A Case of Coil Embolization in a 15-year-old Child with Pulmonary Sequestration (코일 색전술로 치료한 15세 소아의 폐 격리 1례)

  • Pulmonary sequestration is a rare congenital mass of nonfunctional pulmonary tissue that lacks a normal-connection with the bronchial tree or the pulmonary arteries. It is clinically asymptomatic but when it is complicated with recurrent infection, it needs to be treated. Conventionally, surgical removal was recommended, but these days we are trying new and less invasive techniques, such as arterial embolization. There were several reports about successful cases of pulmonary sequestration treated by embolization, but mostly all of them were done to newborns or infants. We report a case of a 15-year-old boy with an asymptomatic pulmonary sequestration on whom was performed coil embolization, and in the follow-up computed tomography(CT), the size of the lesion was decreased. He did not suffer severe respiratory symptoms.

Evaluation of three glucometers for whole blood glucose measurements at the point of care in preterm or low-birth-weight infants

  • Purpose: We evaluated three blood glucose self-monitoring for measuring whole blood glucose levels in preterm and low-birth-weight infants. Methods: Between December 1, 2012 and March 31, 2013, 230 blood samples were collected from 50 newborns, who weighed, ${\leq}2,300g$ or were ${\leq}36$ weeks old, in the the neonatal intensive care unit of Eulji University Hospital. Three blood glucose self-monitoring (A: Precision Pcx, Abbott; B: One-Touch Verio, Johnson & Johnson; C: LifeScan SureStep Flexx, Johnson & Johnson) were used for the blood glucose measurements. The results were compared to those obtained using laboratory equipment (D: Advia chemical analyzer, Siemens Healthcare Diagnostics Inc.). Results: The correlation coefficients between laboratory equipment and the three blood glucose self-monitoring (A, B, and C) were found to be 0.888, 0.884, and 0.900, respectively. For glucose levels ${\leq}60mg/dL$, the correlation coefficients were 0.674, 0.687, and 0.679, respectively. For glucose levels>60 mg/dL, the correlation coefficients were 0.822, 0.819, and 0.839, respectively. All correlation coefficients were statistically significant. And the values from the blood glucose self-monitoring were not significantly different from the value of the laboratory equipment, after correcting for each device's average value (P>0.05). When using laboratory equipment (blood glucose ${\leq}60mg/dL$), each device had a sensitivity of 0.458, 0.604, and 0.688 and a specificity of 0.995, 0.989, and 0.989, respectively. Conclusion: Significant difference is not found between three blood glucose self-monitoring and laboratory equipment. But correlation between the measured values from blood glucose self-monitoring and laboratory equipment is lower in preterm or low-birth-weight infants than adults.

A Correlation Study of Maternal Stress, Anxiety, and Perception of the Newborn in the Early Postpartum Stage (산욕 초기 어머니의 스트레스, 불안 및 신생아 지각 간의 관계)

  • The purpose of this study was to contribute to maternal nursing in early postpartum stage and to neonatal nursing. Data were collected through self-report questionnaires which were constructed to include parental role stress scale, state-trait anxiety scale, and perception of the newborn scale. The subjects consisted of 100 mothers in the early postpartum stage at three hospitals in the Kyoung-In area, from November 8 to December 26, 1997. The data were analyzed by an SPSS program. The results are as follows ; 1. The mean of parental role stress of mothers in the early postpartum stage was 10.70$\pm$2.63. The means of state anxiety and trait anxiety of mothers were 36.29$\pm$8.45 and 38.53$\pm$8.36. The mean of perception of the newborn was 2.65$\pm$5.05, and 59% of mothers rated their newborn as better than the average newborn. 2. The level of parental role stress correlated to the level of state anxiety and trait anxiety. The level of state anxiety and trait anxiety were also related. The level of perception of the newborn was related to the level of state anxiety and trait anxiety. 3. Mothers who did not want the pregnancy, whose newborns were girls, and who already had one child had higher state anxiety than those who did not. Mothers who already had one child, and whose newborn had no specific signs had higher trait anxiety than those who did not. Mothers who professed a religions had a higher perception of the newborn than those who did not. The above findings indicate that the levels of parental role stress, state anxiety, trait anxiety and perception of the newborn of mothers in early postpartum stage were correlated. Therefore nursing intervention for reducing stress and anxiety, and improving perception of the newborn should be provided for mothers in early postpartum stage.

