  • 인터넷의 성장과 더불어 수많은 응용시스템들이 인터넷 기반 시스템으로 바뀌고 있다. 특히, 인터넷 기반 전문가시스템은 다양한 구조와 방법으로 설계될 수 있고 이미 제작되었던 수많은 인터넷 기반 전문가시스템들은 그 나름대로의 장단점을 가지고 있다. 본 연구는 특정 전문가시스템의 개발이 아니라 WWW(World Wide Web)을 사용자 인터페이스로 이용할 수 있는 전문가시스템 쉘에 관한 연구이다. 본 시스템은 WWW 하부구조에 대한 의존성을 제거, 클라이언트와 네크워크에 부담 격감, HTML과 부합하는 텍스트에 기초한 통신과 같은 장점 외에 중앙 집중적 다중 사용자 접근 관리 기능을 제공한다. 본 시스템은 다중사용자를 위한 서버/클라이언트 환경 구축을 위해 소켓을 이용하여 큐잉(queueing)과 직렬화(serialization)를 해결하고 비연결 지향적인 WWW의 특성으로 인한 사용자 관리의 어려움은 사용자의 IP 주소와 Timer를 이용한 휴무기간 검사를 이용하여 해결한다.Abstract The growth of internet drives many applications into internet-based systems. Internet-based expert systems can be designed with various concepts and methodologies, and they have their own merits and demerits. This papers is a study on a development of not expert system itself but expert system shell which is able to use WWW(World Wide Web) as user interface. The suggested system supports functions on multiuser management controlled by a server system as well as independence on development environments, minimization of the load for clients and network, and text-based communications such as HTML. The system uses socket, which solves problems of queuing and serialization, in order to construct multiuser server/client environmen and also the system solves the non-connective property of WWW which makes it difficult to control users and processes by using IP address and idle time which is supported by the timer.

Role of n-3 long-chain polyunsaturated fatty acids in human nutrition and health: review of recent studies and recommendations

  • Long-chain (LC) n-3 polyunsaturated fatty acids (n-3 PUFAs), in particular docosahexaenoic acid (DHA) and eicosapentaenoic acid (EPA), are nutrients involved in many metabolic and physiological processes, and are referred to as n-3 LCPUFA. They have been extensively studied for their effects in human nutrition and health. This paper provides an overview on metabolism, sources, dietary intake, and status of n-3 LCPUFA. A summary of the dietary recommendations for n-3 LCPUFAs for different age groups as well as specific physiological conditions is provided. Evidence for n-3 LCPUFA in cardiovascular diseases, including new studies, is reviewed. Expert recommendations generally support a beneficial effect of n-3 LCPUFA on cardiovascular health and recommend a daily intake of 500 mg as DHA and EPA, or 1-2 servings of fish per week. The role of n-3 LCPUFA on brain health, in particular neurodegenerative disorders and depression, is reviewed. The evidence for beneficial effects of n-3 LCPUFA on neurodegenerative disorders is non-conclusive despite mechanistic support and observational data. Hence, no definite n-3 LCPUFA expert recommendations are made. Data for the beneficial effect of n-3 LCPUFA on depression are generally compelling. Expert recommendations have been established: 200-300 mg/day for depression; up to 1-2 g/day for major depressive disorder. Recent studies support a beneficial role of n-3 LCPUFAs in reducing the risk for premature birth, with a daily intake of 600-800 mg of DHA during pregnancy. Finally, international experts recently reviewed the scientific evidence on DHA and arachidonic acid (ARA) in infant nutrition and concluded that the totality of data support that infant and follow-on formulas should provide both DHA and ARA at levels similar to those in breast milk. In conclusion, the available scientific data support that dietary recommendations for n-3 LCPUFA should be established for the general population and for subjects with specific physiological conditions.

