Influence of glide path on the screw-in effect and torque of nickel-titanium rotary files in simulated resin root canals

  • Objectives: The purpose of this study was to investigate the screw-in effect and torque generation depending on the size of glide path during root canal preparation. Materials and Methods: Forty Endo-Training Blocks (REF A 0177, Dentsply Maillefer) were used. They were divided into 4 groups. For groups 1, 2, 3, and 4, the glide path was established with ISO #13 Path File (Dentsply Maillefer), #15 NiTi K-file NITIFLEX (Dentsply Maillefer), modified #16 Path File (equivalent to #18), and #20 NiTi K-file NITIFLEX, respectively. The screw-in force and resultant torque were measured using a custom-made experimental apparatus while canals were instrumented with ProTaper S1 (Dentsply Maillefer) at a constant speed of 300 rpm with an automated pecking motion. A statistical analysis was performed using one-way analysis of variance and the Duncan post hoc comparison test. Results: Group 4 showed lowest screw-in effect ($2.796{\pm}0.134$) among the groups (p < 0.05). Torque was inversely proportional to the glide path of each group. In #20 glide path group, the screw-in effect and torque decreased at the last 1 mm from the apical terminus. However, in the other groups, the decrease of the screw-in effect and torque did not occur in the last 1 mm from the apical terminus. Conclusions: The establishment of a larger glide path before NiTi rotary instrumentation appears to be appropriate for safely shaping the canal. It is recommended to establish #20 glide path with NiTi file when using ProTaper NiTi rotary instruments system safely.

Screw-in forces during instrumentation by various file systems

  • Objectives: The purpose of this study was to compare the maximum screw-in forces generated during the movement of various Nickel-Titanium (NiTi) file systems. Materials and Methods: Forty simulated canals in resin blocks were randomly divided into 4 groups for the following instruments: Mtwo size 25/0.07 (MTW, VDW GmbH), Reciproc R25 (RPR, VDW GmbH), ProTaper Universal F2 (PTU, Dentsply Maillefer), and ProTaper Next X2 (PTN, Dentsply Maillefer, n = 10). All the artificial canals were prepared to obtain a standardized lumen by using ProTaper Universal F1. Screw-in forces were measured using a custom-made experimental device (AEndoS-k, DMJ system) during instrumentation with each NiTi file system using the designated movement. The rotation speed was set at 350 rpm with an automatic 4 mm pecking motion at a speed of 1 mm/sec. The pecking depth was increased by 1 mm for each pecking motion until the file reach the working length. Forces were recorded during file movement, and the maximum force was extracted from the data. Maximum screw-in forces were analyzed by one-way ANOVA and Tukey's post hoc comparison at a significance level of 95%. Results: Reciproc and ProTaper Universal files generated the highest maximum screw-in forces among all the instruments while M-two and ProTaper Next showed the lowest (p < 0.05). Conclusions: Geometrical differences rather than shaping motion and alloys may affect the screw-in force during canal instrumentation. To reduce screw-in forces, the use of NiTi files with smaller cross-sectional area for higher flexibility is recommended.

Comparison of the cyclic fatigue resistance of VDW.ROTATE, TruNatomy, 2Shape, and HyFlex CM nickel-titanium rotary files at body temperature

  • Objectives: This study aims to compare the cyclic fatigue resistance of VDW.ROTATE, TruNatomy, 2Shape, and HyFlex CM nickel-titanium (NiTi) rotary files at body temperature. Materials and Methods: In total, 80 VDW.ROTATE (25/0.04), TruNatomy (26/0.04), 2Shape (25/0.04), and HyFlex CM (25/0.04) NiTi rotary files (n = 20 in each group) were subjected to static cyclic fatigue testing at body temperature (37℃) in stainless-steel artificial canals prepared according to the size and taper of the instruments until fracture occurred. The number of cycles to fracture (NCF) was calculated, and the lengths of the fractured fragments were measured. The data were statistically analyzed using a 1-way analysis of variance and post hoc Tamhane tests at the 5% significance level (p < 0.05). Results: There were significant differences in the cyclic fatigue resistance among the groups (p < 0.05), with the highest to lowest NCF values of the files as follows: VDW.ROTATE, HyFlex CM, 2Shape, and TruNatomy. There was no significant difference in the lengths of the fractured fragments among the groups. The scanning electron microscope images of the files revealed typical characteristics of fracture due to cyclic fatigue. Conclusions: The VDW.ROTATE files had the highest cyclic fatigue resistance, and the TruNatomy and 2Shape files had the lowest cyclic fatigue resistance in artificial canals at body temperature.

