악교정 수술을 위한 한국 성인 정상교합자의 연조직 기준치 (CEPHALOMETRIC NORMS OF THE SOFT TISSUES OF KOREAN FOR ORTHOGNATHIC SURGERY)

  • This study was performed to evaluate soft tissue cephalometric norms for Korean adults which can be implemented in surgical orthodontic treatment planning using selected horizontal reference plane especially for Koreans (Male: $SN-7.5^{\circ}$, Female: $SN-9.0^{\circ}$) and a simplified analytical method. 70 males and 70 females consisting of freshmen of Yonsei University from 1996 to 1997 and students from the Dental College of Yonsei University were chosen according to clinical examination and cephalometric analysis. The samples had normal profiles, normal anteroposterior skeletal relationship(ANB angle of $0^{\circ}\;to\;4^{\circ}$ and Wits appraisal of -4.0mm to 0mm), and Class I molar and canine relationship. They had no missing or supernumerary teeth and had no experience of orthodontic or prosthetic treatment. After the selection of 15 soft tissue landmarks and the construction of horizontal and vertical reference lines, 25 measurements were taken. These consisted of vertical and horizontal linear measurements and angular measurements. The results were as follows. 1. Mean and standard deviation of the measurements were calculated in males and females. 2. Vertical measurements were comparably bigger in males than females whereas anterior facial height ratio(sN-Sn/Sn-sMe) and lower anterior facial height ratio(Sn-Stms/Stmi-sMe) showed no significant difference between sexes. 3. Most of the horizontal measurements in relation to the vertical reference line(G-perpendicular) showed no significant difference between sexes. 4. Nasofacial angle, columellar angle, nasolabial angle and facial contour angle showed no significant difference between sexes. 5. The upper and lower lip were positioned about $-1.0{\pm}2.0mm$ and $+1.0{\pm}2.0mm$ in relation to the Ricketts' esthetic line in both sexes. In this study, soft tissue cephalometric norms of Korean adults for orthognathic surgery were obtained.

  • Purpose: The aim of this study is to establish anatomical dimension of the nose and nostril shape in Korean twenties. Methods: Authors measured anthropometry of nose in Korean youths 1,000 individuals (male 363, female 637) and analyzed stastically them. Results: The mean measurements are as follows. 1. The shape of nostrils was classified into 7 types by the angle between both long axis of nostril (male / female). Type I ($<40^{\circ}$)1.7% / 1.72%, Type II ($41-70^{\circ}$) 26.17% / 29.35%, Type III ($71-100^{\circ}$) 39.94% / 38.77%, Type IV (unclassified) 3.85% / 5.80%, Type V ($101-130^{\circ}$) 20.93% / 17.89%, Type VI ($131-180^{\circ}$) 7.43% / 5.96%, Type VII ($>180^{\circ}$) 0% / 0.47% 2. 제 III급 부정교합 환자들의 각 치료법에 따른 측모두부방사선사진 계측치의 비교 (Cephalometric difference according to the differential treatment methods in Class III malocclusion;)

  • 제 III급 부정교합 환자의 치료를 위해서는 각 환자의 문제점들과 상태를 다양한 진단방법들을 동원하여 분석한 후에 가장 적절한 치료방법을 선택하여야 한다. 초진시에 치성 또는 기능성 또는 골격성인 원인들의 감별이 분명한 증례에서는 치료계획 수립이 큰 문제가 없지만, 감별이 모호한 증례에서는 많은 어려움을 경험하게 된다. 이에 저자는 각 치료방법을 적용하여 치료한 제III급 부정교합 환자 482명 (남;253,여 ;229)을 대상으로 치료방법에 따라 네 군으로 분류하였다. D군은 가철성 교정장치나 악기능장치를 사용하여 치료한 115명(남;64, 여;51), A군은 악정형장치로 치료한 210명 (남;111, 여 ;99), B군은 고정식 교정장치로 Camouflage 하여 치료한 63명 (남;30, 여 ;33), C군은 악교정 수술로 치료한 94명(남;48, 여;46)이었다. 초진시에 촬영한 측모두부방사선사진에서 전후방, 수직적, 치성, 악골의 형태학적인 계측항목들을 선정하여 각 군에서 측정한 계측치의 평균값 및 표준편차를 구하였고 네 군간의 유의성 여부를 검정하여 아래와 같은 결과를 얻었다. 1. SNA각과 Nasion Perpendicular Plane에서 A점까지의 거리는 네 군간에 유의한 차이가 없었으며, SNB, Wits, Nasion Perpendicular Plane에서 B점까지의 거리, Facial angle, Facial convexity, APDI의 값은 C,A,B,D군의 순이었지만 A군과 B군간에는 유의한 차이가 없었다. 2. 전안면고경, 후안면고경, 하전안면고경의 값이 남자에서는 C,B,A,D의 순으로 유의성 있는 차이를 나타냈으며, 여자에서는 C와 B군간에는 유의한 차이가 없게 나타났다. 3. 남자에서 전두개저에 대한 하악체 길이의 비율, 하악체의 길이와 Symphysis의 길이는 C,B,A,D순으로 나타났으나 B군과 A군사이에는 유의한 차이가 없었고, 여자에서는 Ramus height, 하악체 길이, 전두개저에 대한 하악체 길이의 비율이 네군에서 유의한 차이를 나타냈고 Symphysis의 길이는 C군과 B군 사이에서는 유의한 차이가 없었다. 4. 상악전치 치축은 C,B,A,D순으로 크게 나타났으며 네 군간에서 유의한 차이가 나타났다. 5. Nasolabial angle의 값은 C,A,B,D순으로 작게 나타났으나 유의한 차이는 C와 A,B,D군 간에서만 나타났다.

