두가지 두부 방사선 사진 중첩 방법의 비교 - Nasion-Sella line 중첩과 Anterior cranial base 중첩 (Comparision of two cephalometric superimposition methods - Nasion-sella line method and Anterior cranial base method)

  • 성장 또는 치료로 인한 악안면골의 변화는 두부 방사선을 중첩 함으로써 판단할 수 있다. 중첩에 대한 평가는 중첩방법에 따라 변화되므로 좀더 정확한 중첩 방법을 선택하는 것은 치료 효과에 대한 분석이나 성장 변화에 대한 평가를 정확히하는 선결 요건이 될것이다. 3년 정도의 비교적 짧은 성장기간을 가진 11세 전후의 여자 환자 19명을 대상으로, 가장 널리쓰이는 중첩 방법인 Nasion-Sells line중첩과 Bjork가 주장하는 Anteirior cranial base (ACB)중첩을 비교하여 다음과 같은 결론을 얻었다. 1 Nasion-Sells line중첩과 ACB중첩은 중첩오차(registration error)의 크기가 거의 같아서 reliability에 있어서 큰차이가 없는 것으로 나타났다 2. 검사자간 차이(interobserver difference)에 있어서 Nasion-Sells line중첩은 7개의 측정 항목 모두에서 두 검사자간에 차이가 없었으나 ACB중첩은 7개의 측정 항목중 4개의 측정 항목에서 두 검사자간 유의성 있는 차이가 나타났다. 3. 중첩방법의 차이 (intermethod difference)에 있어서 Nasion-Sella line중첩으로 측정한 계 측치와 ACB 중첩으로 측정한 계측치가 유의성있게 차이가 나지 않았다.

Anchor Plate Efficiency in Postoperative Orthodontic Treatment Following Orthognathic Surgery via Minimal Presurgical Orthodontic Treatment

  • Purpose: The efficiency of an anchor plate placed during orthognathic surgery via minimal presurgical orthodontic treatment was evaluated by analyzing the mandibular relapse rate and dental changes. Methods: The subjects included nine patients with Class III malocclusion who had bilateral sagittal split osteotomy at the Division of Oral and Maxillofacial Surgery, Department of Dentistry in Ajou University Hospital, after minimal presurgical orthodontic treatment. During orthognathic surgery, anchor plates were placed at both maxillary buttresses. The anchor plates were used to move maxillary teeth backward and for maximum anchorage of Class III elastics to minimize mandibular relapse during the postoperative orthodontic treatment. The lateral cephalometric X-ray was taken preoperatively (T0), postoperatively (T1), and one year after the surgery (T2). Seven measurements (distance from Pogonion to line Nasion-Nasion perpendicular [Pog-N Per.], angle of line B point-Nasion and Nasion-Sella [SNB], angle of line maxilla 1 root-maxilla 1 crown and Nasion-Sella [U1 to SN], distance from maxilla 1 crown to line A point-Nasion [U1 to NA], overbite, overjet, and interincisal angle) were taken. Measurements at T0 to T1 and T1 to T2 were compared and differences tested by standard statistical methods. Results: The mean skeletal change was posterior movement by $13.87{\pm}4.95mm$ based on pogonion from T0 to T1, and anterior movement by $1.54{\pm}2.18mm$ from T1 to T2, showing relapse of about 10.2%. There were significant changes from T0 to T1 for both Pog-N Per. and SNB (P<0.05). However, there were no statistically significant changes from T1 to T2 for both Pog-N Per. and SNB. U1 to NA that represents the anterior-posterior changes of maxillary incisor did not differ from T0 to T1, yet there was a significant change from T1 to T2 (P<0.05). Conclusion: This study found that the anchor plate minimizes mandibular relapse and moves the maxillary teeth backward during the postoperative orthodontic treatment. Thus, we conclude that the anchor plate is clinically very useful.

  • The author measured and analyzed the condyle path inclinations of 25 normal Korean adults roentgeno-cephalometrically and following conclusions were obtained. In protrusive movement of mandible, the angle of condyle pation inclination in relation to 1) occlusal plane is 19.86±8.25, 2) Frankfort Horizontal plane is 28.32±8.56, 3) Camper line is 12.22±4.06, 4) Nasion-sella line is 36.80±8.54.

