Zinc Increases Chemotactic Activity of Porcine Peripheral Blood Polymorphonuclear Cells

  • Zinc is necessary for normal functions in the immune system. The objective of the study is to examine the effect of zinc on the chemotactic activity of porcine peripheral blood polymorphonuclear cells (PMNs). A modified Boyden chamber was used to determine the directional migration distance of PMNs. Various concentrations of zinc showed no chemotactic activity to PMNs. However, culture supernatant from peripheral blood mononuclear cells (PBMCs) treated with zinc remarkably increased the chemotactic activity of PMNs when compared with culture supernatant from PBMCs treated without zinc. Culture supernatant from PBMCs treated without zinc also increased the migration distance of PMNs relative to vehicle control (medium alone). Increasing effect in chemotactic activity of PMNs by culture supernatant from PBMCs treated with zinc was inhibited by treatment of porcine anti-interleukin (IL)-8 polyclonal antibody (pAb). This effect was not affected by heat treatment ($4-85^{\circ}C$). This corresponded with heat stable physical characteristics of IL-8. These results suggest that zinc can upregulate the chemotaxis of PMNs, which is primary mediated by IL-8 chemotactic factor released from PBMCs treated with zinc.

A Case of Bilateral Warthin's Tumor (이하선에 발생한 양측성 Warthin씨 종양 치험례)

  • Warthin's tumor is a benign neoplasm that most frequently occurs in the parotid gland. Warthin's tumor shows a tendency of bilateral occurrence, but most of bilateral tumors occur at different time. It usually affects older man and grows slowly. Surgical excision is appropriate treatment for patients with Warthin's tumor and rarely recurs after removal. We report a rare case of bilateral Warthin's tumor which detected at the same time. A 54-year-old man had painless, slowly growing masses on the both mandibular angles. The masses were detected ten years ago. CT scan, ultrasonographic evaluation and fine needle biopsy was performed for preoperative diagnostic study. Superficial parotidectomy was done one after the other at ten-day intervals. After excision, histological examination revealed a mixture of ductal epithelium and a lymphoid stroma. The overlying surface of lymphoid tissue was covered with epithelial cells that was abundant of eosinophilic cytoplasm. Both masses were diagnosed as Warthin's tumor. Postoperatively, the patient did well and had no evidence of recurrence of tumor for 2 years follow-up period. Authors experienced a rare case of bilateral Warthin's tumor which detected at the same time. We recommend stepwise superficial parotidectomy at one or two-week intervals as treatment of bilateral Warthin's tumor.

Evaluation of Peritoneo-venous Shunt Patency with $^{99m}Tc-phytate$ ($^{99m}Tc-phytate$를 이용한 LeVeen 복막대정맥문합폐쇄의 진단)

  • A simple imaging procedure has been devised for patients with peritoneo-venous shunts when ascites reaccumulates and a decision must be made on whether or not to revise the shunt. We recently experienced a patient with reaccumulated ascites in whom obstruction of peritoneo-venous shunt was suspected. 5 mCi of $^{99m}Tc-phytate$ was injected into the peritoneal cavity and imaging of the abdomen was performed $1\sim30$ minutes later. With a proper funtioning shunt, radioactivity in the liver and spleen were easily identifiable in this case. If the shunt is obstructed, tracer activity will remain in the peritoneal cavity and thus can not be identifiable in the liver or spleen. Conclusively, radionuclide methods might be very useful for evaluation of peritoneo-venous shunt patency.

  • The mandibular contour determines the shape of the lower part of the face and thus influences the appearance of the face. A patient with a large, squarish, or broad face who desires a small, round, or slender face can undergo mandibular contouring surgery to reduce the width of the lower face. The successful correction of a prominent mandibular angle by conventional angle ostectomy has been reported. But, in the majority of patients with a widened facial appearance, both the mandibular angle and part of the mandibular body anterior to it are protuberant laterally, so both must be resected. The purpose of this study is to introduce a new method of performing mandibular contouring surgery, more effectively and easily, and to reduce postoperative complication and evaluate its results. We treated 6 patients who has prominent mandibular angle using multiple step osteotomy with angle-splitting ostectomy. The advantages of this new method are as following. (1) easily performable (2) effective mandibular contouring surgery by reducing the width of lower face (3) producing a natural relief of the mandibular angle (4) low risk of soft tissue damage and complications (5) shortening of the operation time. etc.

The Effect of Parity and Calving Seasons on the Reproductive Performance of Korean Native Cows

  • The purpose of this study was to determine the effect of cows parity and calving seasons on the subsequent reproductive performance of the herd of Korean native cows raised under the same condition. With the parity of the cows ranged 1 to 4 (mean: 1.9), significant associations were found between parity and calving interval (p<0.05). Calving interval of the primiparous cows group was 395.0 ${\pm}$ 16.5 days, which was the longest calving interval among the four groups. On the other hand, calving interval of the second parity group was 333.7 ${\pm}$ 3.6 days. The primiparous cows had tendencies that long interval from calving to conception and small number of service per conception relatively when compared with the multiparous cows. In the case of calving season, the interval from calving to first service was short in summer and winter relatively. The interval from calving to conception in summer was the shortest in four seasons. The number of service per conception was larger in spring and winter and smaller in summer and autumn. Calving in spring showed delayed reproductive performance and calving in summer showed desirable reproductive performance.

