Relationship of neck disability index, shoulder pain and disability index, and visual analogue scale in individuals with chronic neck pain

  • Objective: Several muscles surrounding neck are vital not only for neck motion, but for upper extremities motions as well. Neck pain would affect neck and shoulder disability. The Neck Disability Index (NDI), Shoulder Pain and Disability Index (SPADI), and Visual Analogue Scale (VAS) are increasingly used to evaluate treatment effectiveness after chronic neck pain. The purpose of this study was to analyze the correlation of neck pain, shoulder pain, and quality of life in patients with chronic neck pain. Design: Cross-sectional study. Methods: Forty-three patients with neck pain participated in this study. Participants were instructed to place a mark on each item of three clinical measures that best represented their experience of his/her neck and shoulder problem over the last week. The outcome measures of the study were NDI, SPADI, and VAS for neck and shoulder pain and disability. Results: The NDI was significantly correlated with the VAS-Neck, SPADI-Pain, SPADI-Disability, SPADI-Total, and VAS-Shoulder (p<0.05). The VAS-Neck was significantly correlated with the VAS-Neck, SPADI-Pain, SPADI-Disability, SPADI-Total, and VAS-Shoulder (p<0.05). The SPADI-Total was significantly correlated with the NDI, VAS-Neck, SPADI-Pain, SPADI-Disability, and VAS-Shoulder (p<0.05). The SPADI-Pain was significantly correlated with the NDI, VAS-Neck, SPADI-Disability, SPADI-Total, and VAS-Shoulder (p<0.05). The SPADI-Disability was significantly correlated with the NDI, VAS-Neck, SPADI-Total, SPADI-Pain, and VAS-Shoulder (p<0.05). The VAS-Shoulder was significantly correlated with the NDI, VAS-Neck, SPADI-Pain, SPADI-Disability, and SPADI-Total (p<0.05). Conclusions: Neck disability in patients with chronic neck pain should be considered with disability and pain of the shoulder.

경도의 경부장애를 가진 젊은 성인에서 도수교정과 관절가동술이 NDI 및 CROM에 미치는 영향 (The Effects of Manipulation and Mobilization on NDI and CROM in Young Adults with Mild Neck Disability)

  • Purpose : Neck pain is commonly experienced by both adolescents and adults. The purpose of this study was to measure neck disability index(NDI) and the cervical range of motion(CROM) following spinal manipulation and mobilization techniques. Methods : Thirty participants(manipulation=15, mobilization=15) with mild neck disability volunteered for this study. It was used to measure NDI with NDI questionnaire and six motions(neck flexion and extension, left and right lateral flexion, left and right rotation) with the CROM instrument. Independent t-test and paired t-test were used to estimate NDI and CROM, and compared left with right motions. Results : A significant decrease in NDI were found after spinal manipulation and mobilization treatment(p<.05). A trend toward increase in all cervical motions(flexion, extension, left and right lateral flexion, left and right rotation), after interventions, were revealed(p<.05). The significant differences were not found on NDI and CROM between the group(p<.05). It was presented asymmetric motions in cervical lateral flexion(p<.05) before and after treatment. Conclusions : Our results suggest that manipulation and mobilization probably provide at least short-term benefits for patients with mild neck pain. It is needed to be studied the factors and preventive methods influencing the asymmetric cervical motion.

일부 대학생의 경부장애지수에 대한 분포 (The Prevalence of Neck Disability Index among Some College Students)

  • 경부 통증은 청소년과 성인에서 흔히 경험된다. 경부장애지수는 경부통의 상태를 평가하기 위하여 가장 흔히 사용되는 자가보고 방식이다. 이 연구의 목적은 일부 대학생의 경부장애지수에 대한 분포를 조사하는 것이다. 자료는 대학생(남성: 229, 여성: 405)을 위한 NDI 설문지로 수집되었다. 통계학적 분석은 SPSS 12. 0으로 이루어졌고, 기술통계, 독립표본 t-test와 피어슨 상관분석을 사용하였다. 결과는 다음과 같다. 첫째, 경부장애의 정도를 나타내는 두 번째 선택 안에서 높은 항목은 '두통'으로 나타났고, 세 번째 선택 안에서 높은 항목은 통증정도로 나타났다. 둘째, 여성이 남성보다 더 높은 경부장애지수, 백분율, 경부장애등급을 나타냈다(p<0.05). 셋째, 남성은 '장애 없음'이 63.8%, '경도 장애'가 35.4%, '중등도 장애'가 0.9%로 나타났고, 여성은 '장애 없음'이 53.6%, '경도 장애'가 44.7%, '중등도 장애'가 1.5%, '중증 장애'가 0.2%로 나타났다. 마지막으로 경부장애지수등급은 경부장애지수 항목과 상관성이 있는 것으로 나타났다(p<0.05). 본 연구의 결과들은 장래의 임상적, 역학적 연구 자료에 기초를 제공할 수 있을 것이며, 경부장애의 예방과 관리에 대한 적절한 교육과 대책이 필요함을 나타낸다.

