A Case of Advanced Non-seminomatous Germ Cell Tumor of the Testis Cancer with Retroperitoneal Lymph Node Metastasis : Neoplastic Pain & Neoplastic Fever Treated by Sasang Constitutional Medical Treatment (고환통과 발열을 호소하는 고환암환자의 치험 1례)

  • 1. Objectives In this case report, We report a case of advanced non-seminomatous germ tell tumor(NSGCT) of the testis to retroperitoneal lymph node. We successfully managed with sasang medical treatment. 2. Methods We treated a man with NSGCT of testis with retroperitoneal lymph node metastasis. We evaluated pain grade by VAS(visual analogue scale). and fever grade by body temperature(by axillary thermometer) 3. Results Pain and fever were improved with the Sasang constitutional medicine. 4. Conclusions Cancer pain and fever was markedly improved by Sasang Constitutional management. Further studies are needed to evaluate the cause of the improvement.

Treatment decision for cancer patients with fever during the coronavirus disease 2019 (COVID-19) pandemic

  • Background: Cancer patients have been disproportionally affected by the coronavirus disease 2019 (COVID-19) pandemic, with high rates of severe outcomes and mortality. Fever is the most common symptom in COVID-19 patients. During the COVID-19 pandemic, physicians may have difficulty in determining the cause of fever (COVID-19, another infection, or cancer fever) in cancer patients. Furthermore, there are no specific guidelines for managing cancer patients with fever during the COVID-19 pandemic. Thus, this study evaluated the clinical characteristics and outcomes of cancer patients with fever during the COVID-19 pandemic. Methods: This study retrospectively reviewed the medical records of 328 cancer patients with COVID-19 symptoms (fever) admitted to five hospitals in Daegu, Korea from January to October 2020. We obtained data on demographics, clinical manifestations, laboratory test results, chest computed tomography images, cancer history, cancer treatment, and outcomes of all enrolled patients from electronic medical records. Results: The most common COVID-19-like symptoms were fever (n=256, 78%). Among 256 patients with fever, only three (1.2%) were diagnosed with COVID-19. Most patients (253, 98.8%) with fever were not diagnosed with COVID-19. The most common solid malignancies were lung cancer (65, 19.8%) and hepatobiliary cancer (61, 18.6%). Twenty patients with fever experienced a delay in receiving cancer treatment. Eighteen patients discontinued active cancer treatment because of fever. Major events during the treatment delay period included death (2.7%), cancer progression (1.5%), and major organ dysfunction (2.7%). Conclusion: Considering that only 0.9% of patients tested for COVID-19 were positive, screening for COVID-19 in cancer patients with fever should be based on the physician's clinical decision, and patients might not be routinely tested.

A Case of Equine Uterine Angioleiomyoma

  • A 12-year-old thoroughbred mare with four days history of anorexia and high fever was submitted to the Equine Hospital. She failed to conceive in the following the breeding season in spring. In rectal palpation, mass could be palpable surrounding ovary. Because of the poor prognosis, full necropsy for mare was decided. The mass is located between right large and small colon, cranial to ovary using ultrasonography. Grossly, the mass was about $5{\times}3.5{\times}4cm$ in size, and the cut surface of mass showed brownish yellow in color with red-to orange granular areas Histopathologically, neoplastic cells were mixed with well differentiated spindle shape cells arranged in interlacing, interwoven pattern and poorly differentiated spindle cells. Neoplastic cells had abundant pink cytoplasm and bland and cigar-shaped nuclei with low mitotic figures. Many arteriole-like blood vessels with thick muscular wall were present throughout the mass. Masson's trichrome staining revealed muscle origin tumors in this case. Immunohistochemically, the neoplastic spindle cells showed positive reactions for smooth muscle actin and desmin. In our best knowledge, this is the first report of uterine angioleiomyoma in mare in Korea.

Endobronchial Hamartoma - 1 case - (기관지내 발생한 폐과오종 - 1례 보고 -)

  • Pulmonary hamartoma has been considered as rare disease, which consists of lung tumor less than 1 %. Originally described by Albrecht in 1904, hamartoma is tumor like malformation-abnormal mixing of the normal components of organ-and is applied also to tumor found in many organs other than the lung. Lately, the major conclusions are that pulmonary hamartoma is neoplastic rather than developmental error in origin. Because pulmonary hamartoma frequently mimics lung cancer, especially in cancer-risk age groups, its clinical significance is great. Recently, we experienced 1 cases of endobronchial hamartoma which located at the right main stem bronchus. The patient was a 54 year old male who was admitted due to symptoms of fever 4 chilliness and dyspnea. Radiologic studies such as chest x-ray, chest tomogram and chest C-T scan revealed that the nearly total haziness of the right lung was caused by endobronchial tumor. The mass was considered as a benign by bronchoscopic exam, so we removed it surgically by tracheobronchotomy without pulmonary resection. Postoperative chest x-ray revealed satisfactory reexpansion of previous collapsed right lung. The patient discharged uneventfully.

