하시모토 갑상선염과 동반된 갑상선 림프종 1예 (A Case of Thyroid Lymphoma Associated with Hashimoto's Thyroiditis)

  • Malignant lymphoma of the thyroid gland is a reletively rare and constitute up to 1-5% of all thyroid malignancies. It is known to be frequently associated with Hashimoto's thyroiditis. The clinical presentation includes an enlarging neck mass, accompanied by dysphagia, hoarseness or tenderness, with its most common histologic type being the diffuse large B cell non-Hodgkin's lymphoma. We have experienced a case of thyroid lymphoma arising from Hasimoto's thyroiditis in a 57-year-old woman, who presented with an anterior neck mass. We present this case with a review of the literature.

  • Dermoid cysts are benign neoplasm that is derived from both ectoderm and mesoderm. They may be found at various sites of the body, but are extremely rare to occur in the lower neck. We present the case of a 22-year old woman with a midline cystic mass anterior to the cervical trachea. The mass was excised under local anesthesia. Histopathologically the diagnosis was confirmed as a dermoid cyst.

  • The extranodal non-Hodgkin lymphoma is uncommonly occurred in larynx, accounting for less than 1% of all laryngeal neoplasm. In general, the laryngeal lymphoma is appeared as submucosal mass without mucosal ulceration and is most commonly found in supraglottis. The primary laryngeal lymphoma constitute a diagnostic challenge because they are characterized by absence of clinical and gross differential criteria, compared with squamous cell carcinoma (SCC). We encountered a 74-year-old man with hoarseness and lump sensation in the throat. On direct laryngoscope, multiple ulcerative and exudative mass in glottis and supraglottic areas were observed. The patient was finally diagnosed as large B-cell lymphoma through the laryngeal microsurgery. He received radiation therapy and there is no evidence of recurrence. Although the laryngeal mass has superficial mucosal change, primary laryngeal lymphoma must be included in the differential diagnosis.

음성변화를 주증상으로 내원한 유방암의 경부연조직 전이환자 1례 (Neck metastasis of invasive ductal carcinoma of breast causing voice change: a case report)

  • We present a metastatic carcinoma from the breast to the neck soft tissue around common carotid artery, with a rare finding of voice change. A 60 year-old female patient presented with voice change for 7 months. Neck ultrasound revealed a soft tissue mass between internal jugular vein and common carotid artery. Result of fine needle aspiration biopsy was a metastatic carcinoma. Computed tomography and magnetic resonance image revealed $2.5{\times}3.0cm$ sized irregular marginated soft tissue mass in right lower neck encasing common carotid artery and internal jugular vein. Surgical resection was performed and pathologic result with immunohistochemical analysis confirmed the diagnosis of a metastatic invasive ductal carcinoma originated from breast.

낭성 림프관종으로 오인한 전장 낭종 환자 1예 (A case of congenital foregut cyst; misdiagnosed as cystic hygroma)

  • Foregut cystic developmental malformations are rare developmental anomalies. It is important to diagnose the diseases and manage them properly because these cysts may generate feeding or respiratory difficulties depending on the size and location of the lesions. A newborn was referred for a congenital cervical swelling to our clinic on the second day of his life. Neck SONO and MRI showed an about 6cm sized cystic mass at left submandibular area. Aspirations and sclerotherapies were done repeatedly due to recurred cystic mass. Under the suspicious of cystic hygroma, the mass and submandibular gland were excised. Histologically, it was a benign cyst including gastrointestinal and bronchogenic mucosa and pancreatic tissue. Foregut cyst was suggested for the final diagnosis and the patient was discharged at 9 days after the operation without a complication. He has visited our out-patient department. Although several image studies have been introduced to find out foregut cyst, it is difficult to go through differential diagnosis because of similarity of other benign tumor. Further studies for early diagnosis of cervical foregut cyst are needed for preventing possible related problems.

하악골에 발생한 수복성 거대세포 육아종 1예 (A Case of Giant Cell Reparative Granuloma in the Mandible)

  • Giant cell reparative granuloma(GCRG) is a non-neoplastic rapidly expanding and locally destructive tumor that occurs almost exclusively within the mandible and maxilla. A 58-year-old man, complained of a mass on the left infra-auricular area starting 2 months ago. The radiologic finding suggests a mass that originate from mandible, pathology diagnosed the lesion as a giant cell reparative granuloma. The tumor was surgically excised without complications. We report the case with a review of literature.

구후 삼각부 종물 양상의 점액표피암종 1예 (A Case of Mucoepidermoid Carcinoma Presenting as a Retromolar Trigonal Mass)

  • Salivary gland tumors take possession of almost 5% in head and neck malignancies. Among these, mucoepidermoid carcinoma(MEC) is most common malignany in major salivary glands(12-29%) and the parotid gland is most predilection site. Intra-oral MEC has a tendency to various locations, and the predilection sites are palate, cheek, mandible, lip and tongue in order of frequency. A few cases of MEC are occurred in with retromolar trigone, oropharynx, and ectopic salivary gland. Recently, we experienced a-65-year old woman with retromolar trigonal mass, and she was finally diagnosed as MEC. We report it with review of literature.

  • Ganglioneuroma (GN) is benign neurogenic tumor arising from ganglia of the sympathetic nervous system. They are mostly found at posterior mediastinum, retroperitoneum, and adrenal gland, whereas only 1-5% occurred in the cervical region. GN usually present as a single, painless and slow-growing mass, but multiple cervical occurrences are extremely rare. An 80-year-old woman came to our clinic complained of posterior neck mass for three years. We performed surgical excision, and it was finally diagnosed as GN. We report the unique and rare disease entity with a brief literature review.

  • Pleomorphic adenoma (PA) is a benign tumor which usually originates from major and minor salivary glands. This tumor arising outside submandibular gland (SMG) is extremely rare. To author's knowledge, only four cases have been reported so far in English literature. Its pathogenesis is still unclear, but it can be explained by embryologic theory of major salivary gland. A 68-year-old man with an incidental mass on left upper lateral neck visited to our clinic. The radiologic findings showed well-margined round mass outside left SMG. The excisional biopsy revealed a pleomorphic adenoma. We report the rare and unique case with a brief literature review.

  • 경동맥에서 발생하는 가성동맥류는 비교적 드문 질환으로 주로 외상, 감염, 수술의 합병증 또는 방사선 치료 후에 합병증으로 발생할 수 있다. 비정형 결핵성 림프절염에 의한 경동맥 가성동맥류는 거의 보고된 바 없으며 따라서 비정형 결핵성 림프절염이 동반된 환자에서는 급성악화에 의한 농양형성으로 오인될 수 있다. 비정형 결핵성 림프절염으로 치료중인 81세 남자 환자에서 이러한 경험을 하였기에 문헌고찰과 함께 보고하고자 한다.
