between Oxygen Saturation and Color Alteration of a Compromised Skin Flap: Experimental Study on the Rabbit

  • Background The aim of this study was to collect important data on the time of oxygen saturation change in relation to skin flap color alteration using non-invasive pulse oximetry to evaluate its ability to provide continuous monitoring of skin flap perfusion. Methods An experimental study on the monitoring of blood perfusion of 20 tube-island groin flaps of 10 male New Zealand rabbits was performed using pulse oximetry. The animals were randomly assigned to one of two groups representing a blockage of either arterial or venous blood flow. The oxygen saturation change and clinical color alteration were monitored from the beginning of vessel clamping until the saturation became undetectable. The result was analyzed by the t-test using SSPS ver. 10.0. Results The mean times from the vessel clamping until the saturation became undetectable were $20.19{\pm}2.13$ seconds and $74.91{\pm}10.57$ seconds for the artery and vein clamping groups, respectively. The mean time of the clinical alteration from the beginning of vein clamping was $34.5{\pm}11.72$ minutes, while the alteration in flaps with artery clamping could not be detected until 2.5 hours after clamping. Conclusions The use of neonate-type reusable flex sensor-pulse oximetry is objective and effective in early detection of arterial and vein blockage. It provides real-time data on vessel occlusion, which in turn will allow for early salvaging. The detection periods of both arterial occlusion and venous congestion are much earlier than the color alteration one may encounter clinically.

Prediction of Cobb-angle for Monitoring System in Adolescent Girls with Idiopathic Scoliosis using Multiple Regression Analysis

  • Purpose: The purpose of this study was to select standing posture parameters that have a significant difference according to the severity of spinal deformity, and to develop a novel Cobb angle prediction model for adolescent girls with idiopathic scoliosis. Methods: Five normal adolescents girls with no history of musculoskeletal disorders, 13 mild scoliosis patients (Cobb angle: $10^{\circ}-25^{\circ}$), and 14 severe scoliosis patients (Cobb angle: $25^{\circ}-50^{\circ}$) participated in this study. Six infrared cameras (VICON) were used to acquire data and 35 standing parameters of scoliosis patients were extracted from previous studies. Using the ANOVA and post-hoc test, parameters that had significant differences were extracted. In addition, these standing posture parameters were utilized to develop a Cobb-angle prediction model through multiple regression analysis. Results: Twenty two of the parameters showed differences between at least two of the three groups and these parameters were used to develop the multi-linear regression model. This model showed a good agreement ($R^2$ = 0.92) between the predicted and the measured Cobb angle. Also, a blind study was performed using 5 random datasets that had not been used in the model and the errors were approximately $3.2{\pm}1.8$. Conclusions: In this study, we demonstrated the possibility of clinically predicting the Cobb angle using a non-invasive technique. Also, monitoring changes in patients with a progressive disease, such as scoliosis, will make possible to have determine the appropriate treatment and rehabilitation strategies without the need for radiation exposure.

Accuracy Validation of Urinary Flowmetry Technique Based on Pressure Measurement (수압 측정에 기반하는 요류검사의 정확도 검증)

  • Uroflowmetry is a non-invasive clinical test useful for screening benign prostatic hyperplasia(BPH) common in the aged men. The current standard way to obtain the urinary flow rate is to continuously acquire the urine weight signal proportional to volume over time. The present study proposed an alternative technique measuring pressure to overcome noise problems present in the standard weight measuring technique. Experiments were performed to simultaneously acquire both weight and pressure changes during urination of 9 normal men. Noise components were separated from volume signals converted from both weight and pressure signals based on the polynomial signal model. Signal-to-noise ratio was defined as the ratio of the energies between signal and noise components of the measured volume changes, which was 8.5 times larger in the pressure measuring technique, implying that cleaner signal could be obtained, more immune to noisy environments. When four important diagnostic parameters were estimated, excellent correlation coefficients higher than 0.99 were resulted with mean relative errors less than 5%. Therefore, the present pressure measurement seemed valid as an alternative technique for uroflowmetry.

A Study on the Skin Anti-wrinkle Effect of Novel Palmitoyl Tripeptide (신규한 Palmitoyl Tripeptide의 피부 주름개선 효과에 관한 연구)

  • Collagen is the most abundant animal protein in mammals, accounting for about 30 % of all proteins. It is present in connective tissue and contributes to the structural framework of most organs. The tripeptide (glycineproline-hydroxyproline) with the INCI name Tripeptide-29 is main component of collagen type I. The palmitoyl tripeptide (palmitoyl-glycine-proline-hydroxyproline) with the INCI name Paimitoyi Tripeptide-29 is a synthetic material that was designed as a topical agent to stimulate collagen production. We synthesized the palmitoyl tripeptide as a potential anti-wrinkle compound. This compound has been characterized using HPLC. This compound proved, through in vitro tests, to stimulate collagen production and fibroblast proliferation. These results were very promising, so human studies were subsequently performed. We investigated the skin improvement effect of the palrnitoyl tripeptide on human skin by using non-invasive instruments. We measured physiological effects such as skin wrinkles and elasticity after volunteers applied the cosmetic products for 8 weeks. We observed significant improvement in skin wrinkles and elasticity after use of the cosmetic products for 8 weeks. We concluded that the palmitoyl tripeptide had an anti-aging effect on human facial skin.

