Effects of Multipath Electrical Stimulation on the Functional Recovery of Early Stage Patients of Total Knee Arthroplasty

  • PURPOSE: This research was intended to investigate the influence to function recovery at the early stage after surgery, by conducting Multipath Electrical Simulation and isometric exercise treatment as early stage medical treatment method for Total knee arthroplasty patients. METHODS: The subject of 30 patients having Unilateral Total knee arthroplasty over age 65, Multipath Electrical Simulation and isometric exercise (experiment group I), Conventional Electrical Simulation and isometric exercise (experiment group II) and isometric exercise (control group). The intervention was performed in 5 times per a week and 60 minutes per a day during 4 weeks. We performed research by conducting Neuromuscular Electrical Stimulation and isometric exercise together and measured pain, range of motion, muscle strength and gait ability before and after intervention. RESULTS: The result showed therapeutic improvement in experiment group I, experiment group II and control group, but Multipath Electrical Simulation and isometric exercise showed significant improvement in function recovery of early stage compared to Conventional Electrical Simulation and isometric exercise, only isometric exercise. CONCLUSION: Based on research result, in order for early state function recovery of Total knee arthroplasty patients, when conducting neuromuscular electrical stimulation and isometric exercise together, especially when applying Multipath Electrical Stimulation, we could know that it showed more significant improvement to function recovery after surgery. Also, we suggest that Multipath Electrical Simulation may become a useful tool as a method for intervention and performing in various diseases for weakening of Quadriceps muscle.

신경근전기자극에 의한 척수운동신경원의 흥분성 변화 (The Change of Spinal Motor Neuron Excitability by Neuromuscular Electrical Stimulation)

  • The purpose of this study was to determine the effect of neuromuscular electrical stimulation(NMES) on the alteration of spinal motor neuron excitability. In this article, I would like to experiment on a standard capacity of clinical electrophysiology, a difference in applying methods and a clinical efficiency of NMES by Nerve conduction velocity. We used normal eight subjects without neuromuscular disease and all subjects participated 3 session, which at least 1 week between session. Participants classified according to each group in Antagonist, Agonist, Antagonist-Agonist by the NMES. The test was measured continuously pre test, post-test, post 20 minute test by EMG including H reflex, F wave, motor nerve conduction velocity(MNCV). The following results were obtained; 1. H-reflex latencies and H/M intervals were significantly increased in agonist and antagonist-agonist group(p<.01). 2. H-reflex amplitudes and H/M ratios were significantly decreased in agonist and antagonist-agonist group(p<.01). In agonist group, H-reflex amplitudes and H/M ratios were more significantly decreased than antagonist group. 3. F-wave latencies were significantly increased in agonist and antagonist-agonist group(p<.01). F/M intervals were significantly increased in antagonist-agonist group(p<.01). F wave conduction velocities were significantly increased in agonist and antagonist-agonist group(p<.01) but F/M ratios were not significant. 4. MNCV were significantly decreased in agonist(p<.01). These results lead us to the conclusion that agonist and Antagonist-agonist was significantly decreased excitability of spinal motor neuron. Conversely, Antagonist does not decreased. Therefore, A further direction of this study will be to provide more evidence that NMES have an effect on excitability of spinal motor neurons in UMN syndrome.

전기자극이 체중 부하를 제거한 신생 흰쥐 척수조직의 NT-3 및 MAP2 발현에 미치는 영향 (Influence of Electrical Stimulation on NT-3 and MAP2 Expression of Neonatal Rat Spinal Cord During Suspension Unloading)

  • The development of neonatal neuromuscular system is accomplished by the functional interaction between the spinal neurons and its target cells, skeletal muscle cells, and the intrinsic and extrinsic factors affecting this process. The aim of this study was to identify the effect of suspension unloading (SU) and neuromuscular electrical stimulation (NMES) upon the development of the neonatal spinal cord. For this study, the neonatal rats were randomly divided into three groups: a control group, an experimental group I, and an experimental group II. The SU for experimental group I and II was applied from postnatal day (PD) 5 to PD 30, and the NMES for experimental group II was applied from PD 16 to PD 30 using NMES that gave isometric contraction with 10 Hz for 30 minutes twice a day. In order to observe the effect of SU and NMES, this study observed neutrophin-3 (NT-3) and microtubule associated protein 2 (MAP2) immunoreactivity in the lumbar spinal cord (L4-5) at the PD 15 and PD 30. The results are as follows. At PD 15, lumbar spinal cord of experimental group I and II had significantly lower NT-3 and MAP2 immunoreactivity than control group. It proved that a microgravity condition restricted the spinal development. At PD 30, lumbar spinal cord of control group and experimental group II had significantly higher NT-3 and MAP2 immunoreactivity than experimental group I. It proved that the NMES facilitated the spinal development by spinal cord-skeletal muscle interaction. These results suggest that weight bearing during the neonatal developmental period is essential for the development of neuromuscular development. Also, the NMES on its target skeletal muscle can encourage the development of the spinal cord system with a full supplementation of the effect of weight bearing, which is an essential factor in neonatal developmental process.

