The quality of mother-infant interaction during early postpartum period has a significant influence on the child growth and development in many aspects. The mother's initial perception of the newborns might have the lasting influence on the development of the mother-infant attachment and mother-infant relationship. Therefore the proper mother-infant interaction should be developed after infant birth. However, it is acknowledged that the high risk infants influence on the mothers' perception because of their abnormalities or disorders of physical condition and the restricted maternal contacts and interactions. The object of this study is to identify the difference of mother's perception of their normal and high-risk newborn during the early postpartum period. The ultimate goal is to contribute to use this basic data to develop nursing intervention toward the promotion of healthy mother-infant relationship and the helping of growth and development of children. The data were collected for this study from Sep. 21, 1990 to Oct. 1. 1991 at E University Hospital. The sample was 83 of mothers who had normal newborns and 73 mothers who had high risk newborns .The instrument was Neonatal Perception Inventory(NPI) designed by the Broussard for the measurement of mothers' perception of newborns about Average Baby and Your Baby. The data were analysed by using an SPSS Program and include percentage mean, SD, t-test, ANOVA. The results of this study are as follows : 1. The normal newborn mothers' perception is more positive than the high-risk newborn mothers(t=7.94, p=0.000). 2. Mothers' perception of the newborns is not related significantly to mothers' general characteristics. In conclusion, in order to promote positive, healthy mother-infant relationship, the nurse need to support, give information, and educate high-risk newborn mothers through early nursing assessment.
The purposes of this study are to assess iron status in mothers and their newborn infants at birth and to analyze the influence of maternal iron status on their newborn babies. Venous bloods samples were drawn from 144 pregnant women just before delivery and cord bloods of their newborn babies were collected immediately after birth for measurement of hemoglobin, hematocrit, serum iron, ferritin, total binding capacity and transferrin saturation. The values of hemoglobin and hematocrit were significantly lower in the mothers(10.9$\pm$1.43g/dl and 33.7$\pm$3.67%) than in their newborn infants(14.7$\pm$1.43g/dl and 45.3$\pm$4.76%)(p<0.0001). At delivery, serum iron levels in cord blood were about twice as high as those in the maternal blood, and serum ferritin levels in the cord blood were about four times higher than those in the maternal blood. The serum ferritin levels of multigravidas were higher than those of primigravidas,. but there was no difference between the serum ferritin levels of their infants. The serum ferritin levels of the mothers and their infants were higher in maternal group with iron supplement regularly than in other maternal group without iron supplement during pregnancy. Among the mothers, 26.4% had a serum ferritin levels below 12ng/ml(i.e. depleted iron stores)and 78.9% had a hemoglobin below 12g/dl(i.e.iron deficient anemia). When the maternal group was classified according to their serum ferritin levels by 9ng/ml, 12ng/dl or 20ng/ml, there was no significant difference in the iron status of their newborn infants among the three groups. The hemoglobin and serum ferritin levels of the mothers were well correlated with those of their babies. The maternal hemoglobin values negatively correlated with infant birth weight. It is possible that the demands of iron of the mother might be increased in the case of a newborn infant of greater size. The results of this study provide useful information regarding establishment of RDA for iron in pregnant women and guidance about the need for iron supplement during pregnancy.
