갑상샘 초음파 검사에서 K-TIRADS 점수화 체계를 사용한 양성과 악성 갑상샘 결절의 감별진단 (Differential Diagnosis of Benign and Malignant Thyroid Nodules Using the K-TIRADS Scoring System in Thyroid Ultrasound)

  • 본 연구는 갑상샘 초음파에서 K-TIRADS 분류와 K-TIRADS 분류에 따른 Different risk group의 조합을 이용한 방법이 양성과 악성 결절의 감별진단에 유용한지를 평가하였다. 갑상샘 초음파를 시행한 210명을 대상으로 하였으며, 세침흡인세포검사 결과를 바탕으로 후향적인 분석을 하였다. 갑상샘 결절의 양성과 악성에 따른 K-TIRADS와 Different risk group의 점수체계의 차이 분석은 카이제곱 검정을 하였다. ROC 곡선 분석을 통해 악성 결절의 예측을 위한 K-TIRADS 점수와 Different risk group의 최적 cut off 값을 결정하였다. 양성과 악성 결절 군의 분류에 따른 K-TIRADS와 Different risk group의 차이검증 결과에서 각각 통계적으로 유의한 차이를 보였다(p=.001). ROC 곡선 분석에서 양성과 악성 결절의 예측을 위한 K-TIRADS에 따른 점수분류에서는 AUC 0.786, Cut-off value> 2를 나타내었으며, Different risk group에서는 AUC 0.640, Cut-off value> 2로 결정되었다(p=.001). 갑상샘 초음파에서 결절을 발견했을 때 양성과 악성 갑상샘 결절을 감별하는 데 도움을 주는 K-TIRADS에 따른 분류체계가 Different risk group에 따른 분류체계보다 갑상샘 결절의 감별진단에 도움을 줄 것으로 생각하며 K-TIRADS에 따른 분류체계를 적용하면, 불필요한 세포 검사를 줄일 수 있고, 악성 결절의 조기발견에 도움이 될 것으로 생각된다.

세침흡인검사 결과 Atypia of Undetermined Significance로 진단된 갑상선 결절에서 악성을 예측할 수 있는 위험인자 (BRAFV600E Mutation is a Strong Preoperative Indicator for Predicting Malignancy in Thyroid Nodule Patients with Atypia of Undetermined Significance Identified by Fine Needle Aspiration)

  • Background and Objectives This study aimed to identify a reliable preoperative predictive factor for the development of thyroid cancer in patients with atypia of undetermined significance (AUS) identified by fine needle aspiration biopsy (FNAB). Subjects and Method This was a retrospective cohort study. Two hundred and ninety-nine patients diagnosed with AUS by preoperative FNAB who underwent curative thyroid surgery at our institution between September 2005 and February 2014 were analyzed. Clinical, radiological and molecular features were investigated as preoperative predictors for postoperative permanent malignant pathology. Results The final pathologic results revealed 36 benign tumors including nodular hyperplasia, follicular adenoma, adenomatous goiter, nontoxic goiter, and lymphocytic thyroiditis, as well as 263 malignant tumors including 1 follicular carcinoma and 1 invasive follicular carcinoma; the rest were papillary thyroid carcinomas. The malignancy rate was 87.9%. The following were identified as risk factors for malignancy by univariate analysis: $BRAF^{V600E}$ gene mutation, specific ultrasonographic findings including smaller nodule size, low echogenicity of the nodule, and irregular or spiculated margin (p<0.05). Multivariate analysis revealed that only $BRAF^{V600E}$ mutation was a statistically significant risk factor for malignancy (p<0.05). When $BRAF^{V600E}$ mutation was positive, 98.5% of enrolled patients developed malignant tumors. In addition, the diagnostic rate of malignancy in these cases was approximately 16-fold higher than BRAF-negative cases. Conclusion Patients with AUS thyroid nodules should undergo $BRAF^{V600E}$ gene mutation analysis to improve diagnostic accuracy and if the mutation is confirmed, surgery is recommended due to the high risk of malignancy.

  • The major categories of neuroendocrine tumors of lung are typical carcinoid, atypical carcinoid, large cell neuroendocrine carcinoma, and small cell carcinoma. The histologic classification criteria of neuroendocrine tumors are well documented in the "WHO Classification of Tumors" based on mitotic figures and necrosis. Cytologic characteristics of neuroendocrine tumors are trabecular, acinar, and solid arrangement of tumor cells and occasional rosette formation. Nuclear chromatin patterns are characteristically described as "salt and pepper chromatin pattern". Many of cytologic classifications documented in the literature are before the "WHO Classification". In this review, the cytologic features of pulmonary neuroendocrine tumors are documented according to the WHO classification, and recent concepts of neuroendocrine tumors of lung are discussed.

소아에서 발생한 흉선지방종 1 예 (A Case of Thymolipoma in a Child)

  • Thymolipoma is a rare benign mediastinal tumor, composed of mature fatty and thymic tissues. A 9-year-old boy was referred with a one-month history of neck swelling. Preoperative computed tomography scan and fine needle aspiration biopsy suggested thymolipoma. Despite it being rare, thymolipoma should be considered in the differential diagnosis of mediastinal tumors. Characteristics of its clinical feature and radiological findings that can differentiated it from other mediastinal tumors are discussed with a review of the literatures.

후종격동 갑상선종;1례 보고 (Posterior Mediastinal Goiter - A Case Report -)

  • Posterior mediastinal goiter extending to carotid sheath posteriorly is rare repoted case. Recently we experienced huge posterior mediastinal goiter with compressin of trachea in 57-year old male, The patient that complained of dyspnea referred to our hospital for further evaluation of mediasitnal tumor. We confirmed huge secondary posterior mediastinal intrathoracic goiter with diagnostic methods following by chest X-ray, thyroid scan, chest CT, and CT guided fine needle aspiration biopsy in this patient, and performed operation for excision Exision of posterior mediasitnal goiter performed through initial transeverse cervical incision and additional median sternotomy, and the mass removed completely without any complications. The postoperative course were uneventful.

Delayed Diagnosis of Tuberculous Spondylitis Masked by Concomitant Methicillin Resistant Staphylococcus Aureus Infection

  • We present a case of tuberculous spondylitis in which diagnosis was masked by a concomitant pyogenic infection. The patient had undergone percutaneous needle aspiration of an abscess in the cavity of the psoas muscle. Early results from the culture regimen showed isolation of methicillin-resistant Staphylococcus aureus. After eight weeks, mycobacterium tuberculosis was grown at regimen which was cultured at the same site. Initial isolation of pyogenic bacteria, considered to be highly virulent organisms, led to delayed diagnosis and treatment of the tuberculosis.