신경근 전기자극이 흰쥐 위축근 신경근 연접부의 연접재형성에 미치는 영향 (The Effects of Neuromuscular Electrical Stimulation on Synaptic Reorganization of Neuromuscular Junctions in Rat Atrophic Muscle)

  • Purpose: This study was performed to investigate the effects of NMES for recovery of skeletal muscle. Methods: The normal group consisted of healthy rat without cast immobilization. The control group was breeding at standard cage during 7 days after 2 weeks-cast immobilization on hind limb. The experimental group. I. received NMES application during days after 2 weeks-cast immobilization on hind limb. The effects were measured with NT-3 immunoreactivity in neuromuscular junction by light microscope. Results: Immunoreactivity of NT-3 at the neuromuscular junction was higher appeared experimental groups than control group. Then, There was a little detection in the normal and control group. Conclusion: These therapeutic interventions enhance expression of NT-3 at the neuromuscular junction. Also, NMES is considered to effect on a normal structural formation and NT-3 expression at the neuromuscular junction.

  • Purpose : The purpose of this research was to determine the effects of Proprioceptive Neuromuscular Facilitation(PNF) and Functional Electrical Stimulation(FES) of combined on gait ability in hemiplegic gait. Methods : The subjects of this study were 13 hemiplegic patients. Each subjects was taken PNF pattern and FES of combined with 5 times per week for 4weeks. Pre- and Post-intervention change in gait ability were measured using an Timed up and Go test, stride length of the affected side, step length of the affected side. The data were analyzed using the paired t-test. Results : The results of this study were showed significantly improvement in TUG, stride length of the affected side, step length of the affected side after intervention. Conclusion : These results suggest that the Proprioceptive Neuromuscular Facilitation(PNF) and Functional Electrical Stimulation(FES) of combined exercise is an effective way of improving gait ability for hemiplegic patients.

신경근 전기자극의 사전적용이 위축근 기능에 미치는 영향 (A Study of Pre-application of Neuromuscular Electrical Stimulation on Atrophic Muscle Function)

  • 본 연구의 목적은 현수 기간 중 적용한 신경근 전기자극의 사전 적용이 흰쥐 뒷다리 근육의 무게와 후지 기능에 미치는 효과를 알아보는데 있다. 이를 위해서 25마리의 Sprague-Dawley계 흰쥐를 14일 동안 현수한 HLS군(n=5)과 14일 동안 자유롭게 사육한 WB군(n=5), 현수 기간 중 14일간 사전 전기자극을 실시한 ES14군(n=5), 현수 기간 중 11일간 사전 전기자극한 ES11군(n=5), 현수 기간 중 7일간 사전 전기자극한 ES7군(n=5)에 무작위 배치하여 실험을 진행하였다. 실험결과, HLS군과 ES7군에서 비복근의 근무게, 후지 활보장의 유의한 감소와 외향각의 유의한 증가가 관찰되었고, ES14군과 ES11군에서 비복근의 근무게와 후지 활보장, 외향각이 유의하게 유지되었음을 관찰하였다. 즉, 14일 및 11일의 사전 신경근 전기자극이 근위축 지연과 근기능 보존에 효과적임을 보여주고 있다. 이러한 결과는 근위축 예방 및 기능 유지를 위해서는 신경근 전기자극을 가능한 조기 적용해야함을 알려주고 있다.

성견의 하악골 골신장술 후 골 형성에 대한 신경근육의 전기적 자극의 효과 (Effects of Neuromuscular Electrical Stimulation on Distracted Boneafter Mandibular Distraction Osteogenesis in Canine Model)

  • Purpose: This study was designed to examine whether the use of neuromuscular electrical stimulation (NMES) after mandibular distraction osteogenesis accelerated bone formation and consolidation. Methods: Eight adult dogs underwent mandibular left body osteotomy. After a 3 day latency period, a distraction rod device was activated at a rate of 1.0 mm once per day for 10 days. After the completion of mandibular lengthening, NMES group was treated twice daily with 2 hours of NMES for 14, and 28 days, while non-NMES group did not receive NMES. The distracted segment was evaluated radiolgraphically histologically and than immunohistochemically for osteopontin (OPN) to evaluate new bone formation and consolidation. Results: Radiography, did not demonstrate significantly different images between the group and the NMES group. Histological examination however, showed that the new bone formation 14 and 28 days after distraction was better in the NMES group when compared to non-NMES group. Immunohistochemical analysis demonstrated that the staining intensity of OPN increased more in the NMES group than in non-NMES group during early consolidation. Conclusion: The results of this study demonstrated that the use of NMES can promote bone formation and consolidation.