Cyclic fatigue resistance of the WaveOne Gold Glider, ProGlider, and the One G glide path instruments in double-curvature canals

  • Objectives: The aim of this study was to compare the cyclic fatigue resistance of the WaveOne Gold Glider, ProGlider and One G glide path instruments in artificial doublecurvature canals. Materials and Methods: This study included 15 WaveOne Gold Glider (size 15/0.08), 15 ProGlider (size 16/0.08), and 15 One G (size 16/0.06) nickel titanium files. The files were used in accordance with the manufacturer's instructions until they were broken in artificial double-curvature canals made of stainless steel. The time to fracture was recorded via a digital stopwatch and the number of rotations until fracture was also calculated. The data were statistically analyzed via the Kruskal-Wallis test. Results: The highest average number of rotations until fracture of the files was found for the WaveOne Gold Glider, followed by ProGlider and One G in order. Statistically significant differences were present between all groups of files (p < 0.05). Conclusions: In our study, the resistance of the WaveOne Gold Glider nickel-titanium (Ni-Ti) file to cyclic fatigue in S-shaped curved canals was found to be higher than that of the ProGlider and One G Ni-Ti files.

NiTi Rotary Instrumentation이 근관만곡도 변화에 미치는 영향 (THE EFFECT OF NiTi ROTARY INSTRUMENTATION ON THE CHANGE OF APICAL ROOT CANAL CURVATURE)

  • During cleaning and shaping of narrow and curved canals, it is very difficult or nearly impossible to maintain the original canal shape. Procedural accidents such as, ledge, zipping, perforation, and instrument breakage are frequently occurred and even may lead to failure of endodontic therapy. To prevent these kinds of accidents, various instrumentation techniques and materials have been introduced. Recently some nickel titanium (NiTi) files are introduced and it is reported that These NiTi files created rounder preparations with less transportation than conventional instruments in curved canals. This study compared the change of the canal curvature and procedural accidents after instrumentation produced by stainless steel K-flexo file, and NiTi rotary files (Profile 29 and Quantec 2000). Thirty narrow and curved canals (25-45 degree) of extracted human molars were randomly divided into three groups. In group 1, canals were instrumented using a step-back and watch-winding/pull motion with K-flexo files. In group 2, canals were prepared with Profile 29. Group 3, canals were prepared with Quantec 2000 files. Before and after preparation of canals, periapical radiographs were taken and scanned. The change of canal curvature were measured using Photoshop 4.0 program and the incidence of procedural accidents were also evaluated. The results were as follows: 1. All group showed some loss of canal curvature after instrumentation. 2. Average loss of canal curvature was $6.70{\pm}5.31$ degree for group 1, $3.80{\pm}2.57$ degree for group 2, and $5.40{\pm}4.83$ degree for group 3. All group There was significant change in curvature between before and after instrumentation (p<0.05). But there was no statistical difference amoung 3 groups. 3. In group I, there were no procedural accidents, such as ledging, perforation, or instrument fracture. In group 2, two cases of ledge and one case of instrument fracture were produced Goup 3, each one case of ledge, perforation and instrument fracture were occurred. Whthin the limits of above results, It seems that NiTi rotary instrumentation is not All Mighty and if we use uncarefully, it is more dangerous to produce some procedural accidents than conventional hand files. But more studies should be taken to evaluate the exact effects of NiTi rotary instrumentations.

Transparent resin block을 이용한 K3, NRT, PROFILE의 apical transportation 및 working length 변화양상의 비교 (Comparison of apical transportation and change of working length in K3, NRT AND PROFILE rotary instruments using transparent resin block)