Comparative Study of Therapeutic Effect of Needle-free Bee Venom Aqua-acupuncture (BVA) into Zusanli (ST36) in the Rat Formalin Test (포르말린으로 유도된 통증 유발 쥐에서 무침주입기를 이용한 봉독약침의 진통효과)

  • Bee Venom aqua-acupuncture (BVA) simultaneously exerts pharmacological effects of biologically active compounds, existed in the whole bee venom, and medicinal effect of the stimulation of acupuncture points. BVA has been considered as a promising therapeutic method for treating various chronic diseases, mainly accompanying severe pain and inflammation. As a painless injection device, jet injectors have been commercially marketed for various clinical applications including insulin injection and vaccination. Among them, a pressure-driven jet injector system could be used for intradermal delivery of a variety of drugs. The aim of this study was to investigate the analgesic effects of the BVA using a needle-free injector (Biojector $2000^{\circledR}$, Bioject Inc., OR, USA), compared to the conventional BV aqua-acupuncture using a typical syringe. Adult Sprague-Dawley rats were injected with bee venom $(0.08mg/kg,\;50{\mu}l)$ using Biojector $2000^{\circledR}$ (BVA-B) or a syringe (BVA-5) into the Zusanli (ST36) acupoint, 30 minutes before plantar injection of 2% formalin. It was found that BVA-B-, or BVA-5-treated rats, compared to controls, exhibited significantly less licking behavior during the late phase in the rat formalin test, when compared to controls. During early phase, however, those effects were not significant but substantial. The analgesic effect of BVA-B was also compatible with that of the conventional BVA-5. In the immunohistochemical studies, BVA-B significantly suppressed the expression of formalin-evoked c-fos, a biomarker of neuronal activity, in the lumbar dorsal horn of the spinal cord. These results indicated that BVA-B waseffective in the modulation of pain in the rat formalin test, compared to BVA-5. Taken together, the needle-free jet injector system could be substituted for the conventional aqua- acupuncture with the advantage of little pain.

Determinants of Advanced Stage at Initial Diagnosis of Breast Cancer in Pakistan: Adverse Tumor Biology vs Delay in Diagnosis

  • Background: Breast cancer is the most frequent cancer of women in Pakistan with the majority presenting with stage III or IV lesionsat initial diagnosis. Patient and health system related factors are well known determinants of delay in presentation and diagnosis. Additionally, breast cancer being a heterogeneous disease, the various molecular subtypes featuring different aggressiveness also need to be considered. The present study evaluated the association of stage at initial diagnosis of breast cancer with these two factors in local women at a tertiary level health care facility in Lahore, Pakistan. Materials and Methods: Patient and tumor features were recorded separately during diagnostic workup in Breast Clinics at INMOL and at Services Hospital, Lahore. Data were entered in MS Excel and analyzed by descriptive statistics and Chi-Square test. Results: Among the 261 patients, 64% were staged as late breast cancer (LBC), the mean age was 46.8 with standard deviation of 13 years. Some 92% had invasive ductal carcinoma (IDC), 61% had luminal types (LT) of non-aggressive tumor while 39% had the non-luminal types (NLT) of of HER2-enriched or basal aggressive tumors. While 70% of patients presented within one year of symptomatic disease (early report group "ERG"), 30% reported after a mean delay of 4 years with a standard deviation of 3.75 years. The stage distribution among ERG patients was not statistically different from those reporting late (P=0.123). Statistically larger proportion of patients with NLT presented as LBC as compared to the LT (P =0.034). Among the ERG, statistically different stage distribution of disease was observed for the NLT versus LT (P=0.047). Among those presenting late, this difference was insignificant (P=0.416). Conclusions: Breast cancer is a distinct disease in Pakistan with a high frequency of aggressive molecular types affecting younger women, with the majority presenting as LBC. Association of NLT with higher stage at diagnosis is statistically significant whereas time delay in diagnosis is not. Further research is required to define the risk profile and features in local patients. The burden of LBC can be reduced by promoting breast health awareness and by establishing easily accessible dedicated breast care set ups in the hospitals.

Age-Related Fecal Calprotectin Concentrations in Healthy Adults (건강한 성인의 연령별 분변 칼프로텍틴의 농도)

  • Fecal calprotectin (FC) is a marker used for the differential diagnosis of inflammatory bowel disease (IBD). FC is also used to determine the effects of treatment and recurrence prediction because of its non-decomposition by bacteria, relative week stability at room temperature, and its uniform distribution within feces. Healthy male and female adults between the age of 30 and 80 living in Jeju were selected for this study. The FC concentration in the healthy control group (N=45) was distributed widely as 0~545.9 ㎍/g and showed a significant difference with age in healthy adults. The FC concentration in adults over 70 years old (80.6 years on average) was 160.3 ㎍/g. The result is approximately 10 times higher than in adults below 50 years (44 years on average), with FC concentrations at 15.88 ㎍/g. Moreover, adults over 50 years, with an average age of 59.6, had FC concentrations of 35.46 ㎍/g, which were two times higher than the below 50-year-old group, confirming the significant correlation between age and FC concentration. As the FC test is a non-invasive and cost-effective objective marker in IBD tests, a suitable cut-off value is required for different ages. This study provides the baseline data for differential diagnoses.
