Although conservative management of congenital muscular torticollis (CMT) has been well documented, relatively little is known about the response to the treatment. The purposes of this case report were to describe the use of a therapeutic approach based on motor development in physical therapy intervention for an infant with CMT and to report the result of the treatment. The patient was a 20-day-old baby boy with left CMT presenting muscular mass in the left sternocleidomastoid muscle. The angle of the lateral head tilt was 20 degrees. The size of muscular mass was 5.3 mm in ultrasonography. Intervention included ultrasonic therapy, soft tissue massage, passive and active range of motion exercises, motor developmental therapy, and parent instruction. The procedures of motor developmental therapy and changes in the amount of lateral head tilt were documented using photography. The size of the mass was decreased to .3 mm before the 5-month follow-up. The patient also maintained a midline head position in the supine position and a midline head alignment during all functional activities. A therapeutic approach based on motor development is a beneficial method for reducing an asymmetrical head and neck position, and facilitating normal development as a component of physical therapy intervention.
Purpose: Failure of proper migration, fusion, or maturation of the branchial apparatus components results in a variety of congenital defects. Of these, cartilaginous rests are infrequent, while branchial cysts and sinuses are more common, relatively. The purpose of this study is to examine the clinical and pathological features of rare cervical branchial remnants in order to provide basis for its correct diagnosis and treatment. Methods: We report three cases of cervical branchial remnants which were treated in our hospital from December 2004 to December 2009. These cases were examined their clinical features, histologic findings and treatments. The patients had been operated with simple excision, excision of the combined components and preoperative antiboitics. Results: A retrospective review produced 2 cases of the cervical branchial remnants and 1 case of the cervical chondrocutaneous branchial remnant. All cases were on the left side of the neck, and anterior to the sternocleidomastoid muscle. Histopathological examination showed that fistula & sinus were lined with stratified squamous epithelium, additionally, they were consisted of a cutaneous envelope containing sebaceous glands, hair follicles, various amounts of adipose tissue, and elastic fibers. And, One case revealed containing hyaline cartilage. No patient developed complications or reccurences. Conclusion: The authors recommend simple surgical excision of the remnants when discharge, infection, or cosmetic problem occur. Finally, these lesions do not have fistulous tracts or connections with important, deeper organs, and so can be safely transected at the level of the superficial musculature.
In this study, 88 dental students were examined to evaluate the relationship between occlusal states and TM disorders for the epidemiologic study of TM disorders. The clinical evaluation were composed of mandibular movement, TMJ noise, occlusal states and muscle palpation. The following results were disclosed. 1. The frequencies of pain on mandibular movement were 3.4% on maximum opening, 1.13% on protrusion and no pain on laterotrusion. 2. The frequencies on TMJ sound were 21.6% in click, 1.13% in crepitus. 3. The frequency of tenderness on palpation was 12.5% on extra oral, intraoral and neck muscles, tenderness on palpation of TMJ capsule were reported 5 cases, and 4 of them were female. 4. The distribution of Angle's classification was found 79.5% in class I, 4.5% in clasII-div.1 and 15.9% in class III. There was no significant differences on TM disorders between Angles classifications. 5. There was no significant differences on TM disorders between the subjects of canine guided occlusion and group function occlusion, and also for the differences between the subjects of nonworking side interferences and no interferences on laterotrusion. 6. There was no significant differences on TM disorders between the subjects of anterior teeth trauma in C.C. and no anterior teeth trauma, but there were significant differences between the subjects of posterior protrusive contact and no posterior protrusive contact.
Tonsil is rich in lymphatics, blood vessels, muscle, nearby nerve fibers and epithelium. All of these are potential sources of malignancy. Malignancy of the tonsils is an uncommon entity that accounts for little more than 0.5% of new malignancies in the United States every year. Approximately, more than 70% of malignancies in this region are squamous cell carcinoma. In this report, I introduce a case of 47 year male patient who has right tonsillar cancer that is squamous cell carcinoma(T4N3M0). After three times chemotherapy, the patient refused to get conventional western medical treatment including chemotherapy and surgical operation. After 8 month of traditional oriental medical treatment using allegen removed Rhus Verniciflua Stokes(ARV), the size of cancer mass decreased and mass showed necrosis at inner portion. The patient improved pain in the neck. More case will be needed in order to determine the effect of ARV on tonsillar cancer.

