Objectives : This study aims to present a new classification system using evaluation standards to ensure the safety of transport ventilators used in Korea. Due to the rapid advancement of technology, various types of ventilators have been developed and as the range of application increases, so does demand. With quick access to information and a growing economy, the technology used in Korean hospitals is equivalent to that of other countries; however, regulatory and safety consciousness are still lagging in Korea. Methods : Based on researching overseas cases, reference data and standards for improving the current system in Korea are proposed. Results : According to the review of transport ventilator use in Korea, it was found that concerns about safety is due to the absence of a standardized classification system for evaluating the safety and effectiveness of transport ventilators. Conclusions : In order to improve the safety of patients and the quality of medical care, it is essential to establish guidelines and assessment standards guaranteeing the safety of transport ventilators in Korea. Clear definitions and classifications for devices must precede the application of such standards. In addition, effective evaluation standards should be developed in order to resolve problems and improve upon the current system through continuous validation.
Background: Korea National Health Insurance Service (NHIS) is operated as a social insurance system in which people pay a portion of their monthly income as insurance premiums and receive benefits when they experience illness or injury. Since 2005, the national health insurance remained surplus accumulating cumulative reserves each year. However, as of 2018, NHIS revenue recorded 62.11 trillion won and spending of 62.29 trillion won. The deterioration of NHIS finances is expected to accelerate with the aging population, income growth, new medical technology development, and enhanced security policies. Methods: To examine the financial health and sustainability of NHIS, we estimated the future revenue and spending until 2030 using the data from Korea Health Insurance Review and Assessment Service statistical yearbook. 2004-2018 average percentage change in NHIS revenue and spending was calculated. We estimated the future NHIS financial status using two methods. In the first method, we calculated the revenue and spending of the future NHIS by applying the 2004-2018 average percentage change to the subsequent years consecutively. In the second method, we estimated the future NHIS financial status after adjusting for the predicted demographic changes such as the aging population and declining birth rate in South Korea. Results: The estimates from this study suggest that the NHIS's cumulative reserves will run out by 2024. Conclusion: In terms of spending on current health insurance, there should be a search for ways of more efficient spending and funding options.
정량적 미생물 위해평가(Quantitative microbial risk assessment: QMRA)는 국민건강에 영향을 주는 잠재된 위해를 연구하여 식품내 존재하는 병원성미생물과 관련한 위해를 체계적으로 평가하는 것이다. 본 연구는 깁밥에서의 Staphylococcus aureus에 대한 QMRA 모델을 개발하고 이를 식품위생관리에서 이용할 수 있는 기준을 제시하여, 식품안전 분야에서의 QMRA의 필요성과 활용성을 알리기 위해서 실시하였다. QMRA 모델은 매장에서부터 최종소비에 이르기까지 4 단계로 구성되었으며, 미생물 성장모델과 조사자료 그리고 확률분포가 김밥의 최종소비에서의 S. aureus 수준을 평가하기 위하여 이용되었다. S. aureus에 대한 양-반응모델이 없는 관계로 최종 소비단계에서의 S. aureus의 오염수준을 잠재적인 위해를 결정하는데 이용하였다. 이를 위하여 5 log CFU/g이상을 잠재적 유해수준으로 가정하였으며, 시뮬레이션 결과 최종 소비되는 김밥에서 이 유해수준을 초과할 가능성은 30.7%로 나타났다. 김밥에서의 S. aureus의 오염수준은 평균 2.67 log CFU/g으로 나타났으며, 민감도 분석에서는 매장에서의 김밥 보관온도 및 시간이 가장 중요한 요인으로 결정되었다. 이러한 결과를 종합하여 볼 때 김밥 매장에서는 현실적으로 보존시간 관리가 어렵다고 한다면 보관온도를 $10^{\circ}C$ 이하로 유지하는 것이 가장 중요한 것으로 나타났다. 본 연구에서와 같이 QMRA는 식품 내 존재할 수 있는 잠재적인 위해에 영향을 미치는 인자들에 대한 평가에 이용될 수 있으며 이를 식품위생관리에 직접적으로 활용 가능한 것으로 나타났다.삼의 분석방법별 기준인 ginsenoside -Rg1과 -Re의 함량비($Rg1/Re{\Leq}3.87$)에 부합되었다.도에서 MA 저해 효과는 쑥갓>미나리>참깨의 순으로, 각각 54, 48, 29%를 나타냈다. 참깨는 20, $100{\mu}g/mL$의 농도에서처럼 가장 작은 효과를 보여줬고, 쑥갓은 50% 이상의 항산화 효과를 나타냈다. Home modification has come to be recognized as an important intervention strategy to manage health care conditions, maintain or improve functioning, ensure safety, and reduce the wheelchair user's dependency on others. However, the availability of skilled professionals with experience in home modifications for accessibility is limited. A system that enables accurate remote assessments would be an important tool to improve our ability to perform home assessments more easily and at decreased cost. A Remote Wheelchair Accessibility Assessment System (RWAAS) using Virtualized Reality(VR) technology was developed that enabled clinicians to assess the wheelchair accessibility of users' built environments from a remote location. Characteristics of the camera and 3D reconstruction program chosen for the system significantly affect its overall reliability. In this study, we performed two reliability analyses on the hardware and software components: 1) Verification that