Augmentation of Pyriform Margin Using Porous High-Density Polyethylene Sheet In Unilateral Cleft Lip Nasal Deformity (일측성 구순열비변형에서 다공성 폴리에틸렌 판을 이용한 상악골이상구증대술)

  • Purpose: The common deformity after the correction of unilateral cleft lip nasal deformity is nasal asymmetry, and it is caused by the hypoplasia of the pyriform aperture. To correct this, many procedures have been applied, but still many problems are present. Authors performed the inlay and onlay insertion of porous high density polyethylene sheet(1 mm thickness $Medpor{(R)}$ sheet) in the hypoplastic pyriform margin of cleft side and obtained satisfactory results. Methods: 11 cases were performed and the mean follow up period was 15.1 months. Their mean age was 23.6 years. Under general anesthesia, bilateral pyriform margin was exposed. $Medpor{(R)}$ sheets in "match stick" like shaped were inlay inserted, and kidney shaped were onlay inserted fixating with two 6 mm titanium screws. After the surgery, the results was evaluated by photogrammetric analysis. On the basal view, the distance from the subalare and labiale superius' to the transverse baseline connecting the both cheilions was measured from the cleft side and the non-cleft side. Then, the postoperative symmetry was assessed by obtaining the cleft side against the non-cleft side as proportion index, defined as lateral and medial upper lip contour index. Results: There were 2 infections. The cause was because the inserted implant was too long and thus protruded to the base of nasal cavity. The lateral upper lip contour index was from 95.49 to 103.27, and medial upper lip contour index was from 90.92 to 100.49, it was statistically increased, and thus the symmetry was improved. However clinically mild depression remained at nostril floor. Conclusion: Authors performed porous high density polyethylene sheet inlay and onlay insertion for the hypoplasia of the pyriform margin in unilateral cleft lip nasal deformity. It was found that depressed pyriform margin and upper lip were corrected effectively except for the nostril floor, for which an additional soft tissue augmentation would be necessary. The inlay insertion has risk of protrusion, thus the guideline of the use of artificial prosthesis should be observed strictly.

Correlations between anatomic variations of maxillary sinus ostium and postoperative complication after sinus lifting

  • Objectives: The maxillary sinus mucosa is reported to recover to preoperative sterility after sinus floor elevation. However, when drainage of maxillary sinus is impaired, recovery can be delayed and maxillary sinusitis can occur. Therefore, in this study, we investigated the correlations between anatomic variants that can interrupt the ostium of the maxillary sinus and incidence of complication after sinus lifting. Materials and Methods: The subjects are 81 patients who underwent sinus lifting in Wonkwang University Dental Hospital (Iksan, Korea). Computed tomography (CT) images of the subjects were reviewed for presence of nasal septum deviation, anatomic variants of the middle turbinate, and Haller cells. Correlations between anatomic variations and occurrence of maxillary sinusitis were statistically analyzed. Results: Patients with anatomic variants of ostio-meatal units, such as deviated nasal septum, concha bullosa or paradoxical curvature of the middle turbinate, or Haller cells, showed a higher rate of complication. However, only presence of Haller cell showed statistically significant. Conclusion: Before sinus lifting, CT images are recommended to detect anatomic variants of the ostio-meatal complex. If disadvantageous anatomic variants are detected, the use of nasal decongestants should be considered to reduce the risk of postoperative sinusitis.

Anthropometry of Nose in Korean Twenties (20대 한국인 코의 계측치)

  • Purpose: The aim of this study is to establish anatomical dimension of the nose and nostril shape in Korean twenties. Methods: Authors measured anthropometry of nose in Korean youths 1,000 individuals (male 363, female 637) and analyzed stastically them. Results: The mean measurements are as follows. 1. The shape of nostrils was classified into 7 types by the angle between both long axis of nostril (male / female). Type I ($<40^{\circ}$)1.7% / 1.72%, Type II ($41-70^{\circ}$) 26.17% / 29.35%, Type III ($71-100^{\circ}$) 39.94% / 38.77%, Type IV (unclassified) 3.85% / 5.80%, Type V ($101-130^{\circ}$) 20.93% / 17.89%, Type VI ($131-180^{\circ}$) 7.43% / 5.96%, Type VII ($>180^{\circ}$) 0% / 0.47% 2. A LATERAL RHINOTOMY INCISCON : CASE REPORTS AND REVIEW OF LITERATURES (Lateral rhinotomy 접근법에 대한 증례보고 및 문헌고찰)

