종골 골절 시 골절편에 의해 발생한 비복 신경의 포착(1예 보고) (Sural Nerve Entrapment by Fragments of Calcaneal Fracture (A Case Report))

  • Sural nerve is a sensory nerve that innervates the lateral side of ankle and foot, and the injury of this nerve can be usually caused by surgical approch of calaneal fracture or achilles tendon injury. Entrapment neuropahty of sural nerve caused by bony fragment after calcaneal fracture is not reported, yet. Authors experienced one case that sural nerve injury due to bony fragment after calcaneal fracture and we regard that it is a rare case, so we report this case after reviewing literatures.

  • When a patient represents pain in foot, physician can easily overlook compression neuropathy of peripheral nerve as it is uncommon. Among nerve entrapment syndrome encountered in the foot, selective compression in lateral branch of deep peroneal nerve (DPN) is rare. We report a case of a patient with pain and dysesthesia in dorsolateral foot which turned out as lateral branch of deep peroneal nerve entrapment syndrome caused by talonavicular joint effusion. We would like to share diagnostic work up flow and conservative treatment courses. This case manifests the importance of the deep peroneal nerve and its branches in clinical setting of pain and ankle instability.

결절종에 의한 상견갑 신경 포착 증후군 (Suprascapular Nerve Entrapment Neuropathy by Ganglion Cyst)

  • Purpose: The purpose of this study is to describe the characteristic clinical findings and treatment of suprascapular nerve entrapment by ganglion and to evaluate its results. Materials and Methods: Seven paitents with suprascapular nerve entrapment were evaluated on an average 13 months(range, six months to three years two months) after surgical excision and decompression. There were six males and one female. The mean age at operation was 31 years(range, 23 to 40 years), Suprascapular nerve entrapment were caused by compression of ganglion cyst in suprascapular notch or spinoglenoid notch in all cases. All patients complained of pain located over posterolateral area of the shoulder. Two patients had atrophy of both the supraspinatus and infraspinatus muscles, In four patients, only the infraspinatus muscle was involved. Muscle strength on both forward flexion and external rotation was decreased in two patients. In four patients, only external rotation was decreased. All patients underwent open excision of ganglion cyst and decompression. Results: The most dramatic effect of operation was prompt disappearance of pain in all patients. The average visual analog scale had improved from 7.2 to 0.6 point at the latest follow-up evaluation. An atrophy of the supraspinatus or infraspinatus muscle partially disappeared in four of six patients and muscle strength of forward flexion or abduction improved in all of six patients. The overall result was excellent for five patients and good for two. Conclusion: Surpascapular nerve entrapment by ganglionic cyst had clinically unique symptoms and signs on physical examination. Surgical excision is effective for symptomatic and functional outcomes. We believe that early intervention can be one of treatment modality before an irreversible damage occurs if the ganglion is large enough to compress suprascapular nerve, and to develop severe pain and muscular atrophy.

연부조직 연골종에 의한 정중, 척골포착신경병증: 증례보고 (Entrapment Neuropathy of Median and Ulnar Nerve Due to Soft Tissue Chondroma: A Case Report)

  • Purpose: Soft tissue chondroma is a rare benign tumor, found mainly on the palm and sole and grows slowly. Typically, mature hyaline cartilage is the dominant pathological feature. There are reports that assert soft tissue chondromas to be a cause of median nerve entrapment syndrome. However, this is the first case report showing soft tissue chondroma to be a cause of simultaneous median and ulnar neuropathy. Methods: A 62 year-old woman presented with chief complaints of numbness and hypoesthesia of her right palm for 4 to 5 years, and a palpable mass on her right palm that had been increasing in size slowly for 3 years. Physical examination revealed a firm, mobile, non-tender and about $3{\times}3\;cm^2$ sized mass in the center of the right palm. Electromyography showed entrapment neuropathy of the median and ulnar nerve. Ultrasonography showed an approximately $5.7\;cm^2$ mass below the flexor tendon of ring finger. Upon surgical excision, a $3{\times}3\;cm^2$ mass attached to the flexor digitorum profundus of ring finger and redness and hypertrophy of both the median and ulnar nerve were discovered. Mass excision was performed gently and the specimen was referred for histopathologic study. Mass excision resulted in median and ulnar nerve release. Results: The pathology report confirmed the mass to be a soft tissue chondroma with mature hyaline cartilage. The patient exhibited post-operative improvement of her symptoms and did not show any complications. Conclusion: This is the first case report showing soft tissue chondroma to be a cause of simultaneous median and ulnar neuropathy.

Transdural Nerve Rootlet Entrapment in the Intervertebral Disc Space through Minimal Dural Tear : Report of 4 Cases

  • Four patients underwent lumbar surgery. In all four patients, the dura was minimally torn during the operation. However, none exhibited signs of postoperative cerebrospinal fluid leakage. In each case, a few days after the operation, the patient suddenly experienced severe recurring pain in the leg. Repeat magnetic resonance imaging showed transdural nerve rootlets entrapped in the intervertebral disc space. On exploration, ventral dural tears and transdural nerve rootlet entrapment were confirmed. Midline durotomy, herniated rootlet repositioning, and ventral dural tear repair were performed, and patients' symptoms improved after rootlet repositioning. Even with minimal dural tearing, nerve rootlets may become entrapped, resulting in severe recurring symptoms. Therefore, the dural tear must be identified and repaired during the first operation.