commercial software can construct sufficiently accurate 3D models by analyzing the accuracy of dimensional measurements in a virtualized environment; 2) comparison of dimensional measurements with four camera settings. Based on these two analyses, we were able to specify a consumer level digital camera and the Photomodeler Pro software for this system. And we then tested the feasibility of the selected software and hardware in an actual environment. Lastly, A field evaluation was performed to test whether this new system is comparable to the traditional method of accessibility assessment to evaluate its ability to assess the accessibility of a wheelchair user's typical built environment. The results of field trials showed high congruence between the assessments by two methods. Findings suggested that the RWAAS assessments have the potential to enable specialists to assess potential accessibility problems in built environments regardless of the location of the client, home, or specialist.
Osteoporotic fracture (OF), along with bone mineral density (BMD), is an important diagnostic parameter and a clinical predictive risk factor in the assessment of osteoporosis in the elderly population. However, a genome-wide association study (GWAS) on OF has not yet been clarified sufficiently. To identify OF-associated genetic variants and candidate genes, we conducted a GWAS in a population-based cohort (Korean Association Resource [KARE], n=1,427 [case: 288 and control: 1139]) and performed a de novo replication study in hospital-based individuals (Asan and Catholic Medical Center [ACMC], n=1,082 [case: 272 and control: 810]). In a combined meta-analysis, a newly identified genetic locus in an intergenic region at 10p11.2 (near genes FZD8 and ANKRD30A ) showed the most significant association (odd ratio [OR] = 2.00, 95% confidence interval [CI] = 1.47~2.74, p=$1.27{\times}10^{-6}$) in the same direction. We provide the first evidence for a common genetic variant influencing OF and genetic information for further investigation in bone metabolism.
The authors argue that the current Government Information System for asthmatics appears to be non-user friendly due to lack of the cartographic representation for the text based statistical data. Acknowledging these constraints, an operational, user-friendly map for asthmatic prevalence has been generated by combining existing statistical data with the administrative Dong boundary map under GIS environment. The Geographical User Interface, in particular, were ideally suited to deriving the major distribution patterns that more asthmatic prevalence tends to be occurred on conventional commercial district and industrial complex. A visual map using spatial modelling technology were generated to show the fact that some degree of increasing or decreasing trends of asthmatic prevalence already exists in the experimental sites. It could be used as an evidence to restrict initiation of development activities causing negative influence to asthma such as road construction. The result of this study would play a crucial role in improving the quality of environmental health information service if it is operationally introduced into the Government since the highly user-friendly interface provides a completely new means for disseminating information for asthmatics in a visual and interactive manner to the general public.

This study evaluated ecological health, using various biomarkers and bioindicators, of pale chub (Zacco platypus) as a sentinel species, in Daejeon Stream, South Korea, during AprilMay 2011. The biomarkers and bioindicators were compared among three sites of control: Reference ($C_z$), transition ($T_z$), and the urban zones ($U_z$); and the 7-Ethoxyresorufin-O-deethylase (EROD) activity, DNA damage, acetylcholinesterase (AChE) activity, and vitellogenin (VTG) concentrations were more significantly increased in the $U_z$ than in the $C_z$. Also, physiological markers such as condition factor, liver somatic index, visceral somatic index, and gonad somatic index were significantly increased in the $U_z$ than in the $C_z$. For the health assessments, three categorized parameters of blood chemistry, molecular biomarkers, and physiological bioindicators were standardized and calculated as a star-plot, representing values of Integrated Health Response (IHR). Values of IHR had more significant (P<0.05) increases in the $U_z$ than any other zones, indicating an impairment of ecological health by organic matter, nutrients (N, P), and toxic chemicals. This study is based on low levels of biological organization approach of molecular and physiological biomarkers and bioindicators, so further study of high-levels of biological organization approach such as community and population is required for overall range of health assessments. The approach of IHR values, however, may be useful in providing early warning of future impacts on ecological health.
