만성 목통증 노인환자의 라이프케어를 위한 온열치료의 효과 연구 (Effects of Thermotherapy for Life Care in the Elderly with Chronic Neck Pain)

  • 비특이적 만성 목통증은 흔한 질환이며, 흔히 통증, 피로감, 관절가동범위 및 손기능의 감소를 동반한다. 만성 목통증에 의한 증상을 중재하기 위하여 다양한 약물 중재가 사용되고 있지만, 부작용이 적고 적용이 용이한 운동중재 및 대체의학 중재가 요구되고 있다. 본 연구는 비특이적 만성 목통증 노인환자의 라이프케어를 위하여 소금팩과 목운동이 통증, 관절가동범위와 손기능과 같은 운동기능에 미치는 효과를 규명하였다. 16명의 비특이적 만성 목통증 노인환자가 동원되었고, 중재군(8명)과 대조군(8명)으로 무작위 배정되었다. 모든 대상자는 1주일간 30분(일), 5회(주)의 자가 목운동을 수행하였고, 중재군의 대상자는 회당 20분 소금온열팩 중재를 추가적으로 받았다. Visual analogue scale (VAS)와 algometer를 이용한 pain pressure threshold (PPT) 측정을 통해 통증을 평가하였다. 어깨와 손 기능을 평가하기 위하여 어깨 관절의 가동범위 및 악력을 측정하였다. 모든 결과 측정에서 중재군은 유의한 개선을 보였으며(p<.05), 반면 대조군의 피험자는 중재 후 유의한 변화를 보이지 않았다(p>.05). 또한, 중재군은 사후 측정에서 대조군에 비교하여 VAS와 PPT에서 유의한 차이를 나타냈다(p<.05). 따라서 우리의 결과는 소금팩과 목운동이 만성 목통증 노인환자의 통증, 관절가동범위 그리고 손기능 개선에 효과적임을 증명하였다. 또한 소금팩은 목운동의 효과를 증가시키는 중재가 될 수 있음을 확인하였다.

하악편차 교정을 이용한 통증완화 효과에 대한 보고 (A Study on Pain Control Using Balancing Therapy of Occlusal Plane : Case Series)

  • Objectives : This study was performed to evaluate the effect of occulsal plane balancing therapy on pain control without another therapy. Methods : This clinical study was carried out in 6 cases of patients which had neck and shoulder pain. In this study we treated the patients by balancing therapy of occulsal plane using paper. We measured Visual Analogue Scale(V.A.S.) for pain intensity, Range of Motion(R.O.M.) and difference of thermographic temperature between pain site and the opposite before and 15 minutes after treatment and analyzed the change of variables. Also, we analyzed the correlation among hypomovable site of neck rotaion(HS), pain site(PS) and thick site of paper(TS). Results : 1. The average of skin temperature was decreased from $0.45{\pm}0.11$ to $0.24{\pm}0.13$ significantly(p<0.05). 2. The average of V.A.S was decreased from $10.00{\pm}0.00$ to $3.83{\pm}0.99$ significantly(p<0.05). 3. In the correlation analysis among HS, PS and TS, there is negative correlation between HS and TS, but there was no statistical significance. Conclusions : In the study, the balancing therapy of occulsal plane warrants further investigation in the change of skin temperature and R.O.M. of the joint, pain control.

  • Objectives : The purpose of this clinical study was to evaluate the effect of spinal decompression with Korean medicine treatment on the herniation of an intervertebral disc (HIVD) in a patient after fusion surgery of a cervical segment. Methods : A single patient was treated with spinal decompression, acupuncture, pharmacoacupuncture, and herbal medication. We measured the Numerical Rating Scale (NRS), Neck Disability Index (NDI), European Quality of Life-5 Dimension (EQ-5D), Cervical Range of Motion (cervical ROM) and the Beck Depression Inventory (BDI) score to evaluate the treatment effects. Results : The patient's post neck pain & pain in both arms improved significantly as suggested by the scores on the NRS, NDI, EQ5D, cervical ROM and BDI. Conclusions : Thus, spinal decompression, including Korean medicine treatment, could be taken into consideration for HIVD patients after fusion surgery.