The Effect of the Educational Program on Caring-Confidence and Rearing Attitude in Mothers with Newborns (신생아 돌보기 교육이 어머니의 자신감과 양육태도에 미치는 효과)

  • The purpose of this study was to find whether the educational program contributed to the increase of caring confidence and rearing attitude of the newborn mothers. The subjects for this study were 40 newborn mothers-20 control group and 20 experimental group-who delivered a normal neonate and were gotten general informations from the nurses at the K medical center in Seoul. Especially, the experimental group was educated for 30-40 minutes in maternal educational program through lectures, demonstrations, discussion with textbooks and visual materials by a researcher at 2-3 days after delivery. The data were collected two times-at 2-3 days after delivery and at 4 weeks after discharge-from October, 1998 to January, 1999 by investigators interviewing with a structured questionnaire. Pharis(1978)'s self confidence scale was used after some modification to measure caring confidence and Cohler's maternal attitude scale to check mother's rearing attitude. Analysis of data was done by using of frequency, %, x²-test, Independent sample t-test. The results of this study were as follows ; 1. The Confidence of the experimental group was significant higher than the control group (t=2.519, p=.016) and the rearing attitude changed more positively(t=5.545, p=.000). 2. In the confidence scale, the statements having a significant change were holding a baby, changing diapers, feeding, and caring umblicus. 3. In the rearing attitude scale, the statements having a significant change were 12 of 20 statements.

Concept Analysis of Developmental Care for Preterm Infants: Hybrid Model (미숙아 발달지지간호에 대한 개념분석: 혼종모형)

  • Purpose: The purpose of this study was to define and clarify the concept of developmental care for preterm infants. Methods: The hybrid model method was used to identify the main attributes and indicators. In the field work stage, data were collected in Seoul and Cheonan, Korea. The participants were 5 nurses working in the NICU. Results: The concept of developmental care was found to have six attributes and ten indicators in 2 dimensions. For the nursing practice dimension, four attributes were derived. They were being like intrauterine state, individualization, interaction, and integrative care with awareness of development. For the family centeredness dimension, supporting parental attachment and professional alliance were attributes of developmental care. Conclusion: Developmental focused care can facilitate the identification of behavioral responses of newborns and provide individualized interventions for fostering growth and development. This concept analysis could provide guidelines for "developmental care" nursing practice and be useful for research in the neonatal field.

Analysis of Characteristics of Peripheral Arterial Ischemia in Premature Babies and Effects of Nitroglycerin Patch Application

  • Purpose: The aim of this retrospective study was to analyze the characteristics of peripheral arterial ischemia and tissue necrosis in premature babies, as well as the effects of nitroglycerin. Methods: In total, 513 newborns were enrolled who were admitted to the neonatal intensive care unit with a gestational age of 34 weeks or younger. Data were collected on general personal and clinical information, peripheral arterial ischemia, and nitroglycerin patch application in the premature infants. The collected data were analyzed using the χ2 test, t-test, Mann-Whitney U test, logistic regression. Results: Thirty-six (7.0%) infants had peripheral arterial ischemia, while 477 (93.0%) infants did not. Lower gestational age (χ2=35.97, p<.001), lower birth weight (χ2=29.40, p<.001), lower blood pressure (χ2=23.10, p<.001), and insertion of an umbilical artery catheter (p<.001) were significantly associated with the occurrence of peripheral arterial ischemia. Among the preterm infants in whom nitroglycerin patches were applied, 30 (83.3%) premature infants without necrosis improved without complications, 4 (11.1%) showed hypotension, and 2 (5.6%) showed skin damage. Conclusion: Based on a review of our experiences with nitroglycerin patches, we recommend closely observing skin color and using nitroglycerin patches on the skin to help improve flow in premature infants with peripheral arterial ischemia.

A study on the latest trend of researches concerning recurrent pregnancy loss, and controlled experiment on animals about antiphospholipid antibody (습관성 유산에 대한 최신 연구 경향 및 실험적 방법 연구)

  • Objectives : Recurrent pregnancy loss occurs in approximately 0.5-3% of women. There are many studies concerning immunological factor recently. Therefore, aim of this study is to examine the latest trend of researches concerning recurrent pregnancy loss, and controlled experiment on animals about antiphospholipid antibody. Method : We referred Pubmed site by using searching word of 'recurrent pregnancy loss' (Limits : 2000.1-2004.3, animal) Results and conclusions : 1. We searched 29 papers. Immunological factor : 18 ( about antiphospholipid antibody : 10 ), Chromosomal abnormality : 6, The others : 5 2. In five papers about controlled experiment on animals, (1) Materials : 8-12 weeks old mice / 11.5-day old-rat embryos / New Zealand rabbit (2) Inductions : inject intraperitoneally with human IgG containing antiphospholipid antibodies / culture in a solution of 1 ml medium which contained IgG purified from sera of women / inject intradermally with cardiolipin (3) Treatments : inject intraperitoneally with complement component before / culture in a solution of 1 ml medium which contained IgG purified from sera of women with SLE and RPL or from healthy women / inject intradermally with TFX, 0.9% NaCl (4) Measurements : weight fetuses and placentas, calculate frequency of fetal resorption / after culture, examine the embryos / examine platelet counts, APTT and numbers of live and dead newborns, resorbed fetuses, body mass, newborn viability and survival rates.

