한방병원에 입원한 외상성 뇌손상으로 발생한 신경정신학적 장애 환자 1례 (A Case Report of Neuropsychiatric Disorder Induced by Traumatic Brain Injury in Traditional Korean Medicine Hospital)

  • Neuropsychiatric disorder is one of the symptoms developed after traumatic brain injury. The main symptoms of neuropsychiatric disorder are delirium, anxiety, depression and so on. We treated the patient with herbal medicine, acupuncture, moxibustion and other appropriate traditional Korean medical treatment. The patient only appeared neuropsychiatric disorder, not another neurologic symptoms. We consider it is the rare case in traditional Korean medicine hospital. So we report the patient with the review of literatures regarding neuropsychiatric disorder induced by traumatic brain injury.

뇌병변장애 1급 아동에서 발생한 중증유아기우식증 (SEVERE-EARLY CHILDHOOD CARIES IN CHILDREN WITH GRADE 1 NEUROLOGICAL DISORDER)

  • 뇌병변장애인은 뇌성마비, 외상성 뇌손상, 뇌졸중 등 뇌의 기질적 병변에 기인한 신체적 장애로 보행 또는 일상생활의 동작 등에 상당한 제한을 받는 사람으로 정의된다. 최근에 뇌병변 장애아동의 DMFT는 비 장애아동과 유사하거나 낮다는 연구가 있었다. 그러나, 본 증례는 중증유아기우식증이 나타난 뇌병변장애 1급 아동들을 치료한 것으로, 아동들의 공통점을 통해 중증유아기우식 우식증의 특별한 원인을 찾고자 하였다. 뇌병변으로 인한 일차적인 운동장애와 이차적인 치과치료를 받기 어려울 정도의 쇠약함, 잦은 입원, 당분이 많은 음식과 약물의 섭취, 구강위생관리 소홀, 늦은 치과 내원, 협조도 저하 등이 우식 유발인자를 높이고, 우식 예방인자를 낮추어 치아우식증을 유발할 수 있는 것으로 사료되었다. 뇌병변 장애아동을 치료할 때, 치과의사는 개인적인 우식 위험도 평가를 통해 철저한 우식 예방프로그램을 만들고, 부모와 의학적 주치의는 구강관리의 중요성을 인식하여 조기 치과 내원 및 홈케어에 신경을 써야 할 것이다.

  • Sleep disturbances are frequently associated with neurological disorders. Sleep disorders interfere with rehabilitation of patients with neurological disorders such as stroke and may increase the severity of their symptoms and recurrence rate of stroke. The treatment of sleep apnea syndrome is particularly important in managing patients with cerebral infarction of whom 50-80% have moderate to severe sleep apnea. Sleep apnea produces not only poor quality sleep but also excessive daytime sleepiness, fatigue and lack of energy. Sleep problems frequently found in patients with dementia are sleep-wake cycle abnormality, fragmentation of sleep, nocturnal insomnia, decreased slow wave sleep and REM sleep, and sleep disordered breathing. The management of sleep disturbances is very important for controlling symptoms such as nocturnal wandering and sundowning syndrome in patients with dementia. Parkinson's disease and epilepsy are other neurological disorders that may have sleep disturbances.

A New Therapeutic Approach to Dystonia

  • Dystonia is a neurological disorder characterized by involuntary and uncontrollable muscle tonus abnormalities. It is a huge burden not only to the patients and their families, but also to the field of medicine, in that there has hardly been any substantial change in the concept of and approach to this intractable disorder and therefore no breakthrough to its diagnosis, evaluation and treatment. As an effort to solve the current impasse, this review briefly summarizes the current concept, etiology, diagnosis, treatment and management, and then suggests a rather new therapeutic approach to this disorder, based on the concept of neurological balance and TMJ integrative approach. These new approaches will provide a platform for the clinicians and researchers to have a leap in the concept, diagnosis and therapeutics.

  • Cerebrospinal fluid (CSF) is a dynamic metabolically active body fluid that has many important roles and is commonly analyzed in pediatric patients, mainly to diagnose central nervous system infection and inflammation disorders. CSF components have been extensively evaluated as biomarkers of neurological disorders in adult patients. Circulating microRNAs in CSF are a promising class of biomarkers for various neurological diseases. Due to the complexity of pediatric neurological disorders and difficulty in acquiring CSF samples from pediatric patients, there are challenges in developing CSF biomarkers of pediatric neurological disorders. This review aimed to provide an overview of novel CSF biomarkers of seizure disorders, infection, inflammation, tumor, traumatic brain injuries, intraventricular hemorrhage, and congenital hydrocephalus exclusively observed in pediatric patients.

