결핵균 PCR에서 이온교환수지를 이용한 신속한 DNA 분리 (Rapid Extraction of DNA using Ion Exchange Resin for Early Detection of Mycobacterium tuberculosis by the Polymerase Chain Reaction)

  • 연구배경: 본 연구는 결핵의 조기검출을 위하여, PCR을 이용하는데 있어서의 중요한 문제점의 하나인 임상검체로부터 결핵균을 분리 방법을 개선하여, 교차오염의 가능성을 최소화하고 PCR에 의한 결핵균의 DNA검출을 보다 쉽고 안전하게 실시하여 대량의 검체를 처리해야하는 일상 임상검사법으로 정착시키고자 하는데 그 목적이 있다. 방법: 다양한 방법으로 DNA를 검출하여 동일한 방법으로 PCR을 수행하여 이차 PCR 산물의 전기영동 결과를 AFB도말 및 배양검사 결과와 함께 비교하여 감수성, 특이성, 양성예측도 및 음성예측도의 항목으로 비교분석하였다. 실험 1은 Proteinase K 처리후 phenol로 추출하는 방법과 Chelex 100 이온교환 수지를 이용한 InstaGene법을 100예에서 비교하였으며, 실험 2에서는 Microwave 처리후 원침 상청액을 직접 시용하는 방법과 Chelex 100 이온교환 수지를 이용한 InstaGene 법을 98예에서 비교하였다. 결과: 세 DNA 분리법으로 실시한 PCR결과에서 모두 특이성과 양성예측도가 매우 낮았다. 실험 1에서 Proteinase K 법에 의한 경우 보다 Insta Gene을 이용한 경우에 약 20% 높은 감수성과 10%~20% 높은 음성예측도를 보이고 있으며, 특이성은 2%~8% 낮고, 양성예측도는 2.5%~4% 높은 것으로 나타났다. 실험 2에서는 Microwave법과 Insta Gene을 이용한 경우에 거의 비슷한 결과를 보였다. 결론: 결핵진단시 PCR을 위한 객담검체에서의 DNA 분리시에 Microwave법과 $InstaGene^{TM}$ DNA분리 kit가 매우 효율적이며, 특히 InstaGene법이 교차오염의 가능성이 거의 없고, 처리 시간이 짧으며, 조작이 매우 간단하며 매우 경제적인 것으로 나타났다. 다만 특이성을 더욱 높일 수 있도록 연구가 추가되어야 할 것이며 일반 인구집단에서 충분한 임상검체를 대상으로 연구가 추가되면 InstaGene법의 유용성이 더욱 확실히 검증 될 것으로 사료된다.

Prevalence of Human Papillomavirus Types and Phylogenetic Analysis of HPV-16 L1 Variants from Southern India

  • Background: The human papillomavirus (HPV) and its variants show wide geographical distribution and have been reported to cause cervical lesions. With cervical neoplasia as the leading cancer in Indian women, the aim of the present study was to evaluate the multiple infection HPV type distribution and variant genotypes in cervical samples from the coastal Karnataka region, India. Materials and Methods: A total of 212 samples were screened by nested polymerase chain reaction using PGMY9/11 and GP5+/6+ primers. HPV positive samples were sequenced to identify the types and a phylogenetic tree was constructed using the neighbor-joining method. Results: Sequence analysis identified a total of 14 HPV types distributed in 20%, 73.3% and 82.5% of non-malignant, pre-malignant [low grade squamous intraepithelial lesion (LSIL) and high grade squamous intraepithelial lesion (HSIL)] and cervical cancer samples. The distribution of high risk HPV in cancer samples was HPV 16, 76.4%, HPV18, 11.7%, HPV81, 2.9%, HPV31, 1.4%, HPV35, 1.4% and HPV 45, 1.4%. Multiple infections were observed in 11.8% of tumor samples with HPV 16 contributing to 62.5% of cases. In non-malignant samples, 20% of HPV positive samples were detected with HPV16, 82.3%, HPV33, 5.8% and HPV58, 5.8% and very low incidence of multiple infections. Comparative phylogenetic analysis of HPV variants identified 9 HPV sequences as new papillomavirus species, predominantly classified as European lineage type. Conclusions: The findings for HPV infections associated with progression of cervical cancer in coastal Karnataka region and HPV variant analysis provide baseline data for prevention and HPV vaccination programs.