  • The term histiocytic medullary reticulosis first was introduced by Scott and Robb-Smith. It is a clinicopathologic symdrome characterized by wasting, fever, lymphadenopathy, hepatosplenomegaly, pancytopenia, and is often accompanied by jaundice, purpura. Cardinal pathologic features are systemized proliferation of atypical, neoplastic, erytherophagocytic histiocyte. We are here reporting one case which considered compatible for HMR, with a few elementary reviewed literatures.

  • Chronic Myeloblastic Leukemia(CML) is characterised by excessive production of neoplastic myeloid cell, reciprocal translocation between chromosome 9 and 22 called 'Philadelphia chromosome'. This one case of CML with Intracerebral Hemorrhage(ICH) patient is thought of Kidney-Yin and Yang deficiency, by the first clinical symptoms, at admission. So the nourishing Yin and Tonifying kidney, warming and tonifying kidney-yang is used. After the incresing of WBC count, the nourishing Yin and Tonifying kidney, invigorating-Yin and reducing fire is used for treatment of the essence-of-life and blood deficiency, fever due to Yin deficiency.

  • Inhibition of cyclooxygenase(COX), and thus prevention of the formation of prostaglandins, provided a unifying explanation of the therapeutic and toxic actions of nonsteroidal anti-inflammatory drugs (NSAIDs). Recently, the discovery of the two isoforms of COX was made by molecular biologists studying neoplastic transformation in chick embryo cells. The constitutive enzyme, COX-1, is obviously responsible for the production of prostaglandins involved in housekeeping functions such as maintenance of integrity of the gastric mucosa, renal blood flow and platelet aggregation. The inducible form of COX (COX-2) is responsible for the formation of prostaglandins that pathologically affects inflammation, pain and fever. Clearly, all the experimental and clinical data support the hypothesis that the beneficial effects of NSAIDs are due to inhibition of the COX-2 enzyme, whereas the gastrotoxicity is due to inhibition of COX-1. The cox-2/COX-1 ratios of the NSAIDs in common use have been measured and compared with epidemiological data on their side effects. There is little evidence to suggest that one NSAID is clearly more effective than another, But substantial individual variability is present with respect to the pharmacology and pharmacokinetics of these drugs: therefore it is essential to adjust the dosage and choose specific drug to the patient's response.

  • Intravascular lymphomatosis(IVL) which was first described by pfleger and Tappeiner in 1959 is rare malignancy characterized by neoplastic proliferation of lymphoid cell lineage within the vascular lumen with little or no adjacent parenchymal involvement Its usual sites of involvement are central nervous system and skin or infrequently heart, lungs, pancreas, liver, spleen, kidney, adrenal glands, genitourinary tract, and bone marrow. Pulmonary involvement of IVL is not common. Symptoms of pulmonary involvement include dyspnea, cough and fever. Radiologicially, the disease is manifested with diffuse interstitial infiltrates. We report a recently experienced case of pulmonary intravascular lymphomatosis which was manifested with fever and chest pain.

  • Diverticulosis of the small intestine is a rare entity, compared with that of duodenum or colon, and is found in only 1% of autopsied patients. The main complications are diverticulitis with or without a perforation, obstruction and hemorrhage, which are associated with a high mortality. Intussusception is primarily a disease of childhood; with only 5 to 10% of cases occurring in adults. In contrast to childhood intussusception, 90% of adult intussusception cases are had an associated pathologic processes. An inflammatory fibroid polyp is an uncommonly localized non-neoplastic lesion of the gastrointestinal tract. It occurs most often in the stomach and secondly in the ileum. It rarely occurs in other organs such as the colon, jejunum, duodenum and esophagus. We report a case of jejunal diverticulitis with a perforation combined with intussusception caused by an inflammatory fibroid polyp. A 78-year-old female presented with abdominal pain, fever and chill. Contrast CT scan showed intussusception of the ileum. The patient was treated with a small bowel segmental resection. After surgery, the specimen showed jejunal diverticulitis with perforation.

  • Kikuchi-Fujimoto disease was initially described as a self-limiting histiocytic necrotizing lymphadenitis in Japan in 1972, and is predominantly observed in women under the age of 30 year and in Asian populations. The pathogenesis is still poorly understood but is thought to include infections, and autoimmune and neoplastic diseases. The most common clinical manifestations are fever and painless cervical lymphadenitis. Diagnosis is based on the histopathological findings, characterized by focal necrosis in the paracortical region with abundant karyorrhexis, aggregates of atypical mononuclear cells around the zone of necrosis, absence of neutrophils and plasma cells, and usually intact lymph node capsule. There is no specific therapy for the condition, and aseptic meningitis can occur as one of the complications. Here, we report the case of a patient with Kikuchi-Fujimoto disease accompanied with aseptic meningitis, which may be confused as a case of tuberculous meningitis and lymphadenitis.
