The Effect of Continuous Infusion of Ketorolac on Morphine IV PCA (Ketorolac의 지속적 정주가 Morphine정맥 PCA에 미치는 효과)

  • Background: Ketorolac($Tarasyn^{(R)}$) is a non-steroidal anti-inflammatory drug(NSAID) which has shown to be an effective postoperative analgesic available parenterally, and when combined with morphine can reduce its requirement. The analgesic efficacy and adverse effects of continuous infusion of ketorolac added to morphine IV PCA was evaluated in 60 women after abdominal hysterectomy. Methods: Patients were assigned to receive either morphine intravenous(IV) bolus followed by morphine IV patient controlled analgesia(PCA), or ketorolac 30mg IV and continuous IV infusion at 4.0mg/hr in combination with the above regimen. The authors evaluated PCA morphine used, pain assessment(verbal pain intensity score and visual analogue scale) and side effects at 2, 4, 6 and 24hrs during pain control. Results: Continuous infusion of ketorolac decreased the PCA morphine usage significantly(30.4 ---> 19.6 mg : p=0.007) at 24hrs postoperatively. Significant differences were seen favoring ketorolac infusion in pain intensity and visual analogue scale both at rest and during movement. There were no differences in incidences of deep sedation, nausea & vomiting. But the ketorolac group they complained of dizziness more than morphine only group. Little pruritus was recorded in either groups. Conclusions: The authors conclude continuous IV infusion of ketorolac in conjunction with morphine PCA provide effective analgesia after low abdominal surgery.

Educational Drama Skills to Improve the Expressive Ability of Korean Language (한국어 표현능력 향상을 위한 교육연극 기법)

  • The early stage of Korean language education was to understand grammar. However, recent Korean language education aims to improve expressive ability for smooth communication. Hence, educational drama for education has gained attention as one of the effective ways of improving communication skills. Educational drama provides an actual conversational situation in which one can use appropriate words and grammar depending on the situation. Non-verbal expression such as body language and facial expression could be picked up so one also improves one's communicative abilities naturally. This means that educational drama shares Korean language education's goal of improving communicative ability. Moreover, the student becomes the core of the activity and finds solutions through cooperation with others, and this also helps improve social skills and self-esteem. This thesis covers methods of having interviews, re-writing texts to scripts, writing up post-script by imagining the following text, role-playing, and improving expressive ability by debating educational theatrical techniques. To make these activities successful, good quality VOD and learning materials shall be developed and used. Also, efficient training programs shall be created so that teachers make use of what they learn and improve their teaching skills.

Politeness Strategy in German Communication: Focusing on Politeness according to Familiarity (독일어 커뮤니케이션에서의 공손 전략: 친근감 여부에 따른 공손을 중심으로)

  • This paper examines the types and functions of politeness in German communication and how politeness strategy can be realized. 'Politeness' is not a grammatical terminology in German, but it can be found in many places in grammar. The criteria for politeness are not only organized according to the rules of the language system, but the boundaries are ambiguous because non-language factors affect communication. Politeness is an important strategic element as well as social value. The polite expression first appears in the grammatical level of invariant with the form of address according to the familiarity between the conversational parties, verb modus, and modal particle. Modal particle with familiarity is considered to be a positive politeness strategy that limits the listener's speech by weakening or avoiding face threatening act. Modal verbs is classified as polite expressions that do not impose a psychological burden by not forcing the listener to make a direct request. The results of this study are therefore expected to suggest a rationale for empirical research on politeness in German communication.

  • Along with language, socialization is a unique feature of the human being. There is a continuous debate regarding whether the development of socialization is innate, and conducted by the environment in the growing process, or the result of the interaction of both aspects. If socialization is the result of the interaction with the environment or is an acquired developmental process, the following question rises. "Is there a 'critical period' for the development of socialization?" Although there are a huge number of studies seeking for treatment and solutions for developmental delay or deficits of socialization, it is very complicated question to answer. Historical figures such as 'Hugh Blair' of Borgue in England, and 'the wild boy of Aveyron' in France, seem to have innate socialization deficits. Nowadays patients with non-verbal learning disorder, social communication disorder, or autism spectrum disorder seem to have genetic defects. On the other hand, Harry Harlow's monkey experiments, hikikomori of Japan, Romanian orphans and patients with reactive attachment disorder seem to display social deficits due to environmental factors. However, it is not easy to clearly draw a line between innate or acquired factors. Therefore, rather than subdividing the diseases for etiological and pathophysiological approach to heterogenous groups with the common denominator of social deficit, and for the research of pathophysiology and treatment development, the authors suggest a comprehensive concept of "social dysfunction spectrum."

The Effects of Self-Efficacy Promoting Cardiac Rehabilitation Program on Self- Efficacy, Health Behavior, and Quality of Life

