리포머를 이용한 체간 안정화 운동이 만성 뇌졸중 환자의 체간 조절 능력과 균형 및 보행 기능에 미치는 영향 (The Effect of Trunk Stabilization Exercise Using a Reformer on Trunk Control Ability, Balance, and Gait Function in Chronic Stroke Patients)

  • Purpose: This study aimed to evaluate the effects of trunk stabilization exercises using a Reformer on trunk control, balance ability, and gait function in chronic stroke patients. Methods: The participants were 24 chronic stroke patients, randomly divided into two groups: trunk stabilization exercise using the Reformer group (TS-R, n = 12) and general trunk stabilization exercise group (GT-E, n = 12). Assessment methods included the Trunk Impairment Scale for trunk control, the AMTI force platform for static balance, the Timed Up and Go test for dynamic balance, and the Dynamic Gait Index for gait function. Assessments were conducted before and after the intervention. The intervention for the TS-R group consisted of bridging exercises using a Reformer, while the GT-E group performed bridging exercises on a mat. All interventions were performed for 17 minutes per session, five times a week, for a total of 20 sessions over four weeks. Statistical analysis was performed using repeated-measures ANOVA to analyze the interaction between groups and time. Results: The results of the repeated measures ANOVA indicated a significant interaction between the groups and time. The TS-R group showed statistically significant differences in all variables before and after the intervention. In contrast, the GT-E group did not show statistically significant differences in any variables before and after the intervention. Conclusion: The findings of this study suggest that trunk stabilization exercises using a reformer are effective in improving trunk control, balance ability, and gait function in chronic stroke patients.

고유수용성신경근촉진법 내려치기 패턴 훈련이 편측무시가 있는 뇌졸중 환자의 편측무시, 균형능력 및 일상생활수행능력에 미치는 영향: 무작위 임상 시험 (Effect of Proprioceptive Neuromuscular Facilitation Chopping Pattern on Neglect, Balance, and Activity of Daily Living of Stroke Patients with Hemi-Spatial Neglect: A randomized clinical trial)

  • PURPOSE: This study examined the effects of the chopping pattern of proprioceptive neuromuscular facilitation (PNF) on the neglect, balance ability, and activities of daily living in stroke patients with hemi-spatial neglect. METHODS: Twenty stroke patients with neglect volunteered for this study. The subjects were assigned randomly to either the experimental group (EG, n=10) or control group (CG, n=10). The chopping pattern of PNF and exercise therapy were applied in the EG and CG, respectively, for four weeks. Neglect, balance, and activities of daily living were evaluated at the pre-and post-intervention. The assessment tools included a line-bisection test, balance test using Space balance 3D, and modified Barthel index. The pre-and post-intervention values were compared in each group using a paired t-test and the between-group differences were assessed using an independent t-test. Statistical significance was set to ${\alpha}=.05$ for all variables. RESULTS: Significant differences were observed between the EG and CG in terms of neglect, balance, and activities of daily living (p<.05). Both groups showed significant improvement in neglect, balance, and activities of daily living (p<.05). CONCLUSION: These results suggest that the chopping pattern of PNF is effective in improving the hemi-spatial neglect, balance, and activities of daily living in stroke patients with neglect.

시각 통제를 이용한 균형훈련이 만성 뇌졸중 환자의 균형능력과 자세조절, 균형자신감에 미치는 영향 (Effects of Balance Training through Visual Control on Balance Ability, Postural Control, and Balance Confidence in Chronic Stroke Patients)

  • Purpose: The purpose of this study was to conduct balance training through vision control to improve the balance, postural control, and balance confidence and to decrease the visual and sensory dependence of stroke patients. Methods: Twenty-eight chronic stroke patients volunteered to participate in the study. They were randomly assigned to the eyes-closed and the eyes-open training groups. Three times a week for four weeks each group performed an unstable-support session and a balance training session for thirty minutes per set. Their balance, postural control, and balance confidence were assessed using BIO Rescue (BR), the postural assessment scale for stroke (PASS), and the Korean activity-specific balance confidence scale (K-ABC), respectively. All data were analyzed using SPSS version 22.0. Statistical methods before and after working around the average value of each dataset were independent T-test. The significance level for statistical analyses was set at 0.05. Results: Comparison between the groups showed statistically significant effects on all variables before and after the intervention (p < 0.05). Conclusion: This study reflected that balance-training programs involving vision control improve the balance, postural control, and balance confidence of chronic stroke patients. Thus, stroke patients should undergo training programs that increase the use of their other senses with vision control in clinical practice.

