마비성조음장애, 편도 비대, 비폐쇄 및 구개열 환자의 실험 임상 음성학적 연구 (An Experimental Clinical Phonetic Study on Patients of Dysarthria, Tonsilhypertrophy, Nasal Obstruction, and Cleft Palate)

  • The aim of this study is to develop an assessment program of speech rehabilitation for children having some language and speech disorders. Patients of dysarthria, tonsillectomy, tonsilhypertrophy, and nasal obstruction were selected for this experimental clinical phonetic study. Formant variations ($F_1\;&\;F_2$) show pre- and post-operation differences in tonsillectomy and cleft palate patients. Nasal formants ($NF_1\;&\;NF_2$) show pre- and post-operation differences in nasal obstruction. The articulation reaction time (ART) as a parameter was used to assess Voice Onset Time(VOT). It was shown longer duration for hypokinetic dysarthria and shorter for atoxic dysarthria.. The diadochokinetic rate was measured by Visi-pitch. Lower diadochokinetic rate appeared to spastic and dysarthria in comparison with the control group. It was shown that the nasalance of tonsilhypertrophy, nasal obstruction, and cleft palate patients was seen to increase after operation. In addition, the assessment of nasality can be measured only by simple vowels such as /a/ and /i/.

비후성 비염환자에서 음성검사 및 음향비강통기도검사를 이용한 수술전후 비교 (Postoperative Change in Hypertrophic Rhinitis(Study Using Nasometer, CSL and Acoustic Rhinometer))

  • Background and Objectives : With the development of computerized systems, an objective evaluation methods of nasal speech and nasal geometry have become readily available by means of a simple, noninvasive technique. In this study, we assessed the nasality, nasal formant, nasal volume and nasal area in patients with hypertrophic rhinitis before and after turbinate surgery. Material and Method : With the nasometer, we measured nasalance, which reflects the ratio of acoustic energy output of nasal sounds from the nasal and oral cavities. With CSL 4300B, we measured nasal formants. We used acoustic rhinometer to measure nasal area and nasal volume. Postoperative changes of above factors were compared with preoperative values. Paired t-test and Pearson's correlation were used for statistical analysis. Results : The first nasal formant frequency, nasalance scores of three passages(baby, mamma and rabbit passages), minimal cross sectional area(MCA) of narrow side, nasal volume of narrow side and nasal volume of wide side had increased significantly after turbinate surgery (p <0.05). The MCA and nasal volume of narrow side and MCA of wide side showed significant correlation with nasalance score of rabbit passage and baby passage showed significant correlation with nasal volume of narrow side(p<0.05). Conclusion : There were significant increases in nasalance scores, first nasal formant frequency, MCA and nasal volume after turbinate surgery. Thus, we must consider the possibility of voice changes postoperatively in professional voice users.

  • There is close relationship between intraoral structural anomaly and speech- functional problem. Patient with cleft palate patients & ankyloglossia is a typical example, patients with structural anomaly is repaired toward normal structure by operation. Ankyloglossia may cause functional limitation even after adequate surgical treatment speech disorders being one of them. Interindividually, they vary a lot, showing typical articulation specifics. The objective of this study was to evaluate and compare speech for children with ankyloglossia and general public, to determine whether ankyloglossia is associated with articulation problem. We wanted to present criteria for indication of frenectomy. The group of subject is composed of 10 childrens with ankyloglossia and articulation problem, visited the Oral and Maxillofacial surgical unit, dental hospital, Chonbuk university. The average age is 5 Y 7M, M : F ratio is 8 : 2 at the time of speech test. Control group is composed of 10 members without oral structural anomaly. The average age is 5 Y 10M, M : F ratio is 3 : 7 at the time of speech test. Outcomes were measured the PPVT(Peabody Picture Vocabulary Test), PCAT(Picture Consonant Articulation Test), Nasometer II test result obtained each group, statistically measured by Mann-whitney's U Test. There was no difference for 'chronological age-age equivalent' between two group. There was significant difference for 'consonant accuracy' between two group, showed more lower scores in subject group. There was more 'consonant error' in subject group, mostly showed/1/,/s/. A major modality of 'consonant error' was mostly distortion and replacement. There was no significant difference between two group for nasality.

편도 및 아데노이드 수술 후 주관적 음성 변화의 양상과 인자 (Tendency and Parameters of Subjective Voice Change after Adenotonsillectomy or Tonsillectomy)

  • Some patients who have undergone adenotonsillectomy (AT) or tonsillectomy(T) complain of voice change. Research was designed to determine the relationship between subjective voice change and change of parameters in boice analysis and to find the degree of voice change according to age and excised tissue volume. In 34 patients who underwent AT and 17 patients who underwent T, we measured the masalance, the fundamental frequency of /a/phonation and the formants and bandwidths of /a/, /i/, /u/ phonations. These parameters were measured preoperatively and also 7 days after the operation. Excised tissue volume was measured on operation. Postoperative changes were statistically analyzed. Any subjective voice change was asked to be reported at the visit 7 days after the operation. There was analysis for the difference of above parameters between some who answered "voice change" (A group) and the others who answered "no voice change" (B group). 24 patients(71%) who underwent AT and 4 voice change. Nasals nasalance change is an important factor in subjective voice change after AT.

