Factors Associated with the Lumbar Spine and Femoral Neck Bone Mineral Density in Korean Elderly Women (한국 여성 노인의 대퇴경부 및 요추 골밀도 관련 요인)

  • This study was conducted among Korean women over aged 60 who participated in Korea National Health and Nutrition Examination Survey (KNHANES) (the 2nd and 3rd year at the 4th survey, and the 1st year at the 5th survey). The purpose of the study was to figure out a relation between femoral neck and lumbar spine bone mineral density and anthropometry, life style, diet, fracture history, family history of osteoporosis, medical history, menstrual history and reproductive factor. To express the strength of the associations, percent differences were calculated from multiple linear regression models using the formula ${\beta}{\times}$(unit/mean BMD). Unit for continuous variables were chosen to approximate 1 standard deviation(SD). In women aged 60 or older, lean mass(B: 0.257) and fat mass(B: 0.237) greatly influenced bone density in the femoral neck and lumbar spine. Therefore it will be an effective way to prevent osteoporosis for elderly women by increasing lean mass and maintaining proper weight.

Dietary and Non-dietary Factors Related to Bone Mineral Density in Female College Students (여대생의 골밀도와 식이 및 비식이 요인들 간의 상관성)

  • The purpose of this study was to investigate correlations between bone mineral density (BMD) and dietary and non-dietary factors in female college students. The BMD of the subjects (n = 38) was measured using DEXA (Dual Energy X-ray Absorptiometry) at lumbar spine and three femoral sites including femoral neck, ward's triangle, and femoral trochanter. Three-day 24-hour dietary recall data were collected from each subject to assess consumption levels of nutrients and food groups. The mean (${\pm}SD$) values of age, age of menarche, height, weight, body mass index (BMI), waist-to-hip ratio (WHR), fat mass, and % body fat of the subjects were 21.34 (${\pm}1.73$) years, 13.1 (${\pm}1.2$) years, 161.3 (${\pm}5.0$) cm, 53.7 (${\pm}7.2$) kg, 20.6 (${\pm}2.6$) $kg/m^2$, 0.80 (${\pm}0.04$), 15.4 (${\pm}4.4$) kg, and 28.2 (${\pm}4.7$), respectively. The BMD values of lumbar spine, femoral neck, ward's triangle, and femoral trochanter as T-value were $1.150{\pm}0.13\;g/cm^2$, $0.932{\pm}0.11\;g/cm^2$, $0.850{\pm}0.13\;g/cm^2$, and $0.721{\pm}0.10\;g/cm^2$, respectively. The daily mean energy intake of the subjects was 1660.6 kcal. The intake levels of carbohydrate, calcium, iron, vitamin C, and folic acid were lower than the KDRIs, while those of fat, phosphorus, sodium, vitamin A, and vitamin $B_6$ were higher than the KDRIs. Significantly negative correlation were detected between consumption of fat and oils and the BMD of all sites measured (p < 0.05). Potato and starch intake was negatively correlated to the BMD of femoral neck and word's triangle (p < 0.05). The intake of cereals was found to be negatively correlated to the BMD of ward's triangle (p v 0.05). There was also negative correlation of intake of soybeans and products with the BMD of lumbar spine (p < 0.05). Weight and muscle mass were positively correlated to the BMD of both lumbar spine and femurs (p < 0.01). Significantly positive correlations between BMI, and fat mass and the BMD of lumbar spine were observed (p < 0.001). Additionally fat mass was positively correlated to the BMD of femoral neck. WHR and % body fat were positively correlated to the BMD of lumbar spine (p < 0.05). Negative correlation was found between a serum calcium level and the BMD of femoral neck (p < 0.05). In summary non-dietary factors generally showed significant correlations with the BMD of lumbar spine, while several dietary factors showed significant correlations with the BMD of femurs.