경통 환자 평가를 위한 Neck Disability Index의 신뢰도와 타당도 (Reliability and Validity of the Neck Disability Index in Neck Pain Patients)

  • The purpose of this study was to determine reliability and validity of the Neck Disability Index (NDI) now in use for the first time in neck pain patients of Korea. Fifty subjects (26 males and24 female) with neck pain enrolled in the study. They completed a standardized self-administered questionnaire that include pain intensity, personal care, lifting, reading, headaches, concentration, work, driving, sleeping and recreation. Reliability was determined by intra-class correlation coefficient (ICC) and Cronbach's alpha by internal consistency. Validity was examined by correlating the NDI scores to the Visual Analog Scale (VAS) score. The Test-retest reliability of the translated versions of the NDI was good ICC (2,1) = .90 (95%CI .85 .95). Cronbach's alpha value for NDI was found to be .95 and this was statistically significant (p<.05). The criterion-related validity coefficients was .72 (p<.01). We conclude that the Korean version of NDI has shown to be a reliable and valid instrument for the assessment of neck pain.

경부통증대상자에 대한 경부장애지수의 Rasch분석 (Rasch Analysis to Neck disability Index with Neck Pain Subjects)

  • Purpose: The purpose of this study was to examine the functional category, the item structure and the model-data fit of the neck disability index (NDI) of neck pain subjects by performing a Rasch rating scale analysis. Methods: The data was obtained from the assessments of 71 college students (males: 27, females: 44) with neck pain. The data of the NDI was applied to the Rasch's rating scale model to estimate the difficulty of items, the goodness-of-fit of each item, the separation reliability and index, and the rating scale. Results: The 'sleep' item showed misfit and nine items were founds to be fits for self-reporting of disability due to neck pain. The most difficult item of the remaining 9 items was 'work' and the easiest item was 'headache'. The transformation formula score=(logit score+7.10)/(7.10+0.11)$\times$100. The 6 response levels of the NDI were validated according to the structure of the rating scale. The item and subject reliability of the separation reliability was 0.97 and 0.85, respectively. Conclusion: We proved that the NDI for self-reporting of disability of daily activities due to mild neck pain was valid and reliable. This study suggests that individuals with mild neck pain may be assessed by using the modified NDI that does not include the 'sleep' item in the 10 items of NDI.

Relationship of Pain, Disability and Quality of Life in Individual with Neck and Shoulder Pain

  • Purpose: This study was to investigate the correlation between pain, disability and quality of life among adolescents and office workers with neck and shoulder pain. Methods: Twenty-four subjects (mean age=24.92 years, SD=3.94, range=20-37) with neck and shoulder pain participated in this study. The outcome measures of the study were visual analogue scale (VAS), neck disability index (NDI), shoulder pain and disability index (SPADI), short form-36 (SF-36) for neck and shoulder pain and disability, quality of life. Pearson's correlation and Speaman's rank correlation were used to measure the association between VAS and NDI, SPADI, SF-36. Results: The VAS showed a positive correlation between weak to moderate with the NDI and SPADI, but no significantly correlated (${\rho}=0.34$, ${\rho}=0.25$). The moderate positive correlation and significant correlation were observed between NDI and SPADI (${\rho}=0.43$, p<0.05). The PCS of SF-36 was significantly negative correlated with the VAS (${\rho}=-0.24$), NDI and SPADI (${\rho}=-0.63$, ${\rho}=-0.59$, p<0.05). Conclusion: The pain and disability of neck and shoulder has closely relevance. And, the pain and disability of neck and shoulder has a negative impact on the quality of life.

Addition of Myofascial Release Therapy to Therapeutic Exercise for Management of Nonspecific Neck Pain

  • Purpose : It is necessary to demonstrate the effect of non-invasive and non-pharmacological interventions such as manual therapy and therapeutic exercise for the management of nonspecific neck pain. In the present study, we aimed to investigate the efficacy of myofascial release therapy plus therapeutic exercise for disability owing to neck pain and quality of life in individuals with nonspecific neck pain. Methods : Eighteen participants with nonspecific neck pain were randomly allocated to intervention (n=9) and control groups (n=9). The intervention group received a myofascial release therapy for 20 min and performed neck stabilization exercises for 30 min twice a week for 4 weeks. The control group performed neck stabilization exercises for 30 min twice a week for 4 weeks at the same time points as the intervention group. Disability owing to neck pain and quality of life were quantified using the neck disability index (NDI) and the Korean version of the World Health Organization Quality of Life Brief Version (WHOQOL-BREF), respectively. NDI and WHOQOL-BREF were assessed before and after intervention. Results : The disability owing to neck pain significantly changed between the groups over time (total score of NDI, p=.049). There were significant time and group interactions in pain (pain intensity of NDI, p=.035) and concentration (concentration of NDI, p=.049). Personal care, lifting, reading, headaches, work, driving, sleeping, and recreation did not show significant improvement between the groups over time. Total score, overall quality of life and general health, physical health domain, psychological domain, social relationships domain, and environmental domain quantified by WHOQOL-BREF did not show significant improvements between the groups over time. Conclusion : These results suggest the clinical use of myofascial release therapy in addition to therapeutic exercise for the management of nonspecific neck pain. Further studies are needed to generalize the findings of this study.