편도적출술이 구강 및 비강 음향스팩트럼에 미치는 영향 (Effects of Tonsillectomy on Oral and Nasal Spectral Outputs for Sustained Vowel)

  • Background and Objectives: It has been suggested that tonsillectomy possibly causes changes of voice because the morphology of the vocal tract is altered. This may cause serious problems for professional voice users. Materials and Method: Subjects were 26 patients. The oral and nasal sound spectrum of oral vowel /a/, /e/ and /i/ were measured before and after tonsillectomy. The formant frequencies and intensities for oral and nasal spectra were compared. The nasality and fundamental frequencies for oral vowel were measured. Results: The first formant frequencies for oral spectra of all vowels were not changed after surgery, but the second formant frequencies were increased significantly after surgery in the vowel /e/ and /i/. The first and second formant intensities for oral spectra were increased significantly after surgery in the all vowels. The first and second formant frequencies for nasal spectra of all vowels were not changed after surgery, but their intensities for nasal spectra were increased after surgery. The nasalities for oral vowel were not changed after surgery. Conclusion : Tonsillectomy appeared to change the spectral features of oral and nasal components of oral vowel, especially spectral intensities.

비음측정기를 이용한 한국어를 사용하는 정상 성인에서 성별에 따른 비음도의 차이에 관한 연구 (Gender Differences in Nasalance Scores in Korean Speaking Adults)

  • 이 연구의 목적은 한국어를 사용하는 성인에서 성별에 따른 비음도의 차이가 있는지 알아보고, 후천적 구개부 결손환자의 보철치료를 위한 기초자료로 사용을 위해 정상 비음도를 획득하는 것이다. 음성언어 표본으로 선정된 단순모음, 이중모음, 반복자음, 그리고 비음의 비율이 높은 문장, 비음의 비율이 중간인 문장, 비음의 비율이 낮은 문장에 대해 비음도의 평균과 표준편차를 구하고 남녀간의 차이가 있는지를 t-test와 Mann-Whitney test를 이용하여 분석하였다. 피실험자들은 서울 경기 지역에 거주하는 남자 15명과 여자 15명으로 구성되었고 이들 중 남자 피실험자들의 나이는 24세에서 38세까지 분포하였고 평균나이는 26.3세였다. 여자 피실험자들의 나이는 19세부터 33세까지 분포하였고 평균나이는 24.4세였다. 비음측정기 Nasometer II 가 실험에 사용되었다. 연구의 결과로 /이/ 모음을 포함하는 일부 반복 자음을 제외하고는 모음, 반복자음, 문장을 이용한 음성언어표본에서 남녀 간 비음도의 유의성 있는 차이가 발견되지 않았다. 비음측정기는 음성언어표본이 달라짐에 따라 비음도의 유의한 차이를 나타내었다.

언어습득기 이전 청각장애인의 후두소견 및 음성학적 특성 (Laryngeal Findings and Phonetic Characteristics in Prelingually Deaf Patients)

  • Background and Objectives : There are few studies reported that specifically examine the laryngeal function in patients with profound hearing loss or deafness, This study was designed to examine videostroboscopic findings and phonetic characteristics in adult patients with prelingually deaf. Materials and Method: Sixteen patients (seven males, nine females) diagnosed as prelingually deaf aged from 19 to 54 years, and were compared with a 20 normal control group with no laryngeal pathology and normal hearing group, Videostroboscopic evaluations were rated by experienced judges on various parameters describing the structure and function of the laryngeal mechanism during comfortable pitch and loudness phonations. Acoustic analysis test were done, and a nasalance test performed to measure rabbit, baby, and mother passage. CSL were measured to determine the first and two formant frequencies of vowels /a/, /i/, /u/, Statistical analysis was done using Mann-Whitney U or Wilcoxon signed ranks test. Results: Videostroboscopic findings showed phase symmetry but significantly more occurrences decrement in the amplitude of vibration, mucosal wave, irregularity of the vibration and increased glottal gap size during the closed phase of phonation, In addition, group of prelingually deaf patients were observed to have significantly more occurrences of abnormal supraglottic activities during phonation. The percentage of shimmer in the group of prelingually deaf patients were higher than in the control group. Characteristics of vowels were lower of the second formant of the vowel /i/. Nasalance in prelingually deaf patients showed normal nasality for all passages, Conclusion: Prelingually deaf patients show stroboscopic abnormal findings without any mucosal lesion, suggesting that they have considerable functional voice disorder. We suggest that prelingually deaf adults should perform vocal training for normalized laryngeal function after cochlear implantation.

높이가 다른 폐쇄장치에 대한 상악절제술 환자의 만족도 (Patients' satisfaction on the obturators with different extension heights into defects after maxillectomy)

  • 연구 목적: 이 연구의 목적은 결손부 내로 낮게 연장된 폐쇄장치와 높게 연장된 폐쇄장치를 환자의 만족도 및 선호도를 통해 비교하는 것이다. 연구 재료 및 방법: 상악절제술을 받고 보철치료를 받은 11명을 대상으로 결손부 내의 연장부의 높이가 다르고 나머지 형태가 통일한 폐쇄장치를 각 환자 당 2개씩 제작 하였다. 각 폐쇄장치를 2달 간 번갈아 사용하게 한 후 환자에게 폐쇄장치의 사용편의성, 발음, 비음성, 누출, 저작에 대한 폐쇄장치들의 만족도를 설문을 통해 조사하고, 최종적으로 환자들의 선호도와 그 이유를 조사하여 분석하였다. 폐쇄장치에 따른 차이를 Wilcoxon signed rank test를 사용하여 분석하였다 (P<.05). 결과: 구개 결손부 내로의 연장부의 높이가 낮은 폐쇄장치와 높은 폐쇄장치에 대해 환자들의 평가는 차이가 없었다 (P>.05). 그리고 높은 폐쇄장치와 낮은 폐쇄장치에 대한 환자들의 선호도는 다양하였다. 결론: 상악절제술 후 개구가 제한되는 상황이나 보철물의 삽입로가 제한되는 등 특수한 상황에서는 연장부가 낮은 높이의 폐쇄장치가 추천될 수 있다.
