AOM/DSS 유발 대장암에 대한 유근피(楡根皮)와 백화사설초(白花蛇舌草) 단일 및 배합 추출물의 항암 및 항염 효과 (Anti-neoplastic and Anti-inflammatory Effects of Single and Mixed Extracts of Ulmus Davidiana and Oldenlandia Diffusa on Azoxymethane/dextran Sodium Sulfate-induced Colonic Neoplasms)

  • Objectives: The aim of this experimental study was to evaluate the anti-neoplastic and anti-inflammatory effects of single and mixed extracts of Ulmus davidiana (UD) and Oldenlandia diffusa (OD) on azoxymethane/dextran sodium sulfate (AOM/DSS)-induced colonic neoplasms in mice. Methods: AOM/DSS induces colitis-associated colonic neoplasms in mice. Mice were divided into seven groups: normal-no inducement and no treatment; control-colonic neoplasms with no treatment; UD-colonic neoplasms and treatment with UD; OD-colonic neoplasms and treatment with OD; UD1+OD1-colonic neoplasms and treatment with UD1 and OD1. UD1+OD2-colonic neoplasms and treatment with UD1 and OD2; UD2+OD1-colonic neoplasms and treatment with UD2 and OD1. Single and mixed preparations of UD and OD were applied to mice for six weeks. The colon length and weight and histopathologic changes of colon tissue were observed. Serum pro-inflammatory cytokines, including tumor necrosis factor-alpha ($TNF-{\alpha}$) and interleukin-6 (IL-6), were determined by enzyme-linked immunosorbent assay. The mRNA expression levels of Bax, Bcl-2, and interferon-gamma ($INF-{\gamma}$) were measured by RT-PCR. Results: The colon length was significantly increased in OD, UD1+OD2, and UD2+OD1 mice, and the colon weight was significantly decreased in OD and UD1+OD2 mice. The morphological change of colon epithelial cells was more suppressed in complex-treatment groups than in single-treatment groups. The inhibitory effect on inflammatory cell invasion was especially shown in UD1+OD2 mice. The serum level of the pro-inflammatory $TNF-{\alpha}$ was decreased in all complex-treatment groups, and the IL-6 level was decreased in UD1+OD1 mice. Single-treatment groups had an increase in the mRNA expression of the pro-apoptosis regulator Bax, and UD2+OD1 decreased the mRNA expression of the anti-apoptosis regulator Bcl-2. The mRNA expression of $INF-{\gamma}$ associated with inflammation was decreased in OD and UD1+OD2 mice. Conclusions: This study suggests that single and mixed extracts of Ulmus davidiana and Oldenlandia diffusa have anti-neoplastic and anti-inflammatory effects on AOM/DSS-induced colonic neoplasms in mice. Therefore, we conclude that UD, OD, and a mixture of UD and OD are potential therapeutic agents for colitis-associated colonic neoplasms.

Collective review of pancreatic carcinosarcoma, a very rare pancreatic malignancy

  • Pancreatic carcinosarcoma is a very rare malignancy with a poor prognosis. Because of these characteristics, a treatment strategy for it has not been established yet. The aim of this study was to establish a therapeutic strategy for pancreatic carcinosarcoma. We reviewed data of a 65-year-old female patient who was diagnosed with pancreatic carcinosarcoma through endoscopic ultrasound-guided fine needle aspiration biopsy before surgery. For literature review, we searched PubMed using terms of "Pancreatic" or "Pancreas" and "carcinosarcoma" or "carcinosarcomatous". The patient received 11 cycles of neoadjuvant treatment with leucovorin, fluorouracil, irinotecan, oxaliplatin and pembrolizumab because the tumor was borderline resectable. She underwent stereotactic ablative body radiotherapy (SABR) with 35 Gy in 5 fractions, followed by robotic pylorus-preserving pancreaticoduodenectomy. After surgery, the patient received adjuvant chemotherapy in the same regimen as before surgery. She is alive without any recurrence. Among 48 patients within 33 available papers, the median survival time was 15 months. The survival rate of patients who received adjuvant chemotherapy tended to be higher than that of those who did not receive adjuvant chemotherapy, although the difference was not statistically significant (median survival, 47 vs. 15 months; p = 0.485). Three patients who received neoadjuvant chemotherapy had a survival period of 13-23.5 months. Surgery with lymphadenectomy, adjuvant therapy, and neoadjuvant therapy are thought to help improve survival outcomes. Modern treatment approaches for conventional pancreatic ductal adenocarcinoma could be applied to pancreatic carcinosarcoma.

Mammary gland tumors in three male dogs

  • Mammary gland tumors are very rare in male dogs. In this study, four mammary gland tumors from 3 male dogs (2 intact, 1 neutered) were collected from local animal hospitals. The dogs included two purebred Shih Tzu (1 intact, 1 neutered) and one intact purebred Cocker Spaniel. The mean age of dogs with mammary gland tumors was 9 years (5-12 years). Two dogs had a solitary mass, whereas one dog had two mammary masses. Of the four tumor masses, three were observed in the fourth or fifth mammary glands, and one was observed in the third mammary gland. According to histopathologic examinations, all four mammary masses from three dogs were benign tumors including two benign mixed tumors in one case and two complex adenomas. There were no history of obesity, testicular tumors, diabetes, and sex hormonal therapy in any male dogs with mammary tumors. Surgical excision was the only reported treatment for these tumors. No recurrence or metastasis was recorded up to 25 months after surgery.

연골성 분화를 보이는 화생성 유방암의 영상의학적 소견: 증례 보고 (Imaging Findings of Metaplastic Breast Carcinoma with Chondroid Differentiation: A Case Reports)

  • 화생성 유방암은 상피성 암과 다양한 화생성 변화가 혼합된 형태의 암으로, 드물게 발생하나, 높은 재발률과 전이율을 보이는 공격적 성향을 가진 암이다. 연골성 분화를 보이는 화생성 암은 화생성 유방암의 아형 중에서도 상대적으로 드물게 발생하며, 다른 아형에 비해 비교적 좋은 예후를 보인다. 저자들은 연골성 분화를 보이는 화생성 유방암으로 진단된 3명의 환자의 영상의학적 소견을 다음과 같이 보고한다. 유방촬영상 무정형 또는 거친 불균질한 석회화를 동반한 고밀도 종괴를 보이고 초음파상 미세소엽형 또는 불분명한 경계를 가진 복합성 낭성 및 고형성 종괴(complex cystic and solid mass)로 보인다. 자기공명영상에서 비교적 경계가 분명한 또는 부분적으로 경계가 불분명한 종괴로, T2 강조영상에서 비균질한 고신 호강도를 보이고, 빠른 초기 조영증강 및 말기 세척형의 신호강도 감소를 동반한 비균질한 조영증강이나 가장자리 조영증강을 보인다.
