Recurrence and Extraneural Metastasis in 31 Meningeal Hemangiopericytomas (31예 수막 혈관외피세포종에 있어서의 재발 및 신경계외 전이)

  • Purpose : Meningeal hemangiopericytoma(M-HPC), characterized by a high local recurrency and metastatic potential, is a rare neoplasm arising from perivascular pericytes. A retrospective study was performed to identify the recurrence and extraneural metastasis in M-HPC. Materials and Methods : We reviewed the records of 31 M-HPC patients treated from 1982 through 1999 at our institution. The time to recurrence and the various parameters affecting recurrence were determined. Extreneural metastasis was also analyzed. Results : The rate of local recurrency was 38.7%(12/31). The overall average recurrence-free period(RFP) before the first recurrence was 104 months, with overall recurrence-free rates(RFRs) at 5 and 10 years after first surgery of 59.2% and 33.6%, respectively. Of the 12 patients who experienced local recurrence, 4 had recurrences 5 years later after the first surgery. Complete excision at the first operation significantly extended the average time before first recurrence from 43 to 111 months. The 5-year RFRs for the groups of complete excision and incomplete excision were 72.7% and 20.8%, respectively(p=0.0060). Although there was no statistical significance, complete excision followed by adjuvant radiotherapy of more than 50Gy extended the RFP. The 5-year RFRs for the groups of complete excision and complete excision with adjuvant radiotherapy were 70.3% and 100%, respectively(p=0.3359). Four patients(12.9%) presented one or more extraneural metastases that were developed at an average of 107 months after the first operation with the 5- and 10-year metastasis rates of 4.4% and 24.9%, respectively. Conclusions : M-HPC has a propensity to recur either locally or at distant sites after surgical resection. Complete excision is the most important factor to reduce recurrence. However, even with complete excision, adjuvant radiotherapy of more than 50Gy significantly reduces the risk of recurrence. Local and distant recurrences may occur after a prolonged disease-free interval, emphasizing the need for long-term follow-up.

Voice Tremor in Parkinsonism : A Preliminary Study for Differential Diagnosis (파킨슨증의 음성진전 : 감별진단을 위한 예비연구)

  • Tremor is a main factor of parkinsonism. Voice tremor may be the first, later or the only symptom of a neurological disease and its frequency, amplitude, and regularity may differ among the diseases of different neural subsystems. Differential diagnosis between idiopathic Parkinson's disease (IPD) and multiple system atrophy (MSA) has been difficult. This study included three groups: (1) 6 IPD patients; (2) 6 MSA patients; and (3) 20 ageand sex-matched normal controls. The MDVP (Multidimensional Voice Program) was used to analyze the sustained /a/phonation. The results were as follows: (1) frequency perturbation parameters (jitter, sPPQ, Vf0) and FTRI of tremor parameter of two patient groups were statistically different from those of the controls (p < .01); (2) measures were higher in short-term and long-term f0 and amplitude perturbation in MSA than IPD; (3) however, any acoustic parameters between IPD and MSA were not statistically different; except for the rate of frequency tremor, 4$\sim$5 Hz in IPD, 5$\sim$11 Hz in MSA and (4) the pattern of regularity for voice tremor through histogram indicated that amplitude of IPD was irregular while both f0 and amplitude of MSA were irregular. In conclusion, F0, rate of frequency tremor, and pattern of f0 regularity may be predictors for differential diagnosis. These findings might signify that voice tremor of parkinsonism was resulted from modulation of f0.

Influence of Habitual Unilateral Support on Scoliosis Angle and Iliac Crest Height of Lumbago Patients (습관적인 편측지지가 요통환자의 측만각과 장골능 높이에 미치는 영향)

  • This study aims to examine changes from differences in the lumbar scoliosis angle and iliac crest height due to abnormal and habitual posture shown in unilateral weight load at standing posture and suggest data for preventing and treating lumbago. The subjects of this study are 16 lumbago patients between twenties and forties with chronic lumbago over six months, but without neurological symptoms. As a result of photographing front and back with three conditions such as weight load on both sides and left or right unilateral weight load posture in order to examine changes of lumbar scoliosis and iliac crest according to changes of posture at unilateral weight load, while scoliosis angle and iliac crest height by habitual unilateral support were increased, those by opposite support were decreased. In conclusion, it was found that habitual unilateral weight load may cause continuous distortion of spinal angle and change of iliac crest height and these may be a factor of lumbago. Therefore, if habitual unilateral weight load state is kept continuously, distortion of lumbar angle and iliac crest height may be greater and common efforts to change habitual unilateral weight load are needed.