편타 손상으로 인한 경항통 환자를 대상으로 한 약침치료에 대한 임상적 고찰 (The Clinical Study of Herbal Acupuncture for Neck Pain Caused by Whiplash Injury)

  • Objective : This study was performed to evaluate the effect of herbal acupuncture treatment in whiplash injury caused by traffic accident. Methods : The clinical study was carried out 44 cases of whiplash injury patients who had been admitted in Semyung university oriental hospital from March 2004 to December 2006. 24 cases of patients were treated with herbal acupuncture, 20 cases of patients were don't. We compared visual analigue scale(VAS) and range of motion(ROM) of both groups. Results: 1. VAS of herbal acupuncture treated group were significantly decreased compared with non - herbal acupuncture treated group. 2. ROM of herbal acupuncture treated group were significantly improved compared with non - herbal acupuncture treated group. Conclusion : In this study, herbal acupuncture is effective in whiplash injury patients caused by traffic accident.

A Lower T1 Slope as a Predictor of Subsidence in Anterior Cervical Discectomy and Fusion with Stand-Alone Cages

  • Objective : Preoperative parameters including the T1 slope (T1S) and C2-C7 sagittal vertical axis (SVA) have been recognized as predictors of kyphosis after laminoplasty, which is accompanied by posterior neck muscle damage. The importance of preoperative parameters has been under-estimated in anterior cervical discectomy and fusion (ACDF) because there is no posterior neck muscle damage. We aimed to determine whether postoperative subsidence and pseudarthrosis could be predicted according to specific parameters on preoperative plain radiographs. Methods : We retrospectively analyzed 41 consecutive patients (male : female, 22 : 19; mean age, $51.15{\pm}9.25years$) who underwent ACDF with a stand-alone polyether-ether-ketone (PEEK) cage (>1 year follow-up). Parameters including SVA, T1S, segmental angle and range of motion (ROM), C2-C7 cervical angle and ROM, and segmental inter-spinous distance were measured on preoperative plain radiographs. Risk factors of subsidence and pseudarthrosis were determined using multivariate logistic regression. Results : Fifty-five segments (27 single-segment and 14 two-segment fusions) were included. The subsidence and pseudarthrosis rates based on the number of segments were 36.4% and 29.1%, respectively. Demographic data and fusion level were unrelated to subsidence. A greater T1S was associated with a lower risk of subsidence (p=0.017, odds ratio=0.206). A cutoff value of T1S<$28^{\circ}$ significantly predicted subsidence (sensitivity : 70%, specificity : 68.6%). There were no preoperative predictors of pseudarthrosis except old age. Conclusion : A lower T1S (T1S<$28^{\circ}$) could be a risk factor of subsidence following ACDF. Surgeons need to be aware of this risk factor and should consider various supportive procedures to reduce the subsidence rates for such cases.

추나요법을 적용한 후종인대골화증을 동반한 경추척추관협착증 환자의 경과관찰 1례 (A Case Report on Cervical Spinal Stenosis with Ossification of posterior Longitudinal Ligament Applied Chuna Treatment)

  • Objects : This study was to report a clinical effect of Korean Treatment for a patient diagnosed by Computed Tomography as Cervical Spinal Stenosis with Ossification of posterior Longitudinal Ligament. Methods : In order to alleviate left shoulder pain and neck stiffness, the patient was treated by acupuncture therapy, herbal medicine, distilled herbal injection, cervical traction technique of Chuna treatment and conservative managements. To evaluate the effect of the treatment, Recovery rate of Hirabayashi, VAS(Visual Anlaogue Scale), NDI and ROM(Range of Motion) were used. Lhermitte's sign, Valsalva test, Distraction test, Compression test and Spurling test were carried out. Results : VAS, NDI and ROM were improved and Recovery rate was 100%. Conclusions : Korean Treatment can be effectively used for a patient with Cervical Spinal Stenosis with OPLL. Further clinical studies are needed to verify the findings.