A maternity ward in a hospital in Seoul has an educational program for postpartal mothers since its opening, but evaluation on the program has not been done. This study was to find whether the educational program contributed to increase of knowledge and confidence of the post-partal mothers in the area of postpartal self-care and the newborn baby care or not. This study aimed at improvement of the educational program for posrpartal mothers and the newborn babies. Subjects were 40 primiparae who were admitted to the obstetric ward in a general hospitals in Seoul from August 20th to September 10th, 1995. Subjects were those who had no labor pain at the admission time, had no complications during labor and delivery, gave a birth to a healthy baby, and agreed to participate in this study. All subjects were well educated and were in well-to-do group. Most of them received antenatal care adequately. They were tested on knowledge and self-confidence in the area of postpartal self-care and infant care two times, one at the admission time and the other prior to discharge. After the first test, nurses in a maternity ward and nursery taught them on postpartal self-care and infant care. Education consisted of a 1 hour planned program and incident teachings done at bed side. Four tools were developed by authors based literature review. The test tool for knowledge of postpartal self-care consisted of 15 items which included the definition of postpartum, dangerous symptoms in postpartum, lochia, time to begin coitus, postpartum exercise, sitz-bath, and perineal care. The test tool for self-confidence in postpartal self-care included such items as emergency care on dangerous symptoms in postpartum, sexual life and contraception after postpartum, self-confidence in postpartum exercise and perineal care. The test tool for knowledge of newborn baby care consisted of 18 items which included bathing, umbilical cord care, vaccination, breast feeding, abnormal symptoms of neonate. The test tool for self confidence in the newborn baby care included umbilical cord care, vaccination, breast feeding, emergency care for baby. Analysis of demographic data were analyzed with calculation of percentage. Score differences between the first test and the second test were analyzed with paired t-test. SAS / PC (Release 6.04 for DOS) program are as follows. 1. Mothers' knowledge of postpartal self-care increased significantly after education (t=10.04, df=38, p=0.0001). 2. Mothers' self-confidence in postpartal self-care increased significantly after education (t=6.53, df=38, p=0.0001). 3. Mother's knowledge of the newborn abay care increased significantly after education (t=9.74, df=38, p=0.0001). 4. Mothers' self-confidence in the newborn baby care increased significantly after education(t=8.22, df=38, p=0.0001) Suggestions for further studies and nursing practice were as follows. 1. We suggest studies by randomized control-group pretest-posttest design or nonequivalent control group pretest-posttest design will be done. 2. We suggest follow-up studies to find if mothers's confidence will last or not after discharge. 3. We suggest general hospitals to establish a phone-counseling system.
This survey was performed to measure concentrations of airborne microbes, utilizing the six-stage cascade air sampler, according to the sampling site and time in the general hospital and to evaluate the degree of correlations between airborne microbes and environmental factors. Besides the main lobby, airborne microbes and environmental factors were not significantly different between the forenoon(9:00-10:00 AM) and afternoon(2:00-3:00 PM). Concentrations of airborne bacteria and fungi were 404 and $156CFU/m^3$ in the main lobby, 188 and $56CFU/m^3$ in the intensive care unit, 323 and $96CFU/m^3$ in the ward, 239 and $127CFU/m^3$ in the laboratory, and 139 and $33CFU/m^3$ in the newborn baby room, respectively. As a result, the level of airborne microbes was significantly highest in the main lobby and lowest in the newborn baby room(p<0.05). Outdoor airborne microbes concentrations were significantly lower than those of the sampling sites in the general hospital except for the newborn baby room(p>0.05). It was observed that temperature, relative humidity and carbon dioxide in the general hospital had generally positive correlation with airborne microbes. However, there was no correlation between the odor index level and airborne microbes.
Purpose: This study aimed to examine the effect of a newborn care education program using ubiquitous learning (UL-NCEP) on exclusive breastfeeding and maternal role confidence of first-time mothers in Vietnam. Methods: This quasi-experimental study with a nonequivalent control group design was conducted at a university hospital in Hue city, Vietnam, between June and July 2018. Eligible first-time mothers were conveniently allocated to the experimental (n=27) and the control group (n=25). Mothers in the control group received only routine care, whereas mothers in the experimental group received UL-NCEP through tablet personal computers in addition to routine care in the hospital. Then, the educational content was provided to mothers by their smartphone for reviewing at home. UL-NCEP was developed based on the World Health Organization's "Essential Newborn Care Course" guidelines. The exclusive breastfeeding rate and maternal role confidence level after birth and at 4 weeks postpartum were assessed in both groups to assess the effect of UL-NCEP. Results: At 4 weeks postpartum, the experimental group showed a significantly higher level than the control, for exclusive breastfeeding rate (p<.05) as well as mean maternal role confidence (p<.05). Conclusion: UL-NCEP was a feasible and effective intervention in increasing first-time Vietnamese mothers' exclusive breastfeeding rate and maternal role confidence level. This program may be integrated into routine care for postpartum mothers to promote mother and infant health among first-time mothers in Vietnam.