  • Functional dysphonia is a specific voice disorder refers to dysphonia without abnormal anatomical vocal fold findings at larynx. The proportions of this disorder are estimated up to 40% of dysphonia patients at ENT clinics. In this article, we will discuss about other functional dysphonia and neurological dysphonia except for muslce tension dysphonia and spasmodic dysphonia. For details, will describe about phonatory charateristics and treatment options about paradoxical vocal fold motion disorder, mutational dysphonia, essential vocal tremor, conversion dysphonia, and vocal tremor related with parkinson's disease.

지발형 오르니틴 트랜스카바미라제 결핍증 환자들의 신경학적 예후 (Neurological Outcome of Patients with Late-onset Ornithine Transcarbamylase Deficiency)

  • The most common urea cycle disorder is ornithine transcarbamylase deficiency. More than 80 percent of patients with symptomatic ornithine transcarbamylase deficiency are late-onset, which can present various phenotypes from infancy to adulthood. With no regards to the severity of the disease, characteristic fluctuating courses due to hyperammonemia may develop unexpectedly, and can be precipitated by various metabolic stressors. Late-onset ornithine transcarbamylase deficiency is not merely related to a type of genetic variation, but also to the complex relationship between genetic and environmental factors that result in hyperammonemia; therefore, it is difficult to predict the prevalence of neurological symptoms in late-onset ornithine transcarbamylase deficiency. Most common acute neurological manifestations include psychological changes, seizures, cerebral edema, and death; subacute neurological manifestations include developmental delays, learning disabilities, intellectual disabilities, attention-deficit/hyperactivity disorder, executive function deficits, and emotional and behavioral problems. This review aims to increase awareness of late-onset ornithine transcarbamylase deficiency, allowing for an efficient use of biochemical and genetic tests available for diagnosis, ultimately leading to earlier treatment of patients.

입체해석학을 통해 신경과학의 정보를 작업치료학에 어떻게 전달할수 있을까? (How does stereology help to inform translation from neuroscience to OT?)

  • 서론: 작업치료에서는 수행기술을 매우 중요하며, 이런 수행기술에는 감각 인지 기술, 운동실행 기술, 감정 조절기술, 인지기술, 소통 사회 기술 등이 있다. 모든 수행기술은 통합된 신경처리를 통해 이루어진다. 본론: 입체해석학(stereology)란 신경세포, 대뇌 겉질, 기능과 관련되어 뇌의 특정 영역의 형태 변화를 알아 낼 수 있는 과학적 방법이다. 입체해석학은 평면으로 잘려진 생물학적 샘플을 확률이론과 과학적 추출방법을 사용하여 3 차원적으로 분석하는 다학문분야이다. 편견 없는 입체해석학적 방법은 확률이론을 바탕으로하며, 생물학적 영역의 해부학적 형태적 변화를 예측할 수 있다. 부피, 숫자, 길이와 같은 세포구조적변화는 뇌의 기능에 영향을 미치게 된다. 작업치료사들은 신경계환자들의 뇌 기능을 향상시켜 참여를 증진하는 것을 목적으로 한다. 뇌기능 장애 환자들에게 있어 기능적 증진은 뇌의 변화를 의미한다. 그러므로 신경학적 변화와 기능적 증진에 대한 두 가지 증거는 신경계 환자들의 작업치료에 매우 중요한 기초적 치료 근거가 된다. 비록 입체해석학 자체가 실험이나 인체 사후 검사에 많이 쓰일지라도 입체해석학은 신경계 환자들의 작업치료에 근본적 지식들 제공한다. 결론: 그러므로 입체해석학은 신경과학의 지식들을 경험의존가소성과 수행기술에 대한 뇌의 구조-기능관계를 바탕으로 작업치료에 전달해 준다고 볼 수 있다.

  • The periodic limb movement (PLM) disorder is a disease of motor sign mainly in the lower extremities, whereas the restless leg syndrome (RLS) accompanies sensory symptoms in the lower extremities. These two disorders may occur in the one patient, which implies possible common pathophysiological background in those disorders. The aim of this article is to review the clinical features, diagnostic criteria, electrophysiological characteristics of the two disorders and their relation to neurological disorders.

  • Type 2 diabetes mellitus (T2DM) is a progressive disorder caused by a combination of insulin resistance and 𝛽 cell dysfunction. Sogal(消渴) is a traditional Korean medical term referring to a condition pertaining 3 major symptoms - thirst, polyphasia, polyuria. Sogal has been reported to have similar characteristics with DM. This case report demonstrates a patient with T2DM complaining of typical Sogal symptoms. We diagnosed him as So-yang person Sogal and treated him with acupuncture and herbal medicine.