고유수용성신경근촉진법을 이용한 체간 운동과 트레드밀을 결합한 훈련이 파킨슨병 환자의 균형과 보행 능력에 미치는 영향 (Effects of the Trunk Exercise Using PNF Combined with Treadmill on Balance and Walking Ability in Individuals with Parkinson's Disease)

  • Purpose: This study was to explore the effects of trunk exercise using PNF combined with treadmill training on balance and walking ability in patients with Parkinson's disease. Methods: This study included 16 patients with Parkinson's disease. Participants were randomly assigned to 2 groups: an experimental group (n=8) and a control group (n=8). All participants underwent treadmill training for 30 minutes. In addition, the experimental group (trunk exercise using PNF) and control group (conventional training) participated in a 30-minute exercise program. Both groups performed the training 5 times per week for 4 weeks. Disease severity (determined using the unified Parkinson's disease rating scale motor subscale, UPDRS-3), balance (determined using the Berg balance scale, BBS), walking speed (determined using the 10-meter walking test, 10MWT), and walking endurance (determined using the 6-minute walking test, 6MWT) were measured at baseline and after 4 weeks. Results: Pre- to post-intervention improvement was noted on all outcome measures for both groups (p<0.05). Post-intervention, there was a significant improvement in the experimental group as compared to the control group for the following measured outcomes (p<0.05): UPDRS-3 (p=0.03; 95% CI, -5.52 to -0.24), BBS (p=0.04; 95% CI, 0.59 to 6.45), 10MWT (p=0.01; 95% CI, -2.19 to -0.42), and 6MWT (p=0.04; 95% CI, 1.81 to 96.72) Conclusion: The results of this study revealed that trunk exercise using PNF plus treadmill training improves balance and walking ability as compared to conventional training plus treadmill training in patients with Parkinson's disease.

  • Purpose : The purpose of this study was to evaluate the effect of core stability training at deep abdominal muscle for balance control of hemiplegic patient. Method : The subject of this study was a 47-year-old man with right hemiplegia. He was treated five times a week for three weeks with core stability training at deep abdominal muscles. Evaluation tool was used Functional reach test(FRT), timed up and go test(TUG) and one leg standing for stroke patients. Result : The FRT distance increase, TUG time decrease, one leg standing time increase core stability training at deep abdominal muscles for right hemiplegia improved was the ability for maintain balance. Posture and control of trunk stability are changing posture, and so which showed significant improve of total balance control. Conclusion : The result of this study showed that core stability training at deep abdominal muscles is an effective treatment for balance control. Therefore, it could be considered as a treatment method in the rehabilitation of stroke patient with poor postural control and imbalance, although further studies are needed.

Effects of Multipath Electrical Stimulation on the Functional Recovery of Early Stage Patients of Total Knee Arthroplasty

  • PURPOSE: This research was intended to investigate the influence to function recovery at the early stage after surgery, by conducting Multipath Electrical Simulation and isometric exercise treatment as early stage medical treatment method for Total knee arthroplasty patients. METHODS: The subject of 30 patients having Unilateral Total knee arthroplasty over age 65, Multipath Electrical Simulation and isometric exercise (experiment group I), Conventional Electrical Simulation and isometric exercise (experiment group II) and isometric exercise (control group). The intervention was performed in 5 times per a week and 60 minutes per a day during 4 weeks. We performed research by conducting Neuromuscular Electrical Stimulation and isometric exercise together and measured pain, range of motion, muscle strength and gait ability before and after intervention. RESULTS: The result showed therapeutic improvement in experiment group I, experiment group II and control group, but Multipath Electrical Simulation and isometric exercise showed significant improvement in function recovery of early stage compared to Conventional Electrical Simulation and isometric exercise, only isometric exercise. CONCLUSION: Based on research result, in order for early state function recovery of Total knee arthroplasty patients, when conducting neuromuscular electrical stimulation and isometric exercise together, especially when applying Multipath Electrical Stimulation, we could know that it showed more significant improvement to function recovery after surgery. Also, we suggest that Multipath Electrical Simulation may become a useful tool as a method for intervention and performing in various diseases for weakening of Quadriceps muscle.