최근 국내에도 협동로봇이 산업용로봇 시장에 진입하면서 제조업을 중심으로 설치되고 있으나 산업안전보건법 제93조 안전검사에 따른 산업용로봇의 안전규제를 그대로 적용받고 있어 안전보호 대책인 펜스와 매트를 설치하여야 하는 상황이었다. 협동로봇을 설치할 사업장은 ISO 10218-2과 ISO 12100에 따라 로봇-인간, 작업환경, 작업방식에 대한 위험성평가를 실시하고 위험도를 낮추어야 한다. 그러나, 국내 산업현장에 협동로봇의 도입 초기인 관계로 협동로봇에 대한 새로운 위험성도 알려지지 않고 있으며 위험성평가도 활성화되지 않아 사업장에서는 위험성평가가 낯설고 어렵게 받아들여지고 있다. 협동로봇의 위험성평가는 로봇과 인간이 공존하는 개념에서 출발해서 작업자의 이상행동, 인적 오류, 설비결함, 인터록 기능에 초점을 맞춰 실제 일어날 가능성이 높은 위험을 발굴하고 개선하는데 그 목적이 있으며 본 연구는 국내 자동차부품 제조업에 적용된 사례를 통해 입증하고자 하였다. 향후에도 협동로봇의 위험성평가를 다양한 공정 및 작업에 대해 사례 발굴함으로써 중소기업의 안전성 향상에 유용할 것으로 기대한다.
Health monitoring of civil infrastructures, in particular, old bridges that are still in service, has become more than necessary, given the risk that a possible degradation or failure of these infrastructures can induce on the safety of users in addition to the resulting commercial and economic impact. Bridge integrity assessment has attracted significant research efforts over the past forty years with the aim of developing new damage identification methods applicable to real structures. The bridge of Ouled Mimoun (Tlemcen, Algeria) is one of the oldest railway structure in the country. It was built in 1889. This bridge, which is too low with respect to the level of the road, has suffered multiple shocks from various machines that caused considerable damage to its central part. The present work aims to analyze the stability of this bridge by identifying damages and evaluating the damage rate in different parts of the structure on the basis of a finite element model. The applied method is based on an inverse analysis of the normal stress responses that were calculated from the corresponding recorded strains, during the passage of a real train, by means of a set of strain gauges placed on certain elements of the bridge. The results obtained from the inverse analysis made it possible to successfully locate areas that were really damaged and to estimate the damage rate. These results were also used to detect an excessive rigidity in certain elements due to the presence of plates, which were neglected in the numerical reference model. In the case of the continuous bridge monitoring, this developed method will be a very powerful tool as a smart health monitoring system, allowing engineers to take in time decisions in the event of bridge damage.
The correlations between Large Early Release Frequency (LERF) and Early Fatality need to be investigated for risk-informed application and regulation. In Regulatory Guide (RG) -1.174, while there are decision-making criteria using the measures of Core Damage Frequency (CDF) and LERF, there are no specific criteria on LERF. Since there are both huge uncertainties and large costs needed in off-site consequence calculation, a LERF assessment methodology needs to be developed, and its correlation factor needs to be identified, for risk-informed decision-making. A new method for estimating off-site consequence has been presented and performed for assessing health effects caused by radioisotopes released from severe accidents of nuclear power plants in this study. The MACCS2 code is used for validating the source term quantitatively regarding health effects, depending on the release characteristics of radioisotopes during severe accidents. This study developed a method for identifying correlations between LERF and Early Fatality and validates the results of the model using the MACCS2 code. The results of this study may contribute to defining LERF and finding a measure for risk-informed regulations and risk-informed decision-making.