국내에서 분리된 G형 간염바이러스 NS-5 Region 염기서열의 계통학적 분석 (The Phylogenetic Analysis of the NS-5 Region Sequence of Hepatitis G Viruses Isolated in Korea)

  • We examined the hepatitis G virus infections among 227 Koreans who were healthy or were suspected of hepatitis and determined the phylogenetic relationship based on a part of the NS-5 region of 5 positive samples. Viral RNA was extracted from sera and cDNA was synthesized and subsequently amplified by RT-PCR (reverse transcription-polymerase chain reaction) or RT-nested PCR using random hexamer and NS-5 specific primers (470-20-1-77F, 470-20-1-211R, HGVNESTFO, HGVNESTRE). Five positives were found to belong to samples of patients showing symptoms of viral hepatitis. Primers used for PCR or nested PCR were derived from the NS-5 region. On the other hand, no amplification was detected using primers derived from the 5'-NCR (G-146F, G-401R). We performed TA cloning and sequencing of 5 amplified fragments, and their sequences were compared with those of foreign isolates of HGV. The phylogenetic analysis using MegAlign programme of DNAstar has shown that the Korean isolates are clustered on the phylogenetic tree. In summary, we confirmed the hepatitis G virus infection in 5 cases out of 12 patients showing the symptoms of viral hepatitis. The phylogenetic analysis of sequences of 5 amplified fragments showed that their relations to each other were closer than those to the foreign HGV isolates reported.

입원한 폐렴 환아에서 코로나 바이러스 감염 (Human coronavirus infection in hospitalized children with community-acquired pneumonia)

  • 목 적 : 지금까지 코로나바이러스는 대부분 상기도 감염을 일으키며 임상적으로 큰 의미가 없는 것으로 여겨져 왔다. 하지만 최근에 발견된 코로나 바이러스인 hCoV-NL63와 hCoV-HKU1는 하부 호흡기 질환과의 연관성이 있는 것으로 보고되면서 임상적 의의에 대한 연구가 필요한 실정이다. 이에 저자들은 폐렴으로 입원한 소아 환자에서 최근에 발견된 hCoV-NL63과 hCoV-HKU1을 포함한 코로나바이러스 감염의 유병률을 알아보기 위하여 본 연구를 시행하였다. 방 법 : 2006년 3월부터 2007년 1월까지 폐렴으로 상계백병원에 입원한 소아에게서 비인두 흡인물을 수집하였다. Multiplex RT-PCR 방법을 이용하여 RSV, influenza A, influenza B, parainfluenzavirus 및 adenovirus 감염을 진단하였으며, F 유전자 시발체를 이용한 nested RT-PCR에 의해 hMPV 감염을 진단하였다. 코로나바이러스 감염을 진단하기 위해 hCoVNL63, hCoV-OC43, hCoV-229E 및 hCoV-HKU1에 각각 특이적 시발체를 이용하여 nested RT-PCR을 시행하였다. 결 과 : 총 217명의 입원한 15세 이하의 폐렴 환아에게서 수집한 비인두 흡인물에서 multiplex PCR 방법에 의해 RSV 양성은 32명, hMPV는 18명, parainfluenza virus는 10명, influenza virus A는 2명, adenovirus는 6명에서 양성이었다. RT-PCR에 의해 hMPV 양성은 18명에서 확인되었다. 코로나바이러스는 RT-PCR 방법에 의해 8명 (3.7%)에서 양성이었으며, hCoV-229E 1명, hCoV-NL63 3명, 그리고 hCoV-OC43가 4명에서 검출되었다. 하지만 hCoV- HKU1는 연구 대상군에서 검출되지 않았다. 결 론 : 아직 추가 연구가 필요하지만 최근에 발견된 hCoV-HKU1와 hCoV-NL63은 폐렴으로 입원한 국내 소아에서 임상적인 중요성이 적을 것으로 생각된다.

인천지역 급성 설사환자의 group A rotavirus 감염 실태 및 P와 G 유전자형 분포 (The Prevalence and Distribution of the P and G Genotypes of a Group A Rotavirus Detected in Acute Gastroenteritis Patients from Incheon)

  • 인천지역 병 의원에 내원한 급성위장염환자의 대변 검체 총 11,607건을 효소 면역법(ELISA)을 이용하여 group A rotavirus의 계절별 발생 양상을 조사한 결과 2005년부터 2010년간 인천지역에서는 1~2월이 가장 많이 발생하였으나, 기간별로 차이가 존재했다. 또, group A rotavirus 양성 검체 160건에 유전자 분석 결과 VP4는 P8형, VP7는 G1형, G와 P 조합형으로는 G1P8이 가장 많았다. 그러나 이전의 연구와 비교했을 때 연구 지역과 기간에 따라 혈청형과 유전형의 변화가 빈번하였으므로 효율적인 방역을 위해 지속적인 모니터링이 필요하다고 사려된다. 또 이번 연구는 현재 유통 중인 rotavirus 백신의 효율적인 사용의 기초 자료가 될 것이다.

