Clinical validation of the 3-dimensional double-echo steady-state with water excitation sequence of MR neurography for preoperative facial and lingual nerve identification

  • Purpose: This study aimed to evaluate the clinical usefulness of magnetic resonance (MR) neurography using the 3-dimensional double-echo steady-state with water excitation (3D-DESS-WE) sequence for the preoperative delineation of the facial and lingual nerves. Materials and Methods: Patients underwent MR neurography for a tumor in the parotid gland area or lingual neuropathy from January 2020 to December 2021 were reviewed. Preoperative MR neurography using the 3D-DESS-WE sequence was evaluated. The visibility of the facial nerve and lingual nerve was scored on a 5-point scale, with poor visibility as 1 point and excellent as 5 points. The facial nerve course relative to the tumor was identified as superficial, deep, or encased. This was compared to the actual nerve course identified during surgery. The operative findings in lingual nerve surgery were also described. Results: Ten patients with parotid tumors and 3 patients with lingual neuropathy were included. Among 10 parotid tumor patients, 8 were diagnosed with benign tumors and 2 with malignant tumors. The median facial nerve visibility score was 4.5 points. The distribution of scores was as follows: 5 points in 5 cases, 4 points in 1 case, 3 points in 2 cases, and 2 points in 2 cases. The lingual nerve continuity score in the affected area was lower than in the unaffected area in all 3 patients. The average visibility score of the lingual nerve was 2.67 on the affected side and 4 on the unaffected side. Conclusion: This study confirmed that the preoperative localization of the facial and lingual nerves using MR neurography with the 3D-DESS-WE sequence was feasible and contributed to surgical planning for the parotid area and lingual nerve.

Use of Magnetic Resonance Neurography for Evaluating the Distribution and Patterns of Chronic Inflammatory Demyelinating Polyneuropathy

  • Objective: To evaluate the distribution and characteristics of peripheral nerve abnormalities in chronic inflammatory demyelinating polyneuropathy (CIDP) using magnetic resonance neurography (MRN) and to examine the diagnostic efficiency. Materials and Methods: Thirty-one CIDP patients and 21 controls underwent MR scans. Three-dimensional sampling perfections with application-optimized contrasts using different flip-angle evolutions and T1-/T2- weighted turbo spin-echo sequences were performed for neurography of the brachial and lumbosacral (LS) plexus and cauda equina, respectively. Clinical data and scores of the inflammatory Rasch-built overall disability scale (I-RODS) in CIDP were obtained. Results: The bilateral extracranial vagus (n = 11), trigeminal (n = 12), and intercostal nerves (n = 10) were hypertrophic. Plexus hypertrophies were observed in the brachial plexus of 19 patients (61.3%) and in the LS plexus of 25 patients (80.6%). Patterns of hypertrophy included uniform hypertrophy (17 [54.8%] brachial plexuses and 21 [67.7%] LS plexuses), and multifocal fusiform hypertrophy (2 [6.5%] brachial plexuses and 4 [12.9%] LS plexuses) was present. Enlarged and/or contrast-enhanced cauda equina was found in 3 (9.7%) and 13 (41.9%) patients, respectively. Diameters of the brachial and LS nerve roots were significantly larger in CIDP than in controls (p < 0.001). The largest AUC was obtained for the L5 nerve. There were no significant differences in the course duration, I-RODS score, or diameter between patients with and without hypertrophy. Conclusion: MRN is useful for the assessment of distribution and characteristics of the peripheral nerves in CIDP. Compared to other regions, LS plexus neurography is more sensitive for CIDP.

  • A magnetic resonance neurography technique for exclusive imaging of nerve networks is proposed. In the proposed technique, the anisotropy of diffusion coefficients of the molecules inside nerve cells was utilized to effectively suppress the magnetic resonance signal radiating from ones other than nerve tissues. Two images were obtained with diffusionweighting magnetic field gradients applied in the parallel and perpendicular directions with respect to the nerve orientation. One image was then subtract- ted from the other to form the neurogram. Experimentally obtained neurograms of the squld are presented.