Objective : Cricothyrotomy and tracheostomy are performed by physicians in various disciplines. It is important to know the comprehensive anatomy of the laryngotracheal region. Hemorrhage, esophageal injury, recurrent laryngeal nerve injury, pneumothorax, hemothorax, false passage of the tube and tracheal stenosis after decannulation are well known complications of the cricothyrotomy and tracheostomy. Cricothyrotomy and tracheostomy should be performed without complications and as quickly as possible with regards the patients' clinical condition. Methods : A total of 40 cadaver necks were dissected in this study. The trachea and larynx and the relationship between the trachea and larynx and the surrounding structures was investigated. The tracheal cartilages and annular ligaments were counted and the relationship between tracheal cartilages and the thyroid gland and vascular structures was investigated. We performed cricothyrotomy and tracheostomy in eleven cadavers while simulating intensive care unit conditions to determine the duration of those procedures. Results : There were 11 tracheal cartilages and 10 annular ligaments between the cricoid cartilage and sternal notch. The average length of trachea between the cricoid cartilage and the suprasternal notch was 6.9 to 8.2 cm. The cricothyroid muscle and cricothyroid ligament were observed and dissected and no vital anatomic structure detected. The average length and width of the cricothyroid ligament was 8 to 12 mm and 8 to 10 mm, respectively. There was a statistically significant difference between the surgical time required for cricothyrotomy and tracheostomy (p < 0.0001). Conclusion : Tracheostomy and cricothyrotomy have a low complication rate if the person performing the procedure has thorough knowledge of the neck anatomy. The choice of tracheostomy or cricothyrotomy to establish an airway depends on the patients' clinical condition, for instance; cricothyrotomy should be preferred in patients with cervicothoracal injury or dislocation who suffer from respiratory dysfunction. Furthermore; if a patient is under risk of hypoxia or anoxia due to a difficult airway, cricothyrotomy should be preferred rather than tracheostomy.
Objectives : We investigate the characteristics of foot reflex zone acupoint of facial paralysis patients. Methods : In order to make a comparison between facial nerve paralysis patient group and non-facial paralysis group, we measured foot reflex zone acupoint detection in both group of 18 patients who were diagnosticated to facial nerve paralysis and 18 persons who were not. Results : 1. In comparing the means of the foot reflex zone, the measurements of facial nerve paralysis group is different significantly from non-facial paralysis group(p<0.05). 2. The measurement of detection of foot reflex zone acupoints, such as hypophysis(垂體), nose(鼻), cerebrum(大腦), neck(頸項), Trapezius muscle(僧帽筋), eye(眼) and ear(耳) of the facial nerve paralysis group is different significantly in comparison with non-facial paralysis group(p<0.05). But the measurement of detection of foot reflex zone acupoints, such as trigeminal nerve(三叉神經), cerebellum (小腦), kidney(腎), ureter(輸尿管) and urinary bladder(膀胱) of the facial nerve paralysis group is not defferent significantly in comparison with non-facial paralysis group(p>0.05). Conclusions : The results suggest that foot reflex zone can be used in the diagnosis and treatment of facial nerve paralysis.
Background: This study was designed to identify the effect of the taping on the forward head posture during computer work. Methods: Twenty healthy adults were statistically assigned into 2 groups, the taping group (n=10) and the control group (n=10). In order to induce delayed onset muscle soreness (DOMS), twenty subjects performed isometric exercise of the neck on Bobath table for 20 minutes. During the experiment, subjects in taping group were attached Kinesio-tape on their upper trapezius. By using 2-D motion analysis, measurements were taken before taping, at 24 hour, 48 hour, and 72 hour after inducing DOMS. The effects of taping were evaluated by the angle of the head. Results: The results of this study were as follows; 1) There was no significant difference between the taping group and the control group (p>.05). 2) The control group had no significance, but the taping group shown a significance on the angle of head during computer work (p<.01). The interaction of group x period also shown a significance (p<.01). Conclusion: From these result, it was revealed that the taping therapy on upper trapezius could improve the angle of head during computer work.
