RAPID SPREAD NK/T-CELL MALIGNANT LYMPHOMA OF MAXILLA (급속 진행된 상악의 NK/T-세포 악성 임파종)

  • Natural Killer/T-cell(NK/TC) lymphoma is a rare disease of oral and maxillofacial region with an aggressive clinical course, showed unusual clinical manifestations. Prognosis is generally poor and the disease is invariably fatal after systemic dissemination. A case of nasal NK/TC non-Hodgkin's lymphoma in the left maxilla that showed unusual clinical manifestations and a fulminant course of disease, are described with literature reviews. A 81-year-old female patient presented with rapidly growing ulceration and general malaise after extraction of right upper second premolar NK/TC lymphoma was subsequently detected by biopsy. rapid and wide invasion from maxilla to mandible developed within 10 days and multiple metastasis to whole body was after a few weeks. Although the time relationship is not clear, local invasion and multiple metastasis could be dissemination from localized disease of NK/TC. As seen in my case, the course can be excessively aggressive and fulminant even though it first appeared as a localized ulceractive lesion. She is expired 2 months after biopsy. Positivity of immunohistochemical stain (CD56, LCA, UCHL-1, CD3), which is a specific characteristic of NK/TC, may serve as a factor showing a poor prognosis of a malignant lymphoma

  • We present the case of a 45-year-old man with a history of benzene exposure who developed splenic marginal zone lymphoma. For 6 years, he had worked in an enclosed space cleaning instruments with benzene. He was diagnosed with splenic marginal zone lymphoma 19 years after retirement. During his time of working in the laboratory in the 1980s, working environments were not monitored for hazardous materials. We indirectly estimated the cumulative level of past benzene exposure using job-exposure matrices and technical assumptions. Care must be taken in investigating the relevance of occupational benzene exposure in the occurrence of indolent B-cell lymphoma. Because of the long latency period and because occupational measurement data do not exist for the period during the patient's exposure, the epidemiological impact of benzene exposure may be underestimated.

  • Malignant lymphoma affecting the larynx is uncommon, probably accounts for less than 1 percent of malignant lesion of the larynx. Malignant lymphoma of the larynx is predominantly localized to supraglottis such as epiglottis, aryepiglottic fold. Laryngoscopy reveals a irregular submucosal mass with a smooth surface and no ulceration. Most of which belongs to non­Hodgkin's lymphoma. After it has been determined that the disease is localized, radiation therapy is the choice of the treatment. Chemotherapy with or without irradiation is reserved for more advanced tumor. Then life-long follow-up is essential. This paper deals with 5 cases malignant lymphoma of the larynx to review our clinical experience and to suggest the optimum treatment in the course of the disease.

  • The Mantle cell lymphoma(MCL) is a relatively rare subtype of malignant lymphoma characterized by chromosomal translocation t(11 ; 14)(q13 ; q32), positive response for CD5, and nuclear cyclin D1. It is account for an estimated 3-6% of all non-Hodgkin's lymphoma. The involvement of extra-nodal site is not uncommon, whereas salivary glands are rarely affected. It is more commonly occurred in men and old age and approximately 75% of cases are diagnosed with advanced stage. It is usually characterized by an aggressive clinical course, and the prognosis is poorer than other type of head and neck lymphoma. We recently encountered a 69-year-old female with mass in parotid tail and upper neck, and it was diagnosed as mantle cell lymphoma. We report the unique case with a review of literature.

A Case of Primary Large B-cell Lymphoma of Larynx Presenting as Supraglottic Mass (성문상부 종물 양상의 일차성 후두 Large B세포 림프종 1예)

  • The extranodal non-Hodgkin lymphoma is uncommonly occurred in larynx, accounting for less than 1% of all laryngeal neoplasm. In general, the laryngeal lymphoma is appeared as submucosal mass without mucosal ulceration and is most commonly found in supraglottis. The primary laryngeal lymphoma constitute a diagnostic challenge because they are characterized by absence of clinical and gross differential criteria, compared with squamous cell carcinoma (SCC). We encountered a 74-year-old man with hoarseness and lump sensation in the throat. On direct laryngoscope, multiple ulcerative and exudative mass in glottis and supraglottic areas were observed. The patient was finally diagnosed as large B-cell lymphoma through the laryngeal microsurgery. He received radiation therapy and there is no evidence of recurrence. Although the laryngeal mass has superficial mucosal change, primary laryngeal lymphoma must be included in the differential diagnosis.