Purpose: In order to identify the usability of inguinal temperature in a neonatal unit, this study was done to compare inguinal temperature of newborns with rectal, axillary, and tympanic temperatures. Method: Fifty-one normal newborns admitted to the nursery of a university affiliated hospital participated in the study. Tympanic temperatures were taken with a tympanic thermometer. Inguinal, rectal, and axillary temperatures were taken with glass mercury thermometers, and were recorded every 1 minute until the reading remained constant for 2 times. The data were analyzed using the SPSS program. Result: The measurement time for inguinal temperatures in newborns was significantly longer than that for rectal temperatures, but was shorter than that for axillary temperatures. The mean temperature for the newborn's inguinal site was lower than for rectal, axillary, and tympanic temperatures (the lower side), but wasn't different from tympanic temperature (the upper side). The inguinal temperature was significantly correlated with rectal, axillary, and tympanic temperature. The inguinal temperature was not different according to general characteristics of the newborn. Conclusion: These findings indicate that measurement of inguinal temperature is a useful alternative to rectal temperature.
Purpose: The purpose of this study was to compare the nursing needs of parturient women with those of nurses during the perinatal period. Method: The subjects of this study were in two groups, one of which was composed of 244 postpartum women (vaginal delivery) while the other was composed of 144 nurses. The data was collected using a self-report Questionnaire between July and November of 2007, and was analyzed using the SPSS Win 12.0 program. Result: Statistically significant differences were found in nursing needs, specifically in the aspects of physical care, safety of delivery and newborn care between the nurse group and the parturient women group during the perinatal period. Furthermore, the nursing needs of parturient women were higher than those of nurses with regard to environmental needs, safety of delivery, newborn care, and especially among items such as routine examinations and the maintenance of hygiene during labor, as well as a desire for "kind and faithful" care. In addition, it was found that the nursing needs of parturient women in Women's hospitals were higher than those in University hospitals. Conclusion: This study showed that parturient women desire to be educated regarding Perinatal care for themselves and for their newborn.
Objectives: To review the status of newborn hearing screening (NHS) and to investigate the effect of the examiners on NHS tests to help the quality control of NHS at a general hospital in a city. Methods: The charts of newborns from January 2015 to March 2016 and from August 2016 to October 2017 were retrospectively reviewed. We compared the results of tests performed by several examiners(group 1) with those performed by one audiologist (group 2) using the same automated auditory brainstem response test. Results: The screening rate and referral rate were not significantly different between group 1 and group 2. The confirmatory test rate was higher in the group 2, but it was not significant. In group 1, the number of tests performed 3 or more times in one ear at one time was significantly higher. The number of tests performed in only one ear at one time was higher in group 2. The screening rate within one month after birth was 64.21%, referral rate was 7.32%, confirmatory test rate within 3 months after birth was 21.74%, and the prevalence of hearing loss was 1.46%. Conclusions: There was no significant difference of results depending on the examiners. In order to make proper screening test, it is necessary to periodically educate the examiner and to instruct the examiner by the supervisor doctors.
Purpose: Multiple studies have documented that high resting levels of cardiac vagal tone suggest higher levels of self-regulation. The aim of this study was to evaluate cardiac vagal tone as an indicator of autonomic nervous function in healthy newborn and premature infants. Methods: This study was conducted using a descriptive comparison design and a convenience sampling strategy. The participants were 72 healthy and 62 premature infants delivered in a university hospital. Continuous heart rate data recordings from the infant's ECG were analyzed and Mxedit software was used to calculate mean heart period and an index of cardiac vagal tone. Results: The healthy infants had significantly higher cardiac vagal tone than the premature infants, when the influence of gestational age was removed using analysis of covariance. However, there were no significant differences in heart rate and heart period between the two groups when the influence of gestational age was removed using analysis of covariance. Conclusion: The results of this study show that cardiac vagal tone may be used as an index for determining infant's autonomic nervous function. Nursing staff in pediatric departments can use cardiac vagal tone with ease, as this index can be calculated in a noninvasive method from the ECG.