한국 청년층 구순의 사진 계측학적 연구 (Photogrammetric Study of Lip in Young Population in Korean)

  • The aim of this study is to establish anatomical dimension of the lip in young population in Korean, using specially designed soft ware with photographic image. We measure 13 anatomical dimensions of lips in 2,229 young people. (917 male, 1312 female, Aged from 18-33 years. Average age 19.7). Statistical analysis of these measurements of large population could offer useful information in facial plastic surgery. The mean measurements are as follows 1. Lengths (male/female) Widths of philtrum: $1.11{\pm}0.19cm/1.02{\pm}0.21cm$ Heights of philtrum: $1.6{\pm}0.24cm/1.47{\pm}0.21cm$ Heights of cupid bow: $0.88{\pm}0.16cm/0.83{\pm}0.16cm$ Height of upper vermilion: $0.74{\pm}0.16cm/0.70{\pm}0.15cm$ Height of lower vermilion: $1.08{\pm}0.17cm/1.02{\pm}0.15cm$ Height of upper lip(Rt.): $1.24{\pm}0.2cm/1.23{\pm}0.2cm$ Height of upper lip(Lt.): $1.24{\pm}0.2cm/1.17{\pm}0.19cm$ Half horizontal length of lip: $2.2{\pm}0.26cm/2.11{\pm}0.2cm$ Horizontal length of lip: $4.41{\pm}0.4cm/4.25{\pm}0.36cm$ Height of lower face: $7.1{\pm}0.58cm/6.52{\pm}0.6cm$ 2. Angles Nasolabial angle: $97.77{\pm}11.97^{\circ}/95.5{\pm}11.34^{\circ}$ Mentolabial angle: $133.88{\pm}14.65^{\circ}/129.27{\pm}13.67^{\circ}$ Angle of Cupid's bow: $111.65{\pm}13.99^{\circ}/116.75{\pm}16.2^{\circ}$ Previous reported photogrammetric measurements was difficult to implement to surgical practice. Because these were printed photographies of the same size. Therefore, in this study, we can measure a lot of objects and items more conveniently and correctly by using proportional program on computer after taking a digital photograph. Consequently, proportional measurements with photogrammetry of lip could be useful and corrective substitute for anthropometrical measuring. These data could be useful reference for preoperative consultation, surgical planning and learning anatomical measurement of lips and adjacent structures.

Influence of heritability on craniofacial soft tissue characteristics of monozygotic twins, dizygotic twins, and their siblings using Falconer's method and principal components analysis