미세전류신경근자극이 Delayed Onset Muscle Soreness, 혈청 Creatine Kinase, 최대 수의적 등척성 수축에 미치는 영향 (The Effects of Microcurrent Electrical Neuromuscular Stimulation on Delayed Onset Muscle Soreness, Serum Creatine Kinase, and Maximal Voluntary Isometric Contraction: A Preliminary Report)

  • The purpose of this study was to test the microcurrent electrical neuromuscular stimulation on muscle soreness, serum creatine kinase levels and force deficits evident following a high-intensity eccentric exercise bout. 10 volunteer male subjects were randomly assigned to a treatment group or to a control group. Exercise consisted of high-intensity eccentric contractions of the elbow flexors. Resistance was reduced as subjects fatigued, until they reached exhaustion. Muscle soreness rating was determined using a visual analog scale. Serum creatine kinase levels were analyzed using a blood sample. Force deficits were determined by measures of maximal voluntary isometric contraction at $90^{\circ}$ of elbow flexion on a Orthotron II dynamometer. Muscle soreness rating, serum creatine kinase levels and maximal voluntary isometric contraction were determined at the before exercise and again at 24 and 48 hours postexericse. Treatments were applied immediately following exercise. The control group subjects rested following their exercise bout. Statistical analysis showed significant increases in muscle soreness rating and significant decreases in maximal voluntary isometric contraction when the before exercise was compared with 24 and 48 hour measures(p<0.01). No significant effects were observed between groups in muscle soreness rating and maximal voluntary isometric contraction(p>0.05). Highly significants differences in serum creatine kinase levels were found using on Analysis of variance(ANOVA) repeated measures between groups for each time cycles(p<0.001). This modality may have benefits when used early stage in the muscle damage.

미세전류치료의 최근 임상연구논문 고찰 (A Literature Review on the Clinical Application of Microcurrent Electrical Neuromuscular Stimulation(MENS) : articles published for recent 10 years)

  • Objectives : MENS was used for wound healing and pain relief using bioelectric property. There are many articles on in vivo and clinical research. This article reviews articles on MENS published for recent 10 years to consider effectiveness of MENS and other fields to applicate MENS. Articles were collected from MAR, 2008 to JUN, 2008. Methods : We searched Pubmed, KSI, KERIS, KMBASE, and National assembly library using "Microcurrent Electrical Neuromuscular Stimulation" and classified the articles into subsets of foreign, Korean and wound healing, pain, relief, and increasing the range of movement(ROM). Also we evaluated their values according to the Jadad scale. Results : A total of 18 articles (Foreign-9, Korean-9) reconfirmed that MENS reduces pain, increases wound healing, ROM, and strength of muscle. In addition, MENS has effect on stress-hormone and various pain like low back pain, acute lateral epicondylitis, and plantar fascitis. And there are various styles developed. However, there is no standard protocol so the user is usually in trouble at clinic. Conclusions : MENS may be have application to various pain diseases, and further studies analysing its effectiveness are needed.

전기근육자극 훈련이 복부비만 중년 여성의 복부지방, 체간 근 두께와 활성도에 미치는 영향 (Effects of Neuromuscular Electrical Stimulation Training on Abdominal Fat, Trunk Muscle Thickness and Activity in Middle-Aged Women with Abdominal Obesity)

  • PURPOSE: This study examined the effects of low frequency neuromuscular electrical stimulation (NMES) training on abdominal obesity in middle-aged women through electromyography and ultrasound. METHODS: Twenty-two middle aged women with abdominal obesity participated in the study. A low-frequency NMES device was used on the abdomen and waist of each subject for 20 minutes each (a total of 40 minutes) three times a week for eight weeks. The waist-hip ratio (WHR), weight and BMI (Body Mass Index) were measured. Electromyography (EMG) and ultrasound measurements were performed three times in total (pre-intervention, four weeks into the intervention, and eight weeks post-intervention) to examine the effects of low-frequency NMES on the abdominal muscle activity, muscle thickness, and subcutaneous fat. RESULTS: The results indicated a difference in the WHR and waist circumference before and after intervention (p<.05). The external oblique muscle (EO) showed a significant increase in muscle activity during all measurements taken post-intervention (p<.05). The abdominal subcutaneous fat thickness also showed a significant decrease between each measurement (p<.05). The test results showed that the abdominal subcutaneous fat thickness values taken eight weeks post-intervention were significantly lower than those taken pre-intervention and four weeks into the intervention (p<.05). CONCLUSION: These findings show that low-frequency NMES device training can be applied to middle-aged women with abdominal obesity to improve their body shape and exercise performance.
