Clinical Evaluation of the Mediastinal Tumors [26 Cases Report] (종격동 종양의 임상적 고찰: 26례 보고)

  • Mediastinal tumors which are originated in mediastinum or probably metastasized from other organs have long fascinated the surgeon because of difficulty of diagnosis and treatment. This report is the analysis of the 26 cases of mediastinal tumors, experienced in the Department of the Thoracic and Cardiovascular Surgery, Chosun University Hospital from January 1978 to January 1985. The result are as follows; 1. The age distribution was 10 months to 68 years old and the average mean age was 34.7 years old. 2. Subjective symptom were as follows: Dyspnea [69.2%], Coughing [42.3%], Chest pain [30.8%], Back pain [15.4%] and Numbness of extremities [15.4%]. Objective signs were as follows: Decreased breathing sound [53.3%], Pleural effusion and hemothorax [34.6%], Palpable neck mass [34.6%], SVC Syndrome [19.4%] and Bloody sputum [15.4%]. But, there were no definitive symptoms in 2 cases. 3. The malignant tumors were 19 cases [73.1%]. 4. The germ cell tumors were 2 cases [7.7%], the neurogenic tumor were 3 cases [11.5%], lymphoma were 8 cases [30.8%], thymoma were 2 cases [7.7%], mesenchymal tumor was 1 case [3.8%], cyst was 1 case [3.8%] and carcinoma were 5 cases [19.2%] among the 23 cases, histologically analyzed. 5. The successful complete removal was done in 5 cases among 7 cases of benign tumors. In malignant cases, the surgical removal had been 7 cases and inoperable cases were treated to radiation and chemotherapy.

Imaging Manifestations and Misdiagnosis Analysis of Six Cases of Bone Hydatid Disease

  • We retrospectively evaluated the clinical and imaging features of 6 patients with bone hydatid disease confirmed by surgery and pathological examination. Among the 6 patients, 2 were infected with Echinococcosis granulosus metacestode and 4 were infected with E. multilocularis metacestode. The 2 cases with cystic echinococcosis were diagnosed by computed tomographic (CT) examination, and other 4 cases were diagnosed by magnetic resonance (MR) imaging. On the initial evaluation, 1 case each was misdiagnosed as a giant cell tumor or neurogenic tumor, and 2 were misdiagnosed as tuberculosis. The imaging manifestations of bone hydatid disease are complex, but most common findings include expansive osteolytic bone destruction, which may be associated with sclerosing edges or dead bone formation, localized soft tissue masses, and vertebral lesions with wedge-shaped changes and spinal stenosis. Combining imaging findings with the patient's epidemiological history and immunological examinations is of great help in improving the diagnosis and differential diagnosis of bone hydatid disease.

  • For the purpose of clinical analysis In mediastinal tumors, 51 patients with mediastinal tumor who were surgically treated from February 1984 to February 1994 in Chunan hospital of Soonchunhyang university were reviewed. There were 21 male and 30 female patients in the study. Age ranges from 11 months to 75 years, with the mean 39.4$\pm$ 18.8 years. Symptoms and signs were asymptom(21.6%), chest d scomfort(19.6%), dyspnea(15.7%), ptosls(15.7%), general weakness(13.7%) and chest pain(9.8%). The most frequent tumor location was anterosuperior mediastinum(60.8%) followed by posterior(25.5%) and middle(13.7%) mediastinum. In the pathological viewpoint, thymoma(33.3%) was the most frequent type followed by neurogenic tumor(25.5%) an'd germ cell tumor(19.6%). All of benign tumors(46 cases) were completely removed and malignant tumors(5 cases) were treated with rAdiotherapy after operation. The postoperative complications were 2 wound dehesences, 1 pneumothorax, 1 vocal cord palsy and 1 prolonged mechanical ventilation.

  • There has been a marked increase in the utility of aspiration cytology for pathologic diagnosis. It may be applied to any kinds of organs and substitutes surgical biopsy. Be cause of the high risk of complication and difficulties In localization, aspiration cytology in the central nervous system (CNS) has been used with less frequency compared to other sites. However, with the advent of sophisticated imaging instruments, aspiration cytology of lesions in the CNS is being used increasingly. We present cytologic features of brain tumors as well as corresponding histologic findings. Eight types of tumors were aspirated intraoperatively and stained with the Papanicolaou method; 1 anaplastic astrocytoma, 1 glioblastoma multiforme, 1 ependymoma, 1 malignant ependymoma, 1 central neurocytoma, 1 primitive neuroectodermal tumor, 1 benign neurogenic tumor and 1 germinoma. Cytologic features of the CNS neoplasms were quite similar to those of histology except one spindle cell tumor. Reviewing various CNS neoplasms, it appears that cytology may be a useful diagnostic method.

