Rheumatiod Arthritis: An Updated Overview of Latest Therapy and Drug Delivery

  • Rheumatoid arthritis is a severe autoimmune disorder, related to joints. It is associated with serious cartilage destruction. This causes disability and reduces the excellence of life. Numerous treatments are existed to combat this disease, however, they are not very efficient and possess severe side effects, higher doses, and frequent administration. Therefore, newer therapies are developed to overcome all these limitations. These include different monoclonal antibodies, immunoglobulins, small molecules used for immunotherapy and transgenes for gene therapy. One of the main goals of these new generation therapeutics is to address the underlying distressing biological processes by specifically targeting the causative agents with fewer systemic side effects and greater patient console. It is very fortuitous that loads of progressive investigations are going on in this field and many of them have entered into the successful clinical trial. But till date, a limited molecule has got FDA clearance and entered the market for treating this devastating disease. This review highlights the overview of conventional therapy and advancements in newer therapeutics including immunotherapy and gene therapy for rheumatoid arthritis. Further, different novel techniques for the delivery of these therapeutics of active and passive targeting are also described.

  • Although various criteria on the diagnosis of diabetic neuropathy are applied from trial to trial, being tailored in concert with its purpose, the utmost evidences of the diagnosis are subjective symptoms and objective signs of neurologic deficit. The application and interpretation of auxiliary electrophysiological test including nerve conduction study (NCS) should be made on the context of clinical pictures. The evaluation of the functions of small, thinly myelinated or unmyelinated nerve fibers has been increasingly stressed recently with the advent of newer techniques, e.g., measurement of intraepidermal fiber density, quantitative sensory testing, and autonomic function test. And the studies with those techniques have shed light to the nature of the evolution of diabetic neuropathy. The practical application of these techniques to the diagnosis of diabetic neuropathy in the individual patients, however, should be made cautiously due to several shortcomings: limited accessibility, wide overlapping zone between norm and abnormality with resultant unsatisfactory sensitivity and specificity, difficulty in performing subsequent tests, unproven quantitative correlation with clinical deficit, and invasiveness of some technique. NCS, as an extension of clinical examination, is still the most reliable electrophysiological test in evaluating neuropathy and gives the invaluable information about the nature of neuropathy, whereas the newer techniques need more refinement of the procedure and interpretation, and the accumulation of large scaled data of application to be considered as established diagnostic tools of peripheral neuropathy.

Applicability of Existing Fracture Initiation Models to Modern Line Pipe Steels

  • The original fracture criteria developed by Maxey/Kiefner for axial through-wall and surface-cracked pipes have worked well for many industries for a large variety of relatively low strength and toughness materials. However, newer line pipe steels have some unusual characteristics that differ from these older materials. One example is a test data that has demonstrated that X80 line-pipe with an axial through-wall-crack can fail at pressures about 30 percent lower than predicted with commonly used analysis methods for older steels. Thus, it is essential to review the currently available models and investigate the applicability of these models to newer high-strength line pipe materials. In this paper, the available models for predicting the failure behavior of axial-cracked pipes (through-wall-cracked and external surface-cracked pipes) were reviewed. Furthermore, the applicability of these models to high-strength steel pipes was investigated by analyzing limited full-scale pipe fracture initiation test results. Based on the analyzed results, the shortcomings of the available models were identified. For both through-wall and surface cracks, the major shortcomings were related to the characterization of the material toughness, which generally leads to non-conservative predictions in the J-T analyses. The findings in this paper may be limited to the test data that were consider for this study. The requisite characteristics of a potential model were also identified in the present paper.

  • Lung cancer is a serious health problem and leading cause of death worldwide due to its high incidence and mortality. More than 80% of lung cancers feature a non-small cell histology. Over few decades, systemic chemotherapy and surgery are the only treatment options in this type of tumor but due to their limited efficacy and overall poor survival of patients, there is an urge to develop newer therapeutic strategies which circumvent the problems. Enhanced knowledge of translational science and molecular biology have revealed that lung tumors carry diverse driver gene mutations and adopt different intracellular pathways leading to carcinogenesis. Hence, the development of targeted agents against molecular subgroups harboring critical mutations is an attractive approach for therapeutic treatment. Targeted therapies are clearly more preferred nowadays over systemic therapies because they target tumor specific molecules resulting with enhanced activity and reduced toxicity to normal tissues. Thus, this review encompasses comprehensive updates on targeted therapies for the driver mutations in non-small cell lung cancer (NSCLC) and the potential challenges of acquired drug resistance faced i n the field of targeted therapy along with the imminent newer treatment modalities against lung cancer.