  • Objectives : The purpose of study was to investigate the effect of ILlB(intravascular laser irradiation of blood) on serum lipoprotein(a) Methods : The 12 case is diagnosed as cerebrovascular diseases on computed tomograghy. The 18 case is not observed abnormality on computed tomograghy but, they have neurological sign. Blood test was enforced on the first medical examination, The 15 case with plasma lipoprotein(a) greater than 30 mg/dl have been classified into abnormal group and the 15 case of less than 30 mg/dl have been classified into control group. after ten times ILlB, It was observed that the change of plasma lipoprotein(a) and a fibrinogen of blood clotting factor. Conclusions : 1. After ten times ILlB, plasma lipoprotein(a) numerical value was decreased on 13 case of abnormal group. 2. Plasma lipoprotein(a) numerical value was decreased on 5 case of control group. 3. The twenty in fifteen patient's with hyperlipoproteinemias have been fibrinogenemias and after ILIB treatment, in the all of 16 case with hyperfibrinogenemia in the first medical examination to both abnormal and control group, plasma fibrinogen numerical value was decreased.

Inhibition of LPA5 Activity Provides Long-Term Neuroprotection in Mice with Brain Ischemic Stroke

  • Stroke is a leading cause of long-term disability in ischemic survivors who are suffering from motor, cognitive, and memory impairment. Previously, we have reported suppressing LPA5 activity with its specific antagonist can attenuate acute brain injuries after ischemic stroke. However, it is unclear whether suppressing LPA5 activity can also attenuate chronic brain injuries after ischemic stroke. Here, we explored whether effects of LPA5 antagonist, TCLPA5, could persist a longer time after brain ischemic stroke using a mouse model challenged with tMCAO. TCLPA5 was administered to mice every day for 3 days, starting from the time immediately after reperfusion. TCLPA5 administration improved neurological function up to 21 days after tMCAO challenge. It also reduced brain tissue loss and cell apoptosis in mice at 21 days after tMCAO challenge. Such long-term neuroprotection of TCLPA5 was associated with enhanced neurogenesis and angiogenesis in post-ischemic brain, along with upregulated expression levels of vascular endothelial growth factor. Collectively, results of the current study indicates that suppressing LPA5 activity can provide long-term neuroprotection to mice with brain ischemic stroke.

Spontaneous Spinal Epidural Hematoma with Intraoperative Acute Massive Bleeding - Case Report - (수술중 급성 대량 출혈을 동반한 자발성 척추경막외혈종 - 증 례 보 고 -)

  • Spontaneous spinal epidural hematoma is rare disease and usually presents with a progressive neurological syndrome for which surgical decompression is usually indicated. The cause of bleeding in epidural hematoma remains unknown in most of the cases. The most frequently identified risk factor is coagulopathy or treatment with anticoagulants. Recently, authors experienced a case of spontaneous spinal epidural hematoma with intraoperative profuse bleeding at the cervicothoracic location. Laboratory examination showed no evidence of coagulopathy or hepatic disease. On neurologic examination, left hemiparesis(Grade : II) and left side sensory change were noted. On MRI scan, there was a mass of high signal intensity in T2WI and isosignal intensity in T1WI compressing the cord to left side. The patients had good surgical outcome after decompressive laminectomy and hematoma removal.

Effect of Neurotrophic Factors on Neuronal Stem Cell Death

  • Neural cell survival is an essential concern in the aging brain and many diseases of the central nervous system. Neural transplantation of the stem cells are already applied to clinical trials for many degenerative neurological diseases, including Huntington's disease, Parkinson's disease, and strokes. A critical problem of the neural transplantation is how to reduce their apoptosis and improve cell survival. Neurotrophic factors generally contribute as extrinsic cues to promote cell survival of specific neurons in the developing mammalian brains, but the survival factor for neural stem cell is poorly defined. To understand the mechanism controlling stem cell death and improve cell survival of the transplanted stem cells, we investigated the effect of plausible neurotrophic factors on stem cell survival. The neural stem cell, HiB5, when treated with PDGF prior to transplantation, survived better than cells without PDGF. The resulting survival rate was two fold for four weeks and up to three fold for twelve weeks. When transplanted into dorsal hippocampus, they migrated along hippocampal alveus and integrated into pyramidal cell layers and dentate granule cell layers in an inside out sequence, which is perhaps the endogenous pathway that is similar to that in embryonic neurogenesis. Promotion of the long term-survival and differentiation of the transplanted neural precursors by PDGF may facilitate regeneration in the aging adult brain and probably in the injury sites of the brain.