  • Objective: The purpose of this study was to investigate the influence of heritability on the craniofacial soft tissue cephalometric characteristics of monozygotic (MZ) twins, dizygotic (DZ) twins, and their siblings (SIB). Methods: The samples comprised Korean adult twins and their siblings (mean age, 39.8 years; MZ group, n = 36 pairs; DZ group, n = 13 pairs of the same gender; and SIB group, n = 26 pairs of the same gender). Thirty cephalometric variables were measured to characterize facial profile, facial height, soft-tissue thickness, and projection of nose and lip. Falconer's method was used to calculate heritability (low heritability, $h^2$ < 0.2; high heritability, $h^2$ > 0.9). After principal components analysis (PCA) was performed to extract the models, we calculated the intraclass correlation coefficient (ICC) value and heritability of each component. Results: The MZ group exhibited higher ICC values for all cephalometric variables than DZ and SIB groups. Among cephalometric variables, the highest ${h^2}_{(MZ-DZ)}$ and ${h^2}_{(MZ-SIB)}$ values were observed for the nasolabial angle (NLA, 1.544 and 2.036), chin angle (1.342 and 1.112), soft tissue chin thickness (2.872 and 1.226), and upper lip thickness ratio (1.592 and 1.026). PCA derived eight components with 84.5% of a cumulative explanation. The components that exhibited higher values of ${h^2}_{(MZ-DZ)}$ and ${h^2}_{(MZ-SIB)}$ were PCA2, which includes facial convexity, NLA, and nose projection (1.026 and 0.972), and PCA7, which includes chin angle and soft tissue chin thickness (2.107 and 1.169). Conclusions: The nose and soft tissue chin were more influenced by genetic factors than other soft tissues.

두부방사선 분석에 의한 상악전치부 후방이동시 연조직 변화 예측에 대한 연구 (SOFT TISSUE PROFILE CHANGE PREDICTION IN MAXILLARY INCISOR RETRACTION BASED ON CEPHALOMETRICS)

  • 본 연구는 성장에 의한 효과를 배제한 교정치료시의 연조직 변화와 치아 및 악골의 수평적, 수직적 움직임과의 상관 관계를 알아보고 예측 가능성을 검토하기 위해 Angle씨 2급 1류 부정 교합 환자 41명을 대상으로 치아 및 악골, 연조직의 수평, 수직이동의 평균 변화량을 측정하고 연조직에 대한 계단식 다중 회귀분석을 시행하였으며 다음과 같은 결론을 얻었다. 1. 경조직 계측항목 중 상악 중절치의 수평이동 및 기울기, 하악 중절치의 수평 및 수직이동, 수평피개량, 수직피개량, interincisal angle, 하악골 위치, 변화, A, B, 경조직 측모 돌출각 등의 항목에서, 연조직 계측항목 중 상순의 두께 및 후방이동, 기울기, 하순의 후방이동 및 기울기, interlabial angle, 비순각, 이순각, Sn, Ss, Si, 연조직 측모 돌출각등의 항목에서 치료 전과 후에 유의성 있게 변하였다(P<0.05). 2. 치료 전 후 유의한 변화를 보인 연조직 계측항목을 분석한 결과 모든 항목에서 경조직의 변화에 대해 영향을 받는 것으로 나타났다(P<0.05). 3. 연조직 회귀방정식에 중절치 수평피개량, 상악 전치 절단연의 후방이동량, 상악 전치 치근의 후방이동량, Pg의 수평변화량이 가장 높은 빈도로 포함됨으로써 이들은 연조직의 수직 및 수평변화에 영향을 많이 미치는 요소로 나타났으며 하악 전치의 수직이동량은 포함되지 않음으로써 연조직 변화와 관련성이 적은 것으로 나타났다. 4. Sn의 수직 및 수평변화는 경조직의 가장 많은 항목과 연관됨으로써 (A, B, Pg(H), overjet, upper incisor angle, Ur, Is(H)) 여러 경조직 변화의 복합적인 영향을 받는 것으로 나타났다. 5. 연조직 계측항목에 대한 상관계수($R^2$)의 범위는 0.2-0.6으로 중등도의 상관정도를 나타내었다.

상악전방분절절골술과 턱끝전진술 후 안면골격과 연부조직의 변화 (The Changes of Bone and Soft Tissue after Maxillary Anterior Segmental Osteotomy and Advancement Genioplasty)

  • Purpose: Mid and lower facial convexity is more common in Oriental people than in Caucasian. Bimaxillary dentoalveolar protrusion is characterized by procumbent teeth, protruding lips, acute nasolabial angle, gummy smile, receding chin, facial convexity. Especially, pure maxillary dentoalveolar protrusion is less frequent than bimaxillary dentoalveolar protrusion. Therefore, it is important to make an accurate decision for the operation throughout the history taking, cephalogram, dental cast to arrive at accurate diagnosis and surgical plan. Methods: From December 2002 to June 2004, ten patients with maxillary dentoalveolar protrusion and microgenia were corrected by maxillary anterior segmental osteotomy and advancement genioplasty. 10 patients were analyzed by preoperative and postoperative clinical photography, posteroanterior and lateral cephalograms. Results: No major complications were occurred throughout the follow-up period except one of the over-recessed, otherwise most of the patients were satisfied with the result. Conclusion: We could correct the occulusal relationship with teeth and improve lower facial profile, asthetically and functionally, by maxillary anterior segmental osteotomy and advancement genioplasty.