  • Background. Ischemic heart disease results from athesclerotic changes of the coronary artery. These changes are aggravated by hypercholesterolemia, smoking, obesity, lack of exercise, coronary-prone personality, and stress. Because these risk factors affect not only the prevalence of the ischemic heart disease but also recurrence of the disease, cardiac rehabilitation programs were introduced to help patients with ischemic heart disease reduce risk factors. Diverse cardiac rehabilitation programs are needed to motivate participation in cardiac rehabilitation and to enhance patients' quality of life. Objectives. To examine the effect of a self-efficacy promoting cardiac rehabiltation program on self-efficacy, health behavior and quality of life of patients with ischemic heart disease. Methods. Data were collected from 45 hospitalized ischemic heart disease patients. Medical records were reviewed to obtain demographic and clinical characteristics. Data regarding self-efficacy, health behavior, and quality of life were obtained from interviews using structured questionnaires. The nonequivalent control group non-synchronized design was used to conduct this study. One session of conventional group education was given to patients in the control group while they were in the hospital. Patients in the experimental group participated in a newly developed cardiac rehabilitation program. It focused on strengthening self-efficacy with four self-efficacy sources-performance accomplishment, vicarious experiences, verbal persuasion and physical status using two individualized in-hospital education sessions and four weekly telephone counseling follow-up calls after discharge. Results. Four weeks after discharge, the increment of total self-efficacy score was significantly higher in the experimental group than in the control group (p<.0l). There was also a significant difference in the total quality of life scores increments between the two groups (p<.0l). However, no significant changes were noted in the increments of total health behavior scores between the two groups. Conclusion. A cardiac rehabilitation program focusing on promoting self-efficacy was effective in improving self-efficacy, and quality of life of patients with ischemic heart disease.

Oral Presentations as an Alternative Approach to Enhance L2 Learning and Communication Skills (L2 학습과 의사소통 능력 향상을 위한 대안적 접근으로서의 구두 발표)

  • The current study aimed to examine university students' perceptions of the positive effects of L2 oral presentations in terms of communication skills and future employment. The participants were 13 undergraduate students enrolled in L2 oral presentation classes at the beginner or intermediate level. This study followed a qualitative research approach using in-depth and semi-structured interviews as the primary method. The findings revealed that students largely exhibited the benefits of oral presentations in three ways: by enhancing L2 proficiency, building confidence in public speaking, and gaining competitive advantages in their future careers. As a challenge for L2 learners, oral presentations were considered fundamental to develop effective communication skills in academic and professional contexts.

A Study on the Construction of a Real-time Sign-language Communication System between Korean and Japanese Using 3D Model on the Internet (인터넷상에 3차원 모델을 이용한 한-일간 실시간 수화 통신 시스템의 구축을 위한 기초적인 검토)

  • Sign-language communication can be a useful way of exchanging message between people who using different languages. In this paper, we report an experimental survey on the construction of a Korean-Japanese sign-language communication system using 3D model. For real-time communication, we introduced an intelligent communication method and built the system as a client-server architecture on the Internet. A character model is stored previously in the clients and a series of animation parameters are sent instead of real image data. The input-sentence is converted into a series of parameters of Korean sign language or Japanese sign language at server. The parameters are transmitted to clients and used for generating the animation. We also employ the emotional expressions, variable frames allocation method, and a cubic spline interpolation for the purpose of enhancing the reality of animation. The proposed system is implemented with Visual $C^{++}$ and Open Inventor library on Windows platform. Experimental results show a possibility that the system could be used as a non-verbal communication means beyond the linguistic barrier.

  • Objective:This study was conducted to examine the Draw-A-Person test as a non-verbal indicator of concrete and abstract thinking. Method:Thirty child-psychiatric children and 27 normal children were individually assessed with Draw-A-person Test and KEDI-WISC, and the scores were compared between similarity test and DAP. Results:The results showed significant differences between the groups. In clinical group, there were significant correlations with DAP scores and similarity test scores, especially abstract scores on DAP were highly correlated with similarity test scores. Results indicate characteristics of abstract thinking may be more discernable on the DAP than concrete thinking in clinical group.

A Study of the Effectiveness of Token Economy on Children′s Behaviors for Oral Medication (토오컨 강화가 입원 아동의 약 복용행위에 미치는 효과)

  • Oral medication is one of the most frequent treatment in clinical care. and frequently refused by children. Children's refusal spends unuseful time and require nurses'Patience. So this study was attempted to develop token economy Program which Promote children's medication. and to evaluate its' effect to construct the experimental ground. This study was a quasi experimental study under the nonequivalent control group non-synchronized design. The subjects of this study were 48 children who were aged 3-12 years and admitted pediatric ward because of their respiratory disease. Token economy program was consisted of cartoon record paper with stickers which expresses the better medication is taken the more germs die, and gift was given for back up reinforcer. To evaluate this Program. time spent for oral medication was measured six time to both experimental group and control group. and measured the perceived effectiveness of this program by nurses. To determine the effect of the program, the data were analyzed by the SAS 6.12 Program with t-test and mean, standard deviation. The results of this study were as follows: 1 In experimental group, time spent for oral medication was significantly shorter than in control group(t=5.24. p=0.0001). Therefore, the hypothesis of this study was strongly accepted. 2. Nurses perceived this program as an effective one(mean=3.125~3.792). The effectiveness occurred before. during and after medication as well as in parental responses, especially in using verbal dissatisfaction after medication. In conclusion. it was found that the token economy program for admitted children was effective in inducing the children's behavior of oral medication. There was no effectiveness in very highly anxious children Whereas, it is recommended that this program should not be used for such children until they settle down. If the token economy program was made in various situation with creative thinking, it will be very useful nursing measurement, especially in caring for children. To improve the qualify of nursing care, the various programs, which can give joyfulness to stressed patients should be developed.

  • Augmentative and Alternative Communication(AAC) is the widespread supporting plan to solve the communication problem of the people with communication disorder. AAC means various types of non-verbal communication methods that are used to augment or substitute the communications of using speech. With the increasing number of potential Korean AAC users, the demands for AAC are also increasing. In spite of the increasing demands, AAC is not being actively and practically used. In this paper, we implemented a web-based AAC board making system, where the board-makers (special educators and speech therapists) can create and share the customized boards to help the board-users (people with communication disorder). The web-based AAC board making system contributes to the application of AAC in many societies, such as hospitals, schools, companies, or anywhere the AAC intervention is needed.