  • The lateral rhinotomy signifies only an incision and not on operation and a lateral rhinotomy incision with osteotomy of the nasal bones provides access to the entire nasal cavity and maxillary, ethmoid, and sphenoid sinuses as well as the frontal sinus if the floor is removed, permitting removal of benign lesions at these sites and en bloc resection of the ethmoid labyrinth and the party wall between the nasal cavity and antrum with infiltrating tumors. The authors treated a tumor patient and a midfacial bone fracture patient via lateral rhinotomy approach and had a good result. So we report the cases with literature review.

  • Authors experienced a case of intermediate grade mucoepidermoid carcinoma in 29-year-old female. The origin of this tumor might be mucus secreting cells of palatal mucous gland. The characteristic features were as follows: 1. In clinical examination, exopytic mass was observed in left hard palate and similar to pleomorphic adenoma. 2. In radiographic findings, destructive changes of left hard palate, nasal septum, nasal inferior turbinate and floor of maxillary sinus and soft tissue mass were observed. 3. Histopathologically, mucus-secreting cells, epidermoid cells and intermediate cells were observed and diagnosed as intermediate grade mucoepidermoid carcinoma.

Usefulness of Silicone Plate for Sellar Floor Reconstruction (터어키안 저부 재건술시 실리콘 판의 유용성)

  • Objectives : At the closure of the transsphenoidal approach(TSA), the proper sellar floor reconstruction plays an important role in preventing postoperative complications. The septal cartilage, perpendicular plate of nasal septum, and the sphenoid sinus bone are usually used to repair the sellar floor as a bone splint. The authors evaluate the usefulness of a silicone plate as a substitute for bone splint to close a defect of the sellar floor. Materials and Methods : A silicone plate was used to repair the sellar floor in 7 patients with sellar lesions which included four pituitary adenomas, two Rathke's cleft cysts and one metastatic tumor. Among seven cases, five cases underwent a standard TSAs and two received a extended TSAs. The trajectories of the approach were sublabial in four cases and endonasal routes in three cases. The silicone plate for implantation was cut to a size of slightly larger than that of bone window and inserted with a three-pronged fork, and then adjusted precisely. Results : In six patients, there were no complications which related to sellar floor reconstruction. A postoperative cerebrospinal fluid(CSF) rhinorrhea was observed in one patient with pituitary macroadenoma. Conclusions : From the authors' experience, the advantages of the silicone plate are its simplicity of molding to fit any size of sellar floor defects, and easy detection of previously created bone window at reoperation.

Maxillary sinus pneumatization after maxillary molar extraction assessed with cone beam computed tomography (Cone Beam형 전산화단층영상을 이용한 상악대구치 발치 후 상악동 함기화 평가)

  • Purpose : The purpose of this study was to examine the inferior expansion of the maxillary sinus floor following maxillary molar extraction. Materials and Methods : Cone beam computed tomographic images of 59 subjects were used to evaluate the height difference of the maxillary sinus floor between extraction sites and contralateral dentate sites. The height of the maxillary sinus floor was defined as the vertical distance to the Frankfort plane from the level of the anterior nasal spine to the most inferior point of the sinus floor. We examined the difference in sinus pneumatization according to the number of missing teeth and the vertical relationship of the molar roots to the sinus floor. Results : The inferior expansion of the maxillary sinus floor was $1.20{\pm}1.86\;mm$ on the maxillary first molar and $1.90{\pm}2.42\;mm$ on the maxillary second molar. Increased expansion was observed in cases where two proximate molars were extracted. There was no significant difference in sinus pneumatization following extraction according to the vertical relationship of the molar roots to the sinus floor. Conclusion : The results of this study confirm that sinus pneumatization occurs following maxillary molar extraction. In situations where pneumatization can affect treatment after molar extraction, three-dimensional radiography should be considered.