Mandibular shape prediction using cephalometric analysis: applications in craniofacial analysis, forensic anthropology and archaeological reconstruction

  • Background: The human mandible is variable in shape, size and position and any deviation from normal can affect the facial appearance and dental occlusion. Objectives: The objectives of this study were to determine whether the Sassouni cephalometric analysis could help predict two-dimensional mandibular shape in humans using cephalometric planes and landmarks. Materials and methods: A retrospective computerised analysis of 100 lateral cephalometric radiographs taken at Kingston Hospital Orthodontic Department was carried out. Results: Results showed that the Euclidean straight-line mean difference between the estimated position of gonion and traced position of gonion was 7.89 mm and the Euclidean straight-line mean difference between the estimated position of pogonion and the traced position of pogonion was 11.15 mm. The length of the anterior cranial base as measured by sella-nasion was positively correlated with the length of the mandibular body gonion-menton, r = 0.381 and regression analysis showed the length of the anterior cranial base sella-nasion could be predictive of the length of the mandibular body gonion-menton by the equation 22.65 + 0.5426x, where x = length of the anterior cranial base (SN). There was a significant association with convex shaped palates and oblique shaped mandibles, p = 0.0004. Conclusions: The method described in this study can be used to help estimate the position of cephalometric points gonion and pogonion and thereby sagittal mandibular length. This method is more accurate in skeletal class I cases and therefore has potential applications in craniofacial anthropology and the 'missing mandible' problem in forensic and archaeological reconstruction.

자연 두부 위치 및 안정위에서 한국 성인 환자 골격의 전.후 관계 결정 (Assessment of antero-posterior skeletal relationships in adult Korean patients in the natural head position and centric relation)

  • SN 기준선과 FH 기준선이 안모의 심미에 종종 맞지 않아서 모든 치열 교정 및 악교정 수술 증례를 이 기준선을 이용하여 반복적으로 정확히 진단하기가 매우 어렵다. 따라서, 이 연구의 목적은 한국 성인 환자를 대상으로 안정위에서 자연 두부 위치의 개인내 재현성을 확인하고 진성 수평선에 대해 SN 기준선과 FH 기준선의 개인간 다양성을 증명하는 것이다. 그러고, 상기 두 연구를 바탕으로 Na 기준 진수선에서 A점 및 B점까지의 거리가 SNA 및 SNB 각도와 상관 관계가 있는지 여부를 조사하는 것이다. 부정교합 및 악골의 부조화를 치료받기 위해 강남성심병원과 한강성심병원 교정과에 내원한 116 명의 환자(남자 23명, 여자 93명)를 조사하였다. 중심위 왁스 바이트는 모든 대상들로부터 채득하였고 1주일 간격으로 자연 두부 위치와 중심위 상태에서 모든 대상자들의 측모두부방사선사진을 2번 촬영하였다. 3개의 변수에서 위치 선정의 방법 오류는 다른 연구에 비해 낮은 수치를 보였다. 그리고 6개의 변수에서 군내 상관계수를 이용하여 자연두부 위치의 개인간 재현성을 증명하였다 (p < 0.001). 각 환자에서 진성 수평선에 대한 FH 기준선간의 각도는 통계적으로 기준치 0도와 다르지 않았다 (p > 0.05). 하지만 임상적으로 다양한 수치를 보였다 (SD: $3.89^{\circ}$). 반면 각 에서 진성 수평선에 대한 SN 기준선간의 각도는 기준치 7도와 매우 다르게 나타났다 (p < 0.05). Na 기준 진수선에서 A점 및 B점까지의 거리와 SNA 및 SNB 각도와의 피어슨 상관 계수에서 유의하지만 매우 낮은 수치를 보였다 (p < 0.01). 이에 자연 두부 위치는 치열 교정 환자 및 악교정 수술 환자의 진단을 위해 재현적이고 믿을 만한 위치이며 자연두부 위치 및 중심위 상태에서 Na 기준 진수선은 SN과 FH기준선을 대신하여 한국인 성인 환자에서 상악쿨과 하악골의 전후 관계를 결정하는데 매우 유용한 기준선으로 이용될 수 있을 것으로 생각한다.

