Reconstruction of a Large Infected Midline Abdominal Wall Defect Using a Latissimus Dorsi Free Flap

  • Managing large infected midline abdominal defects are clinically challenging and technically demanding. The alloplastic materials, regional flaps, and component separation are usually infeasible because of the size, location, depth, and state of the defects. In these cases, the free flap is the only option with a large well-vascularized tissue that is free to inset regardless of the location. Herein, we report a case of 44-year-old man with a large infected midline abdominal wall defect who was completely treated with a latissimus dorsi myocutaeous free flap followed by negative pressure wound therapy.

Space-Time Characteristics of the Wall Shear-Stress Fluctuations in an Axial Turbulent Boundary Layer with Transverse Curvature

  • Direct numerical simulation database of an axial turbulent boundary layer is used to compute frequency and wave number spectra of the wall shear-stress fluctuations in a low-Reynolds number axial turbulent boundary layer. One-dimensional and two-dimensional power spectra of flow variables are calculated and compared. At low wave numbers and frequencies, the power of streamwise shear stress is larger than that of spanwise shear stress, while the powers of both stresses are almost the same at high wave numbers and frequencies. The frequency/streamwise wave number spectra of the wall flow variables show that large-scale fluctuations to the rms value is largest for the stream wise shear stress, while that of small-scale fluctuations to the rms value is largest for pressure. In the two-point auto-correlations, negative correlation occurs in streamwise separations for pressure, and in span wise correlation for both shear stresses.

Inflammatory Myofibroblastic Tumor Treated with Laparoscopic Proximal Gastrectomy and Double-Tract Anastomosis

  • Inflammatory myofibroblastic tumors (IMTs) of the stomach are extremely rare in adults, and their oncologic prognosis is not well understood. We present a 28-year-old man with a proximal gastric IMT. The patient visited the emergency department of Yeouido St. Mary's Hospital with syncope and hematemesis. Hemoglobin levels were <5.5 g/dl. Gastric fibroscopy showed a protruding mass $4{\times}4cm$ in size, with central ulceration on the posterior wall of the fundus and diffuse wall thickening throughout the cardia and anterior wall of the upper body. Endoscopic biopsy revealed indeterminate spindle cells, along with inflammation. Given the risk of rebleeding, an operation was performed despite the uncertain diagnosis. Because the mass was circumferential, laparoscopic proximal gastrectomy and double-tract anastomosis were performed to ensure a safe resection margin. The pathological diagnosis was consistent with an IMT originating from the stomach, although the tumor was negative for anaplastic lymphoma kinase gene mutation.

  • The vortical structures in a turbulent boundary layer (TBL) developed over a flat plate have been investigated experimentally. The flow conditions tested in this study were Re$_{\theta}$ = 3700, Re$_{\delta}$ = 11${\times}$105 and the shape factor H = 1.3. Instantaneous velocity fields in the streamwise-wall-normal planes were measured by using a dynamic PIV system. A trip-wire and sandpapers were placed behind the leading edge to promote the turbulent transition. 1000 velocity fields were obtained consecutively with a time interval of 1 millisecond. Streamwise u-velocity components were temporally averaged in the measuring plane. In addition, 2000 velocity fields were obtained randomly and ensemble-averaged to get the fully-developed turbulent characteristics. Profiles of the normalized u-component, turbulent intensities and Reynolds shear stress were evaluated. The structures of spanwise vortices were extracted from the instantaneous velocity fields by determining the swirling strength, ${\lambda}_{ci}$. The wall-normalized locations of vortices were temporally averaged in the measuring plane with respect to their rotational direction. The correlations between the temporally averaged u and the temporally averaged $y^+$ of vortices were evaluated. For the case of positive vortices, the correlation is not significant. However, the negative vortices show a strong negative correlation. The y-location of negative vortices tends to increase, as the averaged u decreases and vice versa. These findings indicate that the number of negative vortices in the outer layer increases during the outward bursting events.

현장측정을 통한 터널 내 벽면마찰계수 추정 연구 (The estimation of the wall friction coefficient in tunnels by in-situ measurement)

  • 현재 국내터널에 적용중인 벽면마찰계수는 단순히 외국의 연구결과를 인용하여 사용하고 있는 실정이다. 또한 기존 선행연구들에서는 속도감쇄법을 이용하여 벽면마찰계수를 추정하였으나, 터널 내 수렴풍속이 음수(-)이거나 자연풍의 변화가 있는 경우에는 벽면마찰계수에 대한 추정이 어려운 점이 있었다. 이에 따라 본 연구에서는 기존 속도감쇄법과 더불어 동적 시뮬레이션기법을 적용하여 벽면마찰계수를 추정하였다. 분석결과, 총 9개 터널(양방향 18개 튜브)에 대한 터널 내 마찰계수는 0.011~0.025 정도로 분석되었으며, 평균값은 0.020로 추정되었다. 또한, 본 연구를 통해 정량적으로 획득한 벽면마찰계수를 현재 적용중인 설계기준과 비교하였다.