New Landmark for the Endoscopic Endonasal Transsphenoidal Approach of Pituitary Surgery

  • Objective : To clarify the anatomical correlations of the sphenoid sinus with surrounding structures in the normal Korean population, and to identify surgical landmarks for safe sellar floor dissection in the anterior skull base by endoscopy and microscopy. Methods : We reviewed the 196 brain magnetic resonance imaging findings showing a normal appearance, and measured the distances between anatomical landmarks. Results : The mean distances from the base of the columella to the anterior wall of the sphenoid sinus and the sellar floor were $69.71{\pm}4.25$ mm and $86.26{\pm}4.57$ mm, respectively in the over 15 age group, and showed the smallest degree of variation among the measurements. The mean angles between the floor of the nasal cavity and the straight line connecting the base of the columella and the sellar floor were $29.45{\pm}3.25^{\circ}$ and $24.75{\pm}4.00^{\circ}$ in the over 15 and under 15 age groups, respectively. The mean values of both distances and angles increased with age until 15 years after which no further increases were evident. There were no significant differences in the measurements between males and females or among subjects with different degrees of pneumatization in the over 15 age group. Conclusion : The distances from the base of the columella to the sellar floor and the anterior wall of the sphenoid sinus, which were consistent among individuals, could be used as a surgical indicator to investigate the sellar floor in endoscopic or microscopic transsphenoidal approaches.

A ROENTGENOCEPHALOMETRIC STUDY FOR THE ACCEPTABLE PROFILE IN KOREAN ADULTS (한국성인의 측모에 관한 두부방사선학적 연구)

  • The primary objective of this study was to analyze the hard and soft tissue profile and comparison of the both sexes in Korean adults using roentgenocephalometry. The subjects were 122 males and 116 females from 18 to 22 years of age selected on the basis of their excellent occlusion and harmonious profile. It was an investigation about the interrelationships in the angles which consisted of Frankfort horizontal plane (FH plane), Sella Nasion plane (SN plane), Nasal floor plane (NF plane) and Mandibular plane (Md plane) as reference planes, and of the axial inclinations of the upper anterior tooth to Nasal floor plane (NF Plane) and of the lower anterior tooth to Mandibular plane (Md Plane). Among the 14 categories estabilishied on the soft tissue land marks, the 6 categories were studied for the relationship to Frankfort horizontal plane (FH Plane), and the 8 categories were studied for the soft tissue contour. The results were as follows: 1. Among 8 measured angles in the hard tissue, the angle of FH-Md, NF-Md and SN-Md were greater in females than in males. 2. The axial inclinations of the anterior teeth to NF Plane and Md plane were slightly greater in males than In females. 3. Among 14 measured angles in the soft tissue, the angles of G-Sn-P and E-Pog-Sn were greater in males, and FH-G-Sn, GㆍSn-Sn. Pog and Pog-Sn-P were greater in females. 4. In males, the variability of measured values in both lips region were high.

  • The primary method of diagnosis in craniofacial deformity patient is used by gross inspection. So we studied the frontal photometric measurements for maxillofacial Korean normals 44 male and 48 female ages 19 to 20 years were selected from a group of 92 healthy Kyung Hee university students in Seoul.. The photograph were taken with the subject sitting in head position parallel with interpapillary plane to the floor. During taking pictures, facial anallyser by Obwegeser(Martin Co., Germany) was used for correct measurements. The results of this study was followed. 1. In the linear measurements, all values of males were longer than those of females but middle third height and lower third height, facial width and nasal width of males were longer than those of females in p<0.05 2. The difference between right and left angular and linear measurement was insignificant. 3. In the horizontal facial ratios, interocular distance and nasal width were longer and mouth width were smaller in korean than in american(p<0.05). 4. In the vertical ratios, korean women have shorter facial height of lower third comparing with facial height of middle third than american(p<0.05). 5. In the vertical facial ratios, korean have smaller proportion below the stomion in the lower third department than american. (p<0.05).