Predictors of favorable soft tissue profile outcomes following Class II Twin-block treatment

  • Objective: The aim of this study was to determine cephalometric factors that help predict favorable soft-tissue profile outcomes following treatment with the Class II Twin-block appliance. Methods: Pre- and post-treatment lateral cephalograms of 45 patients treated with the Class II Twin-block appliance were retrospectively analyzed. Profile silhouettes were drawn from the cephalograms and evaluated by three orthodontists in order to determine the extent of improvement. Samples were divided into a favorable group (upper 30% of visual analogue scale [VAS] scores, n = 14) and an unfavorable group (lower 30% of VAS scores, n = 14). Skeletal and soft-tissue measurements were performed on the cephalograms and an intergroup comparison was conducted. Results: An independent t-test revealed that the following pre-treatment values were lower in the favorable group compared to the unfavorable group: lower incisor to mandibular plane angle, lower incisor to pogonion distance, point A-nasion-point B angle, sella-nasion line (SN) to maxillary plane angle, SN to mandibular plane angle, gonial angle, and symphysis inclination. The favorable group had a larger incisor inclination to occlusal plane. Moreover, the favorable group showed larger post-treatment changes in gonial angle, B point projection, and pogonion projection than did the unfavorable group. Conclusions: Class II malocclusion patients with a low divergent skeletal pattern and reduced lower incisor protrusions are likely to show more improvement in soft-tissue profile outcomes following Class II Twin-block treatment.

서 있는 자세와 앉은 자세에서 두부자세의 변화 (Changes of Head Posture in Standing and Sitting Posture)

  • This study was performed to investigate the changes of head posture according to natural standing or sitting posture. Twenty seven healthy dental students without any signs and symptoms of temporomandibular disorders participated in this study. Cervical resting posture (CRP) of the head in sagittal plane was measured by Cervical-Range-of-Motion $^\textregistered$(CROM, U.S.A.) and lateral cephalograph was taken in natural posture. The items related to angle in cephalograph were the angles of cranial and cervical inclination to true vertical line(VER/NSL, VER/AML), the angles of cervical inclination to nasion-sella line(CVT/NSL, OPT/NSL), the angles of comical inclination to horizontal line(CVT/HOR, OPT/HOR), the angle of cervical lordosis(CVT/OPT). The items related to line measurement were the distance from subocciput to Cl(Dl), Cl to C2(D2), C2 to C3(D3), C3 to C4(D4), the upper(PNS to posterior pharyngeal wall) and the lower(tongue base to posterior pharyngeal wall) pharyngeal space, the distance from nation to mention(Na-Me), and the radius of comical curvature from the first comical vertebra(Cl ) to the fifth cervical vertebra(C5). The data were analyzed with SAS/STAT program. The obtained results were as follows : 1. Most items related to angular measurement showed significant difference between in standing and sitting posture. The angles of CRP, CVT/NSL, OPT/NSL, CVT/HOR, OPT/HOR, and CVT/OPT were high in sitting posture, but the angles of VER/NSL, VER/NSL were low in sitting posture. 2. In vertebral distance, only the distance between C3 and C4 was differed by the posture, which decreased in sitting posture. In sitting posture, the distance from nasion to menton(Na-Me) was longer, but the radius was shorter than in standing posture. 3. Correlationship in angular measurements was almost same in both postures. Ceervical resting posture(CRP) was correlated with VER/NSL, VER\ulcornerNSL was correlated with CRP, CVT/NSL, and OPT/NSL, VER/AML was correlated with CVT/HOR, OPT/HOR, CVT/OPT, and the angle of cervical lordosis(CVT/OPT) was correlated with the radius. 4. Correlationship in linear measurement was observed only in among D3, D4, and radius. And the Na-Me was not correlated with any other items. From this results, The author concluded that the head posture in sitting was mote backward extended than in standing.

치아 및 두개골에 대한 두부방사선 계측학적 연구 (ROENTGENOCEPHALOMETRIC STUDY ON THE TEETH AND SKULL)