Current Status of General Thoracic Surgery in Korea (한국의 일반 흉부수술 현황)

  • Overall 25,095 cases of general thoracic surgery were analysed, which were performed by 48 institutes in Korea during recent 6 years[242 hospital-years]. The proportions of tumorous disease and infectious disease to be operated were 6,864 cases[27.4%] and 6,775 cases [27.0%], The most common organ involved for operation was lung-bronchus 16,542 cases [69.5%], and remainders were pleura 2,500 [10.0%], esophagus 2,433[9.7%], mediastinum 1,902[7.6%], chest wall 1,297 [5.2%], and diaphragm 421 [1.7%] in order. Among 6,864 cases of tumorous diseases, the most common causes for operation were lung-bronchus tumor 3132 cases [45.6%] and most of them were lung cancer 2,731 cases [88.7%]. In the 2,019 cases of primary lung cancer with known cell type, squamous cell carcinoma 1,296 cases [64.2%] and adenocarcinoma 460 cases [22.8%] were the most. The common types in the 1,207 cases of mediastinal tumor with known cell type were neurogenic tumor 348 cases [28.8%], thymoma 311 [25.8%], and teratoma 252[20.9%]. The annual cases of operation for tumorous disease including malignant tumor were increased steadily. Operation for infectious lung diseases [including bronchiectasis and tuberculosis] were about twice common than infectious pleural disease [i.e. empyema], and operations for tuberculous disease occupied about half cases of infectious lung disease. In 11,456 cases of other disease entities, excluding tumorous and infectious disease, there were bullous lung disease 9,074 cases[79.2%], benign esophageal disease 484[4.2%], myasthenia gravis 356[3.1%], chest wall deformity 483[4.2%], and diaphragmatic lesion 421[3. 7%] in order. We propose that above results for inquiry can be used as the basic data of general thoracic surgery in Korea.

  • Neurogenic tumors of the brachial plexus region are relatively rare. We report a recent experience of schwannoma of the right brachial plexus in the inferior trunk, which was successfully treated by microsurgical resection. A 38-year- old man presented a dysesthetic pain in the supraclavicular area and the right forearm of C6, 7 dermatome. Rubbery hard mass was palpated in the right supraclavicular area and magnetic resonance imaging showed a well circumscribed, well enhanced ovoid mass with cystic degeneration on the right brachial plexus portion. The patient underwent complete removal of the mass through the anterior cervicothoracic (modified Dartevelle) approach. At the postoperative 3 months, there is no neurologic deficit.

  • Schwannoma is a slow-growing benign tumor of neurogenic origin. The lesion is derived from the Schwann cells surrounding neural tissue. It can grow in almost all regions of the body. Schwannoma of the external auditory canal is a rare finding. Recently, we experienced schwannoma of the external auditory canal of a 29-year-old female who had surgical removal completely and verified histopathologically as schwannoma.

  • A wide variety of histologically or clinically different tumors and cysts can occur from the many anatomical structures located within the mediastinum. We report the analysis of the 26 cases of mediastinal tumors and cysts, experienced in the Department of Thoracic and Cardiovascular Surgery, College of Medicine, Hallym University from March 1983 to February 1990. Sex ratio of male to female was 1:1.16 and mean age was 33.4 years. Malignant tumors were 8 cases[30.7%], benign tumors were 18 cases[69.2%] The most common histologic types were thymoma, 6 cases[23%] and teratoma, 6 cases [23%] followed by neurogenic tumor, 4 cases[15%] and mediastinal tuberculoma, 3 cases [11.5%]. The most frequent symptoms were chest pain and discomfort. Most of benign tumors were completely removed and malignant tumors were treated with anticancer chemotherapy and radiotherapy after operation. Postoperative complications were developed in 4 cases[15.3%] and hospital mortality was 3.8%.

Subarachnoid-Pleural fistula after Excision of Posterior Mediastinal Mass (후종력동종양제거술후 발생한 척추지주막하늑막강루)

  • Subarachnoid-pleural fistula after routine thoracotomy is a rare complication but a very serious problem. Twenty one cases have been reported in the literature. We report a care of subarchnoid-pleural fistula that dveloped after the esecation of posterior mediastinal neurogenic tumor. The patient presented with large amount of clear pleural fluid with mild headache and dizziness. Surgical intervention following a trial of conservative therapy was undertaken because we strongly suspected subarachnoid-pleural fistula. A dural tear was found at the level of resected intercostal nerve root. The dura was closed by way of direct suture and fibrin glue. In this case, the recognition of subarachnoid-pleural fistula formation is difficult because the patient had not presented any neurologic deficit.

A Case of Primary Endobronchial Neurilemmoma Without Intraspinal Extension

  • Neurilemmoma is a benign and slowly growing neurogenic tumor. Intrathoracic neurilemmoma often develops in the chest wall and posterior mediastinum, but endobronchial neurilemmoma is extremely rare. The diagnosis of endobronchial neurilemmoma with preoperative imaging findings is challenging and is usually made via postoperative pathological examination. These authors encountered a case of primary endobronchial neurilemmoma in a 52-year-old woman who had no symptoms. A $3.0{\times}2.6$ cm mass in the right lower lobe projecting into the mediobasal segmental bronchus was shown in the results of the contrast-enhanced computed tomography (CT) and magnetic resonance imaging (MRI) of the chest. Benign neurilemmoma was confirmed via bronchoscopic biopsy, and surgical resection (sleeve bronchial excision and end-to-end anastomosis) was performed.