False Aneurysm of Common Carotid Artery by Trauma (외상에 의한 가경동맥루 1례 보고)

  • Aneurysms of the extracranial carotid artery are rare. This is a case report of the rapidly expanding false carotid aneurysm at left common carotid artery, which was repaired surgically with internal shunt This 20 year old male patient had a large pulsatile mass on left lateral aspect of neck at the time of admission. About 1 month before admission, he had received a trauma on left neck by glass piece and noted massive blood loss. And its skin of lesion was sutured simply at local clinic and well healed. 10 days before the admission, he had the sudden onset of the adult thumb sized and pulsatile mass and the mass had been enlarged more and more to the adult first-sized one. The cervical film showed a egg-sized and soft tissue mass. There was systolic bruit on the mass. The diagnosis was confirmed with the angiogram of left carotid artery and this showed the man`s thumb tip-sized extravasation at the point 2 Cm below the bifurcation of Internal and external carotid arteries. The emergency operation was performed by the internal shunt with carotid artery. The aneurysm was enclosed with the adventitia and carotid sheath, and the intima and media were Intact and had the opening of 0.5 cm in diameter. The opening was sutured by the one-hand mattress suture method and firmed with the Aron Alpha-A "Sankyo." The postoperative course was uneventful and the patient was discharged with good general condition.

Tracheal Stenosis by Extraluminal Compression (외인성 기관협착)

  • Tracheal stenosis can be classified into intrinsic stenosis secondary to tracheal inflammatory lesion or mass effect and extrinsic stenosis secondary tumors of thyroid, esophagus and mediastinum. Extrinsic stenosis which is frequently encountered in clinical setting could be often overlooked due to mild symptom. Recently, even with the increasing interest in extrinsic tracheal stenosis there are limitation in it's diagnosis and treatment. The purpose of this study is to provide guidance in the diagnosis and treaonent of extrinsic tracheal stenosis. Here, we report the etiology, symptoms, radiologic findings, pulmonary fuction finding, treatment and its results in 26 cases of extrinsic tracheal stenosis. Causes of extrinsic tracheal stenosis included compression of aiway by thyroid benign tumor in 13 cases to be the most common, next by thyroid malignancy in 9 cases, metastatic mediastinal turner in 2 cases, 1 case each for esophageal cancer and parathyroid cancer. In 3 cases simple tracheal resection and end to end anastomosis were done, 1 cases underwent total laryngectomy, and 8 cases were treated by conservative management, where all cases failed in treatment. The remaining 14 cases were successfully treated by removing the causes and maintaining tracheal tube insertion for amount of time. Extrinsic tracheal stenosis due to benign conditions were treated satisfactorily by removing mass, however with the malignant causes there was considerable amount of difficulty in treatment.

Malignant Mixed Tumor of Salivary Glands: A Clinical Study (악성 혼합종의 임상적 고찰)

  • For malignant neoplasms of salivary tissues. two of the better determinants of progosis are histologic classification and size of the neoplasm. Proper management of these tumors requires an accurate diagnosis by the pathologist and correct interpretation by the surgeon. Malignant mixed tumors account for between 3 and 13 precent of all cancers of the salivary glands and 2 percent of all tumors in these locations. The typical history of these tumors is that of slowly growing mass demonstrating a sudden increase in growth. The duration of onset of the tumor mass and the diagnosis of malignancy has been demonstrated to be 10 to 18 years. The risk of malignat transformation of a benign mixed tumor increases with the duration of the tumor. We analyzed retrospectively 13 cases of malignant mixed tumor who visited from Jan. 1985 to Dec. 1992. Mean age of the patients was 56.5 years. The origin of tumors were parotid gland 7 cases, submandibular gland 2 cases, and minor salivary gland 4 cases(palate 3 cases, tonsil pillar 1 case). According to the criteria of the AJCC on staging, stage I was 1 case, stage II 1 case, stage III 2 cases, and stage IV 9 cases. Histopathologically, carcinma ex pleomorphic adenoma were 12 cases and the true malignant mixed tumor was 1 case. The major treatment modalities were curative surgery, and radiation therapy followed. In conclusion, aggressive therapy of combined surgery and postoperative radiation therapy is required for these lesions, and patients with known or suspected benign tumor should be encouraged to undergo surgery early on in their disease to avoid malignant degeneration at a later dete.