  • For the purposes of augmentation of the aid for case analysis and diagnosis of malocclusion, a roentgenocephalometric study was made from 84 Korean adolescences. The Subjects consist of 42 males and 42 females aged from 17 to 20 years with normal occlusion and acceptable facial appearance. The author measured 18 angles and 14 linear distances as suggested by Jarabak. The following results were obtained. 1) Each linear measurement of the males' skull was greaten than that of the females. 2) The posterior to anterior facial height was $69.2\%$ in the males and $67.1\%$ in the females. 3) In the relationship of upper lip to esthetic line, the lip of females was more behind than that of males. 4) Saddle angle was $124.7^{\circ}$, articular angle was $148.7^{\circ}$, genial angle was $119.4^{\circ}$ and upper and lower genial angles were $45.1^{\circ}\;(N-Go-a^{\circ})$ and $74.2^{\circ}\;(N-Go-Me^{\circ})$. 5) The ratio of mandibular body to anterior cranial base was about 1:1. 1. 6) The angulations of $SNA^{\circ},\;SNB^{\circ}\;and\;SNP^{\circ}$ were as follows; $SNA^{\circ},\;80.3^{\circ},\;SNB^{\circ},\;79.8^{\circ},\;SNP^{\circ},\;81.1^{\circ}$. 7) The angle of the sella-nasion plane to the mandibular plane $(SNG^{\circ}Me^{\circ})$ was $32.0^{\circ}$ and that of the occlusal plane to the mandibular plane was $18.2^{\circ}$. 8) The angle of the maxillary central incisor to the sellanasion plane $(1-SN^{\circ})$ was $105.6^{\circ}$. That of the mandibular central incisor to the mandibular plane $(1-GoMe^{\circ})$ was $94.0^{\circ}$, and the interincisal angle $(1\;to\;1^{\circ})$ was $127.6^{\circ}$. 9) The linear distance from incisal edge of upper central incisor to facial plane was 8.0mm and that of lower central incisor was 4.6mm. 10) In the relationship of the lower lip to the esthetic line, the lower lip was 0.2mm front of the esthetic line.

  • Purpose : The lateral cephalometric Korean norms of the skeletal and pharyngeal dimension were analyzed for the diagnosis of obstructive sleep apnea syndrome(OSAS). Materials and Methods : The lateral cephalometric radiographies were taken in male(n=53) and female(n=50), who had a normal profile, class I occlusion, normal ANB $(0{\sim}4^{\circ})$ and normal Wits $Appraisal(-4{\sim}0mm)$. The X-rays were traced by two oral and maxillofacial surgeons twice. The significant differences between male-female and examiners were tested. Results : The angle of mandibular line to Nasion-Sella line was $31.12^{\circ}/33.79^{\circ}$ (in male/in female, p=0.0018). The lower Gonion angle was $73.74^{\circ}/73.74^{\circ}(p=0.9978)$. The length of the soft palate was 36.93mm/34.35mm(p=0.0002). The pharyngeal airway space was 13.42mm/11.55mm at mandibular plane level(PAS(ML))(p=0.0025). The hyoid was placed inferiorly to mandibular plane 10.18mm/7.72mm(p=0.0051). The results from this study are to be used for the diagnosis of OSAS.

한국인 유치열기 아동의 두부방사선 계측학적 연구 (ROENTGENOCEPHALOMETRIC STUDY FOR THE CHILDREN IN THE PRIMARY DENTITION OF KOREAN)

  • In order to know cephalometric norms for the preschool children, this roentgeno cephalometric study was undertaken in each 50 Korean male and female children of primary dentition age from 4 to 5 year. The following results were obtained. 1. In the skeletal analysis, there was no significant difference between male and female in angular measurement and the linear measument of the male was generally greater than that of the female. 2. Saddle angle was $122.3^{\circ}$, articular angle was $147.6^{\circ}$, gonial angle was $119.4^{\circ}$ and the sum of each angle was $396.1^{\circ}$ in male and $396.6^{\circ}$ in female. 3. The ratio of mandibular body to anterior cranial base was about 1 : 0.91. 4. In the primary dentition, suggested that the nasion and point A move forward relative to sella turcica in a fashion, pogonion and point B are equal in angular position relative to plan S-N, bony chin and chin button was yet underdeveloped, and the forward growth of mandible was seen rapid than maxilla after 4 years. 5. Suggested that the percentage of anterior facial height to the posterior facial height were 64.4% in male and 64.1% in female. 6. Maxillary primary incisors was more upright than the permanent incisors, mandibular primary incisors was inclined lingually relative to the permanent incisor, and primary incisors was more upright than the permanent incisors. 7. Maxillary primary incisors in female was inclined labially than male. 8. In the the relationship of the upper lip and lower lip to the esthetic line, the upper lip was 2.11mm and the lower lip was 2.33mm front of the esthetic line.
