Background: The success rate of intubation under direct laryngoscopy is greatly influenced by laryngoscopic grade using the Cormack-Lehane classification. However, it is not known whether grade under direct laryngoscopy can also affects the success rate of nasotracheal intubation using a fiberoptic bronchoscpe, so this study investigated the same. In addition, we investigated other factors that influence the success rate of fiberoptic nasotracheal intubation (FNI). Methods: FNI was performed by 18 anesthesiology residents under general anesthesia in patients over 15 years of age who underwent elective oral and maxillofacial operations. In all patients, the Mallampati grade was measured. Laryngeal view grade under direct laryngoscopy, and the degree of secretion and bleeding in the oral cavity was measured and divided into 3 grades. The time required for successful FNI was measured. If the intubation time was > 5 minutes, it was evaluated as a failure and the airway was managed by another method. The failure rate was evaluated using appropriate statistical method. Receiver operating characteristic (ROC) curves and area under the curve (AUC) were also measured. Results: A total of 650 patients were included in the study, and the failure rate of FNI was 4.5%. The patient's sex, age, height, weight, Mallampati, and laryngoscopic view grade did not affect the success rate of FNI (P > 0.05). BMI, the number of FNI performed by residents (P = 0.03), secretion (P < 0.001), and bleeding (P < 0.001) grades influenced the success rate. The AUCs of bleeding and secretion were 0.864 and 0.798, respectively, but the AUC of BMI, the number of FNI performed by residents, Mallampati, and laryngoscopic view grade were 0.527, 0.616, 0.614, and 0.544, respectively. Conclusion: Unlike in intubation under direct laryngoscopy, in the case of FNI, oral secretion and nasal bleeding had a significant effect on FNI difficulty than Mallampati grade or Laryngeal view grade.
The subjects of this study consisted of 58 patients with Le Fort fractures, who were admitted to Chonbuk National University Hospital from Jan. 1988 to Oct. 1995. The author classified the maxillofacial fractures by Le Fort classification and examined the. incidence of Le Fort fractures by age, sex and etiology. The purpose of this study was to compare the imagings of conventional radiograms with those of computed tomograms and to aid in the diagnosis of patients with Le Fort fractures. The obtained results were as follows: 1. The Le Fort fractures occured mainly in 3rd and 4th decades, but there was no significant difference between decades. A ratio of men to women was 5.5 : 1. The major etiologic factors were traffic accident(69%), assault(14%) and fall-down(12%). 2. The most common type of Le Fort fracture was type I(49.3%). The numbers of Le Fort II, ill fractures were 41.3%, 9.4% respectively. And 8.6% were midsagittal splitting fractures. 3. The discontinuity and haziness of the maxillary sinus were easily detected by Waters' view. In skull PI A view, it was difficult to observe fractures due to overlapping of the other structures, but the haziness of the maxillary sinus was sometimes observed. 4. In Le Fort fracture, the fracture pattern of orbit, the wall of maxillary sinus, nasal bone and pterygoid plate were more easily detected in computed tomograms than in conventional radiograms.

Purpose: This study is to evaluate the location of descending palatine artery, the anatomy of pterygomaxillary junction, and the association between the obtained anatomic values and several variables. Methods: We studied 40 patients who were treated for dentofacial deformites from January 2010 to December 2012 in Ulsan University Hospital, Ulsan, Korea. Cone beam computed tomogram (CBCT) was done for all patients. The reference point was approximately 5 to 7 mm above anterior nasal spine on axial image. We evaluated the location of the greater palatine canal (line a: on the coronal view, the shortest line between the center of greater palatine canal and pterygoid fossa; distance a: the distance of line a). We also measured the thickness (line b: on the coronal view, the shortest line between maxillary posterior sinus wall and pterygoid fossa; distance b: distance of line b), width (line c: on the coronal view, the line perpendicular to the line b and the nearest line from the most concave point of lateral pterygoid plate to the medial pterygoid plate; distance c: distance of line c) and height (line d: on sagittal view, the vertically longest line of pterygoid junction; distance d: the distance of line d) in pterygomaxillary junctions. We evaluated the association between the obtained anatomic values and several variables (sex, age, height and weight). Results: The mean distance a was 4.78 mm, mean distance b was 5.53 mm, mean distance c was 8.01 mm and mean distance d was 13.22 mm. The differences between age and mean distance c and weight and mean distance d in pterygomaxillary junctions are statistically significant. Conclusion: There apparently is anatomic variation of pterygomaxillary junctions by various values, particularly weight and age in a Korean clinical population.

