A Case of Advanced Non-seminomatous Germ Cell Tumor of the Testis Cancer with Retroperitoneal Lymph Node Metastasis : Neoplastic Pain & Neoplastic Fever Treated by Sasang Constitutional Medical Treatment (고환통과 발열을 호소하는 고환암환자의 치험 1례)

  • 1. Objectives In this case report, We report a case of advanced non-seminomatous germ tell tumor(NSGCT) of the testis to retroperitoneal lymph node. We successfully managed with sasang medical treatment. 2. Methods We treated a man with NSGCT of testis with retroperitoneal lymph node metastasis. We evaluated pain grade by VAS(visual analogue scale). and fever grade by body temperature(by axillary thermometer) 3. Results Pain and fever were improved with the Sasang constitutional medicine. 4. Conclusions Cancer pain and fever was markedly improved by Sasang Constitutional management. Further studies are needed to evaluate the cause of the improvement.

Nailbed malignant melanoma in three dogs

  • Three 8-12-year-old male cocker spaniels presented with an appendicular mass accompanied by pain, inflammation, lameness, and loss of nail in the digits of the forelimb or hindlimb. A histological examination revealed dermal masses of the digit composed of numerous neoplastic cells with marked pleomorphism and high mitosis. The neoplastic cells showed a strong invasive tendency into the epidermis and adjacent bony tissues, such as distal phalanx. Immunohistochemistry of the neoplastic cells in three masses revealed positive reactions for Melan-A. These three dogs were diagnosed as nailbed malignant melanoma in the digit based on the history, clinical signs, and histopathologic features.

  • Blockade of the ganglion impar was performed as an alternertive means of managing intractable neoplastic perineal pain of sympathetic origin. We successively treated a patient who had suffered from excessive perianal sweating with ganglion impar block using pure alcohol. Eight months after block, the patient has no complaint of perianal sweating. Ganglion impar block is an effective method in the treatment of excessive perianal sweating as well as perineal pain of sympathetic origin.

  • We experienced a case of osteoid osteoma in thoracic vertebra accompanied with herniated nucleus pulposus during the management of back pain. A 32 year old male patient with herniated nucleus pulposus complained of back pain and radiation to right leg. Lumbar epidural block with 1% mepivacaine 5 ml was performed for pain control and it relieved the radiating pain. However patient continued to experience severe exacerated back pain at night which responded to aspirin. Therefore we performed further examination for existence of disease of the spine and diagnosed osteoid osteoma in the right pedicle of T12 vertebra. In conclusion, we recommend physicians when evaluating patients with back pain to be congnizant of possible existence of neoplastic disease of the spine and incorporate it in differential diagnosis.

  • The perineum is comprised of diverse anatomic structures with mixed sympathetic and somatic innervation. The coccyx is innervated by the coccygeal nerves and branches of the fifth sacral root. Recently, ganglion impar block has been introduced as an alternative means of managing intractable pain of sympathetic origin, coccygodynia by trauma, tenesmus and perineal hyperhydrosis. We managed a 59-year-old female patient who had suffered from perineal pain by metastasis of cervical cancer. Approach to impar ganglion through the anococcygeal ligament was impossible because her coccyx was hyperflexed anteriorly by old fracture. But we could perform ganglion impar block successfully by approach through the separation of sacrum and coccyx.

  • Although the vast majority (about 70%) of patients with cancer will have pain directly related to neoplastic invasion into pain-sensitive bone and soft tissue structures,as many as 20% of the adults will have pain caused directly by cancer therapy. Treatment related pain problems are important for several reasons; their appearance may be confused with more omnious pain syndromes associated with tumor recurrence or progression that may be directly lead to death of the patient; when severe, compliance with recommandations for further therapy may be adversely affected. Now, we report a case of postsurgical pain syndrome after radical neck dissection in a patient with tonsilar cancer. The pain after radical neck dissection result from injury to the cervical plexus,cranial nerve,and cervical sympathetics. In our case, we ruled out soft tissue infection and tumor recurrence,and successfully treated with gasserian ganglion block with pure alcohol in the patient having neuropathic pain in the mandible and preauricular region after radical neck dissection.

The Clinical Outcomes of Kyphoplasty for the Treatment of Malignant Vertebral Compression Fractures (전이성 척추 종양으로 인한 압박 골절 환자의 척추 후굴 풍선 복원술의 임상 효과)

  • Background: Kyphoplasty is a minimally invasive procedure that can stabilize osteoporotic and neoplastic vertebral fractures. We retrospectively evaluated the clinical outcomes of kyphoplasty for the treatment of vertebral compression fractures in cancer patients. Methods: We reviewed the clinical data of 27 cancer patients who were treated with kyphoplasty (55 vertebral bodies) between May 2003 and Feb 2008. The clinical parameters, using a visual analog 10 point scale (VAS) and the mobility scores, as well as consumption of analgesic, were evaluated preoperatively and at 1 week after kyphoplasty. Results: A total 55 cases of thoracic and lumbar kyphoplasties were performed without complications. The mean age of the patients was 66 years. All the patients experienced a significant improvement in their subjective pain and mobility immediately after the procedures. The pain scores (VAS), mobility scores and other functional evaluations using the Oswestry disability score and the SF-36 showed significant differences between the pre- and postoperational conditions. Conclusions: Kyphoplasty is an effective, minimally invasive procedure that can relieve the pain of patients with vertebral compression fractures and these fractures are the result of metastasis.

  • The causes of sciatica are variable and include musculoskeletal, dermatologic, infectious, neoplastic, and vascular disorders. In many cases, the symptom is usually caused by degenerative disease in the spine with the compression or irritation of spinal nerve. On the other hands, there are also several announced extra-spinal causes including aneurysm, diabetes, and radiation for sciatica in a low rate. Among the extra-spinal cases, aneurysms arising from iliac vessels are sometimes developing a diagnostic confusion with the spinal causes, and delayed diagnosis can lead to poor prognosis. It is very important to pay attention weather the aneurysmal cause is involved in the symptom of sciatica.

  • Florid cemento-osseous dysplasia (FCOD) is benign, non-neoplastic lesion characterized by multiple sclerosing masses. Cases of bisphosphonate-related osteonecrosis in FCOD have been rarely reported. we present the patient had multiple osteonecrosis with FCOD in the jaw that occurred after receiving bisphosphonates for eight years due to osteoporosis This report discussed the importance of evaluation of the bone disease in the jaw before bisphosphonate treatment and periodic follow-up.

Granulosa cell tumor in a sow (돼지의 과립막세포 종양)

  • A two-year-old mixed breed sow was requested to the Veterinary Pathology Laboratory of Cheju National University with a clinical signs of severe abdominal pain and sudden death. Grossly, there was severe hemorrhage in abdominal cavity. Most of internal parenchymas and subcutaneous muscle showed severe pale discoloration. Both ovaries were enlarged with oval to round protruding multilobular masses and dark red in color. And they were firm and contained multiple small cysts in their cut surface. Histopathologically, numerous neoplastic granulosa cells had spherical-to-oval, hyperchromatic nuclei and scant eosinophilic cytoplasms were distributed with follicular pattern in ovarian masses. And the typical Call-Exner bodies, distinctive microcavityies, were observed in the center of small neoplastic follicles. Based on the gross and histopathologic findings, this case was diagnosed as granulosa cell tumor. In our best knowledge, this is believed to be the first report of granulosa cell tumor in a sow in Korea.