An Analysis of Infection-Related Complications after Epidural Block (경막외 차단술 후 발생한 감염 관련 합병증의 분석)

  • Background: There have been an increasing number of reports about infection-related complications after epidural block, and the analysis of these previous reports may offer valuable information for the prevention and treatment of such complications. Methods: We searched for complications about infection that was related to epidural blockade procedures by using the Medline Search program. We analyzed the types of infection-related complications as well as the potential risk factors, the time course from symptom development to treatment, the causative organisms and the treatment outcomes. Results: Seventeen cases were identified. The types of complications were epidural abscess, subdural abscess, spinal arachnoiditis, bacterial meningitis and aseptic meningitis. Five patients received a single block and twelve patients received a continuous block with catheterization. The most common site of epidural catheterization was the lumbar area and eight patients had indwelling catheters for less than fifteen days. Eight patients had a diabetes mellitus as a risk factor and fourteen patients showed less than seven days from the development of symptoms to treatment. Eleven patients received laminectomy and intravenous antibiotics as a treatment and eight patients had full recovery without neurological deficit. Conclusions: Early diagnosis and treatment is essential for the favorable outcome of infection-related complication after epidural block. In addition, absolute sterile technique should always be performed and patient education concerning these potential complications must be accompanied.

Modulation of LPS-Stimulated Astroglial Activation by Ginseng Total Saponins

  • Ginseng, a traditional medicine in Asian countries, is known to prevent various neuropathologic diseases such as Alzheimer's. Ginseng total saponins (GTS) in particular are one of the most effective ginseng extract compounds for neuroprotection. However, their protective effects on astrocytes are rarely reported. In pathological circumstances, astroglial activation plays a pivotal role in neuroinflammation. Subsequently, neuroinflammation induced by activated astrocytes causes brain damage. The purpose of the present study was to determine the suppressive effects of GTS on astroglial activation in lipopolysaccharide (LPS)-stimulated rat primary astrocytes. Astrocytes treated for 24 h with LPS demonstrated suppressed glialfibrillary acidic protein expression in a dose-dependent manner in the presence of GTS. GTS reduced production of proinflammatory cytokines such as tumor necrosis factor-${\alpha}$ and interleukin-1${\beta}$ and inhibited the level of inducible nitric oxide synthase, and cyclooxygenase-2 in LPS-stimulated astrocytes. Furthermore, GTS suppressed intracellular reactive oxygen species production. These modulations due to GTS may indicate neuroprotective antiinfl ammatory properties which may in turn be related to improvements in neurological performance.

Ameliorative Potential of Rengyolone Against CCI-induced Neuropathic Pain in Rats

  • The sciatic nerve is the largest nerve among the peripheral nerves, and the damage to the sciatic nerve is caused by mechanical and physical pressure. This is an important disease that consumes a lot of time and money in the treatment process. Among them, research on relieving nerve pain caused by damage to the peripheral sciatic nerve has been made efforts to prevent and treat this disease through various methods such as drugs, natural products, electrical stimulation, exercise therapy, and massage. Existing treatments are not very effective in neurological pain, and countermeasures are needed. Forsythia Fructus, used in this study, has been used as a therapeutic agent for infectious diseases and a pain reliever for cancer from the past, and in past studies, it has been known to properly control the inflammatory response. In this study, rengyolone, a physiologically active substance of Forsythiae Fructus, was administered to rats that caused chronic left nerve pain to verify the pain relief effect. As a result of the experiment, it was found that mechanical pain and cold stimulation pain were significantly reduced in the rengyolone-treated group compared to the non-administered group. In addition, it was found that nerve growth factor (NGF) mRNA expression was significantly reduced and Cyclin-dependent kinase 2 (Cdc2) expression was increased in the rengyolone administration group. This increase in NGF expression is thought to be related to rengyolone's anti-inflammatory regulatory mechanism. It is expected that the reduced NGF was directly involved in pain relief.