Decompression of the Sciatic Nerve Entrapment Caused by Post-Inflammatory Scarring

  • A rare case of chronic pain of entrapment neuropathy of the sciatic nerve successfully relieved by surgical decompression is presented. A 71-year-old male suffered a chronic right buttock pain of duration of 7 years which radiating to the right distal leg and foot. His pain developed gradually over one year after underwenting drainage for the gluteal abscess seven years ago. A cramping buttock and intermittently radiating pain to his right foot on sitting, walking, and voiding did not respond to conventional treatment. An MRI suggested a post-inflammatory adhesion encroaching the proximal course of the sciatic nerve beneath the piriformis as it emerges from the sciatic notch. Upon exploration of the sciatic nerve, a fibrotic tendinous scar beneath the piriformis was found and released proximally to the sciatic notch. His chronic intractable pain was completely relieved within days after the decompression. However, thigh weakness and hypesthesia of the foot did not improve. This case suggest a need for of more prompt investigation and decompression of the chronic sciatic entrapment neuropathy which does not improve clinically or electrically over several months.

주관절부 척골신경 포착증후군의 수술적 감압술 및 내상과 성형술 (Decompression and Medial Epicondyloplasty in Ulnar Nerve Entrapment Syndrome at Elbow)

  • As a surgical treatment of ulnar nerve entrapment syndrome includes simple decompression, medial epicondylectomy, and anterior transposition of the ulnar nerve into a subcutaneous or submuscular bed have been widely used. Despite many reports of these surgical procedure, there is little to guide the choice of one surgical technique. The purpose of our study is to analyse clinical and electrodiagnostic result after minimal invasive decompression by decompression and medial epicondyloplasty(deepening of ulnar groove). We have experienced 9 cases of ulnar nerve entrapment syndrome who were treated with decompression and medial epicondyloplasty. Male were five and female were four. The mean age at operation was 36 years ranging from 23 to 47 years. Operative procedure was to incise the medial intermuscular septum and aponeurotic arch of flexor carpiulnaris and to deepen the ulnar groove. Patients are allowed to do range of motion(ROM) exercise on the average 5days. All patient were relieved pain and improved motor and sensory function, and this procedure allows early ROM exercise after operation because the muscle have not been detached.

Ultrasound Guided Transversus Abdominis Plane Block for Anterior Cutaneous Nerve Entrapment Syndrome

  • Anterior cutaneous nerve entrapment syndrome (ACNES) is one the most common cause of chronic abdominal wall pain. The syndrome is mostly misdiagnosed, treated wrongly and inadequately. If diagnosed correctly by history, examination and a positive carnett test, the suffering of the patient can be relieved by addressing the cause i.e. local anaesthetic with steroid injection at the entrapment site. Conventionally, the injection is done by landmark technique. In this report, we have described 2 patients who were diagnosed with ACNES who were offered ultrasound guided transverses abdominis plane (TAP) injection who got significant pain relief for a long duration of time.

Pudendal nerve entrapment syndrome caused by ganglion cysts along the pudendal nerve

  • Pudendal nerve entrapment (PNE) syndrome refers to the condition in which the pudendal nerve is entrapped or compressed. Reported cases of PNE associated with ganglion cysts are rare. Deep gluteal syndrome (DGS) is defined as compression of the sciatic or pudendal nerve due to a non-discogenic pelvic lesion. We report a case of PNE caused by compression from ganglion cysts and treated with steroid injection; we discuss this case in the context of DGS. A 77-year-old woman presented with a 3-month history of tingling and burning sensations in the left buttock and perineal area. Ultrasonography showed ganglion cystic lesions at the subgluteal space. Magnetic resonance imaging revealed cystic lesions along the pudendal nerve from below the piriformis to the Alcock's canal and a full-thickness tear of the proximal hamstring tendon. Aspiration of the cysts did not yield any material. We then injected steroid into the cysts, which resolved her symptoms. Steroid injection into a ganglion cyst should be considered as a treatment option for PNE caused by ganglion cysts.

농구선수에서 발생한 만성 족저근막염이 동반된 외측 족저 신경 압박증 -증례 보고- (Lateral plantar nerve entrapment combined with a chronic plantar fasciitis in a basketball player -A case report-)

  • 발바닥 부위에 반복적인 저 에너지의 자극이 가해지는 선수들에게는 건염, 피로 골절, 또는 과사용 증후군과 같은 병변이 자주 관찰된다. 운동 선수들에게 발생하는 발뒤꿈치 통증의 대부분의 원인은 족저 근막염이고, 이 는 족저 근막의 반복적 자극에 의해 발생한다. 대부분 보존적인 치료 방법으로 증상의 호전을 보이고 운동에 복귀한다. 흔하지는 않지만, 발뒤꿈치 통증의 원인이 되는 신경 압박병증도 반복적인 자극에 따른 신경의 주변조직 비후에 의해 발생할 수도 있다고 한다. 저자들은 발 부위에 저 에너지의 반복적인 자극이 가해지는 농구 선수에게서 만성 족저 근막염이 동반된 외측 족저 신경의 압박병증을 관찰하였고, 발생 가능한 기전과 증례를 문헌 고찰과 함께 보고하고자 한다.
