디지털헬스 정책환경의 국제 동향과 의료기술평가에 대한 시사점 (International Trends of Digital Health and It's Political Implication for Health Technology Assessment)

  • As technologies develop, the digital health sector is gradually expanding. Internationally, the global summit for Digital Health named Global Digital Health Partnership (GDHP) was launched in 2018. Many countries are participating in GDHP and share their policy experiences on digital health and find the ways to cooperate with participating countries (13 countries, including South Korea, and Hong Kong). This article reviewed the international trends in digital health policy environment and evidence assessment focusing on GDHP activities, and derived implications for health technology assessment of digital health. Consequently, to assess the intervention effects of digital health is very complex and the assessment should be considered multidimensional aspects (social, clinical, and technical). In addition the patient experience should be assessed qualitatively. Health technology assessment (HTA) should assess the effect of digital health policies to changes in health care systems resulting from the application of advanced technologies related to the 4th Industrial Revolution. Digital health is also related to new HTA, HTA of existing technologies, and R&D on the promising health technology. Therefore, it is necessary to review the trends of the technology's management policy consistently through the HTA of digital health.

한국 보건의료 연구개발의 현황과 과제 (Current Issues of Healthcare Research and Development in Korea)

  • There has been much efforts to facilitate healthcare innovation and many desirable outcomes were produced in Korea. However, some structural deficiencies were found. They are misalignment of research and development (R&D) subjects with healthcare system, lack of flexibility of current healthcare system to accommodate the radically innovative products, and lack of cooperation among innovation agent. Some suggestions to correct these deficiencies are discussed. The suggestions are as follows: relating Korean healthcare R&D to healthcare system, enhancement of institutional flexibility to allow innovative application of new technology, improvement of the R&D process, and reexamination of the role of academic medical centers.

포괄적 비즈니스 모델 프레임워크 구축 및 적용 : 헬스케어 비즈니스 모델을 중심으로 (Establishment and Application of a Comprehensive Business Model Framework : Focusing on Healthcare Business Models)

  • A business model (BM) can be developed based on a business model framework (BMF) consisting of key components such as value proposition, customers, and resources. To systematically generate and analyze emerging BMs such as healthcare BM, an integration of diverse BMF components is needed. The present study is to establish a comprehensive BMF and evaluate its applicability to healthcare BMs. Based on a review of eight BMF studies, ten BMF components were identified and classified into five major components (value proposition, resources, organizing model, customers, and revenue model) and five minor components (technology, service platform, delivery, competitive strategy, and growth/exit) by analyses of frequency and functional importance. Lastly, the BMs of three emerging healthcare companies (WellDoc, Inc., CFW Shops, and Aravind Eye Care System) were analyzed and compared in terms of the proposed BMF components. The comprehensive BMF components presented in the study can be of help for developing new BMs and analyzing the strengths and weaknesses of BMs.

  • With the rapid changes in healthcare system, the impact on healthcare design is significant and constantly changed. Because of emergence of new technology, far-advanced methods of healthcare delivery, and changing social and environmental issues, the healthcare planners and architects have to make sure that future expansion are taken into consideration when they design the hospitals. This thesis aims to investigate the hospital remodeling cases in Korea and USA and find the solutions for the vertical expansion of the hospitals in the metropolitan area where the buildings may be limited to be horizontally expanded. The M Cancer center in Manhattan, NY is selected to be carefully investigated and analyzed. Based on this analysis, the strategy for the vertical expansion of the hospital will be suggested.

