Frontonasal Bone Flap for Treating a Paranasal Sinus Cyst Diagnosed by CT in a Thoroughbred Racehorse

  • A 3-year-old Thoroughbred gelding was referred to the Jeju National University Equine Hospital with prolonged unilateral nasal discharge for several months. A sinus cyst was suspected based on the history, symptoms, endoscopic findings, and radiographic results. Computed tomography (CT) scan revealed that the cyst (7.5 × 8.0 × 10.0 cm) was located between the rostral maxillary sinus and the frontal sinus, causing distortion and remodeling of the overlying bones, swelling of the overlying cutaneous tissues, and a slight deviation of the nasal septum. Standing surgery under sedation was decided to remove the cyst. A frontonasal bone flap was performed using an oscillating bone saw, and the cyst filled with pus was removed. To support the approach to the maxillary sinus, trephination was performed. The horse was treated with sinus lavage via catheterization, nebulization, antibiotics, and nonsteroidal anti-inflammatory drugs. The horse was discharged 18 days after the surgery without signs of any complication. This case showed that a CT scan could be a valuable tool for the diagnosis and subsequent surgical management of paranasal sinus cysts in horses. Also, the frontonasal bone flap was useful for exposing and removing the large-sized of cyst in the paranasal sinus.

The Efficacy of Nebulized 3 Percent Hypertonic Saline Solution and Fenoterol in Infants with Bronchiolitis (영아 세기관지염에서 3% 고장성 식염액과 Fenoterol 병용흡입 치료의 효과)

  • Purpose : To evaluate the effect of inhaled hypertonic saline solution in hospitalized infants with bronchiolitis. Methods : A randomized double blind trial was performed from October 2003 to May 2004. A total of eighty patients <1 year of age with a clinical diagnosis of acute viral bronchiolitis were enrolled and assigned to receive either of the following : inhalation of 2 mL(0.5 mg) fenoterol added to 2 mL of 0.9 percent saline solution(group 1; n=40) or 2 mL(0.5 mg) fenoterol added to 2 mL of 3 percent saline solution(group 2; n=40). This therapy was repeated at six hours interval after admission. They were evaluated daily just before and 20 minutes after nebulization. The outcome measures included changes in clinical severity score(based on respiratory rate, presence of wheezing, retraction, and general condition) after nebulization and duration of hospitalization. Results : In the clinical severity score, a significant improvement was observed during the 72 hours of hospitalization in both groups(P<0.05). The basic clinical severity scores before inhalation were decreased significantly faster in group 2 as compared to group 1 on each day of treatment(P<0.05). The mean duration of hospital stay was significantly reduced in group 2 than group 1($5.9{\pm}1.9days$ versus $7.4{\pm}2.0days$, P<0.05). No adverse effects were associated with inhaled therapy. Conclusion : These results suggest that a nebulized 3 percent saline solution plus 0.5 mg fenoterol may be more effective than a 0.9 percent saline solution plus 0.5 mg fenoterol in accelerating the clinical recovery of infants with viral bronchiolitis.

Role of Matrix Metalloproteinase in the Pathogenesis of Bronchial Asthma (기관지 천식의 병인에서 Matrix Metalloproteinase의 역할)

  • Background : Toluene diisocyanate(TDI) is a leading cause of occupational asthma. However, the pathogenesis of TDI-induced asthma is largely unknown because there is no suitable animal model. Materials and Methods : We developed a murine model of TDI-induced asthma by performing two sensitization with 3% TDI and one challenge with 1% TDI using ultrasonic nebulization. Results : Similar to occupational asthma in humans, murine TDI-induced asthma includes findings 1) increased inflammatory cells, including neutrophils and eosinophils, 2) histologic changes, including infiltration of inflammatory cells around bronchioles, thickened airway epithelium, contraction of bronchioles, and accumulation of mucus and debris in the bronchioles, 3) increased MMP-9 activity in inflammatory cells in the airway lumen, 4) airway hyperrespnosiveness. Administraion of an MMP inhibitor, MMPI-I, remarkably reduced all these pathophysiological findings. Conclusion : Therefore, we conclude that TDI-induced occupational asthma is associated with the induction of MMP-9 in inflammatory cells, and the inhibition of MMP-9 may be a good therapeutic strategy.

Comparison of the Characteristics of 16 Commercial Nebulizer/Compressor Combinations Used in Korea (국내 시판되는 16가지 연무기/압축기의 성능 평가)

  • Purpose : We assessed the dynamic characteristics of 16 nebulizer/compressor combinations currently available in Korea. Methods : The 16 nebulizer/compressor combinations(Pariboy Type 38/Long life, Pariboy Type N/Long life, Pariboy Type N/Salter 8900, Pariboy Type N/LC, Devilbiss pulmoaid-LT/Hudson, Devilbiss pulmoaid/Hudson, Mesmed neb-300/Own, San-up 3040/Hudson, Midas(Basic)/Own, AirJolie 2/Hudson, Thomas 1127/Salter 8900, Noel NE-2000/Salter 8900, Omron CX3/Hudson, Chang Woo CWN-100/Salter 8900, Voyage/Mefar, Chang Woo ASI-Pro/Medel jet pulse) were evaluated in terms of particle size and mass output. In addition, we determined the effects of nebulizer fill volume on mass output. Results : Pariboy Type N/Long life has the highest respirable mass of 0.184 mg/min and Mesmed Neb-300/Own has the lowest 0.019 mg/min. Pariboy Type N/Long life has the highest mass output of 0.68 mg/min and the shortest mass median aerodynamic diameter(MMAD) of $3.76{\mu}m$. All combinations other than Pariboy Type N/Long life produced a MMAD of over $5{\mu}m$. MMAD over a 5 min nebulization ranged 3.76 to $9.83{\mu}m$. There were no significant effects of fill volume on mass output. Conclusion : We concluded that there is a wide variation in performance of nebulizer/compressor combinations. The characteristics of nebulizer/compressor combinations should be considered in selecting products.