  • Adequate nutrition in early life is proposed to shape a child's future health by launching the growth trajectory in the proper direction, which helps to avoid negative metabolic programming effects. Protein intake during infancy and early childhood is of great importance, as it plays a key role in infant metabolic programming and the future risk of obesity. Breastfeeding provides the best nutrition in early life, with many benefits tailored for the baby, including the appropriate quantity and quality of proteins. Considering the high prevalence of childhood, and subsequent adult, obesity in the region, a virtual Middle East expert consensus meeting was held to discuss an effective approach for managing childhood obesity. Leading pediatric experts from Bahrain, Egypt, Kuwait, Oman, Qatar, Saudi Arabia, and the United Arab Emirates participated in the meeting. The experts discussed, debated, and agreed on certain directions, including the importance of educating parents, endorsing breastfeeding, and ensuring optimum quantity and quality intake of proteins in early life. This expert consensus may serve as the starting point for healthcare professionals in the region who are interested in shaping a healthy future for the generations to come.

  • Transarterial chemoembolization (TACE) was introduced in 1977 with the administration of chemotherapeutic agent to gelatin sponge particles through the hepatic artery in patients with hepatocellular carcinoma (HCC) and was established as conventional TACE using Lipiodol in the 1980s. In the 2000s, drug-eluting beads were developed and applied clinically. Currently, TACE is a commonly used non-surgical treatment modality for patients with HCC who are unsuitable for curative treatment. Considering the vital role of TACE in the management of HCC, it is crucial to organize current knowledge and expert opinions regarding patient preparation, procedural techniques, and post-treatment care in TACE, which can enhance therapeutic efficacy and safety. A group of 12 experts in the fields of interventional radiology and hepatology, convened by the Research Committee of the Korean Liver Cancer Association (KLCA), has developed expert consensus-based practical recommendations in TACE. These recommendations have been endorsed by the Korean Society of Interventional Radiology and provide useful information and direction in performing TACE procedure as well as pre- and post- procedural patient care.

Fifty Years of Innovation in Plastic Surgery

  • Background Innovation has molded the current landscape of plastic surgery. However, documentation of this process only exists scattered throughout the literature as individual articles. The few attempts made to profile innovation in plastic surgery have been narrative, and therefore qualitative and inherently biased. Through the implementation of a novel innovation metric, this work aims to identify and characterise the most prevalent innovations in plastic surgery over the last 50 years. Methods Patents and publications related to plastic surgery (1960 to 2010) were retrieved from patent and MEDLINE databases, respectively. The most active patent codes were identified and grouped into technology areas, which were subsequently plotted graphically against publication data. Expert-derived technologies outside of the top performing patents areas were additionally explored. Results Between 1960 and 2010, 4,651 patents and 43,118 publications related to plastic surgery were identified. The most active patent codes were grouped under reconstructive prostheses, implants, instruments, non-invasive techniques, and tissue engineering. Of these areas and other expert-derived technologies, those currently undergoing growth include surgical instruments, implants, non-invasive practices, transplantation and breast surgery. Innovations related to microvascular surgery, liposuction, tissue engineering, lasers and prostheses have all plateaued. Conclusions The application of a novel metric for evaluating innovation quantitatively outlines the natural history of technologies fundamental to the evolution of plastic surgery. Analysis of current innovation trends provides some insight into which technology domains are the most active.

라디오믹스 기반 직장암 수술 위험도 예측을 위한 MRI 반자동 선택 바이오마커 검증 연구 (A Study on MRI Semi-Automatically Selected Biomarkers for Predicting Risk of Rectal Cancer Surgery Based on Radiomics)

  • Currently, studies to predict the risk of rectal cancer surgery select MRI image slices based on the clinical experience of surgeons. The purpose of this study is to semi-automatically select and classify 2D MRI image slides to predict the risk of rectal cancer surgery using biomarkers. The data used were retrospectively collected MRI imaging data of 50 patients who underwent laparoscopic surgery for rectal cancer at Gachon University Gil Medical Center. Expert-selected MRI image slices and non-selected slices were screened and radiomics was used to extract a total of 102 features. A total of 16 approaches were used, combining 4 classifiers and 4 feature selection methods. The combination of Random Forest and Ridge performed with a sensitivity of 0.83, a specificity of 0.88, an accuracy of 0.85, and an AUC of 0.89±0.09. Differences between expert-selected MRI image slices and non-selected slices were analyzed by extracting the top five significant features. Selected quantitative features help expedite decision making and improve efficiency in studies to predict risk of rectal cancer surgery.