Determinants of Satisfaction in the Usage of Healthcare Information Systems by Hospital Workers in Hyderabad, India: Neural Network and SEM Approach

  • This study focuses on the adoption of Healthcare Information System (HIS) in India's healthcare services, which has led to an increased use of HIS software for managing patient information in hospitals. The study aims to evaluate the factors that influence hospital workers' satisfaction with HIS usage and its impact on their intention to continue in the use of HIS. Primary data was collected through a survey questionnaire from 265 hospital workers. A new framework was developed, and Structural Equation Modeling (SEM) was used for analysis. Sensitivity analysis was also conducted on demographic data using an Artificial Neural Network (ANN) approach. The results indicated that all hypotheses were significant (p < 0.05). Effort expectancy was the most significant factor influencing hospital workers' satisfaction (p < 0.01). Sensitivity analysis showed that education (Model-A) and experience in use of HIS (Model-B) were the most important factors. The study contributes by proposing a new theoretical framework and extending the previous research on HIS usage satisfaction. Overall, the study highlights the importance of easiness and usefulness in predicting HIS usage satisfaction.

고령사회 스마트 헬스케어를 위한 UX 디자인방안 - 사물인터넷 기술을 중심으로 (A Study on the Design Plan of UX for the Smart Healthcare for the Aged Society - Focused on IOT Technology)

  • 생체 감지기술 발달은 그동안 거대한 전문 의료기기에서만 가능했던 다양한 생체 정보 수집을 간편하게 가능케 하였다. 센서 소형화는 진단기기의 휴대성을 높여 기존에 병원에 직접 가서 해야만 했던 다양한 건강 진단을 생활 속에서 간편히 수행할 수 있게 하였다. 스마트폰 같은 휴대기기에 센서를 결합하거나, 첨단 IOT(Internet of Things, 사물인터넷) 기술을 적용해 체내 삽입형, 혹은 부착 가능한 초소형 모듈 등 의료기기의 새로운 폼팩터를 만들어냈다. 또한, 측정결과를 스마트폰 등 휴대용 정보기기를 통해 즉시 확인할 수도 있다. 아직 국내에서는 상용화가 더디게 이루어지고 있지만, 미국 등 원격의료 서비스를 허용한 나라에서는 이 분야에서 다양한 양상으로 신기술 적용이 이루어지고 있다. 세계적으로 가장 빠르게 고령화가 진행 중인 대한민국에서 의료수요, 공급, 그리고 비용 문제는 점점 커지고 있다. 이런 상황에서 새로운 건강관리 개념인 스마트 헬스케어가 기존의 의료체계를 보완할 수 있을지 크게 주목받고 있다. 본 연구는 스마트 헬스케어와 관련된 기술 동향을 확인하고, 다양한 헬스케어 제품을 UX 디자인적 측면에서 유형화하였다. 또한, 스마트 헬스케어 기술을 대상 사용자인 노인에게 적용하기 위한 UX 디자인 접근방법과 지침을 제시하였다. 본 연구가 새로운 사회적 문제인 고령사회 헬스케어 문제 해결에 돌파구가 될 UX 디자인 분야의 기반자료가 되기를 기대한다.

환자중심서비스를 위한 온톨로지 기반의 u-Healthcare 시스템 (Ontology-based u-Healthcare System for Patient-centric Service)

  • U-Healthcare는 홈 네트워크, 휴대용 장치 등에 기반한 정보통신기술과 의료시스템이 서로 융합되어 개인의 생체정보 등을 실시간으로 모니터링하고, 자동으로 병원 및 의사와 연결되어 시공간의 제약을 줄임으로써 언제 어디서나 건강을 관리하고 질병을 예방하는 새로운 형태의 의료서비스이다. 본 논문에서는 진료 중심에서 예방 중심으로 변화되어가고 있는 최근의 U-Healthcare 시스템의 기술 발전 추세에 맞추어 조기 대응이 가능한 Healthcare 정보시스템 구축을 위한 요구분석 사항들에 대해 정리하고, 이를 기반으로 u-Healthcare의 실현을 위한 기존의 단위 시스템인 PACS, OCS, EMR, 응급의료시스템을 통합한 환자중심의 클라이언트 시스템을 설계한다. 특히, 온톨로지는 특정분야의 정보 모델에 이용되어 그 분야에서 공통의 어휘를 제공하고, 그 용어의 의미와 용어간의 관계를 다양한 수준의 형식성을 가지고 제공한다. 본 논문에서는 이러한 온톨로지 및 무질서한 데이터에 대한 관계를 정의하고, 보다 체계적으로 데이터를 군집화하는 클러스터링의 개념을 포함한 환자중심의 서비스를 위한 온톨로지 기반의 시스템을 제안한다.