경남지역 야생들쥐에서 Orientia tsutsugamushi에 대한 항체 조사 및 PCR에 의한 검색 (Survey of antibody on Orientia Tsutsugamushi among wild rodents in Gyeongnam area and detection by nested polymerase chain reaction)

  • As a part of epidemiologic investigation of tsutsugamushi disease, the wild rodents which were captured in Gyeongnam area were diagnosed with indirect immunofluorescent antibody assay (IFA) and Polymerase Chain Reaction (PCR) to find if they have an antibody against Orientia tsutsugamushi. The conclusion was drawn as followings. (1) The captured 58 wild rodents showed that the subspecies distribution of Apodemus agrarius was 86.2%, Microtus fortis was 8.6% and Crocidura lasiura was 5.2%. (2) The antibody positive rate of O. tsutsugamushi Gilliam, Karp, Karto and Boryong by IFA method was 32.0% in Apodemus agrarius among 50 wild rodents and 40.0% in Microtus fortis among 5 wild rodents, respectively. It was negative in the case of all the 3 Crocidura lasiura. (3) The antibody titers on Apodemus agrarius, Microtus fortis and Crocidura lasiura against Gilliam, Karp, Karto and Boryong were measured between 1:20 and 1:640. The antibody titer against each antigen was in the order Boryong>Gilliam>Karp. (4) O. tsutsugamushi was detected from the blood, spleen and kidney from the artificially infected mice by IFA and PCR. IFA showed the positive response between 3 and 18 days after inoculation. On the other hand, positive response was found from all the samples by PCR. (5) From PCR of the genomic DNA extracted from the blood, spleen and kidney samples of the captured wild rodents, Boryong-specific amplification product with size of 210 bp, which is particular in Boryong, was detected from spleen and kidney samples, but not detected in the blood. (6) Boryong-specific amplification product was detected from spleen and kidney samples which were obtained at 3, 6, 12, 18 and 24 days after the infection with Boryong. But, it wasn't detected from the uninfected samples. (7) From PCR of spleen and kidney samples of the captured wild rodents, it was found that positive rate of O. tsutsugamushi in Apodemus agrarius and Microtus fortis were 25.0% (4/16) and 20.0% (1/5), respectively. From the above results, it can be concluded that Apodemus agrarius resided in Gyeongnam area carried O. tsutsugamushi and PCR method might be a simple, precise, rapid and useful diagnostic tool than IFA for the diagnosis of O. tsutsugamushi.

No Detection of 'High-risk' Human Papillomaviruses in a Group of Iranian Women with Breast Cancer

  • The presence of viral DNA in breast cancer cells is controversial. However, some studies have revealed a possible role for the human papillomavirus in the pathogenesis of breast cancer. The aim of the present study was to investigate the presence of HPV-DNA in breast tissue in a group of Iranian women with and without breast cancer and identification of the detected HPV types. Paraffin-embedded specimens from 65 malignant breast cancer cases and 65 cases with benign breast lesions were investigated for presence of HPV-DNA by nested polymerase chain reaction. We found HPV-DNA in 22 (33.8%) of the breast cancer specimens. All non-cancerous specimens were negative. Low and high-risk HPV types, including HPV-6 (26.2%), HPV-16 (1.5%), HPV-35 (1.5%), HPV-52 (1.5%), and HPV-11 (1.5%) were detected in our study. HPV-6 was the most prevalent type in the breast cancer specimens. Although high-risk HPV types have been shown to have a major role in cervix cancer, there have been no data that support the same relevance for other types of malignancies. Furthermore, presence of low-risk HPV types in malignancies still is a matter of debate. The data presented in this study indicates a strong need for epidemiological studies correlating different HPV types in human breast cancer.