청소년의 전방머리자세와 선 자세 균형간의 상관관계 연구 (Correlation between Standing Balance and Forward Head Posture in Adolescents)

  • Purpose: This study investigated the correlation between standing balance and head-forward posture in adolescents. Methods: The participants in this study were 15 female adolescents. We took photographs of the participants' craniovertebral angle with a digital camera to determine the degree of head-forward posture. Standing balance was evaluated using the 30-second Romberg test of BT4. The measurements were taken on a stable-support surface and on an unstable-support surface using a balance pad. The data measured in this study were analyzed using SPSS version 23.0, and the statistical significance level α was established as 0.05. Pearson correlation coefficient analysis was performed to identify the correlation between the degree of head-forward position of the participants and their balance ability. Results: When we measured 30 seconds of the Romberg test, we observed that, with their eyes open, the participants' head-forward posture and balance were not correlated. However, with their eyes closed, there was a strong correlation between the forward position of the head and the balance ability, which declined as the head position increased. Conclusion: Adolescents with severe head-forward posture exhibited problems with balance in posture control with closed eyes when visual information was not available. It can be inferred that an abnormality occurs in proprioceptor sensation due to their habitual head-forward posture. Education on correct posture and preventive activities should be recommended to improve the health of such adolescents and avoid possible future problems of physical deterioration.

경두개직류전류자극이 정상인의 주의집중력에 미치는 영향 (Effect of Transcranial Direct Current Stimulation on Concentration of Attention)

  • Purpose : Study on the effects of attention(working memory) for normal adults by applying transcranial Direct Current Stimulation(tDCS). Methods : There are two groups of 10 random people aged between 20 to 30, one is an experimental group(stimulation) and the other is a comparison group(simulate stimulation). For the 1st da y, pre-stimulation test is executed, and for the 2nd day to the 4th day, tDCS is applied stimulated both on the right prefrontal lobe and the left occipital lobe through the electrode. The stimulation lasts for 15 minutes with voltage power of 1mA for each day. The post-stimulation test is executed on the day of 5th. In this paper, a FAIR attention test is used for measuring the attention. Results : As the result of the experiment, there are significant gaps both on the value of P, the ability of selective attention, and the value of C, the consistent ability after the stimulation on right prefrontal and left occipital lobe. And there is no significant gap on the value Q, the quality(Control). However, all the P, Q, and C values are increased according to the average value between the pre-stimulation test and the post-stimulation. Conclusion : Finally, ability of selective attention and self-control, and consistent attention is progressed by applying the tDCS to the normal people.

기능에서 신체분절의 협응과 기여 (The Coordination and Contribution of Body Segments during Functioning)

  • Purpose: This study investigated the coordination and contribution of body segments during functioning. Methods: The relevant literature related to body segments and function were reviewed. Results: Efficient control of function is considered with regard to a participant's ability to perform a sequence of movements in body segments, which progresses from the head to the arm, trunk, pelvis, and leg segments. Each segment performs a specific role, which environment explorer using visual information for the head, reaching and grasping for the arms, a stabilizer for the trunk, and the distribution of COM in the pelvis and leg. Conclusion: During any of the movements, the momentum generated by the proximal segments is transferred to the adjacent distal segments in an appropriate sequence. In assessing function for clinical intervention strategies, the segment coordination, segment sequence, transfer of the center of body mass, asymmetrical ratio, muscle activity, and compensatory strategies should be considered.

협응이동훈련을 위한 PNF 패턴의 결합 (The Combination of PNF Patterns for Coordinative Locomotor Training)

  • Purpose : To identify importance of coordinative locomotor training, we reviewed the coordinative locomotor training using sprinter & skater with respect to motor control theory. Methods : We reviewed literatures related with sprinter & skater and coordination.. Results : The coordinative locomotor training is useful tool to improve interlimb coordination. A problem of interlimb coordination ability is to minimize the degree of freedoms during walking and to solve with context-condition variability and how to make a interlimb coordinative structures. A way of solving method is coordinative locomotor training using sprinter & skater in PNF. The coordinative locomotor training set to fit the gait steps can be applied with gait tasks and can be extended by applying the irradiation of the PNF. Conclusion : The coordinative locomotor training using sprinter & skater in PNF is a useful way method to improve interlimb coordination during walking.