Comparative Study on the Users' Awareness and Choice Patterns on the Healthcare Environment Color Index, Focused on Koreans and Romanians

  • In contemporary times, there is an increasing demand for a dedicated healthcare environment, all over the world. The main motivation for this demand is the level of awareness manifested by the users and reflected on their expectations. Therefore new research strategies and design actions are investigated in order to answer to the users' needs and to provide objective design tools for the specialist. This study continues a series of previous developments of the Healthcare Environment Color Index that should be used for public facilities in Korea and abroad. The cross-cultural approach is a new opportunity for exchanging know-how and for testing and calibrating the perception of the users on the color index. The gathering of results was made based on the online survey provided in this phase in English version for the Romanian users. The survey was intended as an objective mean of research and an efficient way of disseminating information on the subject thus contributing to increasing the level of awareness of the users. A total of 86 Romanian and 89 Koreans with valid answers took part in the survey. Comparative visualizations of the results showed significant similarities concerning the patterns of color preferences but also cultural and emotional differences. As a result both categories of users proved to be aware of the relation between healthcare environment color and its effects on health. They also showed a common understanding on the potential of the color index. As a result the most appealing colors to be applied to the healthcare environment were the series G and B, and the most effective ones were proved to be the ranges of high lightness and low saturation but also the medium high lightness and medium saturation. Both the similarities and the differences of choice between Romanians and Koreans show that cultural and emotional differences of color recognition are important and that should be taken into consideration in design, in order to generate an inclusive and efficient healthcare environment.

  • Healthcare organizations are overwhelmingly embracing smart value-based care strategies, which focuses on providing superior treatment at a significantly lower cost and quality of service (QoS). For these purposes, fifth generation (5G) of mobile service provides an innumerable improvement that clearly outperforms previous generations e.g., 3G and 4G. However, as with most advancements, 5G is projected to introduce new challenges, prompting the community to think about what comes next. This research was conducted to examine the most recent smart 5G technology applications and the solutions they provide to the healthcare industry. Finally, the paper discusses how the upcoming 6G technology has the potential to transform the future of healthcare sector even beyond the current 5G systems.

RFID 기술을 이용한 u-헬스케어 서비스 인증 프로토콜 (u-Healthcare Service Authentication Protocol based on RFID Technology)

  • 최근 u-헬스케어는 웰빙 시대와 IT 대중화가 맞물려 RFID 기술을 의료 서비스에 접목하는 새로운 비즈니스 모델이 각광을 받고 있다. u-헬스케어 서비스는 RFID 기술이 u-헬스케어에 접목되면서 의료 프로세스 상에 발생하는 수기입력, 중복 데이터 발생, 데이터 위 변조, 정보 버전 상이 등의 품질 저하 요인을 해결할 수 있는 방안이 필요하다. 이 논문에서는 u-헬스케어를 통해 의료 서비스를 제공받는 사용자의 프라이버시를 보호하기 위한 RFID 기반의 사용자 인증 프로토콜을 제안한다. 제안 프로토콜에서는 의료 서비스를 제공받는 사용자의 데이터 위 변조 및 프라이버시와 u-헬스케어 보안시스템의 안전성을 획기적으로 제고하기 위해서 환자에 관한 비밀 정보는 병원에서 생성된 비밀키와 연접하여 DB 테이블 내의 해당 환자의 비밀키 필드에 저장한다. 또한, 불법적인 사용자의 접근이 있을 경우 인증 서버에게 통보하여 u-헬스케어의 불법 유 무 판별한 후 u-헬스케어 서비스 접근을 승인받도록 한다.
