악하선에 발생한 호산성 세포암종 1예 (A Case of Oncocytic Carcinoma Arising in the Submandibular Gland)

  • Oncocytic neoplasm of the head and neck region accounts for approximately 1% of all salivary gland tumors, but only 5% of oncocytic neoplasm is malignant. Oncocytic carcinoma arising in the submandibular gland is exceedingly rare. We encountered a sixty seven-year-old male patient who presented with multiple mass in the right neck. Fine needle aspiration biopsy revealed a salivary gland tumor of predominantly oncocytic form, and a differential diagnosis included oncocytic adenoma or mucoepidermoid carcinoma. A right submandibular gland resection and modified radical neck dissection were performed. Histologically, the tumor cells showed nuclear pleomorphism, and stromal invasion, which were compatible with oncocytic carcinoma. After surgery, the entire neck region was irradiated. Seventeen months after the initial surgery, multiple metastases to the bone and lung were detected from the incidental pathologic bone fracture of the right humerus; palliative chemotherapy was performed to resolve this. We report a case of oncocytic carcinoma in the submandibular gland with a review of literature.

갑상선 종물로 오인된 Killian-Jamieson 게실 1예 (A Case of Killian-Jamieson Diverticulum Simulating Thyroid Mass)

  • Killian-Jamieson and Zenker diverticula are both rare pharyngoesophageal diverticula. Both are outpouching of the mucosal and submucosal layers of the esophageal wall, which protrude through a mucosal gap at the level of the pharyngoesophageal esophagus. When these diverticula are large enough, they can be in proximity to the thyroid gland and may mimic a thyroid nodule. We report a case in which a diverticulum was filled with dietary residue and thus simulated a thyroid cyst on CT scan. And it was finally diagnosed as a Killian-Jamieson diverticulum by the surgery.

비인강암 (Nasopharyngeal Cancer)

  • Thirty nine cases of nasopharyngeal malignant tumor from 1981 to 1989 in Korea univeristy hospital were reviewed retrospectively. The results were as follows; 1) The most common initial symptom was neck mass(41.0%), and cranial nerve involvement was found in 6 cases. Lateral wall of the nasopharynx including $Rosenm\"{u}ller's$ fossa was the frequent predelection site(68.4%). 2) Histopathologically, squamous cell carcinoma was the most common and stage IV were 71.8%. 3) Thirty four cases were treated by radiation therapy alone and 5 cases were treated by combined modality(radiation therapy+chemotherapy). Overall 5 year survival rate was 28.2%(squamous cell carcinoma; 23.5%, undifferentiated carcinoma; 35.7%).

측경부에 발생한 이하선외 Warthin씨 종양 2예 (Two Cases of Extraparotid Warthin's Tumor in Lateral Cervical Region)

  • Extraparotid Warthin's tumor(EPWT) is a rare disease entity; its incidence is about 8% of Warthin's tumor(WT). The periparotid and upper cervical lymph nodes are the most predilection sites. The lymphoid tissue of WT can act like a regional lymph node, and the necrosis and inflammation within the tumor are well known. In our cases, both 81-year-old man and 58-year-old man were visited our clinic, presented with upper lateral neck mass that had been present for the last few months. We operated the excisional biopsy in level ll of neck. The pathologic examination was proven to be EPWT. We report the unique two cases of extraparotid Warthin's tumor with literature review.

  • Spindle cell carcinomas of the head and neck are unusual neoplasms that are histologically biphasic, having both squamous cell carcinoma and apparently malignant spindle cell or sarcomatous element. Controversy about the histogenesis of these tumors has resulted in various terminology ; sarcomatoid carcinoma, pseudosarcoma, carcinosarcoma, and spindle cell carcinoma. The clinical course and natural history of these tumors are also variable. We report a case of 64-year-old male who complained of a neck mass. The patient was treated with extended radical neck dissection and postoperative radiotherapy, but the primary site of spindle cell carcinoma could not be found. We reviewed the clinical course, treatment, prognosis, and histogenesis of spindle cell carcinoma of the head and neck.

  • Ectopic thyroid gland is a defined as thyroid tissue located in a site other than its usual pre-tracheal location. Ultrasound examination, CT scan, thyroid scan, and thyroid iodine uptake test are all valuable modalities for a differential diagnosis. Although most of cases are asymptomatic, treatment is indicated when there are symptoms related to progressive growth of mass. This article reports a ectopic thyroid cyst in the anterior neck that was similar to deep neck infection accompanied by acute airway obstruction.

  • The complications of deep neck infection have become much less common in the antibiotic era. The vascular complications of deep neck infection can have devasting consequences. Most commonly, the internal carotid artery is involved, although the common carotid and external carotid artery can also be affected.0 the cases of patients with a protracted course, recurrent bleeding, cranial neuropathies, or trismus, the presence of vascular complications must be considered. Appropriate imaging should be carried out to allow the localization of the infection and ascertain the status of the vessels in the neck The vascular structures can be imaged with duplex doppler or color doppler flow ultrasound to see the flow between the mass and vessels. Also angiography plays a key role in the diagnosis and management of vascular complication of deep neck infection. Prompt diagnosis and treatment of these patients is necessary to prevent significant hemmorrhagic complications. We experienced a case of pseudoaneurysm of the common carotid artery secondary to deep neck infection treated successfully with surgical excision in 45-year-old-male.

혀 기저부의 골성 분리종 2예 (Two cases of osseous choristoma on tongue base)

  • Osseous choristoma is a rare, benign proliferative osseous lesion, which is defined as the growth of normal tissue in an abnormal location. The etiopathogenesis for its formation is unknown, but various hypotheses have been proposed. Treatment of choice is en-bloc resection, and no recurrence has been reported. Here, we report the two cases of osseous choristoma, presented with a mass on the base of the tongue with/without globus symptom and were treated with surgical excision.

갑상선에서 발생한 원발성 편평상피세포암 1예 (A Case of Primary Squamous Cell Carcinoma of The Thyroid Gland)

  • Primary squamous cell carcinoma of thyroid is a very rare malignant tumor with poor prognosis. It is usually diagnosed as an advanced disease infiltrating adjacent organs, and characterized by aggressive clinical course with an average postoperative survival time of less than 1 year. Recently, we had a 79- year-old woman with a painful neck mass who was diagnosed as primary squamous cell carcinoma of thyroid gland. She underwent total thyroidectomy and selective neck dissection(level III, VI) with no further postoperative managements such as radiation therapy or chemotherapy; she died of poor general condition and pneumonia resulting from rapid progression of the lesion on the 38th day after surgery. We report this case with a review of relevant literatures.

  • Myofibroblastic sarcoma can be classified as low-, intermediate-, and high-grade. Low-grade myofibroblastic sarcoma (LGMS) is uncommon and rarely appears on the skin. LGMS is diagnosed based on histopathological and immunohistochemical findings. Additionally, LGMS metastases are rare as well. Herein, the authors describe a case of cutaneous LGMS in the neck. To the best of our knowledge, there are no cases regarding cutaneous LGMS in the neck present in the existing literature. Surgical resection is considered as the most important treatment for LGMS. Therefore, a complete tumor resection was performed in this case. However, another mass in the uterus was identified on abdominal computed tomography performed post-surgery.