PNF 목 패턴이 전방머리자세를 가진 성인의 목 움직임 및 목 장애 지수에 미치는 영향 (Effect of PNF Neck Patterns on Neck Movement and NDI in Adults with Forward Head Posture)

  • Purpose: The purpose of this study was to investigate the effects of neck patterns in proprioceptive neuromuscular facilitation (PNF) for neck movement and the neck disability index (NDI) among adults with forward head posture. Methods: Thirty-nine subjects were randomly assigned into two groups. Subjects in the proprioceptive neuromuscular facilitation exercise group (PNFG, n = 20) received 20 minutes of PNF neck pattern (flexion-Rt. lateral flexion-Rt. rotation followed by extension-Lt. lateral flexion-Lt. rotation) 3 times weekly for 4 weeks. Outcomes were measured using absolute rotation angle (ARA), anterior weight bearing (AWB), range of flexion and extension motions (RFEM), and neck disability index (NDI) methods before and after the 4-week intervention period. Results: There were significant effects for the PNFG, pre- and post-intervention, in ARA, AWB, RFEM, and NDI. There were significant differences in ARA, AWB, RFEM, and NDI compared with CG. Conclusion: The results of this study suggest the PNF neck pattern could be beneficial for adults with forward head posture.

교통사고로 인한 경추통 환자의 공포 회피반응과 경추통, 목 장애 지수 사이의 관련성 분석 (A Correlation Analysis of Neck Pain, Neck Disability Index and Fear-avoidance Beliefs in Patients with Neck Pain Caused by Traffic Accidents)

  • Objectives : This study aimed to investigate the correlation between fear-avoidance beliefs and neck pain/neck disability caused by traffic accidents. Methods : We surveyed 52 patients who were injured in traffic accidents. The patients completed baseline measures of pain, disability, fear-avoidance beliefs according to the visual analogue scale(VAS), neck disability index(NDI) and a fear-avoidance beliefs questionnaire (FABQ). At the end of medical treatment, VAS and NDI were reassessed. In order to determine the relationship between FABQ(including its subscales), change of VAS and NDI, Pearson correlation coefficients were used. To examine the accuracy of previously reported cut-off scores, we classified the patients into low and high groups and analyzed the change of VAS and NDI. The relationship was measured by Mann-Whitney U-test and a student's T-test. Results : FABQ and its subscales(total, physical activity and work) were significantly correlated with final VAS and NDI. FABQ-T showed low significant negative correlation with change of NDI. Previously reported cut-off scores did not show a statistical significance in this study Conclusions : This study suggests that screening for fear-avoidance beliefs may be useful for identifying patients at risk of prolonged pain after traffic accidents.

대학생의 스마트폰 이용 시간과 경항통과의 관련성 연구 (The Correlation Analysis between Hours of Smartphone Use And Neck Pain in the Gachon University Students)

  • Objectives : The purpose of this study was to analysis the relationship between hours of smartphone use and neck pain in university students. Methods : A survey of 2,353 university students was conducted in Gyeonggi province and Incheon city using a self report questionnaire from April 1 to 11 2013. The questionnaire of questions regarding the hours of using smartphone, mainly used function in participant's smartphone and neck discomfort degree. Neck disability index(NDI) was used to evaluate degree of pain. The collected data were analyzed with the t-test and Kendall's tau test using the SPSS 21.0 program and R 3.1.0, respectively. Results : 1. Almost all of the survey participants(99.07 %) were smartphone users. 66.97 % of them answered that they use their smartphone daily more than 2 hours and 48.18 % of them answered that they use their smartphone 10 to 30 minutes everytime they use it. 2. The overall distribution of NDI scores was 'no disability(0~4 score, 62.92 %)', 'mild disability(5~14 score, 32.85 %)', 'moderate disability(15~24 score, 1.19 %)'. As a result of t-test, we found that the average NDI score for female students was significantly higher than the average NDI score for male students(p<0.05). 3. The Kendall's tau test revealed that total time spent daily using smartphones has a strong positive correlation with 'pain intensity', 'lifting', 'reading' 'headache', 'concentration', and 'driving'(p<0.05) in NDI. Also, time duration of one time smartphone usage has a strong positive correlation with 'pain intensity', 'lifting', 'reading', 'headache', 'concentration', 'work', and 'recreation'(p<0.05). 4. The Kendall's tau test revealed that total NDI scores have a significantly strong positive correlation with both of total time spent daily using smartphones(p<0.05) and time duration for one time smartphone usage(p<0.05). This results imply that long-time use of smartphone has a strong relationship with neck pain. Conclusions : The results of the study would be a good starting point for future studies to reduce the risks of chronic neck pain caused by smartphone usage.