  • 연구목적: 본 연구의 목적은 서로 다른 기하학적 형태를 가진 3가지 Ni-Ti 파일, 즉 K3, NRT, Profle을 사용하여 레진 블록으로 제작된 만곡 근관에서의 apical transportation 양상 및 working length 변화를 서로 비교하는 것이다. 연구 재료 및 방법: 실험 방법은 30개의 레진 블록을 Schneider technque을 사용하여 만곡도 측정 후 각 group당 임의로 10개씩 배분하였다. 이를 각 Ni-Ti 파일을 사용하여 crown down technique으로 근관 성형하였다. 이의 분석을 위해 double radiograph superimposition method (Backman CA 1992)를 사용하고, 분석의 정확성과 일관성을 위해 특별히 고안된 jig, digital X-ray 및 apical transportation의 측정 시 CAD/CAM software를 이용하였다. Apical transportation 양은 '근단공-0.5 mm'부위로부터 0, 1, 3, 5 mm에서 측정 하였으며, working length 변화는 근관 성형 전후의 working length 변화를 측정하였다. 또 통계 처리는 각 group의 apical transportation 양과 working length 변화를 Kruskal-Wallis One Way Analysis로 분석 후 다중 비교는 Mann-Whitney U-test를 사용하였다. 결과: 각 group간 working lengh 변화 및 0, 1, 3 mm 부위에서의 apical transportation 측정 값 사이에는 유의할만한 차이가 관찰되지 않았으나(p = 0.924), 5 mm 부위에서는 K3와 Profile사이에 유의할 만한 차이가 있었다(p = 0.027). 결론: 서로 다른 기하학적인 형태를 가진 3가지 Ni-Ti 파일의 apical transportation 및 working length 변화 정도 평가시, 본 연구의 범위 내에서는 3가지 파일 모두 유의할 만한 차이를 보이지 않은 가운데 본래의 근관 형태를 잘 유지한다고 할 수 있다.

Stainless Steel K-file, Profile .04와 Quantec LX를 이용한 만곡 근관 형성후의 근관형태의 변화에 관한 비교연구 (A Comparison of Stainless Steel K-file, Profile .04, and Quantec LX Instruments to Shape Curved Root Canals in vitro)

  • 목 적: 통상적인 근관성형 과정에서 근관형태의 직선화 경향을 발견할수 있으며 그 결과 만곡이 심한 근관에서 이상적인 근관 형태를 얻기가 어려움으로 인해 이를 극복하기 위한 많은 기구들과 근관 성형법들이 개선되었고 소개되었다. 본 연구에서는 수통형 Stainless Steel K-file, Niti 엔진 구동형 Profile .04와 Quantec LX file를 이용하여 만곡 근관의 성형 후 최종 근관 형태를 비교하고 평가하고자 한다. 방 법: 본 실험은 Bramante등의 방법을 변형하여 술 전의 근관 형태와 술 후의 근관 형태를 비교하였다. Schneider의 방법에 따라 $12^{\circ}$에서 $68^{\circ}$ 이내에 만독도를 가진 45개의 발거된 상하악 대구치의 근심근관들을 선택하여 15개씩 3개의 군으로 나누고 알루미늄으로 제작된 mold에 투명한 교정용 레진으로 매몰하였다. 근첨에서 2.5, 5, 8mm 지점에서 절단하고 각 mold에 재조립한 후 다음과 같이 근관 성형을 시행하였다. 제 1 군은 SS K-file를 이용하여 Step-back 방법; 제 2 군은 NiTi 엔진 구동형인 Profile .04 ; 제 3 군은 NiTi 엔진 구동형인 Quantec LX file로 근관 성형하였다. 술 전과 술 후에, 각 시편들을 입체 현미경으로 사진 촬영하여 근관 중심 위치 이동률, 근관성형 후 면적과 모양, 잔존 상아질의 최 소 두께를 Sigma scan / image software program으로 계산하고 One way ANOVA로 통계적 유의성을 검증하였다. 결 론: 1. Profile .04와 Quantec LX는 SS K-file보다 근관성형시 근관의 본 형태를 유지하는 경향이 있었으나 통계적으로 유의성이 없었다(p>0.05). 2. 근관 성형 후 면적은 Profile .04 엔진 구동형 NiTi file를 이용한 군이 다른 군과 비해 가장 적었으나 통계적으로 유의성이 없었다(p>0.05). 2. 모든 방법들은 같은 부위에서 같은 방향으로 전이되는 양상을 보였다. 즉, 근단부에서는 바깥쪽으로, 중앙부에서는 안쪽으로 전이하려는 경향이 있었다. 그러나, 치관부에는 그러한 법칙이 적용되지 않아 전이되는 양상이 안쪽이나 바깥쪽으로 구별되지 않게 일어났다. 3. 술 후에 근관의 모양은 원형, 타원형, 불균일한 형태들이 다양하게 나타났지만, Profile .04와 Quantec LX를 사용했을 때 주로 원형 형태의 근관을 보여주었으며 Stainless Steel K-file은 타원형이나 불균일한 근관 형태를 보였다.