Mortality Analysis of Surgical Neonates: A 20-year Experience by A Single Surgeon (신생아 외과 환자의 수술 후 사망률 변화에 대한 연구)

  • Pediatric surgery could establish a definitive position in the medical field on the basis of a stable patient population. Neonatal surgery, the core of pediatric surgery, requires highly skilled surgeons. However, recent advancement of prenatal diagnosis followed by intervention and decreased birth rate has resulted in a significant decrease in the neonatal surgical population and the number of surgical operations. The purpose of this study is to examine the outcome of neonatal surgeries and to propose a guide for the future surgeries. A total of 359 neonatal surgical patients operated upon at the Department of Surgery, Ewha Medical Center, during past 21 years were studied. The study period hasbeen divided into two time periods: from 1983 to 1993 and from 1994 to 2004. Analysis was based on the Clinical Classification System and mortality pattern, frequency of disorders, occurrence and cause of death, and other changes. Neonatal surgery was 6.4 % of all pediatric surgery during the total 21 year period, 9.9 % in the first period and 4.8 % in the second. Male to female ratio increased from 2.7:1 to 2.1:1. The overall mortality was 6.7 %, and there was significant decrease from 7.4 % in the first period to 6.0 % in the second. The clinical classification system (CCS) for death cases included class II 2, III 4, and IV 7 during the first period and class III 3, and IV 8 during the second, respectively. According to the mortality pattern by Hazebroek, there were 6 preventable death cases during the first period, and only one in the second, and 2 non-preventable death cases during the first period and 8 in second, respectively. Although the patients in the second period had more serious diseases, surgical mortality has been decreased in the second period, which may be the result of improved surgery methods for newborns and advanced patient care.

Hypothermia and Related Factors in High-Risk Infants (고위험신생아의 저체온증 현황 및 관련요인)

  • Purpose: Maintaining body temperature is a key vital function of human beings, but little is known about how body temperature of highrisk infants is sustained during early life after birth. The aim of this study was to describe hypothermia in high-risk infants during their first week of life and examine demographic, environmental, and clinical attributors of hypothermia. Methods: A retrospective longitudinal study was done from January 1, 2013 to December 31, 2015. Medical records of 570 high-risk infants hospitalized at Neonatal Intensive Care Units (NICU) of a university affiliated hospital were examined. Body temperature and related factors were assessed for seven days after birth. Results: A total of 336 events of hypothermia (212 mild and 124 moderate) occurred in 280 neonates (49.1%) and most events (84.5%) occurred within 24 hours after birth. Logistic regression analysis revealed that phototherapy (aOR=0.28, 95% CI=0.10-0.78), Apgar score at 5 minute (aOR=2.20, 95% CI=1.17-4.12), and intra-uterine growth retardation or small for gestational age (aOR=3.58, 95% CI=1.69-7.58) were statistically significant contributors to hypothermia. Conclusion: Findings indicate that high-risk infants are at risk for hypothermia even when in the NICU. More advanced nursing interventions are necessary to prevent hypothermia of high-risk infants.

Combined Cytogenetic and Molecular Analyses for the Diagnosis of Prader-Willi/Angelman Syndromes

  • Prader-Willi (PWS) and Angelman (AS) are syndromes of developmental impairment that result from the loss of expression of imprinted genes in the paternal (PWS) or maternal (AS) 15q11-q13 chromosome. Diagnosis on a clinical basis is difficult in newborns and young infants; thus, a suitable molecular test capable of revealing chromosomal abnormalities is required. We used a variety of cytogenetic and molecular approaches, such as, chromosome G banding, fluorescent in situ hybridization, a DNA methylation test, and a set of chromosome 15 DNA polymorphisms to characterize a cohort of 27 PWS patients and 24 suspected AS patients. Molecular analysis enabled the reliable diagnosis of 14 PWS and 7 AS patients, and their classification into four groups: (A) 6 of these 14 PWS subjects (44%) had deletions of paternal 15q11-q13; (B) 4 of the 7 AS patients had deletions of maternal 15q11-q13; (C) one PWS patient (8%) had a maternal uniparental disomy (UPD) of chromosome 15; (D) the remaining reliably diagnoses of 7 PWS and 3 AS cases showed abnormal methylation patterns of 15q11-q13 chromosome, but none of the alterations shown by the above groups, although they may have harbored deletions undetected by the markers used. This study highlights the importance of using a combination of cytogenetic and molecular tests for a reliable diagnosis of PWS or AS, and for the identification of genetic alterations.