사람성장호르몬 유전자주입 토끼수정란의 핵이식에 의한 복제 (Cloning of MT -hGH Gene-injected Rabbit Embryos by Nuclear Transplantation)

  • 토끼 수정란의 전핵에 MT-hGH 유전자를 주입하고 핵이식 기법으로 형질전환 복제수정란의 생산효율과 PCR 검색으로 복제수정란에서 유전자존재 여부를 조사한 바 다음과 같은 결론을 얻었다. 1. MT-hGH 유전자를 주입하여 8- 및 16- 세포기로 자란 수정란을 공핵란으로 사용하여 핵이식을 실시하였던 바, 세포융합률은 각각 60.0%, 62.8% 로 비슷하였으나 정상수정란을 공급핵으로 사용한 80.4% 보다 유의적으로 낮은 융합률을 보였다. 그러나 이들 복제수정란의 체외발달률은 처리군간에 유의적인 차이는 인정되지 않았다. 2. 유전자 주입 후 8- 및 16- 세포기로 자란 수정란의 할구를 이용하여 핵이식으로 복제하고 체외에서 배반포까지 자란 수정란을 PCR -screening으로 유전자를 검출한 결과, 각각 23% 와 33% 의 유전자 양성 수정란을 감별하였다.

HPV Detection and Genotyping in Vulvar Squamous Cell Carcinoma in Northern Thailand

  • Background: The study was aimed to evaluate the prevalence and genotype distribution of HPV infection in vulvar squamous cell carcinoma (SCC) in northern Thailand and the clinicopathological difference with regard to HPV infection status. Materials and Methods: Formalin-fixed paraffin-embedded tissue samples of vulvar SCC diagnosed between January 2006 and December 2012 were collected. HPV infection was detected by nested polymerase chain reaction (PCR) with primers MY09/11 and GP5+/6+. HPV genotyping was performed using the Linear Array Genotyping Test, followed by type-specific PCR targeting the E6/E7 region of HPV16/18/52 if the Linear Array test was negative. The histologic slides of vulvar lesions and the medical records were reviewed. Results: There were 47 cases of vulvar SCC included in the study (mean patient age $57.9{\pm}13.2$ years). HPV infection was detected in 29 cases (62%), all of which had single HPV infections. HPV16 accounted for 23 (49%). The patients with HPV-positive SCC had a significantly younger mean age than those with HPV-negative tumors (52.7 years vs 66.2 years, p<0.001). There was no significant difference in tumor stage distribution with regard to the status of HPV infection. The presence of vulvar intraepithelial neoplasia (VIN) of usual type (basaloid or warty) was significantly more frequent in HPV-positive cases compared with HPV-negative cases (62% vs 6%, p<0.001), whereas differentiated-type VIN was more common in HPV-negative cases (24% vs 0%, p=0.019). Conclusions: HPV infection was detected in 62% of vulvar SCC in northern Thailand. HPV16 was the predominant genotype similar to the data reported from other regions. HPV-positive SCC occurred in younger patients compared with HPV-negative SCC, and was associated with usual-type VIN. Vaccination against HPV16/18 may potentially prevent almost one half of vulvar SCC in northern Thailand.

인면역결핍 바이러스 pol 유전자 염기서열 결정에 의한 지도부딘 (ZDV) 내성 돌연변이의 탐지 (Detection of Mutations to Zidovudine in the pol Gene of Human Immunodeficiency Virus-1 by Direct Sequencing)

  • The nested polymerase chain reaction (PCR) assay was used to determine the sequences of reverse transcriptase (RT) codons 41, 67, 70, 210, 215 and 219 of human immunodeficiency virus-1 (HIV-1) pol gene. Template DNA was obtained from uncultured peripheral blood mononuclear cells from 27 Korean HIV-1 infected patients treated with ZDV and Korean red ginseng. The second PCRs were done for 2 separated regions (RT codons $13{\sim}98$ and $152{\sim}259$) with $5\;{\mu}l$ of the first PCR productNucleotide sequences were determined by direct sequencing. In the 27 patients, CD4+ cell count decreased from $230{\pm}117/{\mu}l$ to $152{\pm}162/{\mu}l$ for $46{\pm}26$ months (Mo), and actual duration of ZDV intake was $72{\pm}16$ Mo. In the 16 patients who had been treated with ZDV therapy ${\ge}25$ Mo, the incidences of 70R, 215F/Y, and 41L were 61%, 28% and 22%, respectively and those of 67N, 210W and 219Q were 17%. The incidences of 215F/Y were 6.7% for group ${\le}12$ Mo treatment, 22.7% for group with 13 to 24 Mo, and 27.8% for group ${\ge}25$ Mo. There was no mutation in 9 patients. It might be associated with the interruption of ZDV therapy for more than 6 months in 6 patients. This study shows that the detection of mutation could be useful prognostic marker with other clinical and virological data, and very low mutation rate is dectected compared to overseas reports.