  • Membrane technology has revolutionized the dairy sector. Different types of membranes are used in the industry for various purposes like extending the shelf life of milk without exposure to heat treatment, standardization of the major components of milk for tailoring new products as well increasing yield and quality of the dairy products, and concentrating, fractionation and purification of milk components especially valuable milk proteins in their natural state. In the cheese industry, membranes increase the yield and quality of cheese and control the whey volume, by concentrating the cheese milk. With the advancement of newer technology in membrane processes, it is possible to recover growth factor from whey. With the introduction of superior quality membranes as well as newer technology, the major limitation of membranes, fouling or blockage has been overcome to a greater extent.

Unlinked and Convertible Total Elbow Arthroplasty (비연결형 및 전환형 주관절 전치환술)

  • Designs of total elbow arthroplasty have been evolving with clinical experiences. Newer implants are expected to resolve current limitations and improve long term outcomes. This review article focuses on the basic knowledge of unlinked and convertible total elbow arthroplasty. There have been a variety of designs of unlinked total elbow prostheses. Some implants are still used in the market, while others are no longer commercially available. Modified and newer designs include more congruent contact surface, stemmed implant, and radiocapitellar arthroplasty. Two convertible elbow prostheses have been developed, and one implant is currently available in Korea. Conversion from an unlinked to a linked mode is performed by adding a linking cap. Unlinked total elbow arthroplasty, which restores native elbow kinematics, has a biomechanical rationale of lowering polyethylene wear and loosening of implants. It can be indicated in younger and higher demand patients, who have adequate bone stock and soft tissues. Convertible total elbow arthroplasty broadens implant selection and simplifies revision surgery. These newer prostheses possibly improve the long term outcomes and resolve disadvantages of linked prostheses in total elbow arthroplasty.

  • Since traditional static metadata set like Dublin Core has static metadata attributes about bibliography information, integration of metadata for various metadata, problems about standard and extension of metadata must be considered for applying it to new environment. Specially, as event-driven metadata write method included the notion of e-commerce come out for interoperability in digital libraries, traditional metadata management which cannot distinguish between different kinds of update operations to new extension of metadata set occurs unsuitable waiting of update operation. So, improvement is needed about it. In this paper, we show whether alleviative transaction consistency can be applied to digital library or not. Also it would divide newer metadata into static metadata attribute connected in read operation within user read-only transaction and dynamic metadata attribute in update operation within dynamic(e-commerce) update transactions. We propose newer metadata management algorithm considered in classfication of metadata attributes and dynamic update transaction. Using two version for minimal maintenance cost and ARU(Appended Refresh Unit) for dynamic update transaction, to minimize conflict between read and write operations shows fast response time and high recency ratio. As a result of the performance evaluation, we show our algorithm is proved to be better than other algorithms in newer metadata environments.

  • In this paper we will first give a short overview of the more recent developments in MF, UF and RO. This is followed by a closer look on newer technologies applied in environmental problems. The applications looked an are the recovery of organic components from solvent laden gas streams and the separation of organic volatiles from aqueous waste waters via pervaporation. Technical solutions, the advantages and disadvantages of the processes and, where possible, cost estimations will be presented.

Discovery of Novel 1,4-Benzodioxane Containing Thiazolidinone Derivatives as Potential Antihepatotoxic Agent

  • In continuance of our search for newer antihepatotoxic agents some novel thiazolidinone derivatives containing 1,4-benzodioxane ring system were synthesized starting from 2,3-dihydro-1,4-benzodioxane-2-carbohydrazide. The synthesized compounds were evaluated for antihepatotoxic activity against $CCl_4$-induced hepatotoxicity in rats. Among them some compounds have shown significant antihepatotoxic activity comparable to standard drug silymarin.