Allergic disease(atopic dermatitis, bronchial asthma, rhinitis) were related with lung, spleen, kidney. To prevent allergic disease in born, Oriental medicine taked a serious view of fetal education. In Western medicine, The cause of allergy applied to exopathogen of Oriental medicine. In Oriental medicine, Treatment of allergy used generally strengthening the body resistance to eliminate pathogenic factors.

For this study, 20 hearing-impaired adults and 20 normal hearing adults as a control group were asked to read 7 Korean vowels(/$\alpha$, $\Lambda$, o, u, ɯ, i, $\varepsilon$/). Subjects' readings were recorded by NasalView and analyzed by Praat. Results showed that the means of F0 were significantly higher in the hearing impaired group(HL) than in the normal hearing group(NH), in the female group than in male group, and in high vowels than in low vowels. Second, intensity was significantly higher in the normal hearing group(NH) than in the hearing impaired group(HL), in male group than in female group, and in low vowels than in high vowels. Third, jitter was significantly higher in the normal hearing group(NH) than in the hearing impaired group(HL), and in female group than in male group and in the back vowels than in front vowels. Finally, shimmer was significantly higher in the normal hearing group(NH) than in the hearing impaired group(HL), and in male group than in female group. In particular, the male group showed that front vowels tend to have higher shimmer than back vowels.
Using eighteen text materials from various goners of present-day Japanese, we collected phonologically reduced forms frequently observed in conversational Japanese, and classified them in search of unified explanation of phonological reduction phenomena. We found 7,516 cases of reduced forms which we divided into 43 categories according to the types of phonological changes they have undergone. The general tendencies ale that deletion and fusion of a phoneme or an entire syllable takes place frequently, resulting in the decrease in the number of syllable. Typical examples frequently observed throughout the materials are : $~/noda/{\rightarrow}~/nda/,{\;}-/teiru/{\rightarrow}~/teru/,{\;}~/dewa/{\rightarrow}~/zja/,{\;}~/tesimau/{\rightarrow}~/cjau/$. From morphosyntactic point of view phonological reduction often occurs at the NP and VP morpheme boundaries. The following findings are drawn from phonological observations of reduction. (1) Vowels are more easily deleted than consonants. (2) Bilabials(/m/, /b/, and /w/ are the most likely candidates for deletion. (3) In a concatenation of vowels, closed vowels are absorbed into open vowels, or two adjacent vowels come to create another vowel, in which case reconstruction of the original sequence is not always predictable. (4) Alveolars are palatalized under the influence of front vowels. (5) Regressive assimilation takes place in a syllable starting with ill, changing the entire syllable into phonological choked sound or a syllabic nasal, depending on the voicing of following phoneme.

The parameters include F1, F2, Euclidean Distance between vowels, and vowel triangular area comprised of /i/, /a/ and /u/. For this study, 20 hearing-impaired and normal hearing adults as a control group were asked to read 7 Korean vowels (/a, $\wedge$, o, u, w, i, $\varepsilon$/). Subjects' readings were recorded by NasalView and analyzed by Praat. Results showed that F1 were significantly higher in the hearing impaired group than in the normal hearing group, higher in the female group than in male group, and higher in low vowels than in high vowels. A 54-year-old man, suffering from severe headache and ophthalmoplegia after undergoing endoscopic sinus surgery was referred to a tertiary hospital. Computed tomography (CT) revealed soft tissue density lesions in the left sphenoid sinus. The internal carotid artery was shown to be occluded in brain magnetic resonance imaging (MRI) scans without any other cerebral lesion. Endoscopic view of left nasal cavity shows whitish hyphae in the ethmoid and the sphenoid sinuses. We diagnosed him with cavernous sinus syndrome caused by mucormycosis and conducted endoscopic sinus surgery to remove remaining lesions and decompress orbit and optic nerves. After the revision surgery the patient's headache and ophthalmoplegia were improved. However, multifocal cerebral infarctions were newly discovered in a postoperative CT scan. We experienced a case of mucormycosis of sphenoid sinus resulting in occlusion of internal carotid artery and multifocal cerebral infarction, and report it with a brief review of these disease entities.