PURPOSE. The purpose of this study was to analyze the effects of the splinted implant prosthesis in a reconstructed mandible using three-dimensional finite element analysis. MATERIALS AND METHODS. Three-dimensional finite element models were generated from a patient's computed tomography data. The patient had undergone partial resection of the mandible that covered the area from the left canine to the right condyle. The mandible was reconstructed using a fibula bone graft and dental implants. The left mandibular premolars and molars remained intact. Three types of models were created. The implant-supported prosthesis was splinted and segmented into two or three pieces. Each of these models was further subcategorized into two situations to compare the stress distribution around normal teeth and implants. Oblique loading of 300 N was applied on both sides of the mandible unilaterally. The maximum von Mises stress and displacement of the models were analyzed. RESULTS. The stress distribution of the natural mandible was more uniform than that of the reconstructed fibula. When the loading was applied to the implant prosthesis of reconstructed fibula, stress was concentrated at the cortical bone around the neck of the implants. The three-piece prosthesis model showed less uniform stress distribution compared to the others. Displacement of the components was positively correlated with the distance from areas of muscle attachment. The three-piece prosthesis model showed the greatest displacement. CONCLUSION. The splinted implant prosthesis showed a more favorable stress distribution and less displacement than the separated models in the reconstructed mandible.
Muscle groups that are located in and around the vocal tract can produce audible changes in frequency and/or intensity of the voice. Vocal vibrato is a characteristic feature in the singing of performers trained in the western classical tradition and vibrato is generally considered to result from modulation in frequency amplitude and timbre. Vocal tremor is also characterized by periodic fluctuations in the voice frequency or intensity and vocal tremor is symptom of a neurological disease as Spasmodic dysphonia , Parkinson's disease. Vocal vibrato and Vocal tremor may have many of the same origins and mechanisms in the voice production systems. The purpose of this study is to find acostic character of Korean traditional song Pansori singer's vibrato and Spasmodic dysphonia patient's vocal tremor. twelve Pansori singers and seven Spasmodic dysponia patients participated to this study. Power spectrum and Real time Spectrogram are used to analyze the acoustic characteristics of Pansori singing and Spasmodic dysphonia patient's voice The results are as follows; First, vowel formant differences between Pansori singing and Spasmodic dysphonia patient's voice are higher F1, F3. Second, The vibrato rate show differences between Pansori singing and Spasmodic dysphonia patients;$4^{\sim}6/sec$ and $5{\sim}6/sec$ Vibrato rate of pitch is 5.7 Hz ${\sim}$ 42.4 Hz for Pansori singing , 3.8 Hz ${\sim}$ 27.9 Hz for Spasmodic dysphonia patients ;Vibrato rate of intensity range is 0.07 dB ${\sim}$ 8.26 dB for Pansori singing and 0.07 dB ${\sim}$ 4.81 dB for Spasmodic dysphonia patients
As global interest in clothing spreads over the smart clothing which arouses high added-value in the apparel industry, this study is to develop a smart jacket with an electrically-vibrating device maintaining excellent appearance and comfort. The vibrating device has a massage function that could relieve muscle pain near shoulders and neck. The purposes of this research are to develop jacket pattern for men in their thirties considering body shapes and fashion trend, to develop a wearable device that is composed of motor and controller and integrate it into a jacket, and to assess the external appearance and functional satisfaction of the smart jacket through the wearing test. The results are as follows: 1. In order to develope an appropriate jacket pattern for men in their thirties, several patterns were assessed for their fitting comfort. The final pattern was completed after making alterations some parts, and showed high satisfaction as 3.6(on a five-point scale) in all categories. 2. A vibrating device was developed by connecting motor, controller, battery and switch. Developing this device, focus was maximizing the strength of motor and minimizing the heat generated from motor and controller. Snaps were placed between inner and outer cloth of jacket so that the vibrating device could be easily attachable and detachable. The motor was located around Trapezius where muscles often get stiff. A switch was designed to be used in selecting the modes of Strong, Weak, and Cross Tapping. 3. The wearing test was conducted to examine outer appearance, comfort for motion, and functionality of the smart jacket. The results of assessing outer appearance showed that the location for attaching the vibrating device was not noticeable but looked natural, and there is almost no change in outer appearance when the vibrating device operates. The result of assessing comfort of action revealed that wearer's satisfaction was high in all categories about activity, wearer's comfort, etc. The result also showed that wear's satisfaction for effects of vibrating massage, easiness in using the device, heat generation was not less than 3.5 in all categories except a category about noise.