구순열비변형의 이차 교정에 있어서 Bardach 술식의 유용성 (Usefulness of Bardach's Technique for Secondary Correction of Cleft Lip Nasal Deformity)

  • Purpose: The secondary correction of cleft lip nasal deformity still presents a difficult surgical problems. The present study was aimed to investigate the usefulness of Bardach's technique for secondary correction of cleft lip nasal deformity. Materials and Methods: The subjects were eight patients with unilateral and bilateral cleft lip nasal deformity, who had secondary correction by using Bardach's rhinoplasty technique. Age range was from 2 to 21 years and mean age was 10.6 years. There were 3 boys and 5 girls. Six patients had bilateral and two patients had unilateral cleft lip. Facial photographs were taken before and twenty days after the operation. By using Adobe photoshop, the columella height and the nostril width were measured from the facial frontal photograph and Worm's eye view. The degree of improvement was calculated and statistically analyzed. Results: The degree of improvement of the columella length and the nostril width after Bardach's technique was $70.39{\pm}50.14%$ and $-22.93{\pm}0.15%$ respectively. Bardach's technique resulted in projecting the nasal tip, lengthening the columella, medially advancing the alar bases, restructuring the lower lateral cartilages, and changing orientation of the nostrils from horizontal to oblique. The profile view shows projection of the nasal tip, lengthening of the columella, and the change in the nasolabial angle. The scars remained at the philtrum were matter little in compared with improvement of the nasal appearance. Conclusion: These results indicate that Bardach's technique is an useful surgical technique for secondary correction of cleft lip nasal deformity.

A three-dimensional photogrammetric analysis of the facial esthetics of the Miss Korea pageant contestants

  • Objective: The aims of this study were to measure and compare the facial dimensions of the Miss Korea pageant contestants and a selected group of women from the general population by using three-dimensional (3D) image analysis, as well as to compare various facial ratios to the golden ratio within each group. Methods: Three-dimensional images of 52 Miss Korea pageant contestants (MK group) and 41 young female adults selected from the general population (GP group) were acquired. Fifty-four variables and ratios were measured and calculated. Intergroup comparisons were performed using multivariate analysis of variance. Results: Compared to the GP group, the MK group showed greater total facial height and eye width, lesser lower-facial height, and lesser facial, lower-facial, and nasal widths. Moreover, compared to the GP group, the MK group had more protruded noses with greater nasolabial angle, greater vertical curvature of the foreheads, lesser horizontal curvature of the cheek, and lesser lower-lip-and-chin volume. Conclusions: The MK group had longer faces but smaller lower lips and chins than did the GP group. The golden ratio was not matching the current facial esthetic standards. These data might be beneficial for treatment planning of patients undergoing orthognathic and plastic surgeries.

다양한 방법을 이용한 이차성 구순열 비변형의 비익기저 증대술 (Alar Base Augmentation by Various Methods in Secondary Lip Nasal Deformity)

  • The definitive correction of secondary lip nasal deformities is a great challenge for plastic surgeons. To rectify the secondary lip nasal deformities, various procedures and its modifications have been reported in many centers. However, no universal agreement exist to correct the various components of secondary nasal deformities. The secondary nasal deformity of the unilateral cleft lip has its own characteristic abnormalities including the retroplaced dome of the ipsilateral nasal tip, hooding of the alar rim, a secondary alar-columellar web, short columella, depressed alar base and so forth. Among these components of secondary nasal deformity, maxillary hypoplasia, especially in the area of piriform aperture, and alveolar bone defect can make the alar base depressed, which in turn, leads to wide and flat nasal profile, obtuse nasolabial angle coupled with subnormal nasal tip projection in aspect of aesthetic consideration. Moreover, the maxillary hypoplasia contributes to reduced size of the nasal airway in combination with other component of external nasal deformity and therefore the nasal obstruction may be developed functionally. Therefore, the current authors have performed corrective rhinoplasty with the augmentation of alar base with various methods which include rearrangement of soft tissue, vertical scar tissue flap and use of allogenic or autologous materials in 42 patients between 1998 and 2003. The symmetric alar base could be achieved, which provides the more accurate evaluation and more appropriate management of the various component of any coexisting secondary nasal deformity. In conclusion, the augmentation of alar base, as a single procedure, is a basic and essential to correct the secondary lip nasal deformities.