Accessory Belly of the Piriformis Muscle as a Cause of Piriformis Syndrome: a Case Report with Magnetic Resonance Imaging and Magnetic Resonance Neurography Imaging Findings

  • Piriformis syndrome caused by an accessory belly of the piriformis muscle is very rare. Only a few cases have been reported. Here, we report a case of piriformis syndrome resulting from an extremely rare type of accessory belly of the piriformis muscle originated at the proximal third portion of the main piriformis muscle and attached separately to the greater trochanter inferior to the insertion of the main piriformis muscle. A definitive diagnosis of piriformis syndrome was made based on magnetic resonance imaging and magnetic resonance neurography findings that were consistent with results of nerve conduction study and needle electromyography.

An Updated Review of Magnetic Resonance Neurography for Plexus Imaging

  • Magnetic resonance neurography (MRN) is increasingly used to visualize peripheral nerves in vivo. However, the implementation and interpretation of MRN in the brachial and lumbosacral plexi are challenging because of the anatomical complexity and technical limitations. The purpose of this article was to review the clinical context of MRN, describe advanced magnetic resonance (MR) techniques for plexus imaging, and list the general categories of utility of MRN with pertinent imaging examples. The selection and optimization of MR sequences are centered on the homogeneous suppression of fat and blood vessels while enhancing the visibility of the plexus and its branches. Standard 2D fast spin-echo sequences are essential to assess morphology and signal intensity of nerves. Moreover, nerve-selective 3D isotropic images allow improved visualization of nerves and multiplanar reconstruction along their course. Diffusion-weighted and diffusion-tensor images offer microscopic and functional insights into peripheral nerves. The interpretation of MRN in the brachial and lumbosacral plexi should be based on a thorough understanding of their anatomy and pathophysiology. Anatomical landmarks assist in identifying brachial and lumbosacral plexus components of interest. Thus, understanding the varying patterns of nerve abnormalities facilitates the interpretation of aberrant findings.

Neuroradiologic and Neurophysiologic Findings of Neuralgic Amyotrophy

  • Objective : Neuralgic amyotrophy (NA) is a distinct clinical syndrome that is characterized by the acute onset of shoulder and arm pain, weakness, and sensory loss. The purpose of this study was to assess the clinical characteristics of NA and to determine appropriate diagnostic modalities. Methods : We reviewed the medical and radiologic records of 10 patients diagnosed with NA retrospectively. Neurophysiologic studies were performed in all patients and magnetic resonance neurography was performed in the last three patients. Results : A total of 10 patients were enrolled in our study. All patients had clinical findings compatible with NA. The most common clinical presentation was severe shoulder pain and weakness in seven patients (70%). Neurophysiologic study results were abnormal in all patients. Brachial plexus magnetic resonance neurography showed that the affected brachial plexus showed a thickened and hyper-intense trunk. All patients were managed conservatively with analgesics and physical therapy. The pain and paralysis of all patients improved clinically within 6 months of the initiation of treatment. Conclusion : NA is a rare disease but the symptoms of NA can mimic those of other diseases. Neurophysiologic studies and magnetic resonance neurography are extremely useful tools for the diagnosis of NA.

요추 신경근 평가를 위한 1.5T MR의 단일 방향 경사자장을 사용한 확산강조 자기공명신경조영 (Diffusion-Weighted MR Neurography with Unidirectional Motion-Probing Gradient to Evaluate Lumbar Nerve Roots at 1.5T MR)

  • 목적 최근 확산강조 자기공명신경조영(diffusion-weighted MR neurography; 이하 DW MRN)이 신경근 평가에 도움이 된다고 보고되었다. 본 연구는 1.5T MR에서 단일방향 경사자장을 사용한 DW MRN의 요추 신경근 평가의 유용성을 확인하고자 한다. 대상과 방법 앞뒤 방향 경사자장의 DW MRN을 포함한 64요추 MR을 후향적으로 분석했다. 제3 요추에서 제1 천추까지 총 512개 요추 신경근의 변화를 T2 강조영상, 조영증강 T1 강조영상, 그리고 DW MRN에서 평가하고 일치도와 상관관계 분석을 했다. 결과 T2 강조영상에서 78개의 신경근 압박이 있었고, 조영증강 T1 강조영상에서 52개 신경근이 조영증강되었다. DW MRN에서 67개 신경근의 부종과 고신호강도가 있었다. 조영증강 T1 강조영상과 DW MRN 모두 신경근의 변화가 나타난 경우는 42개였다. DW MRN과 조영증강 T1 강조영상, T2 강조영상 간에 중간 또는 상당한 일치도와 양의 상관관계를 보였다(κ = 0.59-0.65, ρ = 0.600-0.653). 결론 앞뒤 단일방향을 사용한 DW MRN은 척추 신경근의 변화 평가에 도움이 되며, 가돌리늄 조영증강을 대체 또는 보완하는 역할을 할 수 있을 것이다.