The Application of Circular Boundary Overlapping in 3-D Reconstruction of Neck Tumors (두경부 종물의 3차원 재건 영상에서, 원형 경계선 중첩을 이용한 경계선 추출법의 응용)

  • Background and Objectives : Boundary detection and drawing are essential in 3D reconstruction of neck mass. Manual tracing methods are popular for drawing head and neck tumor. To improve manual tracing, circular boundaries overlapping was tried. Materials and Methods : Twenty patients with neck tumors were recruited for study. Representative frames were examined for shapes of outline. They were all single closed curves. Circular boundaries were added to fill the outlines of the tumors. Inserted circles were merged to form single closed curves(Circular boundary overlapping, CBO). After surface rendering, 3 dimensional images with volumes and area data were made. Same procedures were performed with manual tracing from same cases. 3D images were compared with surgical photographs of tumors for shape similarity by 2 doctors. All data were evaluated with Mann-Whitney test(p<0.05). Results : Shapes of boundaries from CBO were similar with boundaries from manual tracing. Tumor outlines could be filled with multiple circular boundaries., While both boundary tracing gave same results in small tumors, the bigger tumors showed different data. Two raters gave the similar high scores for both manual and CBO methods. Conclusion : Circular boundary overlapping is time saver in 3 dimensional reconstruction of CT images.

Metastatic Papillary Thyroid Carcinoma Masquerading as Type II Branchial Cleft Cyst : A Case Report (제2형 새열 낭종으로 오인된 전이성 갑상선 유두상 암종 1예)

  • In case of lateral cervical cystic lesions, the differential diagnoses include branchial cleft cyst(BCC), teratoma, dermoid, hemangioma and lymphangioma etc. But sometimes metastatic cystic lymph nodes may exist in lateral neck. In such circumstance, the primary lesions are known to stem from oropharynx, nasopharynx, salivary and thyroid gland etc. A-66-year-old-male came to our clinic, due to the lateral cervical mass for 5 years. We performed the neck CT, sonography and sono-guided FNAC. He was initially diagnosed with the benign cyst such as BCC. We performed the excisional biopsy on left level II, but the pathologic report was revealed as metastatic papillary thyroid carcinoma(PTC). And then he received the total thyroidectomy with neck dissection. The final diagnosis was cystic metastasis from PTC. We learn a valuable lesson form this case in the following. Even if the simple cervical cyst is presumed with radiology and clinical pattern, more careful considerations on the basis of history and radiologic findings are mandatory.

Parathyroid Adenoma Causing Spontaneous Cervical Hematoma: A Case Report and Review of Literature (급성 경부 혈종을 일으킨 부갑상선선종 1예)

  • Parathyroid adenoma can cause extracapsular bleeding. In 1934, Capps first reported a case of massive hemorrhage secondary to rupture of a parathyroid adenoma. Recently, we experienced a 73-year-old female presented with pharyngeal discomfort and extensive ecchymosis over the neck without history of trauma. Endoscopic investigation revealed submucosal hemorrhage in the posterior wall of the hypopharynx. CT scan and ultrasonography demonstrated the presence of a mass below the left thyroid lobe. Serum calcium level was normal and PTH level was elevated. We underwent left thyroidectomy and parathyroidectomy 2 weeks later from first visit. During the operation, hypopharyngeal mucosa was teared and it was treated with pharyngostoma formation and L-tube feeding. We report a rare case of normocalcemic parathyroid adenoma with spontaneous hemorrhage and propose the proper management period with a literature review.

A Case of Malignant Transformation of Inflammatory Myofibroblastic Tumor of the Neck (육종성 변화를 동반한 경부의 염증성 근섬유모세포종 1례)

  • Iinflammatory myofibroblastic tumor(IMT) is a benign chronic inflammatory mass composed of proliferative myofibroblasts. It is a space occupying lesion which could potentially covert to malignant tumor. Treatment guideline of the disease has not been established due to its rarity. We demonstrate a 60-year old male who had surgical excision for IMT of the cervical esophagus. During the follow-up period, he revealed recurrent tumor which showed sarcomatous change with distant metastasis. We reported this rare case with review of the literature.

  • The most common cause of treatment failure in oral cavity cancer is when it is found to have local recurrence, usually occurring in the ipsilateral cervical lymph node. On the contrary, it is extremely rare to find local recurrence in soft tissue metastasis (STM) in the contralateral neck. Furthermore, lung cancer and malignant lymphoma are most commonly confined to their primary sites. The poor general condition increases the likelihood of STM, which indicates bad prognosis. A 72-year-old man with a hard and fixed mass on the right submandibular space visited our clinic. He had received a wide excision with local flapreconstruction for squamous cell carcinoma in the left corner of lower lip 18 months ago. We performed the wide excision with bilateral selective neck dissection (I-III), and he was finally diagnosed as STM from contralateral lip cancer. We report this unique and rare disease entity with a literature review.