Preference of undergraduate students after first experience on nickel-titanium endodontic instruments

  • Objectives: This study aimed to compare two nickel-titanium systems (rotary vs. reciprocating) for their acceptance by undergraduate students who experienced nickel-titanium (NiTi) instruments for the first time. Materials and Methods: Eighty-one sophomore dental students were first taught on manual root canal preparation with stainless-steel files. After that, they were instructed on the use of ProTaper Universal system (PTU, Dentsply Maillefer), then the WaveOne (WO, Dentsply Maillefer). They practiced with each system on 2 extracted molars, before using those files to shape the buccal or mesial canals of additional first molars. A questionnaire was completed after using each file system, seeking students' perception about 'Ease of use', 'Flexibility', 'Cutting-efficiency', 'Screwing-effect', 'Feeling-safety', and 'Instrumentation-time' of the NiTi files, relative to stainless-steel instrumentation, on a 5-point Likert-type scale. They were also requested to indicate their preference between the two systems. Data was compared between groups using t-test, and with Chi-square test for correlation of each perception value with the preferred choice (p = 0.05). Results: Among the 81 students, 55 indicated their preferred file system as WO and 22 as PTU. All scores were greater than 4 (better) for both systems, compared with stainless-steel files, except for 'Screwing-effect' for PTU. The scores for WO in the categories of 'Flexibility', 'Screwing-effect', and 'Feeling-safety' were significantly higher scores than those of PTU. A significant association between the 'Screwing-effect' and students' preference for WO was observed. Conclusions: Novice operators preferred nickel-titanium instruments to stainless-steel, and majority of them opted for reciprocating file instead of continuous rotating system.

Evaluation of apical canal shapes produced sequentially during instrumentation with stainless steel hand and Ni-Ti rotary instruments using micro-computed tomography

  • I. Objectives Endodontic success depends on thoroughly cleaned and completely obturated root canal system. Effective cleaning and obturation will be achieved by well shaped canal. Numerous methodologies evaluating the efficacy and safety of canal preparation has been developed and the use of micro-computed tomography(MCT) in endodontic research is one of the latest innovations. This scientific tools could overcome the inherent limitations of other methodologies, and possesses the ability to visualize morphological characteristics in a detailed and accurate manner without destruction of the tooth and offers reproducible data in all three dimensions. The purpose of this study was to determine the optimal master apical file size with less transportation and more efficiency in removing the infected dentin. For this purpose we evaluated the transportation of canal center and change of untouched area after preparation sequentially from #25 file through #40 file with 3 different instruments:Stainless steel(SS) K-type hand instruments(MANI, Japan), ProFile.04 instruments (Dentply Tulsa Dental, USA) and Lightspeed instruments(Lightspeed Technology, San Antonio, USA) using micro-computed tomography.(omitted)

근단부 성형 크기에 따른 다양한 전동 니켈티타늄 파일의 중심 변위율 및 만곡도 감소 비교 (Comparison of the centering ratio and canal curvature reduction according to the apical preparation size using various NiTi rotary instruments)

  • The purpose of this study was to compare the centering ratio and reduction of canal curvature according to the preparation sizes of #30, #40 and #50 using three rotary NiTi instruments which have different shaft tapers. Seventy-two simulated root canals in clear resin blocks (Endo Training Bloc; Dentsply Maillefer, Ballaigues, Switzerland) were divided as following 3 groups according to the file system; the 24 canal blocks prepared with each of ProTaper Universal system (Group P), LightSpeed eXtra system (Group L), and K3 (Group K). The pre- and post-instrumented root canals were scanned and superimposed to evaluate and calculate the centering ratio and reduction of canal curvature. Mean scores of each group were statistically analyzed using one-way ANOV A and Duncan's multiple range test for post-hoc comparison. The results were as followings: 1. Group L showed better centering ratio, followed by K and P. And all experimental groups generally showed increasing tendency of centering ratio as the apical size was increasing from #30 to #50, except at 1 mm level of group P where showed reducing tendency of centering ratio. The smaller the ratio, the better the instrument remained centered in the canal. 2. Group P showed more decrease of canal curvature at all apical shaping size (p < 0.05). Under the conditions of this study, the shaft design could affect the quality of canal shaping and the smooth taperless flexible (LightSpeed) shaft design was capable of preparing canals with good morphological characteristics in curved canals.