특발성 신경통으로 오인된 환축추관절 증후군 -증례 보고- (Atlantoaxial Joint Syndrome Misconceived as an Idiopathic Neuralgia -A case report-)

  • Although the atlantoaxial joint is the most common site of rheumatoid arthritis, our patient had no symptoms or signs suggestive of rheumatoid arthritis. The atlantoaxial joint is frequently involved in degenerative osteoarthritis, especially in elderly patients. An 81-year old man presented with severe intermittent electric shock like, lancinating pain from the occipital to the temporal and parietofrontal areas. He also had neck pain and a limited range of motion. After many examinations and laboratory tests, at the department of neurology and neurosurgery, he was diagnosed with idiopathic neuralgia. The diagnosis of atlantoaxial joint syndrome was confirmed, and treated successfully with atlantoaxial joint block.

Biomechanics of stabbing knife attack for trauma surgeons in Korea: a narrative review

  • The aim of this paper was to review the biomechanics of knife injuries, including those that occur during stabbing rampages. In knife stab attacks, axial force and energy were found to be 1,885 N and 69 J, respectively. The mean velocity of a stabbing motion has been reported to range from 5 to 10 m/sec, with knife motions occurring between 0.62 and 1.07 seconds. This speed appears to surpass the defensive capabilities of unarmed, ordinarily trained law enforcement officers. Therefore, it is advisable to maintain a minimum distance of more than an arm's length from an individual visibly armed with a knife. In training for knife defense, particularly in preparation for close-quarter knife attacks, this timing should be kept in mind. Self-inflicted stab wounds exhibited a higher proportion of wounds to the neck and abdomen than assault wounds. Injuries from assault wounds presented a higher Injury Severity Score, but more procedures were performed on self-inflicted stab wounds. Wound characteristics are not different between nonsuicidal self-injury and suicidal self-wrist cutting injuries. Consequently, trauma surgeons cannot determine a patient's suicidal intent based solely on the characteristics of the wound. In Korea, percent of usage of lethal weapon is increasing. In violence as well as murders, the most frequently used weapon is knife. In the crimes using knife, 4.8% of victims are killed. Therefore, the provision of prehospital care by an emergency medical technician is crucial.

척추질환에 도침치료를 포함한 한방복합치료 치험 5례 (The Effect of Acupotomy on Lumbar and Cervical Spine Combined with Oriental Medical Treatment: Report of Five Cases)

  • Objectives : The purpose of this study is to report the improvement in patients with lumbar, cervical or radiating pain to the extremity by means of acupotomy. Methods : We treated 5 patients who have a disease related with lumbar or cervical spine with acupotomy combined with oriental medical treatment. We checked numeric rating scale(NRS), oswestry low back pain disability index(ODI) or neck disability index(NDI), and range of motion(ROM). Results : NRS and ODI/NDI were decreased, and ROM improved at all cases. Conclusions : This study shows acupotomy has a meaningful effect when applied on lumbar and cervical spine.

  • Human cervical spine has to protect the neural components and vascular structures. Also, it must have the flexibility afforded by an extensive range of motion to integrate the head with the body and environment. Because of these two-sided features, human cervical spine has very complicated shapes and their injury mechanisms are not fully understood yet. We have developed analytical model of human CS by using the finite element method. The model has been verified with in vivo and in vitro experimental results. From the qualitative analysis of simulation results, we were able to explain some of the fundamental mechanisms of neck pain. Further more, this FE model of human CS can be used as an analytical tool or biomechanical design of the clinical device and safety restraints.