말초신경질환에서 자기공명영상의 진단적 가치 (The Value of MRI in Diagnosis of Peripheral Nerve Disorders)

  • Objective : The development of magnetic resonance neurography(MRN) has made it possible to produce highresolution images of peripheral nerves themselves, as well as associated intraneural and extraneural lesions. We evaluated the clinical application and utility of high-resolution MRN techniques for the diagnosis and treatment of a variety of peripheral nerve disorder(PND)s. Material and Method : MRN images were obtained using T1-weighted spin echo, T2-weighted fast spin echo with fat suppression, and short tau inversion recovery(STIR) fast spin-echo pulse sequences. Fifteen patients were studied, three with brachial plexus tumors, five with chronic entrapment syndromes, and seven with traumatic peripheral lesions. Ten patients underwent surgery. Results : In MRN with STIR sequences of axial and coronal imagings, signals of the peripheral nerves with various lesions were detected as fairly bright signals and were discerned from signals of the uninvolved nerves. Increased signal with proximal swelling and distal flattening of the median nerve were seen in all patients of carpal tunnel syndrome. Among the eight patients with brachial plexus injury or tumors, T2-weighted MRN showed increased signal intensity in involved roots in five, enhanced mass lesions in three, and traumatic pseudomeningocele in three. Other associated MRI findings were adjacent bony signal change, neuroma, root adhesion and denervated muscle atophy with signal change. Conclusion : MRN with high-resolution imaging can be useful in the preoperative evaluation and surgical planning in patients with peripheral nerve lesions.

요천추 신경총에 대한 자기공명신경조영술의 역할: 주제 범위 문헌고찰 (Role of MR Neurography for Evaluation of the Lumbosacral Plexus: A Scoping Review)

  • 목적 자기공명신경조영술은 말초신경을 시각화하는 데 최적화된 영상 기법이다. 본 주제 범위 문 헌고찰에서는 요천추신경총에서 자기공명신경조영술의 프로토콜을 조사하고, 요추신경총 질환 환자에서 자기공명신경조영술의 임상적 이득에 대해 고찰하고자 한다. 대상과 방법 두 개의 의료 데이터베이스에서 2021년 9월까지 영문으로 출판된 논문에 대해 체계적 문헌검색을 수행하였다. 'Magnetic resonance Imaging', 'lumbosacral plexus', 'neurologic disease'를 포함하는 55편의 논문을 분석하였다. 결과 요천추신경총의 자기공명신경조영술은 말초 신경 질환의 분포 확인, 신경 주변 주사시 유도, 좌골신경통 환자에서는 척추외 원인 평가에 유용하였다. 혈관억제 기법이 적용된 3차원 단시간 반전회복 고속 스핀에코 영상이 주된 자기공명신경조영술 기법이었다. 결론 향후 요천추신경총의 자기공명신경조영술에 대한 기술적 성숙과 임상적 유용성에 대한 검증이 필요하다.

Neuralgic Amyotrophy Manifesting as Mimicking Posterior Interosseous Nerve Palsy

  • The upper trunk of the brachial plexus is the most common area affected by neuralgic amyotrophy (NA), and paresis of the shoulder girdle muscle is the most prevalent manifestation. Posterior interosseous nerve palsy is a rare presentation in patients with NA. It results in dropped finger on the affected side and may be misdiagnosed as entrapment syndrome or compressive neuropathy. We report an unusual case of NA manifested as PIN palsy and suggest that knowledge of clinical NA phenotypes is crucial for early diagnosis of peripheral nerve palsies.