  • This study was executed to evaluate adaptability of dentin bonding agents to dentinal wall with measuring contraction gap on interface between cavity wall and composite resin by SEM study. 6 kinds of dentin bonding agents were investigated for this study : Gluma, Super Bond C & B, All bond 2, Scotchbond multipurpose, Scotchbond 2 and Clearfil photo bond. 30 of fresh extracted teeth were randomly selected and divided into 6 groups with each 5. The round shaped cavities with 3mm dia. and 1.5mm depth were prepared on cementoenamel junction of buccal surface of teeth. Dentin bonding agents were applied to cavity wall and then the composite resin was filled in the cavity. Specimens were sectioned longitudinally on buccal surface. Sectioned aspects of specimens were impressioned with rubber base materials and finally precise replica were made of epoxy resin poured in negative impression. Contraction gaps were examined on interface between cavity wall and composite resin under condition of 200 and 2000 magnification of SEM. The results were as follows. 1. There were no gap on interface between enamel and composite resin in all specimens, but gaps were mainly exhibited on apical side of lateral wall of dentin of cavity. 2. In Gluma, 2 cases of 5 specimens exhibited excellent adaptation to the cavity wall, indicating no gaps on interface between cavity wall and composite resin. The other specimens showed gaps with range of $0{\sim}15{\mu}m$ width. 3. In Super Bond C & B, gaps with range of $0{\sim}10{\mu}m$ width were mainly exhibited on apical side of lateral wall of dentin of cavity. 4. In All bond 2, all specimens showed the most exellent adaptation to cavity wall when compared to the other materials, indicating no gap interface between cavity wall and composite resin. 5. In Scotchbond multipurpose, gaps with range of $0{\sim}10{\mu}m$ width were locally located on cavity wall. 6. In Scotchbond 2, all specimens showed great amount of gap with range of $5{\sim}25{\mu}m$, indicating the worst adaptation to cavity wall compared to the other materials. 7. In Clearfil photo bond, 2 case of 5 specimens exhibited exellent adaptation to cavity wall, indicating no gap. the other specimens showed gap with range of $0{\sim}15{\mu}m$ width on inferface between cavity wall and composite resin.

Toward residential building energy conservation through the Trombe wall and ammonia ground source heat pump retrofit options, applying eQuest model

  • The aim of this research is to apply the eQuest model to investigate the energy conservation in a multifamily building located in Dayton, Ohio by using a Trombe wall and an ammonia ground source heat pump (R-717 GSHP). Integration of the Trombe wall into the building is the first retrofitting measure in this study. Trombe wall as a passive solar system, has a simple structure which may reduce the heating demand of buildings significantly. Utilization of ground source heat pump is an effective approach where conventional air source heat pump doesn't have an efficient performance, especially in cold climates. Furthermore, the type of refrigerant in the heat pumps has a substantial effect on energy efficiency. Natural refrigerant, ammonia (R-717), which has a high performance and no negative impacts on the environment, could be the best choice for using in heat pumps. After implementing the eQUEST model in the said multifamily building, the total annual energy consumption with a conventional R-717 air-source-heat-pump (ASHP) system was estimated as the baseline model. The baseline model results were compared to those of the following scenarios: using R-717 GSHP, R410a GSHP and integration of the Trombe wall into the building. The Results specified that, compared to the baseline model, applying the R-717 GSHP and Trombe wall, led to 20% and 9% of energy conservation in the building, respectively. In addition, it was noticed that by using R-410a instead of R-717 in the GSHP, the energy demand increased by 14%.

난류유동의 Large-Eddy Simulation 기법의 알고리즘 향상에 관한 연구 (Improvement on Large-Eddy Simulation Technique of Turbulent Flow)

  • Two aspects of Large-Eddy Simulation(LES) are investigated in order to improve its performance. The first one is on how to determine the model coefficient in conjunction with a dynamic subgrid-scale model, and the second one is on a wall-layer model(WLM) which allows one to skip near-wall regions to save a large number of grid points otherwise required. Especially, a WLM suitable for a separated flow is considered. Firstly, an averaging technique to calculate the model coefficient of dynamic subgrid-scale modeling(DSGSM) is introduced. The technique is based on the concept of local averaging, and useful to stabilize numerical solution in conjunction with LES of complex turbulent flows using DSGSM. It is relatively simple to implement, and takes very low overhead in CPU time. It is also able to detect the region of negative model coefficient where the "backscattering" of turbulence energy occurs. Secondly, a wall-layer model based on a local turbulence intensity is considered. It locally determines wall-shear stresses depending on the local flow situations including separation, and yields better predictions in separated regions than the conventional WLM. The two techniques are tested for a turbulent obstacle flow, and show the direction of further improvements.rovements.

Effects of Natural Selection, Mutagenesis, and Protoplast Formation and Cell Wall Regeneration on the Production of Aminoglycoside Antibiotics

  • High producers or blocked mutants of aminoglycoside antibiotic-producing Streptomyces spp. were selected by application of an agar plug method and by culturing individual colonies in broth. The productivities of aminoglycoside antibiotic producing organisms were increased by selection of a high producer from colonies obtained by spreading spores of wild strain, or survived from treatment of a mutagen or from the colonies regenerated from protoplast-formation and cell-wall regenerations. Some mutagen treated colonies lost the ability to produce antibiotics (5-8%). Some A-factor negative and deostreptamine or streptidine negative mutants were obtained by N-methyl-N'-nitro-N-nitrosomethylguanidine (MNNG) treatment. Many of the survivors from the MNNG treatment lost the ability to produce antibiotics. Major colonies produced less amount of antibiotics ; only few survived colonies produced more antibiotics than the parent. Resistance of Streptomyces spp. against the antibiotics produced by itself was also markedly affected by mutagen treatment.