Factors Predicting Difficult Biliary Cannulation during Endoscopic Retrograde Cholangiopancreatography for Common Bile Duct Stones

  • Background/Aims: Difficult biliary cannulation is an important risk factor for post-endoscopic retrograde cholangiopancreatography (ERCP) pancreatitis (PEP). Therefore, this study aimed to identify the factors that predict difficult cannulation for common bile duct stones (CBDS) to reduce the risk for PEP. Methods: This multicenter retrospective study included 1,406 consecutive patients with native papillae who underwent ERCP for CBDS. Factors predicting difficult cannulation for CBDS were identified using univariate and multivariate analyses. Results: Univariate analysis showed that six factors significantly predicted difficult cannulation: ERCP performed by non-expert endoscopists, low-volume center, absence of acute cholangitis, normal serum bilirubin, intradiverticular papilla, and type of major duodenal papilla. Multivariate analysis identified ERCP performed by non-expert endoscopists (odds ratio [OR], 2.5; p<0.001), low-volume center (OR, 1.6; p<0.001), intradiverticular papilla (OR, 1.3; p=0.007), normal serum bilirubin (OR, 1.3; p=0.038), and absence of acute cholangitis (OR, 1.3; p=0.049) as factors significantly predicting difficult cannulation for CBDS. Conclusions: Initial cannulation by an experienced endoscopist, early rescue cannulation, or early takeover by an experienced endoscopist should be considered when performing ERCP for CBDS in the presence of factors predicting difficult cannulation.

  • The helicopter system is non-linear and complex. Futhermore, because of absence of an accurate mathematical model, it is difficult accurately to control its attitude. But we can control the non-modeled system with the uncertainty and unstructure using the fuzzy control algorithm. Therefore, we apply optimized fuzzy controllers for the control of its elevation angle and azimuth one using expert's intuitions and knowledges. The simulation and experimental results of the hellicopter simulator CE150 with MATLAB shall be introduced.

  • The dynamic characteristics of turning processes are complex, non-linear and time-varying. Consequently, the conventional techniques based on crisp mathematical model may not guarantee surface roughness regulation. This paper presents a fuzzy controller which can regulate surface roughness in milling process using end-mill under varying cutting condition. The fuzzy control rules are established from operator experience and expert knowledge about the process dynamics. regulation which increases productivity and tool life is achieved by adjusting feed-rate according to the variation of cutting conditions. The performance of the proposed controller is evaluated by cutting experiments in the converted CNC milling machine. The result of experiments show that the proposed fuzzy controller has a good surface roughness regulation capability in spite of the variation of cutting conditions.

Super Strict Quality Requirements of the 21st Century and Its Influence on the Industry of CHINA

  • The famous US quality management expert J. M. Juran pointed out on the American Quality Congress of ASQC in 1994, that the 20th century is a century of productive force and the 21st century would be a century of quality. In the past dozens of years; science and technology have made rapid progress. Take electronic products for example, the non-conformity rate has dropped from 1%($10^{-2}$) and 1%($10^{-3}$) to ppm (parts per million, $10^{-6}$), even to ppb (parts per billion, $10^{-9}$). Such a low non-conformity rate can be called the super strict quality requirements of products of the $21^{st}$ Century. Super strict quality requirements can cause a series of new changes: Therefore, super strict quality requirements are unavoidable to produce significant influences to the industry of the $21^{st}$ century. This paper analyzed these influences and pointed out that which quality sciences must be paid attention to first.