  • Lymphoproliferative disorders, manifesting either as a primary lesion or as part of a generalized process, rarely involve the breast. The primary malignant lymphoma of the breast is rare. The cytologic features of this neoplasm have not been reported in Korea thus far. We described a case of primary malignant lymphoma of the breast, as diagnosed by fine needle aspiration cytology (FNAC) in an 84-year-old female. FNAC smears showed high cellularity. The main cytologic feature observed was a carpeting of monotonous, atypical, enlarged lymphoid cells. These lymphocytes showed relatively round elliptical nuclei with some indentation. One or more prominent nucleoli were seen. Biopy from the breast mass revealed diagnostic features of non-Hodgkin's lymphoma, diffuse large B-cell type.

  • Mucosa-associated lymphoid tissue (MALT) lymphoma is one of the most common lymphomas and accounts for about 7% of all newly diagnosed non-Hodgkin's lymphoma (NHL). The clinical course of MALT lymphoma is relatively indolent and, in the majority of cases (50%), the lymphoma arises within the stomach. Primary central nervous system lymphoma (PCNSL), an uncommon variant of extranodal NHL, can affect any part of the neuraxis, including the eyes, brain, leptomeninges, or spinal cord. Herein, we present a rare case of PCNSL, which occurred one year after radiochemotherapy of gastric MALT lymphoma. A 62-year-old man presented with a 3-day history of left facial palsy. One year ago, he underwent antibiotic eradication therapy of Helicobacter pylori, local stomach fractional radiotherapy, and chemotherapy for gastric MALT lymphoma. Magnetic resonance imaging revealed a strong enhancing solid mass in the right frontal lobe. The tumor was completely removed, and the histological diagnosis of PCNSL developing from diffuse large B-cell lymphoma was made. Although elucidating the correlation between the first gastric MALT lymphoma and the second PCNSL seemed difficult, we have postulated and discussed some possible pathogeneses, together with a review of literature.

  • 전신적인 침범없이 후두에만 국한되는 악성 림프종은 매우 드믄 예로서 일반적으로 두경부에만 국한된 방사선 치료로 근치 가능하다. 그러나 때때로 급성 호흡 곤란을 초래하여 응급 기관 절개와 복합 항암제를 투여해야 되는 경우를 간혹 경험하게 된다. 본 증례는 수 개월간 지속되어온 애성과 최근에 갑자기 심해진 호흡 곤란으로 급성 후두염이란 임상 진단명으로 보존적인 치료를 하였으나 증상이 호전되지 않아서 후두조직 검사 결과에서 비 호즈킨 림프종으로 확진되어 6회의 복합 항암제를 투여한 다음 남아있는 병변에 대한 45 Gy의 외부 방사선 치료후 촬영한 전산화 단층 촬영에서 완전 관해를 보였으며 진단후 10개월이 지난 현재 무병 상태를 보이는 환자를 경험하였기에 간단한 문헌 고찰과 함께 보고한다.

Primary Laryngeal Lymphoma (후두에 발생한 원발성 림프종)

  • Primary lymphoma of the larynx is rare, accounting for less than 1% of all laryngeal neoplasm. Early symptoms are ambiguous and nonspecific, and confirmation of the diagnosis is often difficult. The aim of this study was to report the experience of our department in the management of these aggressive lesions, as they require special diagnostic and therapeutic attention. We enrolled 3 patients who diagnosis of lymphoma involving the larynx were retrospectively reviewed. The histopathological diagnosis revealed 1 case of diffuse large B cell lymphoma, and 2 case of NK-T cell lymphoma. Details of the presentation, diagnostic procedures, treatment, and outcome of these patients were presented. Primary laryngeal lymphoma is a rare entity. Early symptoms are subtle and nonspecific, and confirmation of the diagnosis is often difficult. Because of the rarity of this tumor type, the optimal management remains controversial and it seems that should be managed not as a distinct disease entity but as an unusual presentation of non-Hodgkin lymphoma, according to the recent treatment trends.

Fine Needle Aspiration Cytologic Findings of Thyroid Lymphoma -Report of Two Cases- (갑상선 림프종의 세침흡인 세포학적 소견 -2 예 보고-)

  • We report two different types of thyroid lymphoma associated with Hashimoto's thyroiditis. Both showed autoantibodies and were compatible with Hashimoto's thyroiditis according to their clinical backgrounds. A 76-year-old female noted a painless, rapidly growing mass in her neck which was diagnosed as diffuse non-Hodgkin's lymphoma, large cell type, after the fine needle aspiration cytology of the thyroid. She underwent chemo-radiotherapy and is free of the disease 10 months after diagnosis. The other patient, a 73-year-old female with a diffuse golfer, was diagnosed on fine needle aspiration cytology as having Hashimoto's thyroiditis. Three years later she developed a hard nodular growth in the both lobes of the thyroid. This was subjected to fine needle aspiration cytology and needle biopsy and was diagnosed as a MALT lymphoma. She refused any treatment and died 12 months after the diagnosis.