A Case Study of a Serious Antibiotics-associated diarrhea Patient Treated with Modified Hyangsayukgunja-tang (중증 항생제 유발 설사 환자에 대한 향사육군자탕(香砂六君子湯) 가감방(加減方) 투여 1예)

  • Objectives : The purpose of this study is to report a therapeutic effects of a Korean herbal prescription for antibiotics-associated diarrhea. Methods : Modified Hyangsayukgunja-tang was prescribed once or twice a day to a antibiotics-associated diarrhea patient. To observe the therapeutic effects, the frequency of defecation was counted and Bristol Stool Form Index was used. Laboratory tests were conducted periodically. Results : The frequency of defecation was decreased from 9 times a day to once a day. According to Bristol Stool Form Index, stool form was improved from Type 7 to Type 5. The level of red blood cell, hemoglobin and albumin increased slightly from the day of admission to discharge. Conclusions : Hyangsayukgunja-tang can be a choosable treatment option for a serious antibiotics-associated diarrhea.

The Clinical Study on the Effect of Positional Release Therapy on Patients of Neck Pain (경항통 환자의 치료시 자세이완치료 효과에 대한 임상적 연구)

  • Objectives: The purpose of this study is to evaluate the therapeutic effect of Positional Release Therapy on patients of neck pain, Methods: 12 patients was treated by Positional Release Therapy(PRT) combined with acupuncture, We measured the Visual Analog Scale(VAS) and Neck Disability Index(NDI) before and after treatment The statistical analysis was performed by using the Wilcoxon signed rank test. Results: 1. Patients showed significant improvement in VAS after treatrnent(p<0.05). 2. Patients showed significant improvement in NDI after treatment(p<0.05). Conclusion: These results imply that Positional Release Therapy combined with acupuncture would be effective on relieving pain and increasing functional ability in daily-life of patients of neck pain.

A Clinical Study on Effectiveness of Acupuncture Treatment of Acute Ankle Sprain Combined with Positional Release Therapy (자세이완치료를 병용한 침구치료가 급성기 족관절 염좌의 치료에 미치는 영향에 대한 임상적 연구)

  • Objectives: The purpose of this study is to evaluate the therapeutic effect of acupuncture treatment of acute ankle sprain combined with positional release therapy. Methods: A prospective randomized single blind study between positional release group and knee flexed supine position group was conducted. Patients with ankle sprain within 48 hours were evaluated by Ankle injury grade chart(AIGC). In group A, positional release therapy was combined with acupuncture, whereas in group B, acupuncture was conducted in knee flexed supine position. The treatment was planned for a duration of 1 week, 3times a week. In AIGC scores, VAS, weight bearing time and weight bearing time in blind were followed up and compared. Results and Conclusion: The VAS score decreased in both group. Weight bearing time increased in group B, weight bearing time in blind increased in group A. Comparing the therapeutic effect of each group, group A had significant effectiveness in weight bearing time in blind. So we may conclude that Acupuncture treatment combined with positional release therapy is effective and recommandable at early stage of ankle sprain.

  • Deep neck infections mean infection in the potential spaces and facial planes of the neck, either abscess formation or cellulitis. Deep neck infections are caused by dental, salivary gland, pharyngeal and tonsillar infections. Sometimes, deep neck infection may be caused by tuberculosis in case of immunodefiecient patients. Acquired immunodeficiency syndrome(AIDS) is a disease associated with defective cell-mediated immunity after infected with human immunodeficiency virus(HIV). The chance of opportunistic infection in patients of AIDS increases as the level of immunodeficienty progresses. Human immunodeficiency virus infection is the most single significant risk factor for progression of pulmonary tuberculosis to extrapulmonary sites. In patients infected with HIV, the rate of extrapulomonary tuberculosis rises upto $60\%$. We report a case of a 47 year old male patient with AIDS associated with deep neck infection by tuberculosis.

Hematogenous Osteomyelitis Following Open Fracture in a Cat

  • A 6-month-old, weighing 2.8 kg, female, domestic short-haired cat presented with open fracture at right distal radius about a month ago. Based on radiological findings, hyperplasia at the right radioulna and left humerus was found. Results of the cytological examination were inflammatory reaction and reactive osteoblast. Distal radial ostectomy proceeded with necrotic bone debridement. Three weeks after operation, the radial bone lysis was seen on radiograph but clinical condition improved. Hyperplasia at the right radioulna and left humerus was dissolved. Two months after operation, she can use her both forelimbs despite right elbow have been diagnosed as arthrosclerosis because of periosteal reaction. Six months after operation, clinical symptoms of limbs were not detected. In this case, open fracture was treated through delayed surgical debridement procedure, therefore hematogenous osteomyelitis occurs at another forelimb. The longer a wound remains open, the more likely it is that infection will develop. The infection can occur to fracture bone and seed from hematogenous spread to another normal bone. In conclusion, when open fracture occurs, early antibiotic treatment and urgent surgical intervention are recommended.
